Eric first entered the advertising world as an illustrator using the traditional methods. He was looking for something new after freelancing for a while and knew his skills in letterpress printing and wood engraving wouldn't pay the bills. However, in 1995 he was quick to adapt to the new era brought by the internet and started his first business. Back then, all his clients were just scrambling to get a website for their businesses. Finding clients was as easy as sending them an email offering his services. This agency grew quickly to 12 employees and then was hit by two major events throughout the years: the dotcom bubble burst and 9/11, prompting a dramatic downsizing. Though the agency gradually recovered, Eric ultimately decided to sell in 2013 looking for a fresh start doing some consulting work. He wanted to help business owners learn how to run their business. One of his clients was Cuberis, whose (then) owner needed guidance in managing the business. What began as a consulting relationship evolved into an unexpected opportunity and Eric eventually purchased the agency. With this, round two of agency ownership began. Learning to Diversify Clients as the Key to Agency Survival That first blow during the dotcom bubble burst helped Eric see the initial model of direct client engagement was no longer viable. Whereas before the referrals just poured in as everyone tried to beat the competition to get a brand new website, he now needs to forge strategic alliances allowing him to continue generating business. He also needed to rethink his focus, so far marked by working primarily with small, brick-and-mortar clients. Instead, the experience gained during several difficult times and subsequent economic downturns taught him that a diverse client base can serve as a buffer against market volatility. An Unexpected Exit: What Decisions Led to Selling the Agency? Eric's decision to sell his agency emerged organically from a series of strategic decisions that began in 2000 when he hired an consultant to enhance his business management skills. In hindsight, investing in professional guidance was the beginning of a journey he hadn't anticipated. Following the consultant's advice, Eric started transitioning from an active role in his agency to developing a resilient organizational structure and empowering employees to operate independently. Initially, this move didn't have an exit strategy in mind—just sound business practices aimed at improving the agency's efficiency. However, by 2008 he felt there wasn't much for him to do at the agency, which made him restless. While he contemplated changing up things in the agency to satisfy his entrepreneurial drive, he knew it would just divert from the things that were already working. Ultimately, it became clear that instead of introducing changes just to scratch his entrepreneurial itch, it would be better to sell and move on to new things. Building a Bridge from Employee to Agency Owner When Eric decided to sell his agency, he identified an employee with  the ambition and capability to take over the business. The challenge then became structuring a deal that would make the purchase feasible for this successor. The plan was a five-year buyout with an element of owner's financing. Basically, Eric increased the employee's salary so that he could take a portion of this new salary each month and buy shares according to a distribution schedule. Over a five-year period he continued to buy shares as his equity increased. Once he hit a 45% ownership, he would buy the rest all that once through a loan. This structure not only provided the employee with a clear pathway to ownership but also allowed him to acclimate to the responsibilities of ownership without the pressure of an immediate buyout. He was able to learn about the business and develop his leadership skills under Eric's mentorship. For him, the key to succeeding with this type of structure is to take your time with the process. Ultimately, this was the best decision for the agency and for himself. Eric knew the business was in good hands and he also knew there were other things he wanted to do. He wanted to focus on helping other people run their businesses more efficiently. Not Your Time to Sell? Here are the 5 Roles of a CEO For Eric, not working in the business and feeling isolated from the work being done helped him realize he wanted to sell and move to other things. However, this doesn't have to be the case for all agency owners. You can successfully make the transition from owner to CEO and find meaning in your new role as long as you understand what that role is. The 5 roles of a CEO are: Grow and mentor the leadership team. To be the face of the company. To set vision and direction. Manage the financials. Be available for key relationships. Whatever you're doing, make sure they are part of these five roles. If it's something outside of these, you need to assess whether or not you are the one that should be doing it. If this is something you're ready to do for your agency, then selling is not the right move and you can continue being part of the business growth. The ST segment on an ECG represents the interval between the end of ventricular depolarization (QRS) and the beginning of ventricular repolarization (T-wave).  