On this episode, we are joined by April O'Connell to discuss exercise dosing for patients who have sustained injuries of their upper extremity. She shares with us factors that should be considered when designing an exercise program for a patient and how to progress them through their plan of care to achieve their goals and beyond. Guest Bio: April O'Connell, OTR/L, CHT, ACSM, is an occupational therapist at Cedars Sinai Hospital in Beverly Hills, CA. She works with a variety of sports teams including the NFL, MLB and NBA and recently spoke at ASHT and AAHT on Rehabilitation of the Overhead Athlete. She is one of the main contributors to the fellowship program for hand and upper extremity rehabilitation as well as the co-director for Cedars Sinai Hand Symposium. April got her start as the Clinical Specialist in the Hand and Upper Extremity Therapy Department at NYU Langone Health. She has lectured to orthopedic surgeons and therapists nationally and internationally in current concepts of orthopedic rehabilitation of the upper extremity. Some of her most recent lectures include being a guest speaker for Dr. Radio on Sirius Satellite on topics for rehabilitation of the hand and upper extremity as well as injuries to the overhead athlete. In addition, she has presented on Flexor Tendon Repair Advancement at AAOS in 2015 and was later asked to speak at the ASSH national conference in 2016 and ASHT in 2017. She recently presented her latest research at the ASHT National Conference on flexor tendon rehabilitation in 2019. April is a founding member of the NYU Langone Golf and Pitching Labs and was the head hand therapist for the first successful bilateral hand and face allotransplantation. She earned her Occupational Therapy degree at Boston University, is a Certified Hand Therapist and is certified by the American Council of Sports Medicine (ACSM) as a Clinical Exercise Specialist.

In this episode we discuss the exertion types and what uses energy, we recap on what pacing is and why it is important when you live with a chronic condition such as ME and long covid. Robin discusses the principles of pacing and the varying forms of pacing, we discuss listening to your body and the challenges of this with PEM/PESE. Robin explains how monitoring your HR/HRV is important, what is available to monitor this data and how to understand what level to set your alerts at to remain within the anaerobic threshold. We discuss the lactate threshold and Robin explains the zones of this threshold and how important it is to remain within the lactate threshold, especially those with PEM. Robin advises how the lactate threshold can be tested at home, and clinical testing. We additionally discuss how those with Orthostatic intolerance, PoTs can find it challenging to maintain their heart rate within their threshold due to heart rate spikes. In this episode we discuss pacing and micro pacing, how it can be challenging to understand and to navigate. Robin explains the principles of pacing, identifying triggers, the lactate threshold and how to break down tasks and why this is important. Robin shares his own journey and how he identified his activity triggers and applied micro pacing. He explains how certain tasks can use the same energy requirement however the mechanics/movement of certain tasks can be more of a trigger spiking the heart rate and causing PEM/PESE. Robin discusses the difference between a strength issue and a fatigue issue. How it is not always understood that PEM is a physiological issue, not a muscle wasting issue.We talk about the lactate threshold and the protocol Robin has developed to test a person's lactate threshold. Season 2, Episode 29, Release Date: 5-4-2024 IFIO with guest Toni Herbine-Blank   Today I have the pleasure of sharing my conversation with Toni Herbine-Blank, MS. RN, C-SP. She is a Clinical Specialist, Psychotherapist, and Senior Trainer for the Center for Self-Leadership. Toni began training students in Internal Family Systems Therapy in 2004 and since then has been instumental in designing programs and writing curricula for Levels 1 and 2 IFS training programs.   Toni has been in private practice since 1996 specializing in the treatment of couples and individuals. In 2009, after designing a training for IFS therapists to apply the model to couples therapy, Toni offered the first Intimacy from the Inside Out (c) Training Program in Boston, MA. Since then the program has been received with great enthusiasm across the United States.   Whereas IFS focused on the internal aspects of an individual, IFIO focuses on the interdependance our internal system and external relationships. We turn inward (known as a "YOU-turn") to help us in our connections ("RE turn" to relationship).    Toni shared how the heart of her model is "Courageous Communication." When we feel healed, we open up to conversations. In this way we can stay connected with our minds and hearts, and are able to tune into how we might be impacting other people in the conversations.   Within intimate relationships, there is mutuality, as well as exploration. Although skills and tools could be extremely helpful, we need to have a critical mass of self energy in order to access these skills. Self-energy helps us listen and speak from our hearts.   We spoke about how Shame is a verb. When we have difficulty connecting and commnuicating with one another, we can explore what is coming up within us, and there is always some path that connects us back to shame, shamfulness or shaming behaviors. Shame is a "rolling" energy, where we sometimes replicate within our systems what is done to us. Additionally, what we do to ourselves, we do to others.   In IFIO we are "helping people free themselves from the myth of Unloveability."   Through IFIO we earn the trust of our internal system, and in this way recognize who we can extend this trust to in our external world. As these people are welcomed into our internal universe, we co-constuct authentic and meaningful connection.

