With the opening of Bar RBC in Bend, we return to our conversation with Garrett Peck from August of last year... following the opening of RBC... and the talk of what would become Bar RBC. Original Post:   It's always a pleasure to have Garrett Peck on our podcast.  For years, Garrett was at the center of the Portland food world as GM and host at some of Portland's most prolific restaurants.  He managed the Heathman, later Headwaters, as well as The Paley Group's other hotel properties, Imperial, Rosa Rosa and The Crown.  When turmoil of 2020 put beyond a damper on the viability of those restaurants, Garrett and his wife Jonie sought greener pastures, which led them to Sisters, OR.  There, they connected with some of Portland's other most successful restaurateurs, who also found central Oregon more to their liking. In 2022, the three opened RBC (Rancher Butcher Chef) with other investors.  Now with a year under their belts, RBC is lauded as one of the states most successful restaurants, outside of Portland. Dan Quayle — Former Vice President of the United States and former United States Senator from Indiana. Golfer at DePauw. Mark Rolfing — Former PGA Tour pro. Longtime golf commentator. Having that vision fuels your optimism, which also fuels how you lead the team. Surround yourself with people who have a high level of optimism and stay away from those who are fond of using the victim card. [16:47] How to Spread Positivity in Leadership As a leader, be vulnerable enough to talk out loud to your team about some things in your weekly meetings. Tell them not to take things personally if, at times, you just don't feel at your 100% best. That little dialogue with them wipes out any perception, judgment, or defensiveness. Understand the psychology of positivity in the workplace because it leads to a higher level of performance as well as better relationships. Take subtle risks to show somebody or a team that there is some good out there. Kindness on display is not a bad thing. There are a lot of crises happening right now and people need a bit of level-headedness. Even a little pick-me-up or telling them you got their back is so subtle. That level of optimism could change the nature of a crisis and how you work through it. Be able to see your team from 20,000 feet up to know what's going on in your organization. See how you can connect people with each other and foster diversity in thinking. At the same time, don't just stay at the 20,000-foot perspective. Go down and get the frontline perspective and see it from there to get a richer picture. That probably means more listening and watching than talking. [50:38] Working with Different Generations If you have Gen Z members in your team and they're not used to struggles, give them challenging experiences in a safe environment. And to reveal some of the secrets to earning more while working less, Kirk Behrendt brings back Angela Heathman, one of many amazing lead coaches at ACT Dental. With her coaching expertise, you can learn how to plan more strategically to create the practice and life that you want. For tips on working smarter, not harder, listen to Episode 428 of The Best Practices Show! Main Takeaways: Plan strategically for the year. Mitigate schedule conflicts early. Look at your calendar quarterly. It's not always about adding more. Plan your life, not just vacations. Quotes: “One of my teams that I was talking to this week was talking about how the doctor is going to be gone for a couple weeks next month. And they were still talking about their goal as their whole yearly goal divided by 12, essentially. And I brought up the fact that they probably won't hit it with the doctor being gone for so long. And what we were then talking about is maybe setting goals on a per-hour or per-week or per-day basis instead of just that monthly goal. So, it was a little bit of a change in thinking for them, because they already planned on not hitting that goal and were just planning, this month, to blow it out of the water.” (2:25—3:05) “If you start by planning out the year strategically, then you can make it go a lot smoother for the whole year. You don't have to really make up time or try to hit these summer goals later in the year, because if you've planned for it like in January, February, March on an hourly or daily basis, you've already accounted for the time off that you're going to take.” (7:24—7:50) “Start by marking off — you could use any type of calendar. If you want to do a digital calendar or a big at-a-glance calendar on the wall, it doesn't matter. But what you need to do is figure out what days you're going to be gone for family vacations, for holidays, days off around holidays, CE, and also figure out when your team members are going to be gone too. And then, you'll know how many days your practice is going to be open for the year. And sometimes, that visual is nice to see, like, ‘Oh, gosh, we're not really going to be here in July,' or, ‘We're all taking different weeks off in