It should appear isoelectric (flat) in a normal ECG. What if the ST segment is elevated? This is evidence that there is an injury that goes all the way through the muscular wall of the heart (transmural) This is very concerning for a heart attack (STEMI) but can be occasionally caused by other pathology, such as pericarditis What if the ST segment is depressed? This is evidence that only the innermost part of the muscular wall of the heart is becoming ischemic This has a much broader differential and includes a partial occlusion of a coronary artery but also any other stress on the body that could cause a supply-and-demand mismatch between the oxygen the coronaries can deliver and the oxygen the heart needs This is called subendocardial ischemia What else should you look for in the ECG to identify subendocardial ischemia? The ST-depressions should be at least 1 mm The ST depressions should be present in leads I, II, V4-6 and a variable number of additional leads. There is often reciprocal ST elevation in aVR > 1 mm The most important thing to remember when you see subendocardial ischemia is…history Still, keep all cardiac causes on your differential, such as unstable angina, stable angina, Prinzmetal angina, etc. Also consider a wide array of non-cardiac causes such as severe anemia, severe hypertension, pulmonary embolism, COPD, severe pneumonia, sepsis, shock, thyrotoxicosis, stimulant use, DKA, or any other state that lead to reduced oxygen supply to the subendocardium and/or increased myocardial oxygen demand. Anxious Behaviors Linked to Gut Dysfunction in Dogs Dr. Jason Gagne, board certified veterinary nutritionist, joins host Laura Reeves for a deep dive into dog food feeding trials, anxious behaviors linked to gut dysfunction and more. “It's one thing to open up the AAFCO book,” Gagne said. “The Association of American Feed Control Officials who set this model bill, as it's termed and says you need this much zinc, you need this much selenium and so forth. Then pet food company XYZ, whoever that may be, goes ahead and puts that much in or hopefully a little bit more to meet those requirements. “It's another thing to actually be the company (Purina) that does these feeding trials. Again, as I mentioned before, we're obtaining these ingredients, we're formulating our diets. We have very tight control over our ingredients specifications and have very high standards for who our suppliers are. But it's another thing to then take that diet that you make and feed it out to a colony of dogs and we do do that and we're proud of that. “The purpose of doing that and having that colony is to feed these diets. Because when we do that, we can measure the digestibility of our diets. We can assess the fecal quality and the fecal score, which I think is important to a lot of breeders and sporting enthusiasts. And we can even assess the performance on the diet. “It's not just important to meet the AAFCO requirements, but it's more important really to optimize those recommendations for the betterment of the dog, right? We put those diets out there on the market after we've assessed, hey, we put this in the diet, we want to make sure it's actually in the diet. We can analyze the diet itself, but then again, we're analyzing what's going in and coming out of the dog as well. “This microbiome, what else is it doing? It's actually been linked to behavioral issues as well when it goes out of whack, that dysbiosis as you mentioned earlier. “We actually did do a study with dogs displaying anxious behaviors, and paired it with physiologic as well, so salivary, cortisol and we also had Holter monitors on them for heart rate. We were able to show that when the dogs were being supplemented with the bl999, they (had) a decrease in barking, jumping, spinning and pacing, which was anxious behaviors that they were displaying, versus when they were on the placebo.”

Pearls and Quotes - Channelopathies One cannot equate the presence of type 1 Brugada ECG pattern to the diagnosis of Brugada syndrome. Clinical history, family history, and/or genetic testing results are required to make a definitive diagnosis. The loss-of-function variants in the SCN5A gene, which encodes for the α-subunit of the NaV1.5 sodium channel, is the only Brugada susceptibility gene with sufficient evidence supporting pathogenicity. Exertional syncope is an “alarm” symptom that demands a comprehensive evaluation with 4 diagnostic tests: ECG, echocardiography, exercise treadmill test, and Holter monitor. Think of catecholaminergic polymorphic ventricular tachycardia (CPVT) in a patient with exertional syncope and normal EKG! ICD therapy is never prescribed as monotherapy in patients with CPVT. Medical therapy with a combination of nadolol plus flecainide is the current standard of care. Long QT syndrome is one of the few clinical scenarios where genetic testing clearly guides management, particularly with respect to variability in beta-blocker responsiveness. Notes - Channelopathies 1. What are the diagnostic criteria for Brugada syndrome (BrS)? Three repolarization patterns are associated with Brugada syndrome in the right precordial leads (V1-V2): Type 1: Prominent coved ST-segment elevation displaying J-point amplitude or ST-segment elevation ≥2 mm, followed by a negative T wave. Type 2/3: Saddleback ST-segment configuration with variable levels of ST-segment elevation. It is important to note that only a type 1 pattern is diagnostic for Brugada syndrome, whereas patients with type 2/3 patterns may benefit from further testing. The Shanghai score acknowledges that relying solely on induced type 1 ECG changes has limitations. Therefore, one cannot equate the presence of a type 1 Brugada ECG pattern alone to the diagnosis of Brugada syndrome. The score suggests incorporating additional information—such as clinical history, family history, and/or genetic testing results—to achieve a definitive diagnosis. 2. What is the significance of genetic testing in Brugada syndrome? There are 23 alleged Brugada syndrome susceptibility genes published with varying levels of evidence. However, only one gene mutation, the loss-of-function variants in the SCN5A gene encoding for the α-subunit of the NaV1.5 sodium channel, is considered to have sufficient evidence. The overall yield of BrS genetic testing is 20%. The presence of PR prolongation (>200 ms) along with type I EKG pattern increases the yield to 40%. On the contrary, in the presence of a normal PR interval, the likelihood of SCN5A positivity drops to

