Commotio Cordis - A life threatening trauma

One of the scariest scenes unfolded this past Monday night for the world to see. There was great excitement for this game, as two of the best teams in the NFL were competing head-to-head with playoff implications. A packed stadium and a buzz that only great teams can generate. Shortly after the game had started the unthinkable occurred… 24-year-old Damar Hamlin was injured on the field. Just a routine tackle. A play that you've seen numerous times in a game and one that you would never think twice about. As Hamlin stood up, he soon then collapsed back down and lay motionless on the field. Doctors rushed by his side, administered CPR, and 20 minutes later he was rushed to the hospital by ambulance. Dr. Grant Garcia wanted to share some information about what happened and explain what Commotio Cordis is: a trauma to your chest, a type of injury that is usually associated with Baseball, Lacrosse, Hockey, and combat sports. Dr. Garcia wants to let coaches and families know how to be prepared and recognize the difference between a common injury and a life-threatening one. --- Support this podcast: https://anchor.fm/nyseasportstalk/support

Happy New Year!!! As we get excited about all the possibilities and new shows for 2023, we look back at some of our favorite topics and conversations of 2022. In 2022, we talked about several hot topics including the future of cartilage, meniscus, and ACL surgeries. We discussed internal bracing, fantastic tech wearables, and what it takes to make it to the next level. We thought a fun show looking back at some of our top segments would allow our listeners to hear some of the highlights they may have missed or some they may want to hear again. In response to some of these segments, we have had a few updates including the introduction of one of the first cartilage stimulation products with Cartiheal Agili-C Implant (tag) which may be the future of cartilage transplants. Also, ACL repair (tag) has taken off including the tightrope and internal brace combination that has been impressing Dr. Garcia and his patients on their speedy recovery as we enter 2023. Finally, wearable technology was taken one step further at the World Cup in which high-tech soccer balls and player tracking apps were used to provide the best experience yet and allowed a fairer and safer tournament. (Read: https://www.jumpstartmag.com/4-cutting-edge-technologies-used-in-fifa-world-cup-qatar-22/) Thank you to all our listeners. Robotics, cameras, and rehab. A perfect combination. Orthopedic surgeon, Dr. Grant Garcia discusses a new technology that will revolutionize how patients can rehab and how surgeons can teach. We start out discussing the Avail system. A camera system that utilizes technology, networking, and software to combine the perfect partnership. This Avail system will allow doctors to learn and teach in a way they haven't done before. One technology that has the industry buzzing is coming from the company Hrobotics. They have come out with an FDA-registered robotic, physical therapy device for both the upper and lower extremities called Rebless. Rebles allows for passive, active, active-assisted, and resisted training and range of motion measurement, so therapy can be customized based on each patient's condition and progress. To simplify, as a patient you can take this machine home, and your doctor can program, monitor, and adjust your rehab in real-time. This device can do much more and Dr. Garcia explains all of it. Like any other night for football fans, Thursday Night Football is a time to sit back, root for your teams, and enjoy the game. A fast-moving, action-packed, adrenaline-filled sport that is fascinating to watch. We watch the game in awe, we connect, and it reminds us of our communities and love of the sport at every stage of our childhood. We know with this violent game comes injuries, we expect them, we hope not to see them, but they are inevitable. Blown ACLs, shoulders, ankles, and sometimes frightening compound fractures. These injuries are obvious and then there are those that are by far the scariest of injuries...Head injuries. Concussions. The dirty little word that football leagues don't want us to talk about. An injury that for years the NFL had lied about. The repercussions of this type of injury, until Dr. Bennet Omalu exposed the NFL and the catastrophic results of these injuries. We know the risk, the players know the risk and so does the NFL. In this show, Dr. Grant Garcia discusses the injury that quarterback Tua Tangoviola suffered, and the concerns with concussions. Especially, Second Impact Syndrome. He discusses Fencing, Gross Motor Instability, and the doctors' responsibilities to his patient. What can or should be done moving forward to fix these protocols that seem to have loopholes in them? How does the responsibility of a doctor to his patient affect the decision-making when it comes to million-dollar athletes and their billion-dollar owners? If anything, we hope that this show reminds parents and coaches of the big picture especially when it comes to the health of your kids, and players. --- Support this podcast: https://anchor.fm/nyseasportstalk/support

