Luanda acolhe a 21.ª Conferência Extraordinária de Chefes de Estado e de Governo da União Africana, centrada no reforço da prevenção e gestão de conflitos. O especialista em relações internacionais Osvaldo Mboco analisa os limites dos mecanismos da organização, o peso do financiamento externo e o papel que Angola procura consolidar na diplomacia de paz africana. Luanda acolhe a 21.ª Conferência Extraordinária de Chefes de Estado e de Governo da União Africana num momento em que o continente continua marcado por conflitos prolongados, novas ameaças à segurança e uma crescente pressão geopolítica. A cimeira quer reforçar a capacidade da União Africana para agir antes de as crises se transformarem em guerras. A chamada Decisão de Luanda pretende ligar compromissos a metas, responsáveis, financiamento e prazos. Para Angola, o encontro é também uma oportunidade para consolidar o seu papel na diplomacia de paz africana. Para analisar o alcance desta cimeira e o papel de Angola, conversámos com Osvaldo Boco, especialista em relações internacionais, que acompanha a política externa angolana e a dinâmica diplomática da União Africana. RFI: O que é que torna esta cimeira diferente das anteriores? Osvaldo Mboco: De forma directa, nada a torna diferente. Mas o facto de os Chefes de Estado e de Governo do continente africano entenderem que devem reforçar os mecanismos de prevenção e gestão de conflitos quer dizer que é um passo. Eles percebem que esses instrumentos, esses mecanismos, apesar de serem actuais, não são eficientes nem eficazes para resolver os problemas que o continente africano enfrenta. É importante analisar de que forma se pretende reforçar esses mecanismos, porque há vários: há os políticos, os jurídicos e os institucionais. Há uma série de instrumentos e mecanismos ao nível da União Africana. O nosso grande problema tem sido, primeiro, a eficiência e a eficácia, o facto de serem accionados quando a situação já decorre. Depois também, por exemplo, agora falamos muito do Fundo de Paz. O Fundo de Paz da União Africana dá nota de que os países signatários devem, de facto, doar 0,2% das suas exportações, mas, ainda assim, não é suficiente para aquilo que se pretende. A União Africana já tem mecanismos de prevenção de conflitos, como referiu. Por que motivo continuam a falhar? Há muitas variáveis no meio dessas falhas. Primeiro, a vontade política dos actores envolvidos num determinado conflito e a falta de capacidade dos Estados para contribuírem para que esses mecanismos, de facto, funcionem. Estou a falar aqui do ponto de vista do financiamento. O financiamento é um dos grandes problemas que o continente africano, ou as organizações, têm. E, se reparar, temos o Fundo de Paz, como referi inicialmente, que é um fundo importante, mas para o qual os Estados não têm estado a contribuir como deveriam. Estima-se que 0,2% das exportações dos Estados deva ser direccionado para este Fundo de Paz. Penso que agora são entre 16 e 17 países que são membros desse Fundo de Paz. Isto tem estado a inviabilizar uma série de questões. É crucial que os mecanismos não fiquem simplesmente ao nível das organizações, ao nível da União Africana. Se olharmos para as sanções económicas, que também são um mecanismo numa situação de crise, elas dependem, em grande medida, da capacidade unilateral dos Estados para aplicar uma ou outra sanção a alguns Estados. Então, temos, ao nível do continente africano, uma série de questões que julgo que deveriam ser reflectidas e para as quais se deveriam procurar soluções no continente, em função do que vai decorrendo. Claramente, ao nível da própria organização, não vamos conseguir todas as soluções. É fundamental que também se olhe para a necessidade de os Estados ou os governos terem uma gestão parcimoniosa, porque muitos dos problemas que acontecem resultam da governação dos governos: uma boa governação, a questão do amadurecimento das democracias nos Estados, a questão do respeito pelas instituições dos Estados e pelas próprias Constituições, que muitas vezes são alteradas no interesse da manutenção de um determinado grupo que tem intenção de permanecer no poder. São também essas questões que penso que o continente africano deveria trabalhar e articular da melhor forma possível, para evitar que esses assuntos internos dos Estados tenham sérias implicações na própria segurança colectiva, porque depois desencadeiam conflitos e crises, com muitas implicações no próprio funcionamento das instituições. Até onde pode ir a União Africana na prevenção de conflitos sem entrar em choque com a soberania dos Estados? É quase impossível não entrar em choque com os interesses e a soberania dos Estados. O princípio da não ingerência nos assuntos internos dos Estados, ao nível do continente africano, continua a ser, por um lado, um elemento que salvaguarda e respeita a soberania e, por outro, um elemento que inibe a União Africana de actuar sobre determinados assuntos. É fundamental olhar para os mecanismos. Por exemplo, uma força de manutenção da paz para um determinado Estado: há ou não qualquer violação? É fundamental olhar para uma situação de crise humanitária ou para uma actuação num país onde as questões ligadas aos direitos humanos estão a ser violadas: até que ponto a União Africana pode intervir? Claro que, sempre que se coloca em causa a vida, ou perante uma situação de genocídio em curso, há esta visão de que a União Africana pode intervir. Mas essa intervenção da União Africana acaba por não ganhar a dimensão que pretendemos, porque a União Africana tem limitações, sobretudo do ponto de vista dos recursos, e não consegue manter um contingente militar durante muito tempo numa missão de manutenção da paz ou numa missão de verificação, por exemplo, de desarmamento ou de acantonamento que esteja a decorrer. As limitações da União Africana, em primeiro lugar, estão nos mecanismos a serem utilizados para alterar uma determinada situação e também na capacidade financeira para custear determinadas operações. Mas, ainda assim, a União Africana não tem força suficiente para alterar uma situação que decorre num determinado Estado. Daí falarmos do Sistema Continental de Alerta Precoce. Ou seja, esse sistema ou instrumento permite que, sempre que exista a possibilidade, indícios ou indicadores de que poderá decorrer uma situação que ponha em causa a própria estabilidade do país, ele seja