My guest today worked for Club Med from 1990 to 1992. Her first season was in Club Med Playa Blanca with CDV Andrea Livingstone, as a G.O. Nurse. She is originally from Queens, NY and now lives in the Sunshine State. She went to Club Med as a GM in Haiti and Copper Mountain in the early 70s. She then went to Club Med Cancun as a GM in 1987 and realized that Club Med employed nurses too. Two years later she got the Club Med magazine in the mail and inside was an article called, “Become a G.O.” and that's what she did. Please help me welcome, Andrea Chassen! Before Club Med, Andrea attended the University of Buffalo and then obtained a master's in nursing at NYU. Andrea was a Clinical Specialist at a teaching hospital in Philadelphia and had a month to put everything in order before she left for Club Med after her very positive interview. Andrea also worked at Club Med Punta Cana, Sonora Bay, and Eleuthera. Do you like Club Med nursing stories? Well, Andrea has some! In this episode we talk to Dr. Jacob Long about their 2019 Neilsen-ASIA Research Award project titled "Does the Speed of Robotic Leg Movements During Tilt-Table Verticalization Mitigate Orthostatic Hypotension in Subacute SCI?" Note: this episode was recorded live in New Orleans at the 2022 American Spinal Injury Association (ASIA) annual scientific meeting. Dr. Long received his doctorate in Physical Therapy from the University of Mississippi Medical Center in 2013. He joined Methodist Rehabilitation Center in 2014, soon transitioning to the Spinal Cord Injury program to advance his education and clinical expertise with this complex condition. In parallel to his clinical duties, he achieved two assistive technology certifications from the Rehabilitation Engineering and Assistive Technology Society of North America (RESNA), and the Assistive Technology Professional (ATP) and Seating & Mobility Specialist (SMS). After passing the American Board of Physical Therapy Specialties board examination in Neurologic Physical Therapy (NCS) in 2018, he has been promoted to the position of Clinical Specialist. In this role, he serves as a resource and mentor for other therapists and students, and actively collaborates with the investigators at the Methodist research division, the Center for Neuroscience and Neurological Recovery.