March.' So, sometimes, it's nice to be able to mitigate any issues that might be coming up too.” (8:15—9:01) “A lot of our offices will have team members that will provide some coverage as well, like maybe the hygiene is doing general supervision, or there's an admin team member there who's answering the phone. So, a lot of our doctors are able to check out.” (9:22—9:38) “I think that it's important to figure this out with your team, because sometimes they think that we're just asking for all their days off for the year because we're micromanaging, or we don't want them to take a vacation. But that's not true. The reason we want them to put their time off in advance is so we can strategically plan what the goal should be for the year.” (11:55—12:22) “Even if [dentists] do the planning in advance for the year, they don't always look at it on a quarterly basis on if they did work the amount of days that they thought they were going to. And they might have had some unforeseen snow days, or COVID-19, or something like that that they didn't account for. We talk about countermeasures at ACT a lot. So, you will want to put in place some countermeasures. Maybe that's adjusting your daily goal going forward, or maybe that's that you have to work another day that you thought you were going to have off, to make up for those unforeseen changes.” (13:46—14:25) “I think a lot of [dentists] are just like, ‘Oh, I think I work about 16 days a month,' and they just think, ‘Oh, I work four days a week, roughly four weeks a month.' And that's where I see the biggest issue, is that they just estimate. And so, I know it sounds a little tedious to go...

How Many Patients Do I Need? Episode #395 with Angela Heathman To be a profitable practice, you need to have the right number of patients. But how do you know how many are too much, and how many are too few? In today's episode, Kirk Behrendt brings on Angela Heathman, Lead Practice Coach at ACT, to discuss how many patients a practice should have. Kirk and Heather talk about when to add a new doctor, the dangers of exhaustion, tips on how to get the most out of the patients you have, and much more. To answer the question “How many patients do I need?” listen to Episode 395 of the Best Practices Show!   Main Takeaways:   More patients doesn't mean better.   Determine what you want out of your practice—how much do you want to be working?   For one doctor working four days a week, 1200-1500 patients is the capacity.    Capacity for hygienists is around 750 patients.   A doctor's schedule should be full 95% of the time, and a hygienist's 92% of the time.   You can expect to lose 15% of clients due to natural causes—figure out how many you need to add to offset that.   Quotes:   “If you are a single practice and had 3100 patients, I would expect that you would be exhausted. I would expect that you would be exhausted or that your patients weren't really getting probably the standard of care that you really wanted them to, because you probably just don't have the capacity in your office to take care of that many folks. The simple answer is usually about 1200-1500 for one doctor working four days a week.” (03:50—04:20) “Typically, that office that has 1200-1500 patients has two full-time hygienists then, working four days a week. So, the number that we think about for hygienists is usually around 750. Again, longer story is that it could be anywhere between 750-1000, but typically the offices that we're coaching are spending 60 minutes with their patients, they're doing perio, which means they're needing time for scaling and root planning, and also three and four-month re-care appointment, so that's why that number is more around 750 instead of 1000.” (05:23—06:00)   “You can be anything that you want to be, but figuring out how to make that work for your own life is what's most important, and a lot of times you don't have to do that, and sometimes we get kind of caught up, like you said, in the ‘more, more, more,' but you can have a very good life and not have to do that if you don't want to, so figuring that out first is important.” (07:02—07:24)   “If you're feeling like ‘Oh, we need to add more hygiene hours, we need to add another hygiene day,' one of the things you'll want to look at is your capacity; like how full is your schedule already? Because if you're measuring that and every day you have two hours of open time, then I would argue that you don't need another hygienist—at least not right now—first you need to fix that problem of having all the open time.” (08:18—08:48)   “I always want to look at [capacity] in terms of percentages. So, for a hygienist we want their schedule to be full about 92% of the time, and for a doctor, 95% of the time. And so, what that shakes out to in the matter of a week is only about two cancellations—so only having about two hours open to really hit that benchmark.” (10:21—10:50)   “One of the things we coach our teams to do is something called an RFR. I would like to