When technology and creativity intertwine great discoveries are explored. This is what is occurring in the orthopedic field. The discussion of restoration versus reconstruction of a torn ACL at times has been debated, but now more than ever restoration has given patients a great option. Orthopedic Surgeon, Dr. Grant Garcia discusses the BEAR Implant. The Bridge Enhanced ACL Restoration (BEAR) technique is a procedure used to treat ruptured or torn ACLs. This new, FDA-approved procedure serves as a much better alternative than traditional ACL reconstruction. It's less invasive and less complex in nature. Dr. Garcia talks about how the BEAR Implant enables your body to heal its own torn anterior cruciate ligament (ACL). The implant is different because it works with your own blood to heal the torn ends of your ACL back together. We discuss who is a prime candidate, and how soon after your injury should you have this procedure done, pros and cons of restoration versus reconstruction, affordability and getting insurance to approve this procedure. Dr. Garcia covers all patients' questions. Real topics, realistic questions, and answers. Dr. Garcia is always at the forefront of technology and the BEAR implant has him excited for his patients as they recover and get back to normality faster than ever. Why might the BEAR implant be good for you? There's a lot more that we discuss, but I guess you need to listen and enjoy. Patients can now achieve complete functional restoration without the consequences of traditional ACL surgery. A few fun facts. the BEAR implant is FDA-approved. A tiny incision can place the implant as it will be situated in the space between the two ACL ends. Also, the patient's blood will be added to the BEAR implant to activate clot formation (Dr. Garcia explains all of this). --- Support this podcast: https://anchor.fm/nyseasportstalk/support

Athletes are a big part of the conversation because as Dr. Garcia said, consumer wearables are the “Holy grail of athletics” in how this tech will help athletes, trainers, doctors, and the overall organization when it comes down to the players and how they are monitored. Athletes are competing longer, are aware of their sleep patterns, mental and physical stresses, and more. Teams also monitor hundreds of thousands of algorithms to make decisions that they determine is best for an overall winning outcome. There is an abundance of tech such as pitchers' sleeves, helmet monitors, heads-up displays goggles for swimmers, and much more. This tech doesn't just benefit athletes, but patients like you and me. As Dr. Garcia said, they can use this technology to “treat patients with a holistic approach.” We cover a lot in this show…Concerns over HIPPA compliance, telemedicine, also, how surgeons themselves are monitored, and how certain places are making sure they are up to an optimum standard before they go into surgery. The dreaded three letters that an athlete or a person, in general, doesn't want to hear from their doctor is ACL. Better yet, torn ACL. It's an injury that is all too common especially with athletes. An injury that can usually take anywhere from 8 months to a year to recover from. However, Dr. Chad Lavender of Marshall Orthopaedics has developed a new approach, a new technique to ACL surgery that has the orthopedic community buzzing with excitement. It is known as the “Lavender Fertilized ACL.” this technique uses a mixture of bone marrow concentrate, autograft bone, and DBM (demineralized bone matrix) to fill the tunnels within the knee joint during traditional ACL reconstruction. This technique along with utilizing an internal brace reinforcement to improve the stability of the healing ACL patients have reported the same results as the standard ACL reconstruction with shorter recovery times, significantly less pain, and no known additional risks. “My hypothesis was if we improved the biologics and healing process into the tunnel, where we put the ACL, patients would have a better long-term outcome. If we also improve the stability of the ACL with an internal brace, patients will have a better short-term outcome and be able to enhance their rehabilitation – which is exactly what we've seen.” Chad Lavender, MD I had a great time talking with Dr. Lavender as we discussed at length the “Lavender Fertilized ACL.” Technique, but also his love of West Virginia and the University. Dr. Lavender knows first-hand how injuries can really affect athletes and why it's so important for him to help people get back to normality as soon as possible. It's not just helping them to get back faster physically, but what it means to get back mentally. He credits his peers and everyone at Marshall Health for their continuous support, and we also discuss a little college football and Mountaineer rivalries. Imagine being a college football fan and not seeing Ohio State, a rising Utah, and perhaps even an Alabama not being in the playoffs? There are fans that would love it, but I'm not sure if the NCAA does. We all know they love their big names that bring in big money, but maybe this is exactly what football needs in order to finally make changes. They have until Dec. 12th to decide on expansion for next season. We'll see what happens. We also discuss if this season is a trend or if this can be the norm. Dori is passionate while discussing her Huskies and the loss they sustained at the hands of the cougars. It's a great back and forth. Shout out to our buddy Derek Sparks who has continued his journey where we will all eventually end up one day. I know the former Apple Cup MVP for Washington State loved the game. RIP Derek Sparks.

