Send us a textKaren discusses her recent trip to Bakersfield, California, and her preference for taking the "road less traveled" rather than the main highway. She describes the beauty and serenity of driving along Highway 178, but also the anxiety she experienced about potentially breaking down in a remote area. She emphasizes the importance of having a sense of adventure and humor when taking the road less traveled.Overcoming anxiety and staying present When Karen starts to feel anxious about potential car trouble on the remote road, she describes her techniques for managing her PTSD-related anxiety, including deep breathing, body relaxation, and self-reassurance. She highlights the importance of staying present and mindful, rather than letting anxiety ruin the enjoyment of the journey.Discovering unexpected sights and experiences As Karen continues her drive, she comes across unexpected natural wonders like Lake Isabella and the "Canyon of Delights" with the dramatic Kern River. She expresses a desire to return and explore these areas more thoroughly, especially with her camera. Karen emphasizes how taking the road less traveled can lead to discovering beautiful and unexpected experiences. The benefits of an optimistic mindset Karen contrasts the benefits of an optimistic, "Pollyanna" mindset versus a pessimistic one when taking the road less traveled. She argues that an optimistic outlook, grounded in the principles of Science of Mind, allows one to fully embrace the uncertainty and possibilities of the journey, rather than getting stuck in anxiety and fear.Recap and encouragement to try the road less traveled In closing, Karen reflects on her successful journey down Highway 178 and the sense of empowerment and accomplishment she felt. She encourages listeners to consider taking the road less traveled, either literally or metaphorically, as a way to open themselves up to new experiences and personal growth.Support the PodcastDoSupport the Show.

The discovery prompted a huge law enforcement response, with more than 30 patrol cars and motorcycles from various agencies gathering at the scene, and eventually forming a single file procession to the coroner's office. Services for Benjamin Harmonson are pending. With suicide all too common among first responders it's critical that resources are available – and a handful of such resources are right here in Kern County. 17's Robert Price reached out to one such organization. We now know the names of the two people who died in a house fire in Inyokern back in March. The fire broke out around 3 a-m on March 29th on North Calvert Boulevard near Johnston Avenue. However... in a phone call with 17 News... the AG's Office clarified they're simply reviewing the case right now. They have not decided whether to take on the prosecution. The Kern County Sheriff's Office tells us they've completed the investigation and handed over all information to the AG's Office. Scrivner's whereabouts meantime are still unknown. He has missed the last two Board of Supervisors meetingsNow to a sad start to the Memorial Day weekend. Rescue crews have recovered a body in Isabella Lake. According to the Kern County Sheriff's office, a witness called 9-1-1 around 7:30 yesterday evening, to report seeing someone in the water near 'French Gulch' boat launch area. Emergency crews responded and pulled the person out out of the water. Lifesaving measures were performed, but KCSO says the person was pronounced dead at the scene. It's unknown the age or gender of the person and how long he or she had been in the water. The coroner's office will release the name and cause of death at a later date. The water may seem calm...and as the days get warmer...it can be quite inviting. But...below the surface...the Kern River is really very dangerous. Yesterday ...rescuers updated the number of drownings on the warning sign that sits at the mouth of the canyon off Highway 178. 10 deaths were added to the sign, bringing it to 335 drownings in the Kern River in the past 56 years. Episode 143: Pulmonary Cocci BasicsDr. Lovedip Kooner explains the history, diagnosis, and treatment of pulmonary coccidioidomycosis (cocci for short.) Disseminated cocci infection was also discussed. Dr. Arreaza added some anecdotes of patients seen with this infection. Written by Lovedip Kooner, MD. Comments by Hector Arreaza, MD. You are listening to Rio Bravo qWeek Podcast, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California, a UCLA-affiliated program sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home. This podcast was created for educational purposes only. Visit your primary care provider for additional medical advice.Definition:Coccidioidomycosis, also known as Valley Fever, is an infection caused by the fungi Coccidioides immitis and Coccidioides posadasii. Coccidioides is also referred to as cocci. Generally speaking, C. immitis is found in California and C. posadasii is found in Arizona, and Central and South America. More recently Cocci has also been found as far north as Washington and British Columbia. History:The fungal infection was first reported by Wernicke and Posadas in Argentina in 1892 where they described a case where a man had cutaneous cocci of the head, arm, and trunk. To this day, the head is preserved in Argentina. 4 For many years, only disseminated cases were recognized and described as “coccidioidal granulomas.” The work of Dixon and Gifford in 1935 elucidated that a pneumonic disease of unknown cause termed “San Joaquin Valley Fever” was, in fact, the primary coccidioidal infection and the port of entry of almost all coccidioidal disease. Initial infection occurs predominantly by inhalation of aerosolized arthroconidia and rarely by direct cutaneous inoculation.1,2Coccidioides spp. survive best in areas with low rainfall (12–50 cm per year), limited winter freezes, and alkaline soils. With climate change models, predicting the geographical range expansion. These dimorphic fungi exist in a mycelial form in the soil. Coccidioides species have been found in animal burrows near the Kern River and in Armadillo burrows in South American countries like Brazil. The mycelia produce arthroconidia (spores) that are ultimately airborne and inhaled. The inoculum required for infection is low and in animal models as few as a single arthroconidium may cause infection.3 Infection:Once arthroconidia are inhaled into the lung, there is typically a 1-3-week incubation period. The arthroconidia undergo morphologic changes into spherules, which are large structures that contain endospores.4 As spherules mature, they rupture and release endospores. Endospores can be spread hematogenous or through lymphatics to essentially any organ, leading to the development of new spherules and potentially disseminated disease.5 Not everyone who inhales the arthroconidia gets the infection. Clinical Manifestations.About 60% of patients who inhale arthroconidia are asymptomatic. 30% have a mild respiratory illness, like the flu. 10% have a more serious disease course and are diagnosed. Other symptoms may include fever, drenching night sweats, and weight loss. Extreme fatigue that limits baseline activity may also raise concerns. Symptom onset up to 2 months after endemic exposure should lead to coccidioidomycosis on the differential. Coccidioidomycosis cases have been documented in Michigan, Europe, and China. These cases were of people who traveled to endemic areas for as little as a few days and then were later diagnosed. 1-3% of all coccidioidomycosis cases are disseminated, severe, or chronic pulmonary infections. If undiagnosed, coccidioidomycosis may lead to significant morbidity and mortality. Dissemination sites include the skin, lymph nodes, bones, and Central Nervous System (CNS) which is the most severe. Any organ can be infected, including documented cases of the prostate and adrenal gland. Arreaza: Recap: 60% are subclinical, 30% are mild, 10% serious, 1-3% are disseminated. What are some risk factors for severe infection? Should I stop biking?Risk factors for severe infection:Severe pulmonary infections can happen in anyone but occur more commonly in diabetics, tobacco users, and people older than 65 years of age.Oceanic or Filipino ethnicity and black or African American have a higher rate of dissemination. Immunosuppression, including HIV, transplant patients, and immunosuppressive medications like corticosteroids or TNF-alpha inhibitors have been shown to be risk factors for dissemination. Pregnant patients, particularly in the third trimester have higher rates of severe infection as well.Arreaza: How do we diagnose the disease?Diagnosis:Diagnosis is commonly made serologically. EIA (enzyme immunoassay) is used more often. There are more false positives than false negatives and varies by manufacturer. Kern County Health Department uses Immunodiffusion IgG and IgM and Complement Fixation are used. Immunodiffusion IgG and IgM are scaled by non-reactive, weakly reactive, reactive and strongly reactive. Compliment fixations are scaled by a ratio/dilution. Serum Compliment fixations

