Concussion is a 2015 American biographical sports drama film written and directed by Peter Landesman, based on the exposé "Game Brain" by Jeanne Marie Laskas, published in 2009 by GQ magazine. Set during the 2000s, the film stars Will Smith as Bennet Omalu, a forensic pathologist who fights against the National Football League trying to suppress his research on chronic traumatic encephalopathy (CTE) brain degeneration suffered by professional football players. The film premiered at AFI Fest on November 11, 2015 and was released by Columbia Pictures on December 25, 2015. The film grossed $48 million worldwide and received mixed reviews, although Smith earned a Golden Globe nomination.

In 2005, Dr Bennet Omalu would publish a report in the United States on the autopsy brain findings of a former professional footballer from the NFL. His conclusion was the former athlete suffered from Chronic Traumatic Encephalopathy (CTE); a disease caused by repeated brain injuries most likely due to sport. The response was swift and severe claiming that the findings were incorrect, the conclusions erroneous, and a retraction of this report was demanded. Surprisingly, this was not the first time medical literature has noted the link between sport, repeated head injuries and brain damage. The first publication was written in 1928 by Dr Harrison Martland who was the Chief Medical Examiner of Essex County in New Jersey. Harrison performed hundreds of autopsies on people with head injuries including boxers. He noted twenty-three examples of boxers with permanent brain damage that he called ‘punch drunk'. Other researchers found this was not limited to boxing but other contact sports such as football. Later, this condition became known as ‘Dementia pugilistica'. This disease has surged into the spotlight in the last decade due to a number of high-profile athletes who have been diagnosed with CTE. Unfortunately, both professional and amateur athletes are at risk. This condition also greatly impacts the family and friends of those who suffer from it. However, a great deal of misinformation also exists about the risks and causes of this disease. These two episodes (of which this is part 1) are designed to explain what we know, what we don't, and what can be done about this debilitating condition. Our special guests: Renee Tuck – sister of Shane Tuck who was a professional footballer in the Australian Football League (AFL) playing 173 games and a brief career as a boxer. Later in Shane's career, he suffered from mental health issues and in July 2020 took his own life. Shane was diagnosed with a severe form of CTE. He was only 38 years old. Associate Professor Michael Buckland – Head of the Neuropathology department at Royal Prince Alfred Hospital, founder and director of the Australian Sports Brain Bank (ASBB), co-director of the Multiple Sclerosis Australian Brain Bank, and Head of the Molecular Neuropathology Program at the Brain & Mind Centre. Are some sports too violent for children? Kids and teens who play full- or high-contact sports like rugby, American football and mixed martial arts have higher rates of concussions, which can lead to traumatic brain injuries. On the flip side, there are also huge benefits to playing sports when you're young, including community and confidence.  Dr. Bennet Omalu, a pioneer in understanding chronic traumatic encephalopathy (CTE), says that children should not be allowed to play certain sports, and argues instead that we should invent new, safer sports for kids. Babalwa Latsha, Africa's first female professional rugby player, contends that existing sports can and should be modified to be safer for children, and holds that participating in sports is a great way to learn valuable life skills. These injuries are obvious and then there are those that are by far the scariest of injuries...Head injuries. Concussions. The dirty little word that football leagues don't want us to talk about. An injury that for years the NFL had lied about. The repercussions of this type of injury, until Dr. Bennet Omalu exposed the NFL and the catastrophic results of these injuries. We know the risk, the players know the risk and so does the NFL. In this show, Dr. Grant Garcia discusses the injury that quarterback Tua Tangoviola suffered, and the concerns with concussions. In the movie Concussion we can watch the story of Dr. Bennet Omalu, the physician who discovered that many football players in the USA were receiving a form of brain damage known as chronic traumatic encephalopathy. The repeated blows to the head and resulting concussions over many years lead to many problems with mental health in the players. Many people attempt to prevent Dr. Omalu's efforts, asking him to have some respect for the players' bodies and families. However, Omalu had a higher ideal of respect - that pursuing the truth and perhaps