DNA has become the gold standard in forensic investigation. As its use is increasing, so is our understanding of its limitations. Jae Gerhard is the principal forensic scientist at Independent Forensic Services. Starting out as a forensic scientist at the Australian Federal Police in 2002, Jae is an expert in body fluid detection, bloodstain pattern analysis, and DNA analysis and interpretation. When I see a runner who's had a sprain or a trip or a fall or something, the #1 concern is to make sure no bones are broken. Most of the time, what I do is I take a portable ultrasound unit with me so that I can look at the foot on the spot and try to figure out whether or not there is a fracture. I had a question about this as a comment on one of the Doc On The Run YouTube video episodes where I was talking about ultrasound imaging in the foot when you're a runner. The question was, “Can you get a false negative? Can you get a false positive?” Can you get a false negative if you're looking for a fracture with an ultrasound unit? Well, that's a great question and that's what we're talking about today on the Doc On The Run Podcast.
There's no such thing as a “false positive syphilis confirmation test”, is there? After all… It's right there in the name, CONFIRMATION test! Well, not all positive confirmation tests are true positives, especially when that test is the FTA-ABS. Yes, it's true, the specificity for this is very good between 95 to 98%. But there are cases of BFPs (Biologic False Positives), which have been reported and published in the literature. This is why putting the test results in clinical context is very important. In this episode, we will highlight a real clinical scenario where a “low level“ confirmation test did not fit the clinical picture (asymptomatic, monogamous, low risk patient). Is there something else going on here? What about autoimmune conditions? Can pregnancy itself cause a false positive FTA-ABS? And more importantly… What are the options for management in this case? We'll discuss all of this, and also review the “reverse syphilis algorithm” in this episode.
