One underappreciated fact about the explosion in genetic databases, like consumer sites that provide information about ancestry and health, is that they unlock valuable insights not only into an individual's past and future, but also for that individual's entire family. This raises serious concerns about privacy for people who have never submitted their genetic information for analysis, yet share much the same code as one who did. This week on If/Then, we're sharing an interview from The Future of Everything with Stanford GSB Professor Kuang Xu. This week, a data breach was disclosed by 23andMe, affecting 6.9 million users and compromising their genetic data information. The incident raises significant concerns about data privacy and security. It's important to note that while some perceive this as a violation of HIPAA, it typically doesn't apply to direct-to-consumer genetic testing services like 23andMe, as they don't involve covered entities such as physicians. This leaves a gap in privacy protection, and alternative legal frameworks, such as those enforced by the Federal Trade Commission, may come into play. However, actions against direct-to-consumer genetic testing companies by the FTC have been rare. The breach highlights the need for increased awareness and potentially new laws and regulations to safeguard sensitive genetic data in the rapidly evolving digital landscape.

Infertility. It's real. It affects 10-15% of couples in the US. After a thorough investigation, 30% of couples will be left without an identifiable cause for their infertility. This has been termed unexplained infertility. While traditionally this may have been considered a reassuring result, new data is raising the yellow flag of caution in these patients. Research has now established the link between infertility and some potential future negative health outcomes. As “next generation“ genetic tests become more integrated into clinical practice, new data (released ahead of print on March 16, 2023 in the New England Journal of Medicine) has revealed some life-changing realities for couples struggling with unexplained infertility. In this episode, we will highlight the advances in genetic medicine and summarize the key findings of this eye-opening new publication.

Throughout the U.S., usually right after a baby is born, a doctor or nurse takes a pinprick of blood from the newborn — often from its cute, little feet — to screen for diseases or genetic defects. These blood tests are mandatory and create a health and genetic database. But who gets access to that genetic information, and for what purpose, is at the center of several legal fights. In New Jersey, the Office of the Public Defender recently sued the state testing program after police allegedly used a newborn’s blood sample to help them obtain a warrant for the father’s DNA. “Marketplace Tech’s” Kimberly Adams spoke with reporter Nikita Biryukov at the New Jersey Monitor, who broke the story.

When a great idea is born out of frustration, you best believe that it will result in an amazing company. One that is passionate about helping people get ahold of information to improve their health and life. If you are interested in learning about your health and body, this will be a fascinating discussion for you. Joe and I talked about how he founded his companies and how the data from DNA testing can help improve your life significantly.   Joe Cohen won the genetic lottery of bad genes. As a kid, he suffered from inflammation, brain fog, fatigue, digestive problems, anxiety, depression, and other poorly understood issues in conventional and alternative medicine. Frustrated by the lack of good information and tools, Joe decided to embark on a journey of self-experimentation and self-learning to improve his health—something that has since become known as “biohacking.”   Joe has studied health sciences for 17 years and has read over 30,000 PubMed articles. He's given consultations to over 1000 people who have sought his health advice. After completing the pre-med requirements at university, he founded SelfHacked because he wanted to make a big impact in improving global health. He also founded SelfDecode and LabTestAnalyzer, popular genetic and lab software tools to improve health.   Now, Joe regularly speaks at events alongside the world's top health experts and continues to work on enhancing his company's products to provide people with access to cutting-edge tools and information designed to optimize health.   Learn more about Joe and SelfDecode: Web: https://selfdecode.com/ IG: https://www.instagram.com/mrbiohacker/   In This Episode: [1:23] Who is Joe Cohen, and how SelfDecode started [4:58] Diving deeper into DNA testing [8:47] How genes play a role in our body [20:15] More examples of the genes' role in the body [24:43] The gene related to inflammation—does it exist? The Future of Everything with Russ Altman: E 163 | Kuang Xu: How to make (and keep) genetic data private An expert in genetic privacy says there's a fine line between one's right to know and another's right to not know. Today's guest, Kuang Xu, is an expert in how genetic information can and should be used. He says that the DNA problem weighs heavily on privacy experts in fields ranging from law and engineering to public health and criminal justice. The fundamental question is: Can we create methods for accessing genetic data while maximizing the privacy of all involved? The problems will only grow more intense as time and data accumulate, Xu says, unless we resolve them now, as he explains on this episode of Stanford Engineering's The Future of Everything podcast with host Russ Altman. Dr. Bupp will break these intersections down, explain genetics in a manner that you can also explain it to others, and highlight the importance of genetic testing with consent.    Let's listen to Dr. Bupp to learn more about genetics in behavioral health!   [00:01 - 06:55] Opening Segment Welcoming today's guest, Dr. Caleb Bupp Why Dr. Bupp chose genetics 06:56 - 16:49] Genetic Issues  We talk about the 3 intersections of genetics and mental health Understand genetics better with this analogy from Dr. Bupp Dr. Bupp tells us why every health issue is genetic  [16:50 - 27:49] Genetic Testing What is Dr. Bupp's advice for children with autism  Genetics will be of no use under one certain scenario Dr. Bupp elaborates Genetic testing will be valuable in learning about behavioral health issues Dr. Bupp explains The worst part of being a geneticist for Dr. Bupp [27:50 - 39:22] Genetic Data  We talk about the importance of consent in genetic testing The value of a person's genetic data as explained by Dr. Bupp What is pharmacogenomics and why we should understand it  [39:23 - 47:22] Closing Segment Listen to Dr. Bupp's reminder for us  What's his message for his resident self?  Finals takeaways Consent is a must in genetic testing Genetic testing has become mainstream  The different types of genetic testing Children with autism should be offered genetic testing or consultation about it A main barrier right now in genetic testing is cost Tweetable Quotes: “When you're working underneath somebody who loves what they do, it's just infectious.” - Dr. Caleb Bupp “... every health issue is genetic. In some way, shape, or form, we just have to get granular enough to figure out what exactly the genetic cause for that [issue] is.” - Dr. Caleb Bupp Resources Mentioned: https://bit.ly/2NPrqeE (Genetic Information Nondiscrimination Act of 2008) https://bit.ly/3j0fCSg (What is informed consent?) Researchers seeking information about genes and genetic variants face the challenge of needing to search multiple databases, each with their own unique set of formatting issues. To unlock the information they are seeking, they often must spend hours wading through databases, restructuring data, and addressing nonstandard annotations. A groups of scientists at The Scripps Research Institute is addressing that problem through the development of two web services—MyGene.info and MyVariant.info—that pull data from multiple databases and provide a uniform structure for the information. We spoke to Chunlei Wu, associate professor of molecular medicine at Scripps Research Institute, about the services, the challenges in maintaining and keeping current these rapidly growing data sets, and how they are changing research into genetic-based disorders. The team's study in Genome Biology about the services can be found at http://bit.ly/1U88uAW.
