«Einheitliche Finanzierung von ambulant und stationär (EFAS)», über eine Vorlage mit diesem technischen Titel stimmt die Schweizer Stimmbevölkerung am 24. November 2024 ab. Kurz erklärt: Ambulante Behandlungen, etwa Hausarztbesuche oder Operationen im Spital ohne anschliessende Übernachtung, werden anders finanziert als stationäre Behandlungen, etwa Operationen mit anschliessender Übernachtung oder psychiatrische Aufenthalte. Nochmals anders werden die Kosten in der Langzeitpflege, etwa bei der Spitex oder in Pflegeheimen, verteilt. EFAS soll dies nun vereinheitlichen. Nach dem persönlichen Anteil an den Kosten sollen die Kantone ab 2032 von allen drei Behandlungsarten mindestens 26.1 Prozent bezahlen, die Krankenkassen maximal 73.1 Prozent. Laut den Befürwortern der Vorlage können so bis zu 400 Millionen Franken im Jahr gespart werden. «EFAS hat das Potenzial gewachsene Strukturen zu vereinfachen und so die Kostensteigerung zu dämpfen.», sagt Dr. Reto Dahinden, CEO des Krankenversicherers SWICA. Die einheitliche Finanzierung könne Fehlanreize beseitigen, Strukturen vereinfachen und die Qualität zu fördern. Am 24. November wird über die EFAS-Reform zur Finanzierung von Gesundheitsleistungen abgestimmt. Katharina Prelicz-Huber, Nationalrätin der Grünen Partei, lehnt EFAS ab. Lorenz Hess, Nationalrat der Mitte-Partei ist ein Befürworter. In der Abstimmungskontroverse kommt es zum Schlagabtausch. Wer bezahlt die Rechnung im Gesundheitswesen? Heute spielt es eine Rolle, wo eine Behandlung stattfindet – ob zu Hause, im Spital mit Übernachtung oder im Pflegeheim (sogenannt stationär) oder ambulant, also eine Behandlung in einer Praxis, sei es bei der Ärztin, dem Physiotherapeuten oder Psychotherapeuten. Die unterschiedlichen Finanzierungen führen zu Fehlanreizen – etwa, dass nicht zwingend das medizinisch Sinnvolle gemacht wird, sondern das finanziell Attraktivere. Die einheitliche Finanzierung will das ändern: Künftig sollen die Kantone rund 27 % und die Krankenkassen rund 73 % der Kosten tragen. Am 24. November kommt die Reform zur Abstimmung. Die Ja-Seite verspricht sich von der Reform eine bessere Gesundheitsversorgung, eine Entlastung bei den Prämien und Vorteile für das Gesundheitspersonal. Die Nein-Seite befürchtet genau das Gegenteil: schlechtere Versorgung, höhere Prämien und größeren Druck auf die Pflegefachpersonen. Il 24 novembre siamo chiamati a votare anche sulla cosiddetta riforma EFAS, una riforma sostanziale del finanziamento dei costi della salute. Propone che i tre grandi settori del sistema sanitario (ospedaliero stazionario, ambulatoriale ospedaliero e non, cure per gli anziani) siano finanziati nello stesso modo: 27% dai cantoni (quindi dai contribuenti), 63% dalle casse malati (quindi dagli assicurati). Oggi non è così. Con questa riforma si spera di contenere l'aumento dei premi delle casse malati e di favorire un maggiore coordinamento tra gli attori della sanità, con potenziali risparmi all'orizzonte. Il sindacato dei servizi pubblici (Vpod) ha lanciato un referendum. Considera che la riforma non avrà un'incidenza sui premi assicurativi e teme che consegni agli assicuratori privati il controllo del sistema sanitario elvetico. In der einheitlichen Finanzierung von ambulanten und stationären Leistungen («Efas») sehen Bundesrat und Parlament ein Mittel zur Kostensenkung im Gesundheitswesen. Dagegen hat die Gewerkschaft VPOD das Referendum ergriffen: Sie fürchtet einen beschleunigten Anstieg der Krankenkassenprämien. Als Bundesrätin Elisabeth Baume-Schneider vor wenigen Wochen einen erneuten Anstieg der Krankenkassenprämien verkündete, erwähnte sie auch Massnahmen zur Kostendämpfung: «Mit der einheitlichen Finanzierung werden die Prämienzahler entlastet.» Die Rede war dabei von der Reform des Bundesgesetzes über die Krankenversicherung, mit der eine einheitliche Finanzierung von ambulanten und stationären Leistungen eingeführt werden soll. Diese Vorlage, kurz auch «Efas» genannt, kommt am 24. November zur Abstimmung. Behandlungen sollen vermehrt ambulant durchgeführt werden Aktuell gelten je nach Behandlungsart verschiedene Finanzierungschlüssel: Bei ambulanten Leistungen kommen die Krankenkassen