Podcast: NYSEA radio on Spotify, Apple, iheart, Audacy, and many other platforms. Utah dismantled Oregon with a dominating performance this past weekend, and once again for the fifth consecutive season, the Pac-12 will not be a representative in the college football playoffs. A concern for college football as even Greg Sankey the SEC commissioner has been on record that their absence has not been good for the sport. Kudos to Kyle Whittingham and his Utes, as they show us once again that they are the class of the Pac-12. In his 18th season, coach Whittingham has led his team to the Pac-12 title game for the 3rdtime out of the last 4 seasons. Former football coach Scott Laigo travels to Happy Valley and shares his experiences being there. One of the great stories he shares is Penn State coach James Franklin speaking to his team about the Zac Stacey incident and his message to his players. In this week's Student-Athletes Advocates tidbits, Scott gives advice to the 2023 class and what student-athletes need to be prepared for. A must listen. As we wind down the regular season for college football, we look at one of the great rivalry weeks in sports. A game that began in 1900 and ended in a 5-5 tie. The Apple Cup is back. After not playing last year for the first time since 1944, Washington and Washington State renew their rivalry. A game that means everything for these two universities and their fan bases. This season a win for Washington State could lead to a Pac-12 title game. We share great stories and thoughts, from Drew Bledsoe, snowball fights, stuck cars in the snow, and frozen butt cheeks. What?! We also mentioned former 1994 Apple Cup MVP Derek Sparks and how we are thinking of our friend. We hope you enjoy the show and as always, we are thankful for our listeners.