preventing further injury was valuable. We can see, in a Hollywood version of events, how the good doctor showed reverence for his patients, praying and asking for their help before he performed an autopsy. Dr. Omalu's commitment to truth and completion presented him with many difficulties. Finally, his work was recognized as true. How can the virtues of one person, fully embodied, make a difference in the world? In this episode, the two compare their different results with ECT and discuss the need for better regulation of this procedure to ensure good outcomes for all patients.See original interview. SHOW NOTES I sat down with Greg Threadgold, an ECT patient who feels ECT saved his life. We had a sincere discussion about the differences in treatment practices and the need to prevent injury among ECT patients. Greg Threadgold discusses his ECT Success and shares how he felt about my failed treatment. Repetitive head injury has delayed effects as does low-voltage diffuse electrical injury. ECT must be considered through the lens of both repetitive head trauma and an electrical injury (900 milliamps/450 volts/504mC). To standardize, regulate, audit ECT and provide rehabilitation to patients if/when injured, please sign and share the international patient safety petition www.change.org/patientsafetyECT Your signature could save a life. Sadly, Greg's experienced his first episode of symptoms associated with delayed electrical injury eight years after having 13 treatments. His experience speaks to the reality that even "successful ECT outcomes" carry the risk of developing low-voltage diffuse electrical injury symptoms years after treatment. Paroxysmal neurological disorders from electrical injuries acquired channelopathies can be difficult to recognize and diagnose. We desperately need researchers who study acquired brain injuries and/or repetitive head injuries to do a retrospective study on people with a history of shock treatment to identify needs associated with the delayed effects of repeatedly exposing the brain to 900 milliamps/450 volts of electricity. Dr. Bennet Omalu, the neuropathologist who first identified and published on Chronic Traumatic Encephalopathy among American Football Players, stated in California's Department of Rehabilitation TBI Advisory Board Meeting that electroconvulsive therapy must be considered through the lens of a repetitive electrical injury to the head. The neuropathology of ECT is clear. It's both a repetitive head injury and an electrical injury--and must be considered as such when considering the functional acquired brain injury to develop appropriate interventional rehabilitative care. Dr. Omalu received his MB, BS [M.D.] degree from the University of Nigeria in 1991. He received his MPH [Masters in Public Health] degree in Epidemiology from University of Pittsburgh in 2004. He also received his MBA [Masters in Business Administration] degree from Carnegie Mellon University in 2008. Dr. Omalu holds four board certifications in Anatomic Pathology, Clinical Pathology, Forensic Pathology and Neuropathology. Dr. Omalu is also board certified in Medical Management and is a Certified Physician Executive [CPE]. Dr. Bennet Omalu, identified a disease he called Chronic Traumatic Encephalopathy [CTE] in the brains of deceased athletes and military veterans; and received the “Distinguished Service Award” in 2016, which is the highest award of the American Medical Association. In December 2015, Sony Pictures produced and released a major Hollywood film titled “Concussion” on Bennet's life and work. Will Smith played Dr. Omalu in the film. Bennet's memoir “Truth Doesn't Have a Side” was published in 2017 by Zondervan, HarperCollins. Last week, Andrew Luck shocked the NFL world by announcing his retirement. In his retirement speech, he stated "I've been in this cycle of injury, pain, rehab, injury, pain, rehab, and it's been unceasing, unrelenting... The only way I see out of it is to no longer play football." This is probably one of the most high profile retirements in the history of the NFL and adds to the growing list of players retiring in or before their "primes". The NFL has dealt with a lot of fall out over chronic traumatic encephalopathy (CTE) since Dr. Bennet Omalu published a case report of the disease in an NFL player (Mike Webster) in 2005. Since then, there have been numerous players that have reconsidered football as a long term career. Listen in as Dr. Raman Gurai and Dr. Brian Joves discuss this and what may be coming down the road for the NFL. This is a discussion between two long time sport fans, who happen to be physical medicine and rehabilitation physicians. The doctors do not know the details of the NFL policies or standards, specific players' injuries and are simply talking it through from an informed fan's perspective. Maroon, M.D., FACS, is Professor and Vice