Dermatologist Dr David Reid joins Ask Dr Jessica to talk about common skin conditions that are chronic (stay around for a long time), like dandruff, Keratosis Pilaris and Moles.   Dr Reid explains the best treatment options for dandruff--like using selenium sulfide, Ketaconazole, or zinc pyrithione (or a combination thereof). He also gives tips on how to treat keratosis pilaris -- a common, harmless skin condition that causes dry, tiny bumps found often on the upper arms, thighs and cheeks.  And lastly, we talk about moles!  He gives a great overview on what moles are, and signs when to see a dermatologist. Dr David Reid is currently the Chief of Dermatology at Rush Medical Center in Chicago.  He completed his medical school degree at Harvard Medical School and his dermatology residency at Northwestern University.   He has been well published and has lectured internationally. Dr Jessica Hochman is a board certified pediatrician, mom to three children, and she is very passionate about the health and well being of children. Eating Organs to Fix Certain Organs [15:48] Scotty says: I was wondering if you have come across any study’s that have taken someone with example a heart condition, and told them to eat beaf heart (or any animal) everyday to see if it had any reaction to their condition. Or a liver disease and they ate a bunch of liver and got better? Nutrition has become overly complicated now in some circles and it seems just so simple for your body to absorb those nutrients to heal itself. Any thoughts about this? Heart Palpitations [20:23] Bret says: Hi, Wolf Pack! Long-time listener (since the Greg Everett days), first-time Rebel and question-asker. I remember a few podcasts ago that Nicki mentioned (in passing) having to quit coffee due to heart palpitations. I've been experience palps for about 1 year and have quit coffee multiple times, with nebulous results: Once for 2 weeks, and once for 3 weeks (no significant decrease in palps either time). Now, alongside the 7DCT and Rebel Reset, I've given up coffee PLUS alcohol (I like a little red wine - weekends only) for the last 4+ weeks, with palpitation frequency and intensity dropping but certainly not eliminated. I'm going to keep up the quit (both coffee and alcohol) for a little while longer to see this thing through, but I'm wondering... If the quit doesn't eliminate the palps, are there any other "usual suspects" that I can bring in for questioning, i.e. other stuff to eliminate or add to my diet/lifestyle? Stats: - Age 53; height 6' 1"; weight 170 lbs; athletic build; very happily married - EKG and Holter monitor test results - normal; I'm thinking about an echo but they're pricey - Strict-but-not-perfect ancestral diet for the last ~9 years (thanks Robb!) - Active (workout 2 or 3 x / week, plus daily movement and/or mobility work) - Excellent sleep - Good community, but could improve - Recently upped sodium intake since learning about electrolytes from you guys. (I'll also be starting an LMNT regimen soon). P.S. - There might be a positional component to my palps (low-grade adult-onset POTS?), but it's difficult for me to QA/QC my own subjective symptom observations to determine that. Bret   4. 13 Year Old Son's Keratosis Pilaris [26:22] Lil says: Hey Robb and Nikki, thanks for all of your great podcasts! I have been a follower of your work for 12+ years. My 13 year old son, Roman, has had quiet the health journey. A year and a half ago he came to me and said he wanted to "get healthier". At that time he was 5'3 and weighed 186 lbs. Both my husband and I live a primal life. We prioritize sleep, sunlight, movement, whole Paleo foods, social connection, limited screen time etc. At our home that is the norm. I am divorced from Roman's dad. Although he is aligned with our health values he did not support that in our kids. (Mostly out of convenience) When Roman was with his dad his diet mostly consisted of fast food! Roman's dad finally got on board when Roman himself showed interest in becoming healthier. We hired a Primal Health coach (we love you Amy Taft). Since working with her Roman is no longer pre-diabetic, has corrected his cholesterol issues, has grown more than 5 inches and has lost 40ish pounds. He feels great about who he is now! He has so much confidence in himself and his abilities! Such a wonderful transformation. Anyhow, since he was 5 or so he has dealt with Keratosis pilaris. We have tried AIP as dietary intervention along with many random suggestions found on-line. (Soaps, scrubs, etc) His bumps haven't gotten better with anything we have tried. The dermatologist we took him too said there isn't much to do besides heavy duty pharmaceuticals. I myself have tinkered around with carnivore. What do you think about Roman experimenting with carnivore on a short term basis? Or, do you have any other suggestions? Thank you in advance!   