On June 29, a pipeline ruptured in West Tennessee, spilling approximately 200,000 gallons of crude oil. It was the second-largest spill in state history. But at the time, there was no public announcement of the spill, which came very close to contaminating Tennessee's largest aquifer. Environmental activists say this incident goes to show that the state needs to do a better job of maintaining its existing oil and gas pipelines, rather than building new ones. But a new state law just made it easier to build this kind of infrastructure. What do we know about the state of pipelines in Tennessee? What is physician burnout? Why is this important for to CardioNerds? Physician burnout is a syndrome of chronic workplace stress that leads to emotional exhaustion and a sense of dissatisfaction and disconnection personally and professionally. 30-45% of cardiologists have reported physician burnout. 3. What factors affect physician burnout? Emotional and physical exhaustion often lead to physician burnout. First year of training as an intern or fellow and first year of serving as an attending are particularly high-risk periods. This is largely due to learning a new system and responsibilities mixed with a sense of decreased accomplishment.The sense of decreased accomplishment can lead to physicians suffering from impostor syndrome.Grit can be defined as a perseverance for long-term goals. The level of grit was not associated with burnout among first-year Internal Medicine residents. 4. What are some of the solutions to prevent or address physician burnout? Physicians need to feel a sense of belonging and should be supported and celebrated when they have accomplished something by their colleagues and administrators. Fellows and attendings want to feel listened to and supported.Destigmatizing this idea of “perfection in medicine”. Pediatricians get up every day thinking about how to meet the needs of children. And very often we forget about our own health. In this episode of Pediatric Meltdown, Dr. Lia and Dr. Michelle Fiscus discusses physician burnout, what it means to face identity challenges as a physician, and overcoming these challenges. Dr. Michelle Fiscus is a board-certified pediatrician who practiced general pediatrics in Franklin, TN for 17 years before joining the Tennessee Department of Health in 2016. She most recently served as medical director of the Tennessee Vaccine-Preventable Diseases and Immunization Program until she was terminated from that position for sharing a memo regarding a 34-year-old case law that allows certain minors to consent for their own medical care. Dr. Fiscus was thrust into the national spotlight after she released a statement shedding light on the choice of those in leadership in Tennessee state government to place their political agendas over the best interest of the people they serve. Determining Family Resilience with Dr. Jason Yaun, Medical Director of the Family Resilience Initiative at Le Bonheur Children's Outpatient Clinic Join me for an informative conversation with Dr. Jason Yaun. Dr. Yaun is the Medical Director of the Family Resilience Initiative (FRI) at Le Bonheur Children's Outpatient Clinic in Memphis, Tennessee. The FRI is an initiative that aims to intervene with children and their families in the Social Determinants of Health (SDoH) and Adverse Childhood Experiences, or ACEs. The FRI serves families with children 9 months to 5 years of age. These families are connected with Outreach Coordinators as part of routine care and various interventions are implemented based on answers from the ACEs questionnaire. As part of the care, families may be connected with a psychologist or other services offered by Le Bonheur Children's Hospital and other services available in Memphis and surrounding communities. Dr. Yaun talks about the goals of the FRI and supporting families and “meeting them where they are” in their life and circumstance stage. The FRI is making connections for families and hopes to provide a model for other clinics to be able to follow. More About Dr. Yaun:Dr. Jason Yaun is an Associate Professor of Pediatrics at the University of Tennessee Health Science Center in Memphis, Tennessee. He is the Outpatient General Pediatrics Division Chief and the Medical Director of the UT Le Bonheur Pediatric Specialists General Pediatrics Clinic at Le Bonheur Children's Hospital, which serves as the academic teaching practice for UTHSC. Dr. Yaun was born and raised in Memphis, completing medical school and residency training at the University of Tennessee before joining the faculty there. Today, we are setting the table with an organization called the Farmer Veteran Coalition. This is a National Organization with a TN chapter and the work of this organization is pretty straight forward. They help veterans become farmers with support, education, networking, and resources. We visit with Jim Artman, who is a Veteran having served 8 years in the Army, a former sniper, and founder of Appalachian Grit, a yard to table agricultural company located in Knoxville. He battled his own serious case of PTSD with nutrition, exercise and gratitude and he feels it is a mission to help other people in the community to gain greater access to nutritious foods by helping to build and advocate for healthy foods, community food gardens, urban agriculture and food systems. Jim is also Vice President of the TN Chapter of the FVC and he's going to let us know of the important work of this organization. Jim will also let us know about a FVC conference that takes place in Knoxville in just a couple of weeks November 17, 18,and 19th.

Dr. Wells has been featured in numerous print and broadcast features on oral health and cosmetic dentistry, including an episode of The Learning Channel's A Personal Story, and ABC's hit reality TV show, Extreme Makeover. In addition, he is active in many charitable efforts, including Smiles for Life, Give Back a Smile, the Dustin J. Wells Foundation, Smiles for Hope, and the Interfaith Dental Clinic. For five consecutive years, Dr. Wells was selected as one of the Top Dentists in America. In 2001, Dr. Wells received the American Academy of Cosmetic Dentistry (AACD) award for Outstanding Contribution to the Art and Science of Cosmetic Dentistry, and in 2008, was awarded the AACD award for Outstanding Scientific Advancement in Cosmetic Dentistry. And in 2018 Dr Wells was named to the Dental Hall of Fame.

Mark Campen expresses the importance of farmer's markets in discussing the book Gaining Ground: A Story Of Farmers' Markets, Local Food, And Saving The Family Farm by Forrest Pritchard. Pritchard grew up on a farm but didn’t know much about farming. Fresh out of college with a degree in English and geology, he decided he could save the family farm from financial ruin, a farm his parents have been barely keeping up by using all expendable income from their full time jobs in the city. Beginning with his initial attempt to raise money selling firewood, he tells an engaging story of his career as a nonconventional farmer. Mr. Campen explains, "This book is about family, community and a will to successfully carry on the farming lifestyle, albeit in a different way. Knowing where our food comes from and our potential role in the food system is ‘gaining ground’ here in Knoxville." November is lung cancer awareness month. Lung cancer is the leading cause of cancer death in the U.S., due in part to the fact that so few cases are diagnosed at an early stage when cancer is most curable. Today we're hoping to shed light on a type of cancer that carries a stigma—even though 80% of people diagnosed with lung cancer are former or never-smokers. Joining us for the show is Amy Copeland, the Associate Director of Medical Outreach at Lung Cancer Alliance (LCA), and Jenny White, a lung cancer survivor who never smoked who is now the Director of LCA's Tennessee Chapter.

In this episode I had the pleasure of speaking with Jennifer, Director of Loudon County 9-1-1 in Tennessee and she is also the NENA President for the Tennessee Chapter. Jennifer shared her 9-1-1 story beginning with her time as a telecommunicator to her rise up to the position of director. We also touch on how important 9-1-1 is as the main link in each call and how 9-1-1 is pushed to the back of the emergency services chain.
