Participants in the usual diet group will be encouraged but not required to follow the Dietary Guidelines for Americans diet. The study will also measure the impact of the study diets on walking function, hand function, vision, serum neurofilament (blood biomarker), and brain imaging (presence of enhancing lesions and changes in brain volume).  The study will be held at the University of Iowa Hospitals & Clinics over two years. It will consist of three visits to Iowa City, Iowa. The first visit occurs at month 0. The second visit is at month 3, and third and final visit is at month 24. The first visit may last 4-6 hours. The second visit will last about 2-3 hour. The third visit may last approximately 4-6 hours. Participants will be randomly assigned to follow one of the three study diets. Participants will continue to take their current medications, including any disease-modifying treatments for multiple sclerosis. Participants may take recommended supplements, eat more non-starchy vegetables, complete daily food logs (3 questions) on a free smartphone application, complete online surveys every three months, watch videos to learn about the study diet to which they are assigned, meet with the study dietitian to learn about the study diet to which they are assigned, and attend optional online support meetings. If a participant is assigned to the ketogenic diet, they will be asked to take a blood ketone measurement using a drop of blood daily for the first month, then twice per week. They will have a device that pricks the finger to conduct the blood ketone measurement. Participants will have a fasting blood draw at each of the study visits and may bring a snack after the blood draw. They will complete assessments of walking, hand, and vision function at each visit. They will have a NON-CONTRAST MRI of the brain at the first and last visits. Key requirements to participate in the study include the following: 1) be willing to share medical records with the study team, 2) do not have clinically significant heart disease, liver disease, kidney disease or type 1 diabetes, 3) do not have a serious psychiatric disease that would make adopting a study diet more difficult (for example schizophrenia or disabling depression), 4) are not taking insulin, coumadin, or weight loss medications, 5) have a smart phone or tablet to download a free app, and 6) have access to high-speed internet and a computer, tablet or smart phone to complete surveys and participate in video conferences via the Zoom platform. Multiple sclerosis (MS), is an auto-immune condition affecting more than 1 million Americans. And like other autoimmune conditions, it it's becoming more prevalent. The role of genetics in MS is not significant. Lifestyle choices however may play important roles in regulating the immune system, and therefore should certainly be looked at as it relates to this and other autoimmune conditions.   My guest today, Dr. Terry Wahls, is herself a multiple sclerosis patient. On the podcast she describes how aggressive her multiple sclerosis became, how she herself did not seem to realize any dramatic improvement from pharmaceutical intervention, and how she ultimately discovered the important role of nutrition in terms of regulating her immune system and as such, the disease course of her multiple sclerosis situation.   Dr. Wahls then took these observations and created programs for other  MS patients, applying rigorous scientific principles to study the effects of nutritional interventions.   Her latest study compares her nutritional protocol with a long-standing nutritional approach to multiple sclerosis called the Swank diet that actually began being utilized in the late 1940s. I will say that the results she has obtained, which we discussed today, are extremely impressive.    There is certainly good reason to consider the use of pharmaceutical intervention in multiple sclerosis. Nonetheless, that's one intervention that, in my opinion, should certainly be coupled with other proven therapies, like nutrition. Dr. Wahls makes it very clear that there are multiple mechanisms by which this dietary change may prove helpful in the MS patient;  by targeting things like inflammation, the functionality of the gut bacteria, and even the function of mitochondria.   I am certain you will enjoy today's interview. Medical notes this week… A growing number of young adults have substance use disorders. In fact, many have more than one. A study in the Journal of American College Health shows that about 40% of college-age Americans have been dealing with a substance abuse/use disorder in the last year, and about 15% have more than one. Even on college campuses, where help is available, few seek treatment, and researchers say only one in 100 is able to stop using. People with multiple sclerosis often suffer from crushing fatigue. But a new study shows that following a specific diet could help alleviate it. The study in the journal PLoS One shows that the so-called "Wahls Diet" drives up good cholesterol, which appears to east fatigue in MS patients. The Wahls diet excludes gluten, dairy and eggs, and is high in fruits, vegetables, meat, and plant protein. And finally, women who are trying to get pregnant often stop drinking, but a new study suggests men should stop drinking, too. The study in the European Journal of Preventive Cardiology shows that drinking among women pre-pregnancy raises the risk of congenital heart disease by 16%. But men who drink alcohol in the three months before conception increase the risk of heart disease in their offspring by 44%.

In 3 months, she was out of her wheelchair and could walk with a cane. Just 5 months later, she could walk without a cane. And just 9 months later she was able to do an 18-mile bike ride!