chairman of the Department of Neurological Surgery and Heindl Scholar in Neuroscience at the University of Pittsburgh Medical Center and is the current medical director of WWE. He obtained his medical and neurosurgical training at Indiana University, Georgetown University, Oxford University in England and the University of Vermont. He had done extensive research into brain tumors, concussions and diseases of the spine that have led to many innovative techniques for diagnosing and treating these disorders. In late 2016, the Supreme Court rejected challenges to the settlement of a landmark lawsuit against the National Football League. That cleared the last hurdle to a billion-dollar agreement — former players could finally be compensated for brain ailments they suffered playing professional football. While the lawsuit and settlement rocked the sports world, the way it all started was almost unbelievable. It involved an unlikely friendship between two men from vastly different backgrounds. One was Dr. Bennet Omalu, a Nigerian-born forensic pathologist who had discovered CTE in the brain of former Pittsburgh Steelers lineman Mike Webster. The other was Jason Luckasevic, a lawyer who grew up worshiping his heroes in black and gold. During a meeting in 2006, Omalu told Luckasevic that the NFL was unhappy with his work. They were coming after him, trying to discredit him and ruin his career. “And I said ‘well do something about it, look into it, figure it out, fight back,’” Luckasevic says. “And he said ‘well you’re a good lawyer you figure it out.’” And so, in an effort to defend his friend and help players who had been used and discarded by the NFL, Luckasevic went to work. The 2015 film Concussion, which was nominated for an MTV Movie Award in the true story category, stars Will Smith as Dr. Bennet Omalu, a forensic pathologist who becomes embroiled in a battle with the NFL after his research indicates that a football career can lead to lasting brain damage .The real Dr. Omalu, a physician originally from Nigeria, began his research somewhat accidentally, when he was assigned to perform an autopsy on former NFL star Mike Webster, according to a 2009 GQ profile of Omalu that reportedly inspired the 2015 film. Webster had been a famous football player who, by the time his life ended at age 50, was "pissing in his own oven and squirting Super Glue on his rotting teeth," according to GQ. When Omalu examined Webster's brain, things appeared normal on the surface, but he had a suspicion that there was unseen damage. After getting permission to examine Webster's brain in depth, Dr.Omalu discovered something unnerving — a build-up of proteins, splattered around the brain and gunking up the works, seemed like they might have been the cause of Webster's erratic and dysfunctional behavior. And Omalu believed it was caused by a lifetime of repeated head injuries. He named the disease chronic traumatic encephalopathy, or CTE. The film also stars Alec Baldwin as Dr. Julian Bailes, the chairman of neurosurgery at West Virginia University Hospitals who teamed up with Omalu to support his CTE research. The real Bailes and Omalu fought to share their findings, despite the fact that the NFL not only refused to cooperate with their research but actively tried to isolate Omalu: When the NFL held a summit to discuss concussions in 2007, 300 doctors, scientists and trainers, both from within the NFL and outside of the organization, were invited to participate, reports GQ, but Omalu wasn't. Despite the attempts by the NFL to dismiss Omalu, he was able to publish and advocate for his research, eventually forcing the league to begin to acknowledge the dangers of CTE and the risks that players face. But even though the movie has been made, Omalu's real-life work isn't over. "Anybody still denying the disease is out of his mind," Omalu told GQ in 2009. "The issue now is treatment. I followed the games along with my fantasy stats, cheered when my players scored, cursed when my opponents did. Then the injuries happened. Tony Romo out with a fractured clavicle, Alshon Jeffrey with a hurt hammy. And Luke Kuechly with a concussion. These days, it seems at least one player all 32 teams is going through the “concussion protocol,” a series of tests to protect players’ health. There are many sides to this debate, whether the onus is on the players or league to protect the players’ health. In a time where former NFL player Chris Borland retired after a single year citing the health risks as his reason. He had a bright future and could have made a lot of money as an NFL player. He chose his health. In a recent study, 76 of 79 deceased players’ brains were shown to have some sort of degenerative disease[1]. It’s really hard not to understand why Chris made the decision he did. A new development in treating brain injuries is on the horizon. Kannalife, a