Hosted by Michael Tetreault | Editor-in-Chief, Concierge Medicine Today Episode Overview In one of the most comprehensive episodes in DocPreneur Leadership Podcast history, host Michael Tetreault takes an honest, evidence-based, and encouraging look at the cash-pay and subscription-based primary care landscape — who it serves, how it works, where it's heading, and what every physician and advanced practice clinician needs to understand before making a career-defining decision. This episode doesn't take sides. It takes a clear-eyed look at the full picture — including the parts that don't always make it into the conference keynote. What's Covered in This Episode The Foundation Not all subscription-based primary care models are the same. Two models operating in this space share surface-level similarities but are structurally distinct businesses with different economic logic, different patient populations, and different long-term trajectories. Understanding which one you're considering — and why — changes everything about how you plan. A Lesson From Healthcare History Before committing to any practice model, it helps to understand what happened to the movements that came before it. This episode traces three instructive parallels: the micropractice and ideal medical practice movement of the early 2000s; the decades-long fight for healthcare price transparency and what happened when physicians finally got it; and the rise and reality check of retail health — what scaled, what didn't, and why. The common thread in every model that has achieved durable scale in American healthcare is the same: structural fit with the economic environment, not ideological purity. Two Pathways, One Brand Name The episode walks through both economic models in the cash-pay primary care space — the purist, cash-only, no-insurance model and the employer-integrated model — explaining how each works, who each serves, and what the financial picture actually looks like for physicians considering either path. The revenue math is done out loud. The sustainability data from peer-reviewed research is cited. The patient demographic fit for each model is examined honestly and specifically. Who Each Model Serves — and Where Other Models Fit Better A detailed breakdown of the patient populations each model genuinely serves well — and an honest, evidence-based look at the patient populations where other models may be a better structural fit. Including Medicare-eligible patients, patients with complex chronic disease, lower-income households, and employees of small and mid-sized businesses. The Overlooked Opportunity — NPs, PAs, and Advanced Practice Clinicians One of the most significant and underexplored opportunities in subscription-based healthcare delivery today is the direct-care model as a pathway for nurse practitioners, physician assistants, and other advanced practice clinicians. The evidence on NP and PA-led primary care outcomes is strong and peer-reviewed. The physician shortage projections make the need urgent. And the organizational infrastructure for advanced practice clinician-led direct-care practices is largely unbuilt — which means the opportunity belongs to whoever moves first. The Organizational Landscape An honest look at what the multiplicity of organizations, coalitions, and alliances in the cash-pay primary care space tells us — and what research on professional association dynamics says about the long-term implications of organizational fragmentation for legislative effectiveness and individual practice planning. One Brand, Two Directions Drawing on four documented historical parallels from the history of American medicine — the AMA and managed care, osteopathic medicine's identity divide, family medicine's emergence as a separate specialty, and the micropractice movement — the episode makes the case that two communities with genuinely different economic interests and regulatory priorities currently sharing a brand name may, consistent with historical precedent, find their own distinct professional homes over time. This is presented as pattern recognition grounded in verified historical evidence — and as practical planning context for physicians building practices today. The Tax and Structuring Update A clear, practical summary of the 2025 "One Big Beautiful Bill" Act changes — effective January 2026 — and what they mean for HSA eligibility of cash-pay membership fees. What qualifies, what doesn't, and why legal counsel is essential before making any representations to patients about tax-advantaged payment options. Eight Questions Before You Commit A practical pre-decision checklist — eight specific questions every physician or advanced practice clinician should be able to answer clearly before committing to any cash-pay practice pathway. Key Takeaways Cash-pay primary care and concierge medicine are not the same model, do not serve the same patient populations, and should not be evaluated as interchangeable alternatives. The purist cash-pay model has grown from approximately 100 practices in 2009 to over 2,100 by 2023 — real and meaningful growth. The financial sustainability data, however, reflects consistent challenges that peer-reviewed research has documented specifically in lower-income markets and solo practice settings. The employer-integrated pathway has stronger structural sustainability — multiple revenue streams, embedded benefit relationships, and documented employer cost reductions of 12 to 20 percent over three to five years. A December 2025 Johns Hopkins study found concierge and cash-pay primary care practices combined grew 