How prepared do you feel to care for children and youth who have experienced abuse or assault? 1 in 10 Canadians (7.8%) over 15 report at least one instance of sexual abuse before age 15 (Stats Canada) Up to 88% of people being trafficked access health care (Polaris Project) - many don't disclose they are being trafficked Highest rates of intimate partner violence occur between age 12-24 (Stats Canada) Young people aged 15-24 years have the highest rate of sexual assault (Gov't of Canada) In this episode, we will explore the presentation of sexual abuse, sexual assault and sex trafficking in the pediatric setting and discuss trauma and violence informed care practices to guide paramedic care. We will also discuss:  - gaps and barriers to care in this population - practical tips to guide trauma and violence informed care including what not to ask  - role of the Sexual Assault Nurse Examiner and care available in Ontario through the Network of Sexual Assault and Domestic Violence Treatment Centers (SADVTC's)  - mandatory and consent based reporting  - prevelence and risks of strangulation in the context of sexual assault and intimate partner violence and the importance of thorough assessment and imaging

Tammy is a published author and subject matter expert on human trafficking and trauma-responsive care. Not many have as many credentials attached to their names as this amazing woman. Please allow me to introduce you to Laura Clary, BSN, RN, FNE-A/P, SANE-A. There is much to be learned from someone who knows so much.  Laura has been a registered nurse for 11 years. In 2010, she completed her Forensic Nursing training at Greater Baltimore Medical Center and joined the GBMC team as a Forensic Nurse Examiner. She has been the Clinical Program Manager of the GBMC SAFE (Sexual Assault Forensic Examination) & Domestic Violence Program since 2014. Laura is certified to care for patients who have been victims of sexual assault, rape, child abuse/neglect, intimate partner violence, and human trafficking. Laura obtained her National professional certification as a Sexual Assault Nurse Examiner from the International Association of Forensic Nurses in 2013. Laura is a Board of Nursing approved Forensic Nurse Examiner Instructor and an active member of the International Association of Forensic Nurses, as well as the Maryland Child Abuse Medical Providers (CHAMP), and is Chair of the Baltimore County Sexual Assault Response team. Years of Service: 1977-PresentDeb began her nursing career in 1977, and for 42 years delivered babies and worked in nursing leadership roles in the maternal-child area. After her husband passed away, Deb was trying to decide what the next chapter of her life would look like and learned about a new program with the Idaho State Police. ISP was looking for a nurse to develop a state-wide role to teach nurses how to perform the medical forensic examination after sexual assault. Deb was hired as the coordinator and became a certified Sexual Assault Nurse Examiner where she now teaches about sexual assault, strangulation, trauma response, and how it affects behavior for nurses, law enforcement, and Sexual Assault Response Teams in the state. Since beginning her role, Idaho has increased the sexual assault collection sites from 3 to 32 with the goal of ensuring there is a site in every part of Idaho.

In episode one of our 2023 Sexual Assault Awareness Month mini season, Just Science sat down with Dr. Julie Valentine, a certified Sexual Assault Nurse Examiner, to discuss how large-scale data collection can provide a better understanding of sexual assault cases and improve sexual assault response practices, policy, and education. Throughout her career, Dr. Valentine identified a gap in data-driven practices for responding to sexual assault cases. Using her experience providing care to survivors, Dr. Valentine embarked on research to collect comprehensive data regarding which factors assist in developing an assailant's DNA profile so that it can be entered into local, state, and national databases. Listen along as Dr. Valentine describes the relationship between research and providing care, her goals for developing best practices and public policy, and some of her specific research findings. Wade and granting states the authority to impose restrictions on abortion care. We speak with Amy Perez, a nurse and abortion care provider living in Texas, about what the decision means for reproductive health and Amy Perez is a queer activist from Madison, Wisconsin. She graduated with a Bachelor of Science in Nursing and a minor in Chican@ & Latinx studies from the University of Wisconsin-Madison. As an undergrad, the Obama administration awarded her the President's service medal in recognition for 200+ hours of volunteer service for Planned Parenthood Federation of America. Upon graduation, Amy moved to Austin, TX to complete a Residency in Pediatric Intensive Care. After SB8 took effect, a bounty law that allows private citizens to sue whoever helps people obtaining abortions beyond 6 weeks of gestation, Amy decided to switch specialties to serve those affected by this law. Amy currently works as an Abortion Services Nurse for Planned Parenthood of greater TX at the South Austin Surgical Services Center. Amy intends to become a Sexual Assault Nurse Examiner to help victims of sexual violence when she becomes eligible to certify. Amy's goal is to become a Midwife and a Women's Health Nurse Practitioner and open her own sexual and reproductive health center. Eventually, she will also get a DNP in psychiatry & mental health. She has been the Clinical Program Manager of the GBMC SAFE (Sexual Assault Forensic Examination) & Domestic Violence Program since 2014. Laura is certified to care for patients who have been victims of sexual assault, rape, child abuse/neglect, intimate partner violence, and human trafficking. Laura obtained her National professional certification as a Sexual Assault Nurse Examiner from the International Association of Forensic Nurses in 2013. Her life goal is to help people understand that their relationship with their past traumas affects their current life and what to do about it. Although her focus is sexual violence, she understands that all traumas have an effect on the body, mind, and soul.

Today we speak with Ketura'h Edwards-Robinson, a native Bostonian who is currently working in her home city as a Nurse Practitioner, primarily in the area of Women's Health. One thing that makes Ketura'h special is that in the past year, Ketura'h trained to become a birth doula and is now serving clients in her community with Accompany Doula Care, a local doula agency that has developed a model of care where their doulas are reimbursed by the client's health insurance, which is helping address health disparities.Ketura'h  has served as a women's health nurse practitioner at the Suffolk county jail, a Primary Care clinical care manager, OB-GYN nurse and even a pediatric nurse. She also has experience providing acute treatment for patients experiencing substance use disorders and is a Sexual Assault Nurse Examiner for the Massachusetts Department of Public Health.During today's conversation, Ketura'h explains what a nurse practitioner is and how their expertise and scope of practice differs from other clinicians and she describes her path to becoming an NP.  She talks about what she loves about being a nurse practitioner and what aspects of OB/GYN care she does and doesn't do.We talked with Ketura'h about her her training in breastfeeding and infant feeding and what led her to become a birth doula (and how she manages being a clinician while attending births as a doula).We end our discussion with her providing listeners with several final words of wisdom for pregnant listeners.