5. I’ve started listening only within the last few months, and I am hooked. I appreciate your valuable insight on everything from nutrition to politics and Medicare. I was listening to the March 4th episode and heard Robb say something about protein intake from collagen powder not “counting” toward total protein intake. I had to listen to that a few times...WHAT?! Insert “mind blown” emoji here. I did a little research and think I sort of understand why, but I’m also disappointed because collagen was my go-to protein powder for smoothies, coffee, etc. My question for you is - if collagen isn’t going to do the job, then what is the best, most bioavailable protein powder out there? I realize the protein you chew is the best kind, but I am looking to use it as a supplement to bridge the gap. There’s so much information on the interwebs, so I’m hoping you can help clarify. I am a 34 year old female, 5’9” and 140 lbs. I teach (hard...not gentle) yoga during the week and I also do a bit of running. I’m generally pretty fit and my meals are typically paleo. Welcome back to our weekend Cabral HouseCall shows! This is where we answer our community's wellness, weight loss, and anti-aging questions to help people get back on track! Check out today's questions:  Cara: Hi Dr. Cabral! Yesterday (August 31) about 45 minutes after a hard workout at 10:30am; dead lift holds, overhead holds, kettlebell swings, assault bike, I got home and I had wavy lights in my peripheral vision. It freaked me out and this had happened to me a couple of months ago and I remembered a nurse friend said it was likely low blood sugar. I ate some mango and about 20 minutes later it went away. I had a DNS smoothie with blueberries and avocado about an hour before my workout and by the time I was done working out, I had half a gallon of water. This has happened 3 times in the last 4 years, each time was after a work out. I'm super healthy, I don't drink alcohol. I do the Dr Cabral Foundation Level 1 plus cal/mag, 3,000mg of Alkalized vitamin c, lysine, Vitamin D, CBD. I eat wild salmon twice a week and sardines 3 times a week, so I only occasionally take EN fish oil. I started to google search this but stopped because things like mini-strokes came up. All of my recent bloodwork was all where it should be according to your podcast guidance and I just had an eye exam and other than my life long near-sightedness, my eye health is perfectly healthy. Does this sound like low blood sugar? If so, why would it be the case if I ate breakfast and worked out at 10:30, plus had 32 oz of water? I do not want to go to a doctor because it'll be at least an MRI ordered. Im in your IHP Level 2 right now. Thank you for ALL you do! Cara: Follow up: I received a reply from our Dr Cabral Support Group from an optometrist and she said that what I experienced could be a visual aura. She said most visual auras precede a migraine or headache but a good amount of people never get the headache and just experience the aura. I never had a headache or a migraine after any of these occurrences. Then my next question, why would I be getting these aura migraines? Gabriela: Hi Dr. Cabral First I love your podcasts, very informative. Now for my question do you believe women should fast differently than men, are you familiar whith Dr. Beth who is all about women's health. She believes women should have snacks during the fast because of our hormones. Would really love to hear what you have to say about this. Thank you! Luke: Hi Dr. Cabral. Would you be able to tell me what you typically feed your dog. As with humans, diet for pets seems to be a topic that has many differing opinions. What does your dog's diet look like (types of foods and amount), and do you give him any supplements? I know you would have done your research on this and I appreciate your response. Thank you. Richelle: Hi Dr, Cabral,I’ve done your 7 day detox and am currently using the deep sleep protocol. I love using the support group and Michelle advised me to ask you my specific question. I have Blepharitis and my doctor currently has me using tea tree oil wipes which has helped. She says it will never be cured but by doing this it will keep it under control. Because of this, I rarely wear any makeup. I also suspect that I have a parasite issue and plan on doing that protocol in October after my trip to Jamaica.I also have been diagnosed with Afib and aflutter and had open heart surgery as a baby back in 1960. I’m currently on a blood thinner,Eliquis, and take diltiazem for high blood pressure. I’m taking magnesium and only half the recommendation for the blood thinner. At my request, my doctor is gradually lowering my blood pressure medicine as I don’t want