83.1 percent between 2018 and 2023. The employer-integrated model is the primary driver of that growth trajectory. Concierge medicine — particularly the PCM model — is not retreating. The global concierge medicine market is projected to surpass $34 billion by 2032 and is growing at a compound annual rate that outpaces most healthcare market segments. The National Academy of Medicine's 2021 Future of Nursing report, AAMC physician shortage projections, and peer-reviewed NP/PA outcomes research collectively point to advanced practice clinician-led direct-care models as one of the most significant underexplored opportunities in subscription-based healthcare delivery. Pattern recognition from healthcare history — price transparency, retail health, the micropractice movement — consistently shows that the distance between a compelling healthcare idea and durable scaled impact is longer and more complicated than early advocacy suggests. Models that have achieved durable scale in American primary care share one characteristic: structural fit with the economic environment, not independence from it. At least five Native American men were detained January by Immigration and Customs Enforcement (ICE) during raids in Minneapolis, Minn. As other reports of Native Americans being mistaken for undocumented immigrants continue, federal lawmakers have introduced a bipartisan bill to improve the Department of Homeland Security (DHS)'s interactions with Native Americans when they are proving citizenship. KUNM's Jeanette DeDios (Jicarilla Apache and Diné) has more. The Respect Tribal IDs Act would require DHS working with the Bureau of Indian Affairs (BIA) and tribal nations to create training for officers to better detect and respect current tribal IDs. U.S. Sen. Ben Ray Luján (D-NM) says ICE agents are breaking the law. “My Native American brothers and sisters, who are American citizens, are being held by ICE agents because these agents do not recognize tribal IDs, it’s disrespectful, it’s wrong, I would argue, illegal, and that’s why we need this legislation.” Luján says tribal leaders have voiced their concerns about ICE agents detaining their tribal members near their reservations. “I’ll remind you that some of the first awareness that we had about ICE agents going after Native American communities, happened in New Mexico, down in Mescalero and also on the Navajo Nation, and it’s happening in other parts of the country as well.” DHS said in a statement that ICE agents acknowledge and recognize tribal ID cards as proof of citizenship and there have been no ICE operations on tribal lands. Lujan says it is hard to collect data on the number of Native Americans who have been detained by ICE because DHS will not release the data. Meanwhile, some legal scholars are raising concerns about a case brought by the Trump administration that is before the Supreme Court and how it might undermine birthright citizenship among Native Americans. (Courtesy Data for Indigenous Justice) A national organization called the Courage Project shines a light on acts of bravery, both big and small. This year, a group that works to bring attention to Alaska's missing and murdered Indigenous people (MMIP) is in the spotlight. As KNBA's Rhonda McBride tells us, Data for Indigenous Justice (DIJ) is one of sixteen organizations nationwide to receive this award. The steady beat of the drum is what you hear at many events involving DIJ and while their presence is felt more than seen, they are a force for change. Funders for the Courage Project like the MacArthur Foundation say the award was created to recognize neighbors helping neighbors, people who perform everyday acts of civic courage, that speak to the American spirit and strengthen democracy. “When I first started doing this work, people wouldn’t even meet with me.” Charlene Apok, known by her Iñupiaq name, Aqpik, founded DIJ to bring attention to missing and murdered Alaska Natives. She saw breakdowns and inequities in how law enforcement handled their investigations and pushed for a database to better track those cases and expose systemic failures. “Organizations didn’t want to talk about it. It was too hot. It was too political. It was too uncomfortable. It shouldn’t be courageous to say the things that we’re saying, and to ask the questions that we’re asking. These should be things that are expected for the safety of our people.” Doug Modig, a traditional healer, says it is never easy to speak truth to power, especially for a small organization like Data for Indigenous Justice. “Real lives are at stake her. Real people are experiencing hurt. There aren't many people that have that courage, because it's so rare. It reminds me of a wolverine. They'll take on a bear, a full-grown bear.” Wolverines, Modig says, are fearless when it comes to protecting their territory. “Why don't they just give up? They're not going to make it, because they're so small. But the truth is, courage isn't about size. It's the content of your heart.” Aqpik says heart is exactly what her team brings to their work. Their commitment has helped to uncover critical information about unsolved cases. “I’ve come to learn, with a lot of guidance from my elders, that this role is called being a story keeper.” Aqpik says it is a sacred responsibility to listen to the stories that families share. She says they are the bravest of all. Long after the marches are over and the drumbeats fade, they must live with these stories. Get National Native News delivered to your inbox daily. Understanding the Legal Principle of ‘Standing': Who Can Bring a Lawsuit?