On November 14th, Jennifer Cohen, Athletic Director at the University of Washington fired football head coach Jimmy Lake after 13 games. The last time a coach at Washington had a shorter tenure was in 1956. Jimmy Lake, the successor to Chris Peterson, was supposed to pave the way for future Huskie's glory. A reputation as being one of the top defensive coaches in college football was a young up-and-coming coach that should have cemented himself in Husky lore for years to come. That expectation came to a stunning halt after 13 games. There is much to unpack with this bold decision. Was Coach Lake over his head or was this decision made way too hastily? Did Coach Lake ever have a chance to succeed here in the Pacific Northwest, or were there certain individuals waiting for the first signs of failure to give them the excuse to be rid of Jimmy Lake? A great discussion covering many avenues, thoughts, and frustrations. Disclaimer: Due to technical difficulties with one of the three microphones audio is a bit off, but it is understandable. Also, the other two microphones are completely clear. We felt it was such a great discussion that we posted as-is. Shorty has written multiple articles on the fundamentals of throwing the football, training the quarterback, and passing schemes. In addition, he has developed training videos on pass catching and proper route running techniques for receivers. The All-Northwest Football Camps have received national recognition from ESPN, the New England Sports Network, and the NFL Game Day programs. In 1972, Shorty and Mac Bledsoe (Drew Bledsoe's father) started the All-Northwest Football Camp, which specialized in the training of quarterbacks and receivers. Throughout the camp's 34 years, scores of quarterbacks and receivers went on to play at the collegiate level and several others were drafted and played in the NFL. A few from the state of Washington include Clint Didier - Washington Redskins, Steve Pelluer - Dallas Cowboys, Lake Dawson – Kansas City Chiefs, and Drew Bledsoe -New England Patriots. Drew was the #1 pick in the 1993 NFL draft. We travel down memory lane, share laughs, and ask questions that we think are pertinent to today's topics regarding football and more. Also, Scott Laigo, president of Student-Athletes Advocates discusses what student-athletes are doing to get noticed by coaches and what are some of those tools. Some will say…if you don't like it stop it. Others will say a coach is a teacher and their responsibility is to have their athletes learn bigger lessons than the final score. It's a hot topic after head coach Mil'Von James from Inglewood High School ran up the score and refused to have a running clock after being up 59-0. We discuss coaches' roles and sportsmanship. The battle of Michigan versus Michigan State lived up to the hype. The Heisman hype… as Kenneth Walker III showcased himself on the biggest stage so far this season with a five-touchdown performance. As the Spartans rise up the charts, or should I say... poll. Harbaugh and his Wolverines once again are being asked can Jim get it done. Is he the right man for the job? As we've seen this season already, coaches don't last. Coaches such as Clay Helton, Nick Rolovich, and recently Gary Patterson have all lost their jobs for a litany of reasons. We asked the question and discussed what happens to programs, student-athletes, and coaches' families when fired during a season? Speaking of possibly losing a job. Jimmy Lake and the Washington Huskies have struggled. Not only with their performance on the field, but also coaches comments off of it. As if the Pac-12 conference didn't have enough problems, attendance is down and so is viewership. Why is that and what can be done? Scott Laigo, president of Student-Athletes Advocates discusses what student-athletes are doing to get noticed by coaches and what are some of those tools. Great having coaches Dori Bennett and Scott Laigo on the show… Another college football week is in the books, and we start with the Washington Huskies. As they continue to disappoint. Once again there was a failure at the coaching and quarterback position. At this point, Dylan Morris is not capable of leading his team to a win as he continues to turn the ball over. The bread and butter of this team year after year has been their defense. A defense that is unrecognizable at this juncture of the season. As UCLA steamrolled them to a tune of 237 rushing yards. Is time running out for this staff on Montlake? Speaking of time running out. It seems the clock has struck midnight on Nick Rolovich and his unvaccinated coaches. The Cougars coach may have won against the Cardinals this weekend, but his desire to remain unvaccinated has been a divisive topic for all Cougar fans at Pullman, WA. What do we do? We talk sports. It's as simple as that. What a weekend for college football. The "whiteout" game lived up to the hype with over a hundred thousand fans in attendance. Penn State and their beleaguered quarterback put on a great performance as Sean Clifford hand his best night as a Nittany lion as they go on to defeat the Auburn Tigers. The party might have gone on long into the night but concerned fans might have to worry about their coach James Franklin being looked at by other teams. A rough day/night for the SEC officials. Do we ignore it? Of course not. We have plenty to say. The Big Ten has 6 teams that are undefeated. The conference as played great and look no further than Mel Tucker's Spartans. It's been an incredible turnaround. We discuss the coach, players, and the impact that the transfer portal has had not only one the Spartans, but inevitably the entire sport of college football. College football week 2 is in the books and it was an emotional one. Our coaches Dori Bennett, Scott Laigo, and I remember 9/11 and recognize the wonderful halftime performance by the Michigan Band as they paid tribute to the lives lost 20 years ago. The 2 biggest wins in the first two weeks of this College Football season have come from the Pac-12. As Oregon defeated #3 Oh io State in the best game of the weekend. The Ducks averaged 7.07 yards per rush as the Buckeyes give up over 200 rushing yards. The conference of champions is having a great start to their season so far. On the other end of the spectrum are the Huskies. As they have struggled mightily on offense. After scoring a touchdown on their opening drive in week one, they proceeded to not score on their next 19 drives. The loss to Michigan has put their coaching staff on the hot seat and for good reason. On a positive note, Dori's positive player spotlight focuses on Race Porter. The Washington punter has been raising mental health awareness. Discussing his struggles and his passion. Former coach Scott Laigo, now president of Student-Athletes Advocate discusses the SAT/ACT dilemma and how it can affect you the student-athlete. For more information check out SAAdvocates.org. Starting July 23rdthrough August 8th the world's greatest athletes will compete for the ultimate glory…to be recognized as the best at their sport and represent their country while doing so. This Olympics will introduce a few new sports to their games and one of these sports is Skateboarding. Once upon a time, this sport in the '50s was referred to as “Asphalt Surfers” and then in 1978, Alan Gelfand invented a maneuver that gave skateboarding another revolutionary jump: the “Ollie”, which counts as the greatest trick ever invented and completely revolutionized skateboarding. That was the birth of street skateboarding. Ever since then the sport grew immensely. From Skate videos and magazines in the 80's to the rise of Extreme Games (X games) in 1995 and in 1999 the Hawk soared through the air to land the first 900 as Tony Hawk became the face of Skateboarding. Skateboarding is uber-popular today and as the popularity of this sport continues to rise, so do injuries. Dr. Garcia discusses some of these common injuries that skateboarders sustain. Preventative measures to reduce those risks and the importance of wearing the proper equipment to avoid/reduce such injuries. Faceplants, long bone fractures, attempting tricks above your skill level, ankle and foot injuries are just a few of the things that we discuss. Not everyone goes to college to play sports so most of us High School sports bring the highest level that we achieve. It's one of the few things that Coach Sherlock and I discuss in our interview. The uncertainty of a pandemic, the “here we go again” feeling of no fall sports, the euphoria of getting back on that field and playing a sport that so many love. We discuss and tease him about how to manage all school sports in the spring. Also, how important it is for other Metro schools to have success and how they help elevate the league as a whole. He loves the Cleveland H.S. story and how they rebounded to have a successful season in 2019-20. He wants to see his former school Franklin be successful and get back to its glory days. We discuss “real” home field advantage, public versus non-public schools, and play a little game called “Go for it or Punt?” I can't thank Coach Sherlock enough for his time, and especially on game day. It's a great interview with emotion, information, and hope. Music: Bensound