allein für die anfallenden Kosten auf. Bei stationären Behandlungen und Pflegeleistungen hingegen übernehmen die Kantone jeweils einen Teil. Gemäss der Vorlage sollen neu alle Gesundheitsleistungen einheitlich finanziert werden. Der Bundesrat und das Parlament wollen so Fehlanreize aus dem Weg schaffen: Weil die Kantone neu auch ambulante und nicht nur stationäre Behandlungen mitfinanzieren würden, erhoffen sie sich eine stärkere Förderung von ambulanten, günstigeren Behandlungen. So würden die Kosten im Gesundheitswesen gesenkt und die Prämienzahlenden entlastet, sagen die Befürworterinnen. Höherer Beitrag der Krankenversicherungen? Ganz anders sehen es die Gewerkschaften, die gegen diese Vorlage das Referendum ergriffen haben. Ihnen ist vor allem der Einbezug der Langzeit-Pflegeleistungen ein Dorn im Auge: Weil die Kantone in diesem Bereich neu einen geringeren Anteil finanzieren müssten als bisher, entfiele ein höherer Betrag auf die Krankenversicherungen. Die Folge davon wären laut den Gewerkschaften höhere Krankenkassenprämien. Ist die «Efas»-Vorlage eine längst überfällige Reform des Gesundheitssystems, die eine Entlastung der Krankenkassenprämien zur Folge hätte? Oder führt sie im Gegenteil zu höheren Prämien und einem Leistungsabbau im Pflegebereich? Zu diesen Fragen begrüsst Mario Grossniklaus am 25. Wie soll in Zukunft die Gesundheitsfinanzierung funktionieren? 14 Jahre arbeitete das Parlament an einer Reform. Kompromisse wurden gemacht. Bundesrat und eine Mehrheit im Parlament sagten «Ja» zur Vorlage. Der VPOD ergriff das Referendum und warnt vor einer «Scheinreform». Am 24. November stimmt die Schweiz über die einheitliche Finanzierung von ambulanten und stationären Leistungen (EFAS) im Gesundheitswesen ab. Darum geht es Leistenbruch oder Operation des Hallux valgus, was früher einen mehrtägigen Spitalaufenthalt zur Folge hatte, kann heute auch ambulant operiert werden. Will heissen, der Patient kommt morgens und kann einige Stunden nach der Operation wieder nach Hause gehen. Das kostet definitiv weniger als eine oder zwei Übernachtungen im Spital. Wer bezahlt heute? Bei ambulanten Eingriffen zu 100 Prozent die Krankenkasse. Beim Spitalaufenthalt bezahlt die Krankenkasse etwas weniger als die Hälfte, der grosse Rest übernimmt der Kanton. Das soll sich ändern EFAS, die einheitliche Finanzierung von ambulanten und stationären Leistungen will, so wie es im Namen steht, eine einheitliche Finanzierung. Rund dreiviertel der Kosten soll in Zukunft die Krankenkasse bezahlen, der Rest der Kanton. Zudem will man auch die Langzeitpflege in dieses neue Finanzierungsmodell einbinden. Ben Azadi interviews Paul Beatty for a discussion all about how cancer grows in the body, and how to reverse aggressive cancer. Visit Paul's website: https://naturalcancerrecovery.com/ Paul Beatty, B.A., B.P.H.E., M.B.A., was the national account manager for Canada, for a Fortune 500 company, who in the 1980s came down with a metabolic disease that doctors could not diagnose. After severe flu like feelings and high fever for days, his left side broke out in many patches of bleeding eczema. Neurological symptoms followed and overwhelming fatigue with pain. Doctors first thought it was Multiple Sclerosis, but after specialists in infectious diseases picked up high titres of Epstein Barr virus, Cytomyclo Virus, and HHV6, the diagnosis became -Myalgiac Encephalomyelitis plus fibromyalgia plus malignant hypothermia. Because it was viral, the doctors said they could do nothing at this time but would treat the symptoms. Later, Paul started losing hair, had cysts that had grown in his back cut out and lost 8-10 teeth due to root caries. Paul, an inveterate researcher, continued to get sicker by the day, but he never stopped looking for answers. Finally, he discovered a clinical trial run by Dr. David Horrobin, the Canadian biochemical researcher who eventually healed him. The trial was overseen by Dr. Peter Behan and his wife. It was published in "Acte Neurological Scandinavia" using EFAs as treatment for "Post Viral Syndromes like Epstein Barr Virus". Dr. Behan was a Harvard Trained Neurologist and head of the Neurology Department at