Our destination is fly fishing the wild and scenic Kern River with Guy Jeans, owner of Kern River Fly Shop, Kernville, California. Emerging from the shadow of Mt Whitney in Sequoia National Forrest, the Kern flows 165 miles through some of the most beautiful country in California and is home to 3 native species including the world-famous Golden Trout. Guy grew up as a surfer kid in Ventura, playing music, washing windows, and guiding part-time on the Kern while sleeping in his truck. After selling his business, he started his fly shop and now guides, writes fly fishing articles, teaches, designs flies, plays in a “ska” band, and even hosts the Guy Jeans podcast. We break down the North, South, and tailwater sections of the Kern, along with tales of carp fishing, top flies and seasons, and wilderness wanderings. I'm thrilled to introduce you to Wendy H., an Ambassador high adventure troop leader from GS San Diego. Her troop has taken some incredible adventures, including backpacking in the Canadian Yukon, High Sierras, and this year, Mt. Whitney. She is also a certified canoe instructor and her troop has canoed parts of the Colorado River, kayaked in an Alaskan bay and whitewater rafted down the Kern River. The crazy part? She wasn't an experienced adventurer before becoming a troop leader - she built those skills right alongside her girls, and now, she's done things she previously wouldn't have even dreamed of. We hope this interview inspires you that you, too, can design a high adventure lifestyle as a Girl Scout Volunteer.

Nate writes "We decided to go night fishing for catfish in the Kern River canyon because it was too hot during the day. I decided not to go too deep into the canyon but to the first or second campground off the 178hwy. We arrived to lower richbar around 12:30am just after midnight. All three of us were armed, just for protection like we always do. Haul our fishing gear and started fishing with chicken liver as bait. By 1:30am we caught at least 6 catfish in a stringer. We were happy that we didn't really had to use the flashlight since the moon was out. As we were talking, we heard this roar. Roar so loud that we felt instant fear as it vibrate through our chest. I have heard many animals nothing like this. Then we all noticed across the river by a tree, this thing stood up, all black, tall 9-10ft, muscular. We all froze. It proceeded to push the tree next to it into the river. The tree snapped and echo throughout the canyon. I grabbed my knife and cut my fishing line. My friend and brother also in a hurry brought their poles to cut our lines. As I'm finishing up with that, it jumped into the river and was swimming (more like walking through the water) towards us. At that moment, I drop the poles, pulled out my gun, and keys. Told the guys to run to the car. Understand that this river, Kern River, have killed many people, trying to swim across or drowned. We all ran to the car with left our poles, the fish and stringer, and tackle box. Running in fear to the Lexus and drove off. I must've been going 70mph. After a couple miles we came out of the canyons towards Bakersfield, I slowed down, and utter the first words. Wtf was that? We were in shock but asking that it snap the guys back. My friend said the .45 wouldn't do a thing to it. We laugh but we're all still in shock, how big it was, how it was swimming through those rapids, like it was in a indoor swimming pool. Like the river current wasn't an obstacle to get across. It shook all of us. Let me know if you like to hear the second story.”