accionado. Agora, o que é discutível é até que ponto este sistema tem capacidade de impedir que esta situação aconteça, porque mesmo os contingentes militares que podem ser desdobrados ao nível da União Africana dependem essencialmente de quê? Dependem dos Estados, que vão, de facto, ceder os militares ou os efectivos. Dependem do financiamento dessa mesma operação. E depois há uma série de questões logísticas, salários que devem ser pagos e tudo mais, que dificultam a acção da própria União Africana. Mas não estamos a desmerecer uma conferência desta natureza. Quando se reflecte sobre estes assuntos, julgo que é importante para se encontrarem soluções que vão ao encontro daquilo que é a matriz dos conflitos ao nível do continente africano. A Decisão de Luanda pode produzir resultados concretos ou corre o risco de ficar apenas no papel? Dependerá muito da vontade política e dos compromissos assumidos por parte dos actores. Mas como sempre? Sim, como sempre. Agora, Luanda tem a vantagem de ter um histórico, de ser um actor importante em matéria de gestão e resolução de conflitos. É um actor com aceitação. A sua diplomacia tem demonstrado capacidade de organização deste tipo de eventos, desta natureza. E é também o momento em que Luanda fica, mais uma vez, sob os "holofotes", com algumas aspas, da imprensa internacional. É necessário que se produzam resoluções e soluções que vão ao encontro da resolução dos problemas que o continente atravessa. Que peso político e diplomático pode Angola ganhar ao liderar esta agenda? Bem, o peso político e diplomático que Angola pode ganhar ao liderar essa agenda é continuar a consolidar o seu papel como um dos actores centrais na pacificação dos conflitos africanos, consolidar a sua imagem como um dos Estados a que é reconhecida capacidade de liderança e de congregar outros actores sobre o mesmo assunto. São mais ganhos diplomáticos que Angola vai obtendo, bem como de imagem e reconhecimento internacional sobre esta matéria. Daí olharmos para determinados actores internacionais que vêem Angola como um país que, para além de ser estável, é um actor importante em matéria de gestão e resolução de conflitos ao nível do continente africano. Penso que Angola, ao posicionar-se desta forma, vai mantendo aquilo que é a sua imagem e a sua projecção internacional. O financiamento externo continua a limitar a autonomia da União Africana na gestão de conflitos? Claramente que sim, porque, em grande medida, os africanos dependem do financiamento externo. O financiamento externo, às vezes, vem com condicionalismos, e esses condicionalismos devem ser, digamos, tratados, devem ser, até certo ponto, equacionados no âmbito da resolução que se pretende. Ouvindo o porta-voz da conferência, que é o embaixador Bembe, ele deu nota de que o grande objectivo de não convidar os parceiros é "arrumar a casa", respeitando e reconhecendo aquilo que tem sido a parceria, mas, neste caso concreto, pretende-se ir buscar soluções africanas para os problemas africanos. É uma boa nota, mas é importante que a busca de soluções para os problemas africanos passe também pela capacidade de executar esses instrumentos de forma independente e de ter capacidade para pagar e custear todas as acções que julgamos serem importantes para a busca da paz e da estabilidade. Penso que África deve deixar de ser o parente pobre do sistema internacional, procurar formas de andar sozinha e encontrar soluções estruturais para os seus próprios problemas. Claramente, a cooperação e a parceria são extremamente importantes para as próprias dinâmicas das relações internacionais, das relações do continente africano com outros actores internacionais. Mas, ainda assim, é fundamental que África encontre soluções que se adequem ao seu contexto e procure, sem sombra de dúvida, dar respostas às questões de estabilidade no continente africano. Porque grande parte dos conflitos que decorrem no mundo decorre no continente africano. Muitas das questões discutidas no Conselho de Segurança das Nações Unidas e que ocupam a sua agenda são, maioritariamente, problemas africanos ligados à estabilidade e ao conflito. Então, é altura de olhar com maior profundidade, estudar as dinâmicas dos conflitos em África, estudar os diferentes intervenientes e procurar soluções que se adequem a esta matriz de conflito.

Understanding which one you're considering — and why — changes everything about how you plan. A Lesson From Healthcare History Before committing to any practice model, it helps to understand what happened to the movements that came before it. This episode traces three instructive parallels: the micropractice and ideal medical practice movement of the early 2000s; the decades-long fight for healthcare price transparency and what happened when physicians finally got it; and the rise and reality check of retail health — what scaled, what didn't, and why. The common thread in every model that has achieved durable scale in American healthcare is the same: structural fit with the economic environment, not ideological purity. Two Pathways, One Brand Name The episode walks through both economic models in the cash-pay primary care space — the purist, cash-only, no-insurance model and the employer-integrated model — explaining how each works, who each serves, and what the financial picture actually looks like for physicians considering either path. The revenue math is done out loud. The sustainability data from peer-reviewed research is cited. The patient demographic fit for each model is examined honestly and specifically. Who Each Model Serves — and Where Other Models Fit Better A detailed breakdown of the patient populations each model genuinely serves well — and an honest, evidence-based look at the patient populations where other models may be a better structural fit. Including Medicare-eligible patients, patients with complex chronic disease, lower-income households, and employees of small and mid-sized businesses. The Overlooked Opportunity — NPs, PAs, and Advanced Practice Clinicians One of the most significant and underexplored opportunities in subscription-based healthcare delivery today is the direct-care model as a pathway for nurse practitioners, physician assistants, and other advanced practice clinicians. The evidence on NP and PA-led primary care outcomes is strong and peer-reviewed. The physician shortage projections make the need urgent.