Chris Darland, President and CEO of Peerbridge Health, discusses technology breakthroughs that enable early, low-cost detection of heart issues with a cardiac monitor that uses AI to analyze heart rhythms over time to detect patterns that may indicate heart failure. The device is based on ECG technology that drives the Holter monitor and delivers a full 12-lead ECG with a volumetric 3D view of the heart. The goal is to reach more patients who may not proactively seek medical attention and provide early intervention and treatment.  Chris explains, " I think the problem with the current state is largely that it's too late. And I say that to mean that 80% of all heart failure cases today are getting caught after an emergency room visit. So, it's not necessarily that we don't have the tools to find heart failure and the cardiologists and expertise to identify it, but we're not getting it in time. So if you are one of those patients who, let's say, are caught earlier or maybe unfortunately when you come through the ER, a lot of the tools are existing in the hospital. So you'll get an ultrasound, an echo, to get a view of your heart. We'll do an ECG in the hospital and potentially a cardiac MRI. There's a suite of tools available in the acute care setting that can get to this conclusion relatively quickly if you have all the tools right in front of you." "The nice part about it is it's relatively accepted from a workflow perspective, a tool that's used quite commonly and has become exceedingly convenient over the last five to 10 years. There is a lot of innovation in a handful of companies to make it easy to wear, light, waterproof, etc. And when we think through the innovation we're working through right now, it's essentially using that tool and the output of this ambulatory cardiac monitor that we have in Peerbridge. Then doing some interesting AI advanced diagnostics on top of it to pick up patterns that were maybe lost historically or potentially missed based on signal fidelity of devices that were in the market. "The Beatles blew my mind." The LA artist talks about her new album—Something in the Room She Moves—writing music while pregnant and how she works in the studio. The LA-based composer and musician Julia Holter has garnered a reputation for releases that toe the hazy edge of shoegaze and dream pop, always skirting the contours of the electronic music world. On today's RA Exchange, she takes a deep dive into her sixth studio album, Something in the Room She Moves, with music editor Andrew Ryce. Holter talks about writing all but one of the songs while she was pregnant ("Evening Mood" even captures a sample of her child's ultrasound, recorded from her phone), which lends the album a warmth and tenderness unparalleled in any of her past releases. But it's also a somber record; her nephew died while she was recording it, an event that forced her to process the complexity and gravity of two conflicting, converging experiences while composing. For the technically inclined, Holter also discusses how she works in the studio and manages post-production. As the artist tells Ryce, she likes to play with resonant frequencies and processing that puts each of the sounds she records into the same sonic world on a record, using effects and composition tricks to give everything what she calls a "sensuous vibe." Deepthy Varghese, MSN, ACNP, FNP, of Northside Hospital, is joined by Sammy Khatib, MD, Ochsner Clinic, Foundation Program, and William Whang, MD, FHRS, Mount Sinai Hospital, to discuss the PULSED AF study investigated the impact of monitoring strategies on atrial arrhythmia (AA) recurrence after pulsed field ablation (PFA) in atrial fibrillation (AF) patients. The study employed various monitoring approaches, including 24-hour Holter monitoring and symptomatic trans telephonic monitoring, revealing that freedom from all AAs varied significantly based on monitoring strategy. PFA demonstrated substantial efficacy, yielding zero burden in 69.4% of paroxysmal AF and 62.2% of persistent AF patients, with a median burden below 9%. Notably, patients with PAF and less than 10% AA burden experienced clinically meaningful quality of life (QoL) improvements, while persistent AF patients showed QoL enhancements regardless of burden. The study suggests that the choice of monitoring protocol influences the ≥30-second AA recurrence endpoint and highlights the association between low AA burden post-PFA, improved QoL, and reduced healthcare utilization related to AAs. It was a picture of Adam and his son holding a great looking Kokanee Salmon that they had just caught in Holter Reservoir. The Captain and Downrigger were surprised since normally you just don't see many Kokanees being caught out of Holter anymore. Oh, but that has changed, and Adam tells all about it, including where and when you can find them and even gives some info on what you might want to use to catch them! In this VETgirl online veterinary continuing education podcast, we discuss atrial fibrillation (AF) in dogs. AF is a common arrhythmia in our canine patients, characterized by that classic “tennis shoes in a dryer” sound on auscultation. This arrhythmia is caused by disorganized, rapid electrical activity of the atria (Pedro). After identification of this abnormality on physical examination, it is important to confirm the arrhythmia with an electrocardiogram (ECG) and consider a full cardiac assessment to better understand the cause and severity of this finding. AF can itself lead to structural remodeling, and in humans with AF, an increased risk of sudden cardiac death (SCD) is observed (Waldmann). However, it is unclear whether this risk for SCD is also true in dogs. So, Borgeat et al wanted to evaluate this in a study entitled "Prevalence of sudden cardiac death in dogs with atrial fibrillation." In this study, the authors hypothesize that a higher proportion of dogs with AF suffer from SCD than those with sinus rhythm and that SCD would be associated with complex ventricular arrhythmias as identified on a Holter monitor.