Law School

Standing is the legal ability to file a lawsuit.Only those directly involved and affected by an issue have standing.Article III of the Constitution gives federal courts the right to preside over "cases" and "controversies."A three-part test must be met for standing: Injury in Fact, Causation (Traceability), and Redressability.Injury in Fact: The plaintiff must have a concrete and particularized injury.Causation: There must be a direct link between the defendant's conduct and the plaintiff's injury.Redressability: The court must be able to remedy the plaintiff's injury.Individual Standing: Lawsuits for personal harms suffered.Third-Party Standing: Suing on behalf of someone else under specific conditions.Organizational Standing: Organizations suing on behalf of their members.Taxpayer Standing: Limited exceptions for challenging government spending.Lujan v. Defenders of Wildlife (1992): Mere ideological interest is insufficient for standing.Massachusetts v. EPA (2007): States can sue with specific harms caused by federal inaction.Hollingsworth v. Perry (2013): General interest in enforcing a law is insufficient for standing.Challenges to standing include arguing no real injury, no direct causation, or the case is moot.Ensure a direct, personal injury when considering a lawsuit.Show a clear link between the defendant's actions and the harm.Courts do not hear cases based purely on moral or political beliefs.Check if an organization can file on your behalf if affected.Standing ensures courts hear cases where plaintiffs are directly involved and affected by the outcome.Plaintiffs must demonstrate injury, causation, and redressability.Understanding standing is crucial for legal action and following high-profile cases.

Ever want to know what it sounds like to go vintage shopping with a friend who spills all the details about their shitty ex in a distinctive mid-range vibrato?  Well this is the episode for you!Come join as we spend time with Portland, OR based, singer/songwriter Kendall Lujan.  Her music grabbed my ears from the first time I heard it, and I've had it on heavy rotation ever since.  Extremely fresh and new take on things, and her voice and vocalizations are distinctly her own.In addition to diving in to her catalog (hard to decide which tracks to play!), we also learn that music was her comfort and happy place, even as a baby, and the universe orchestrated a chance meeting with one Carlos Santana who gave her encouragement and advice (albeit a little weird) at 11.However struggles with anxiety and performing her own music kept her from playing her originals publicly until 2020.  But she was recognized for her unique talent rather quickly by Dominic Schmidt of Rola and helped her record her music and get on tour in Europe.  She now has a handle on the anxiety, but it needs to be worked like a muscle, she says, and if she doesn't exercise it, it can become difficult again. Her debut album Lucky Penny drops this week!  And Kendall will perform live on the show with us, so don't miss it!Text us your thoughts on this episode, and who should be OUR #NextFavBand...As always, our hope is to bring you "your next favorite band". I start by asking for a big favor!! I haven't asked in about 10 years – but if you get a moment, could you drop a positive review for WFNP on iTunes, or a good rating on Spotify, or tell a friend who you think would like the show? Thank you in advance!   This show is part of the effort to refresh the WFNP catalog, based on the ideas of Patron Chris C, who did a ton of work to help me come up with a list of things he thought I could re-do, given that it's been 14 years since I covered some of this stuff. If you have any ideas, please drop a DM on Patreon or send me ideas through the site!   This show has my recommendations for filling a case of wine with classics from around the world. These wines will give you a great idea of what these regions represent and why they are so famed. Even for experienced wine drinkers, I think it's good to go back to the basics and try stuff you haven't had in a while to remind ourselves why they are so good!   