give credit to whoever started that, but I don't even know who it was, but RFR stands for Reason For Return. So, making sure it's clearly documented in your clinical notes the importance of why that patient needs to come next time. So, if next time you're going to be taking a PA to check a tooth, or a pano, or looking at an implant, or something like that, putting a little something additional in that clinical note so that whoever is fielding that cancellation call can have something very tangible to refer back to. They want to know how they're doing, where they stand, and just have an opportunity to communicate. One of the best systems to improve communication is with a team member check-in, and Kirk Behrendt brings in two ACT Dental coaches, Angela Heathman and Courtney Dalton, to share how to create this system. You don't just want to attract and retain the right people for your practice — you also want to grow them! To learn how by improving communication, listen to Episode 370 of The Best Practices Show! Main Takeaways: The people component is important in your practice. Your team members are actually craving more communication with you. Team member check-ins should be frequent, short, quick, and casual. Have team members run the agenda and the meeting. Let your team know you care about them with your behavior. Quotes: “[The team member check-in is] a time for self-reflection. So, the team member will come prepared. They'll fill out some highs and lows personally, professionally, where they feel they have strengths within the practice, and then where they feel like they can identify some opportunities of growth or of improvement. It's typically quick, maybe 10, 15 minutes or so. They give all that information to their doctor or their doctors, and it sparks this really great conversation about, ‘Here's the value I see in you. Here's where I think and agree that you're doing really well. And I agree, here's where we can maybe give you some tips or help you improve,' and the doctor can give some really constructive feedback as well to help to continue to encourage growth in that team member. And then, the cycle continues. When you leave it, you schedule your next one so that there's never a question from that team member, ‘Where do I stand? Am I doing well? Am I not doing well?'” (5:30—6:29) “What we've learned over the past few years is that employees really want more constant, less pressure type of feedback. So, that's how and why we adapted the check-in like Courtney was talking about, more frequent, short, quick, casual. Your team members really want to have that time with you.” (7:03—7:24) “[Team members are] usually craving that communication, and they're always usually a little bit nervous [about the team member check-in], because a lot of them haven't had that one-on-one time before. So, before the first one, they might be a little bit nervous, ‘I'm kind of scared going into this.' And then, they find out, ‘Oh, this is no pressure. This is just a conversation.'” (10:04—10:24) “I remember being an employee at a dental office and never having a review the entire time that I worked there. So, I can kind of speak to that too, that we want to talk to you. I would always ask, ‘Am I going to have a review at some point?' And I was always told if I don't, that that's a good sign. But that's not a good sign. You want to have these conversations so your employees aren't wondering.” (10:28—10:58) “The team members are actually craving communication. That's 110% true. As a clinician, when you go that long, it's a year — or sometimes, more likely, more than a year — between your reviews, you have no idea where you stand, if you're doing well, if you could be doing something differently or changing your perspective on something. By having that frequent cadence check-in, you know that the doctor and you have this really great open dialogue, and that craving is satisfied because you just know. And it's not always about getting a pat on the back. And it's definitely not always financially related. It's really just about, ‘I want to be seen. I want you to look at me, talk to me, and just really satisfy that craving of communication.'” (11:08—11:59) “Everybody thinks they're too busy, but they just have to schedule [team member check-ins]....

It's the second part of our conversation with Chef Philippe Boulot of Multnomah Athletic Club. Chef Boulot is one of the true pioneers and icons of the Portland Food world, having taken over the Heathman restaurant in 1994, leading to four straight James Beard Best Chef Northwest nominations, which he won in 2001.   Chef Boulot's pedigree is most impressive, having learned to cook as young boy in Normandy.  He studied under renowned Chef Paul Robichon before seasoning his skills at numerous significant Paris, London, New York and San Francisco hotel restaurants before coming to Portland.  Chef Boulot has also been named Officer of the prestigious Order of Agricultural Merit by the Ambassodor of France.