bio-pharmaceutical and phyto-medical company, has been studying Cannabinoids (CBD) for use in treatment[2]. We were lucky enough to sit down with former NFL defensive end and Super Bowl XXXIII champion, Marvin Washington to talk about CBD, Kannalife, NFL player safety, and a controversial new movie, Concussion, starring Will Smith as Dr. Bennet Omalu[3]. Highlights of the conversation will be discussed. To expand on the conversation with Marvin, CTE stands for chronic traumatic encephalopathy. In brief, its slang is NFL Concussion Syndrome, and in one study, 96% of former NFL players have it[4]. That’s a very scary number, especially when you see how popular the sport is. It’s essentially saying, if you play football at a professional level, you will get this disease and it will affect your life when you retire. How many of us have had to deal with a parent or grandparent with a cognitive disability? It’s painful. And there is no reason for it. Marvin mentioned that the numbers of young children and high school students playing football is down, but millions and millions of people are still playing this sport[5]. Whether or not to play the game is a conversation that needs to happen within families. As Marvin said, families and players are more educated about the risks. Marvin and I love football. And we want to see it grow, but safely, without a 96% likelihood of getting CTE. Hopefully, these issues will be brought to light this December when Concussion starring Will Smith is released on Christmas Day. Concussion is the story of Dr. Bennet Omalu, the forensic pathologist that discovered CTE. From the trailer (see below), it has Will Smith’s Oscar nom written all over it. This movie has the NFL running scared. They already are planning a rebuttal to the film[6]. This isn’t the first time the NFL has been involved in some concussion related scandal. Late in 2013, there were rumors the NFL pressured ESPN to distance itself from a PBS Frontline documentary, League of Denial: The NFL’s Concussion Crisis[7]. Regardless of whether or not the NFL pressured anyone, and regardless on the success of Will Smith’s movie. Concussions and CTE are problems. They warrant solutions, wherever they may be found. I met Marvin through contacts at the Cannabis World Congress and Business Expo in Los Angeles. Marvin spoke about the work of Kannalife, a company doing R&D on the medical benefits of CBD, of which he serves on the advisory board. Kannalife holds the exclusive license for the commercialization of CBD as an antioxidant and neuroprotectant. There is limited research showing the benefits of CBD to reduce short term brain damage[8]. For more on Kannalife, check them out here. CBD is a non-psychoactive aspect of marijuana. Meaning, it doesn’t get you high[9]. This gives CBD many positive medical aspects. Mostly because this limits the side effects. It isn’t used for recreational purposes like THC (the psychoactive ingredient in marijuana.). Even in states where medical and recreational marijuana use is legal, it doesn’t change the fact that marijuana is a banned substance in every sport! A UFC fighter just got suspended for five years because of it[10]! We are on the verge of some very exciting discoveries. In time, not only will CTE be a thing of the past, but very possibly dementia, Alzheimer’s, and ALS. What are your thoughts? Bennet Omalu Bennet Ifeakandu Omalu is a Nigerian American physician, forensic pathologist, and neuropathologist who was the first to discover and publish findings of chronic traumatic encephalopathy in American football players while working at the Allegheny County Coroner's Office in Pittsburgh. Bennet Omalu discovered Chronic Traumatic Encephalopathy in former football players, sparking years of denial from the NFL and the creation of a movie about his life's work. For Omalu, the release of Concussion served as the ultimate vindication for years of hard work, and provided a spotlight for other endeavors. Along with his position as chief medical examiner for San Joaquin County, he serves as president of Bennet Omalu Pathology, as well as associate clinical professor of pathology at UC Davis Medical Center. In this episode we explore emerging radiology technologies and the new field of Preventative Radiology The info in this show will help you understand your risk for Alzheimer's and cognitive impairment and possibly even concussion related brain changes - and what to ask your doctor to get it ordered correctly.   Our guest, Cyrus Raji MD PhD, is a clinical fellow at the University of California - San Francisco. He completed his combined MD and PhD at the NIH-funded Medical Scientist Training Program at the University of Pittsburgh School of Medicine in 2010.  He has worked with leaders in the field including Dr Bennet Omalu (the pathologist played by Will Smith from the movie, Concussion).  