On June 30, 2020, Zack joined me for a great conversation (https://apple.co/30jV3aZ) to discuss the uncertainty of fall football, the effect, and conversations revolving around the BLM movement, diversity, and his commitment to Ingraham H.S. June seems like it was years ago. Flash forward to March 5, 2021. Spring football is in the air and the Metro league is finally kicking off. Coach Elvig and I discuss the disappointment of fall football being canceled, how that circumstance was used as a positive, and how those positives will continue to be used going forward as well as his gratefulness toward his upperclassmen in how they have helped with incoming freshmen. We discuss the importance of excelling off the field, not just on it, and the people and the process that make that work. Yes. football is back, but being back on the field with his coaches and student-athletes is really what matters the most to Coach Zachary Elvig.

Derek Sparks has many titles. He is a coach, mentor, motivator, author, founder, former Washington State University running back, husband, and father. Derek currently is the running back coach at Pacific Lutheran University, and their season is about to start against the University of Puget Sound. It will be the first time they play in 448 days. It will mark the longest stretch of days for the Lutes without playing a football game since WWII. Derek and I talk about many of the challenges of this past year: his love and appreciation of the game of football and how it has been put in perspective not only for him but also for so many others during the absence of the game that so many love. Getting ready for a college football season has been tough, but trying to get ready for a season as a high school coach seems impossible. We discuss cancer versus coaches, the pandemic, recruiting, and transfer portals. It's a great show and it's always a pleasure having Derek spend time talking with me. The former Metro League high school coach of the year shares his thoughts on getting ready for a season, the obstacles and challenges that he and his staff would endure if he were still coaching high school football, and the leadership that the leagues need to show. The challenges of HS student-athletes getting noticed at the next level can be scary for so many; Derek shares his thoughts and advice with them. Sparks began his coaching career in the junior college ranks, serving as the running backs coach in 1997 at Compton College in southern California. Sparks played at Washington State, helping the Cougars win the 1992 Copper Bowl and the 1994 Alamo Bowl. Sparks was the MVP of the 1994 Apple Cup and was a Doak Walker Award nominee in 1995. He continued his career with a free-agent contract with the 49ers in 1998. A motivational speaker, Sparks won the 49th Annual Coach-of-the-Year Award by the NFL National Football Foundation in 2014-2015. He is a best-selling author, writing Lessons of the Game in 2000, and is the founder of the House of Champions for Homeless High School Student-Athletes in 2014. Music: Bensound

President of Student Athletes Advocate Scott Laigo is my guest this week. Scott helps student-athletes and their parents navigate through the recruiting and NCAA eligibility process. Saadvocates.org is a non-profit organization that will help you navigate the system, scholarships, grades, and more. Scott and I discuss this past season in college football, and especially how the playoff system needs to be fixed and why. February 3rd is decision day for HS seniors, but the transfer portal is a new wrinkle that student-athletes and coaches now have to take into account. Transferring may sound like an opportunity, but be aware of the pitfalls that lay ahead. Larry Scott is out, but who's in? We discuss why it's imperative to get someone in as President sooner than later before certain opportunities are lost to other conferences. This spring the FCS will be playing, and local schools will be trying to complete a season. Can fall, winter, and spring sports all coexist? What health concerns may lie ahead, and whom does that burden fall on? It's a great show with the former football coach. The NFL is in playoff mode and as we enjoy the sport that so many of us love, we are reminded about the dangers of head injuries as two of the biggest stars in the league suffer arguably the most difficult injuries to endure. This past weekend, former NFL MVP Lamar Jackson, and Super Bowl MVP Patrick Mahomes sustained concussions that saw them exit their games. A concussion is an injury that sometimes is not so clear to see, and at other times it is very obvious. Dr. Grant Garcia discusses the various levels of concussion injuries that can be sustained, and how all levels of head trauma are significant. Concussion injuries happen at the highest and lowest level of sport, not just football, and although significant advances are being made it can still sometimes be difficult to catch, especially to the untrained eye, or someone that didn't see the injury occur. We discuss the “signs” to look for. The importance of a player or parent coming forward when a player is exhibiting such signs is paramount and they should never be ignored. Dr. Garcia discusses these signs, the steps or “concussion protocol” that should be followed when returning to sport, and the importance of teams using data forms such as SCAT (Sports Concussion Assessment Tool.) This tool represents a standardized method of evaluating injured athletes for concussion and can be used in athletes aged from 10 years and older. Another informative show that we hope you will enjoy and share.