Her research focuses on the experience of children with medical complexity and their caregivers, especially those who are tracheostomy and ventilator dependent. She has received funding support from the Society for Simulation in Healthcare to explore how family caregivers perceive the use of high-fidelity simulation in learning tracheostomy emergency management. She also received funding from Sigma Theta Tau Zeta Pi Chapter to examine the challenges of caring for a child with medical complexity during a global pandemic. Her other research interest is in minimizing pediatric health disparities, improving the rapid response of critically ill children, increasing cultural competence in care, and merging research and quality improvement methods to implement interventions with a positive, sustained impact on the care of hospitalized children. Leah Holderbaum, OTR, CBIS, ATP received both her Bachelor of Science in Occupational Science, and Master of Science in Occupational Therapy from Eastern Kentucky University. Leah has 10 years of clinical experience, which began with Kettering Health Network in Ohio at the NeuroRehab and Balance Center. Leah expanded her career to Texas in 2012, as an occupational therapist on the Spinal Cord Injury Team at Baylor Scott & White Institute for Rehabilitation-Dallas, where she served as the clinical specialist. While at Baylor, Leah facilitated a national research study, in conjunction with the University of Pennsylvania, regarding compliance with intermittent cathing. Leah has developed and instructed continuing education courses on the therapeutic management of the spinal cord injured patient, and the distinct role of the OT in bladder management, and has provided lectures at Texas A&M, the University of St. Augustine, and TWU on SCI. Her webinar entitled Clean Intermittent Cathing:  An Interdisciplinary Approach to Best Practice launched nationally in 2020. Currently, Leah is serving on a national expert panel regarding establishing a standard protocol for clean intermittent cathing. She developed a passion for education, and bladder management as the Clinical Specialist on the SCI team, based on knowledge and experience with quality of life improvements with patients, which earned her the acclaimed status of “Cath Queen”. Leah was recently published in the 2nd edition of the WOCN Core Curriculum, and is also contributing faculty at the University of St. Augustine for the Occupational Therapy Department.Leah currently works for Numotion as Medical Supply Account Manager, where she has the opportunity to share her love for bladder management, and promote quality of life and independence for customers she encounters.65646454yt

Along with providing Mummy MOTs for ladies who are six weeks post-partum. Zoe worked in the NHS for 8 years and has been working exclusively in private practice since 2016. Understanding what the pelvic floor is, how big it is and why it is so important are the first questions I ask Zoe. Naturally if you have had children you are more than likely aware of some of this, but are you actually exercising it regularly?  If not, then do listen as Zoe shares exactly how you should be exercising it, where, how long for and how many times per week.  Good news too, that it's never too late to start (if you haven't been doing it) or you feel like it's not as good as it once was! This month we are joined by physical therapist Dr. Janice Ying. She shared with us how she has fit nine lives worth of achievements into just one life, her work with musicians in physical rehabilitation and injury prevention, and her work with collegiate and graduate-level musicians. We pepper her with controversial questions, and she shoots back with a stellar answer every time. For example, are musicians small muscle athletes? Should we care about our joint range of motion? Show Notes: Bio: Dr. Janice Ying is the owner and founder of Opus Physical Therapy and Performance Inc.  She is an ABPTS board certified Clinical Specialist in Orthopedic Physical Therapy serving the Greater Los Angeles area and is internationally recognized as an expert in the field of Performing Arts Medicine. Most recently, she has been awarded the 2021 Emerging Leader award by the American Physical Therapy Association - Academy of Orthopaedic Physical Therapy.  She is currently the head physical therapist at The Colburn School - working with elite collegiate and graduate level musicians on injury prevention techniques and addressing playing-related injuries. Originally from Kansas City, MO, Dr. Ying attended Pepperdine University where she received her Bachelor of Arts in Music Education and Piano Performance while on multiple scholarships for her achievements as both a pianist and violinist. She worked professionally as a pianist for several years, performing throughout the country before returning to school to complete her graduate studies in physical therapy.   Dr. Ying received her Doctorate in Physical Therapy from the top-ranked physical therapy program, University of Southern California. She completed her orthopedic residency training at Glendale Adventist Medical Center. She currently serves as Adjunct Clinical Instructor for the University of Southern California in the department of Physical Therapy and Biokinesiology.   In the past, she has served as a mentor, lecturer and member of the clinical faculty for the Glendale Adventist Medical Center - Orthopedic Residency as well as taught courses on clinical management of the elbow, wrist and hand, and functional ergonomics.  Additionally, she has obtained advanced certifications in ergonomics, Kinesiotaping, Graston Technique and manual physical therapy techniques. Dr. Ying is internationally recognized for her work with treating performing artists.  She is a frequent guest speaker and lecturer at national and international conferences/events. Dr. Ying's professional interests include clinical research and the application of injury prevention techniques and rehabilitation of instrumental musicians and dancers. She frequently provides courses and educational sessions for medical professionals on topics relating to the management and treatment of health conditions found in performing artists. Dr. Ying is an active member of the American Physical Therapy Association, where she has held several positions as both an elected and appointed officer for the Orthopedic Academy, and Performing Arts Special Interest Group of the American Physical Therapy Association.