His current research involves Diffusion Magnetic Resonance Imaging based Edge Density Connectome Mapping for Prediction of Alzheimer's Disease. He is an advisor to the NeuroReader.   Neuroradiology is the branch of radiology focusing on the brain and spinal cord.  Ironically, very few radiologists actually have deep expertise in neurodegenerative conditions like Alzheimer's.   An MRI of the Brain- looks at brain structures and is readily available to most anyone through a local doctor. Typically they are simply interpreted by an individual radiologist which may or may not be useful in assessing your risk of Alzheimer's.   A better assessment may be the individual radiologist joining forces with computer programs that perform volumetric analyses of specific brain regions (explained in min 26:38).  These volumetric analyses programs assist the radiologist by comparing domains of your brain to a database of normal brains and may give you a more detailed analysis and detect areas of early concern before it is obvious to the naked eye (even a trained one sometimes!).  It can also identify other patterns of brain shrinkage - like vascular disease (e.g. strokes, mini-strokes, and suboptimal blood pressure issues), alcoholism, and psychiatric disorders.   The Different Types of Imaging Used for Alzheimer's 1) MRI (Magnetic Resonance Imaging). Looks at the structure of the brain alone. Does not look at function. A brain MRI contains no radiation if it is done without IV contrast. 2) fMRI (functional MRI) measures oxygen reflecting blood flow in the brain. Can identify problematic areas before structural changes are evident.  It is overall a more sensitive tool than regular MRI, but for the most part is only available in research setting. 3) "Nuclear imaging" - looks at metabolism in the brain using radioactive tracers that have patterns consistent with Alzheimer's and other dementias.  2 examples of nuclear imaging  a) PET-fdg assesses glucose utilization, high cost if not covered by insurance and not as available as MRI  b) SPECT scans - not widely available and cost is somewhat prohibitive at this time.   What insights imaging can give you about Alzheimer's Brains Classic brain atrophy (shrinkage) in Alzheimer's happens in the hippocampal regions in the medial temporal lobes near the middle of the brain.  The hippocampi are important in the making (encoding) of new memories.  The hippocampi start to shrink up to 3 years before actual symptoms of Alzheimer's begins. And in that area one can get a loss of volume in the temporal lobe. Posterior cingulate gyrus and precuneus in the brain's parietal lobes can develop problematic changes as well. These areas are important in tracking information and cognitive integration. What do my MRI volumetric numbers mean? To be considered abnormal and potentially consistent with Alzheimer's diagnosis - the "magic number" for hippocampal volumes on MRI of the brain (using the Neuroreader or Neuroquant tool) is a number below the 25 percentile  (this information is included in the report provided by Neuroreader or Neuroquant).  Ventricular volumes, if higher than 75th percentile, may be too big and suggestive of Alzheimer's or another disorder as well. An Introduction to the field of "Preventative Neuroradiology" (discussed at minute 23:30). Knowing your risk for Alzheimer's with imaging.  It's not the whole story but its good data. If your MRI  is read as "normal" but is noted to have some "periventricular white matter" or white matter changes suggesting "chronic ischemia" and low blood flow - this may not be exactly normal. Remember T2 FLAIR (one way the radiologist views the MRI images where fluids lights up brighter) is the easiest place to see these changes on your MRI.  These changes may represent issues like hypertension, diabetes, or artery disease.  When there is white matter problems then gray matter disease (or problems with brain cells like neurons) is probably not far behind. Lifestyle changes seem to be able to impact the hippocampus in a positive direction. Brain changes classic for CTE (Chronic Traumatic Encephalopathy) that happen as a consequence of concussion/traumatic brain injury may be found within the brain stem. Ventral diencephalon and frontal lobe may be affected as well.     How to Get a Volumetric MRI of your Brain To get a Volumetric Brain MRI study done so that it can be computer-read by Neuroreader (by the Brainreader company) or NeuroQuant (by Cortechs labs).  Make sure to have it done in a "volumetric acquisition" (ie really "thin slices").  Ask the imaging center's MRI technologist: if the MRI Volumetric scan is done in the "sagittal" orientation with General Electric machine- "acquisition in SBGR" or if the machine is a Siemens machine-acquisition ask for it to be done in "MP-RAGE" or whatever settings the institution uses for volumetric acquisitions.