The list contains 12 still wines (no sparkling or fortified this time!), whites, reds, and one rosé. The wines are:   Whites: White Burgundy, which is Chardonnay. I recommend the wine especially from the Hautes Côtes de Beaune or Hautes Côtes de Nuits, Côte d'Or Blanc, or Chablis 2. New Zealand Sauvignon Blanc – with a twist. Look for wines from the Awatere or Wairau Valleys. 3. German Riesling from the Mosel (for off-dry wines) or Rheingau. I also mention Pfalz for great dry wines. 4. Portuguese Alvarinho – Vinho Verde, specifically from sub-regions of Monção and Melgaço   Rosé 5. Tavel from the Rhône Reds 6. Bordeaux – especially from the Médoc or Côtes de Bordeaux for Merlot-driven wines (Pomerol and St. Émilion) and for bolder, more ageworthy wines with more Cabernet -- Haut-Médoc or Saint-Estèphe or for spendier versions – Graves, Margaux, Saint Julien, Pauillac 7.   Rioja from Spain   8. Italian reds either Chianti Classico (have food with it!) or Roero/Langhe Nebbiolo/Nebbiolo d'Alba/Barbaresco   9. California – Santa Barbara County Pinot Noir (or more specifically Santa Maria Valley or Sta. Rita Hills)   10. Barossa Valley Shiraz (take another look if you haven't in a while!). For more restraint, you can look for Shiraz from Eden Valley, a sub region of the Barossa Valley   11. Chilean Cabernet Sauvignon, especially Maipo Andes/ Alto Maipo is best, due to the higher altitude in this area 12. Argentinean Malbec, more specifically from Gualtallary, Tupungato, Altamira or, more generally the Uco Valley and Lujan de Cuyo       Full show notes and all back episodes are on Patreon. Become a member today! www.patreon.com/winefornormalpeople _______________________________________________________________   Check out my exclusive sponsor, Wine Access.  They have an amazing selection -- once you get hooked on their wines, they will be your go-to! This episode was inspired by Joanna Macy, Ph.D., author and teacher, a scholar of Buddhism, systems thinking, and deep ecology. Our episode follows her philosophy, a spiral sequence flowing through four stages: beginning with gratitude, honoring our pain for the world, seeing with fresh eyes, and finally, going forth. Call to Action: Share an experience that gave you gratitude for nature. Once you take care of your basic needs, Learn all that you can about life and yourself. Your expansion of consciousness will give you renewed hope for connection, generosity, and peace. This is Fanachu Season 8, Episode 322This episode of Fanachu premiered on October 2, 2024. Support the show

Dixie is back, and so is the Podcast! In this episode: You've been told ‘Don't do Bondage when you're High', but have you ever heard a true story that illustrated that point? Well, Science tells us that we learn best from a story, so this week, we have an informative ‘Don't Try This at Home' story for you. Punk Rock Leather Daddy Al Rahm Lujan was a mere 20 years old and fresh out of the Navy, and just beginning to explore his dominant nature. Young and unskilled at bondage (but eager to play), Al meets a submissive man with a unique fetish for tasseled loafers and business suits, so he picks up a pair of dress shoes at Kmart and they take their party to a secret lair. Al has his victim on a St Andrews cross and is using whips, floggers, nipple clamps and more on him - when the evening takes an unexpected turn. The submissive turns blue, the EMTs, Firefighters and Police are called, and their dangerous experience becomes a life lesson. Will Al get arrested for being Drunk and Kinky? Listen to discover the consequences of an impulsive (and intoxicated) adventure that helped Al get Sober. #GetSmart   Song: ‘All in the Suit that You Wear' (Stone Temple Pilots)   About our Storyteller:   Al Rahm Lujan is a Bay Area Renaissance Daddy.