to be on any meds. I’ve lost 11 pounds recently, retired from teaching after 36 years and need to lose more weight as I know it will help lower blood pressure. If you've been really frustrated with that chicken skin (also known as Keratosis Pilaris), you're not alone. There's no medication for it, but there is a way to get rid of KP naturally. There are a few important considerations though which I want to highlight for you today. I'm sure your doctor has really only said that this is a skin problem. But the truth is, Keratosis Pilaris is actually a symptom of something else! Fortunately, you've got options to support your skin both with diet tweaks and supplements. In today's episode, I'm sharing what you need to know to get rid of Keratosis Pilaris naturally! In this episode: Why Keratosis Pilaris is not a skin issue The importance of Vitamin A for your skin and other body systems What's the best test? Keratosis Pilaris diet recommendations Supplements for Keratosis Pilaris Quotes: If you have struggled with Keratosis Pilaris, know this — you don't have a skin problem. You have a gut problem. KP is a symptom of something else directing you to investigate. And that something else happens to be low or deficient Vitamin A status. To up your Vitamin A game in your diet, the easiest and most obvious answer is to add sources that contain it! These foods include liver and cod liver oil, both of which give you way more than your daily value of active vitamin A. You can also choose from other food options that offer a much lower amount but could still be really helpful such as king mackerel, salmon, Bluefin tuna, goat cheese, and butter. Three dermatology residents — Dr. Elisabeth Tracey, Dr. Julie Croley, and Dr. Daniel Mazori — discuss tips for clear communication with patients in this special resident takeover of the podcast. Beginning at 6:11, they talk about challenges with topical therapies and setting expectations with patients. “We, as dermatologists, can optimize patient management by being effective communicators,” said Dr. Croley. They provide communication strategies for improving compliance with therapy and ensuring patients have the correct instructions, as well as clarifying patient misconceptions and the importance of maintenance treatment. We also bring you the latest in dermatology news and research: 1. Topical ruxolitinib looks good for facial vitiligo in phase 2 study. About half of patients on the two highest doses had a 50% improvement after 6 months of treatment. 2. Patients concerned about clinician burnout. Almost three-quarters of Americans are concerned about burnout among health care professionals. 3. Antimalarial may be effective, safe for erosive oral lichen planus. Hydroxychloroquine sulfate may be an effective and relatively safe treatment option for moderate to severe oral lichen planus. Things you will learn in this episode: Review expectations of therapy with patients, such as an intense inflammatory response to topical 5-fluorouracil for actinic keratosis, to ensure that patients remain compliant with the therapy but also feel they can trust you as their physician. If patients are hesitant to use topical minoxidil because they are concerned with the length of time they’ll have to use it, use a metaphor for another lifelong commitment such as brushing your teeth. “What I started actually doing is calling topical minoxidil toothpaste for your hair,” said Dr. Mazori. Talk to patients about spot-treating with acne or applying topical medication appropriately for psoriasis. “A particular challenge in dermatology with topical medications is not just whether or not they use it or pick up the prescription but how they use it,” said Dr. Tracey. Talk to patients about underapplication of sunscreen. Recommend a physical blocker if patients express concerns about systemic absorption. Write down instructions to ensure patients have the relevant information. The teach-back method of communicating with patients often is taught in medical school and ensures that the patients have understood what you’ve said, but it doesn’t ensure that they retained it. Strategies such as having medical students write the instructions or copying notes from your electronic medical record to print for patients can help save time. Emphasize the importance of maintenance treatment for conditions such as intertrigo, seborrheic dermatitis, or onychomycosis to prevent recurrence. Give patients both the trade name and generic name to ensure they use the correct medication. I recently noticed a big flare up of my Keratosis pilaris (KP) and it’s spread to my thighs and calves when it has remained mostly on my arms since I was diagnosed. Previously on my ketogenic diet, I’d see closer to 0.5 blood ketones and blood glucose levels in the 90’s.