On this edition of No Holds Barred, host Eddie Goldman once again spoke with Dr. Bennet Omalu. He was in New York Monday, August 7, to give a presentation at the CUNY Graduate School of Journalism in New York, and hosted by the New York Press Club, about his new book, Truth Doesn't Have a Side. Dr. Omalu is the pioneering forensic pathologist who was the first to prove a connection between the head trauma and brain injuries which inherently result from playing American football, and the disease CTE, chronic traumatic encephalopathy. We spoke with him right after this presentation, and mainly about why he included wrestling in his list of high-contact sports which children under the age of 18 should avoid, which are football, boxing, MMA, ice hockey, and rugby. A study published September 2015 in the American Journal of Sports Medicine found that college wrestling has the highest rate of concussions of any college sport, including football. But as to rules changes in wrestling to make it safer, Dr. Omalu said, "I tend to defer to the sports experts, because I'm not an expert in many of these sports." Dr. Omalu, however, sees wrestling's problems as being far deeper than just the levels of head trauma and brain injuries. "The problem with wrestling is," he said, "there's no professional wrestling outside WWE. "So if you, say, allow wrestling in children, that means you're encouraging children eventually to go into MMA or WWE. Does that make sense? So that is why I lump all of them together, because I see it as a spectrum." In addition to his comments on the combat and contact sports, we add commentaries on some of the issues related to CTE, concussions, and combat sports. On this edition of No Holds Barred, host Eddie Goldman once again spoke with Dr. Margaret Goodman, a neurologist, the former chair of the Medical Advisory Board of the Nevada State Athletic Commission, and the president and board chairman of VADA, the Voluntary Anti-Doping Association. This is another in a series of discussions celebrating the tenth anniversary of this No Holds Barred podcast, which began in March 2006. These interviews will focus both on the legacy and issues raised over the years on this show, as well as contemporary issues and the future of the combat sports and martial arts. We spoke with Dr. Margaret Goodman by phone Monday. There is hardly a day without a report, discussion, or analysis of doping in sports. Even a decade ago, this was not the case, particularly in the combat and contact sports. Yet today, issues like doping, as well as athlete safety, brain trauma, and CTE (chronic traumatic encephalopathy), regularly appear in the headlines and are the main topics in the news and on discussion shows. It has taken time for public awareness of these issues and the crises for sport they entail to grow to a critical mass, but that is where we are today. "I think that public awareness, and the public, the fans that watch these sports, asking questions, demanding answers to those questions, keeping on top of the sports, and even with the athletic commissions and the media asking questions, I think changes do come," said Dr. Goodman. "The public is more and more aware of this," she said, and urged, "I think that it's really important for the average boxing fan and MMA fan to ask questions, to find out as much information as they can." More and more transparency on these issues and more and more people asking questions, she said, "this is what really will evoke changes." We discussed many specific issues, including that of the many suspensions of athletes for using the banned substance meldonium; why many of these suspensions have now been lifted; drug-testing in MMA including the UFC-USADA program; the World Boxing Council (WBC) working directly with VADA on testing fighters and the WBC Clean Boxing Program; the growing list of top-rated boxers who are part of VADA's anti-doping program; the lack of uniformity in drug and medical testing of fighters, both around the U.S. and the world; Bellator MMA's booking of Kimbo Slice for a fight in the U.K. right after he tested positive for using a steroid from his February fight and was suspended for three months and had his fighter's license revoked by the Texas commission; the dangers of brain injuries in. and the relative safety of, boxing and MMA; whether or not bare knuckle boxing can be safer than boxing with gloves; the ethics of watching and covering such dangerous sports as boxing and MMA; the need for a national commission for boxing and MMA, for MMA to be included in the Muhammad Ali Boxing Reform Act, and for international regulation of these sports; the importance of the recent film "Concussion" about Dr. Bennet Omalu; how repeated sub-concussive blows to the head must be examined and not just full concussions; an upcoming WBC webinar; and much, much more.
