Twenty-six percent of rural hospitals in Texas are at risk of closure, according to a report issued late last year by the firm Kaufman Hall. The number is up 10% from 2021 and the risk is due in part to pandemic relief money running out. The Texas Organization of Rural and Community Hospitals defines a rural hospital as one in a community of fewer than 60,000 people. Henderson pointed to a closure in Bowie as an example of how important hospitals are not just to a city's health care, but to its economy. "It wasn't just that the hospital closed and you lost access and lost 100 or more good jobs. It's that local sales tax revenue dipped by a third the following year, school enrollment dropped. They went through around a teacher layoffs," TORCH president and CEO John Henderson said. "So you see all those cascading economic impacts that are all negative and harmful to a community." Dr. Kristie Loescher, assistant dean for instructional innovation for the McCombs School of Business at the University of Texas and the academic director for the Healthcare Innovation Initiative, said one of the ways rural hospitals can make smart decisions by limiting the services it offers. For example, she pointed to an emergency room. Due to a federal law, a hospital is required to treat any patient who walks through its emergency room doors -- whether or not they can pay for services. "In Austin or Dallas, if a hospital sees a few patients like that, they can absorb it. They can absorb the bad debt. But in a rural situation, they're living so close to the wire, they don't have that extra margin to be able to provide the amount of charity care required," she said. Hospital Flash Report: January was Devastating for Finances In February, Kaufman Hall published their Hospital Flash report that showed hospital finances in major distress mode, following the end of the Coronavirus Aid, Relief, and Economic Security Act or, CARES Act, funding. They reported that in January the median change in operating margin without CARES decreased 71.3 percent; operating room minutes fell by 15.7 percent; the average length of stay rose 8.6 percent; labor expense per adjusted discharge jumped 14.6 percent; and total expense per adjusted discharge increased 11.6 percent. AHA commissioned the AHA to do a report on the future revenue/losses of healthcare and they are projecting $50B - $120B in losses in 2021.  Big gap right.  Well, the gap is due to assumptions, Vaccine distribution and return to pre-COVD habits.  What would your assumptions be?FTAHospitals could lose between $53 billion and $122 billion due to the lingering effects of COVID-19, depending on the speed of vaccine distribution and complete recovery of patient volumes, according to a new report from Kaufman Hall.Under an optimistic scenario, hospitals would lose $53 billion in revenue this year. The loss would primarily come from a $27 billion decline in outpatient revenue and $17 billion for inpatient as well as $9 billion in emergency department revenue.Under a more pessimistic scenario, hospitals would lose $122 billion thanks to a $64 billion decline in outpatient revenue.Hospitals could lose billions more if consumers and hospitals adapt to a “new normal” of slower or only partial recovery of patient volumes. Staffing has long been one of the Achilles heels of the nursing profession, confounding nurse managers and nurse leaders alike, and taking up an inordinate amount of time. Our guest for this episode is an expert on the topic and has taken her cues from other industries to treat this as a data science problem.  Early in her career, Therese Fitzpatrick was a chief nursing officer for various hospitals and health systems in the midwest before taking a turn as an entrepreneur, receiving her PhD in sociology and becoming a professor in the University of Illinois at Chicago's School of Public Health. Today she is an executive at consulting firm Kaufman Hall, where she helps hospitals across the country assess their clinical and operational performance, and optimize their staffing. In this episode Therese walks us through how she applies big data to the problems of staffing and workforce optimization, some low-tech ways that nurse managers can approach scheduling, and how COVID-19 will impact the evolution of the nursing workforce. Therese believes that robust float pools driven by Millenial and Gen Z nurses are the future and encourages nurse leaders to include them in conversations around staffing and workforce. According to a national study undertaken by healthcare consulting firm Kaufman Hall in May 2020, April was a devastating month for hospitals across the country from a financial perspective.  Despite the expectations of some that hospitals would be overflowing with patients due to the spread of COVID, in actuality a sharp decline in overall patient volume was noted.  This, along with a prolonged suspension of so-called elective surgeries, have translated to record-setting declines in hospital revenue.  The following seeks to illustrate the scope of the fiscal damage and to assess where hospitals may go from here.

The nursing community has a unique opportunity to inform the public and policy makers of the vital role nurse practitioners play in advancing health access, equity, and quality care. Recently, Becker’s Hospital Review published a notice stating about 15 physicians at Edward Elmhurst Hospital were to be terminated and replaced by nurse practitioners. The implications made in response from local physician groups were that nurse practitioners are viewed as mid-level providers, and this practice could put patients in danger.In response, in December 2019, nurses from Rush University College of Nursing and Rush Health in Chicago submitted a Letter to the Editor to Becker’s Hospital Review, expressing dismay at the description of nurse practitioners as mid-level providers. The Nursing Economic$ Editorial Board believes all nurses have an ethical obligation to correct inaccurate descriptions of advanced practice colleagues, and in support of the nurses from Rush University and Rush Health, submitted a follow-up Letter to the Editor to Becker’s Hospital Review. Today’s discussion, led by Nursing Economic$ Editorial Board member Dr. Therese Fitzpatrick, focuses on the events at Edward Elmhurst Hospital as reported in Becker’s Hospital Review and elaborates on why advanced practice registered nurses and nurse practitioners must become informants to policy makers and the public.