Royal Glasgow Hospital in Scotland. Dr. Horrobin was the collaborator behind the "Double Blind Crossover Trial". At the time of Dr. Horrobin's passing Paul promised his mentor's wife he would carry on the work as best he could. For over 25 years Paul has been quietly helping cancer victims heal themselves one at a time. During this time Paul has helped hundreds and many of their testimonials are listed on the site. Ninety percent of the requests Paul receives come from people who were told there was nothing more oncologists could offer and they should go home and put their affairs in order. Paul has gathered decades of research, both theoretical and practical, since he has had first-hand experience in successfully treating a wide variety of serious diseases. These include even the most difficult forms of cancer such as pancreatic and ovarian. His confidence is founded in success. Paul has a production credit and appeared in a "Nature of Things" David Suzuki episode called "Lives in Limbo" in 1990s about CFS/Fibromy algia and EFAs. Also during the 1990s, Paul appeared on W5, CBC Journal, Dini Petty Show, and the Shirley Show as well as being published in the Canadian Journal of Health. Besides cancer, the protocol shows good results with other serious diseases (autoimmune, cardio-vascular, diabetic complications, dermatological issues, women's issues etc.) Paul believes that if we can heal the cell, we can heal anything because every part of our body is comprised of cells. Today, I have here with me Prof. Brian Peskin, a world-leading scientist specializing in parent EFAs — termed PEOs — and their direct relationship to cardiovascular disease, cancer, and wound care (both traumatic and chronic). [include file=get-in-itunes.html]I warned you that professor Brian Peskin was a fireball. Talk about a guy who is passionate about his work! But in reality how could you not be when you see how the "mainstream" medical science is killing people by telling them that fish oils are healthy? It would be similar to seeing injustice being done and not really caring. Brian is passionate about what he does because he actually cares about people. He cares that people are being lied to and are taking supplements that are actually harming people. Like he says it take about 10 years or so until the damage is known from the concentrated forms of fish oils. According to Mr. Peskin and Otto Warburg, cancer results when a cell is deprived of 35% of its oxygen. What are cell membranes made from? If my memory serves me correct they're mostly constructed of lipids, fats. These cell membranes built from fish oils prevent oxygen coming into the cells and thus making sure they stay healthy. He makes the argument during the show that athletes and people who exercise a lot of high amounts of oxygen in the blood but not inside the cells. So somehow the oxygen from exercise isn't making it into the cell itself. He also states that a lack of parent essential oils (PEO) is also the main issue with heart disease and even diabetes. These oils are everything. We need oil but we need the right kinds of oil. Butter, coconuts and other oils are good but not as good as parent essential oils according to Brian Peskin. Olive oil he says is simply the lack of a negative (fish oil) but doesn't really benefit you very much. So right after the show we ordered some parent essential oils and we're going to see what they do for us. If we like them and see results we're going to start selling them on the website. Listener Tina asks: Hey Ben, in training for my Ironman I finally decided to try the dreaded ice baths and I was amazed how well I felt the next day. I've tried it by running down to the lake which is close to my home, and standing in up to my hips for 10 minutes. I've also sat in luke warm water in my tub and slowly added cold water up to the big dump of the ice tray from my refrigerator. So my question is, is there a certain temperature that ice baths work better in? If I'm still sore on the second day of a hard weekend, would another ice bath be beneficial? Listener Fitz asks: Ben - how do you feel about paleo/primal diet proponents who advise skipping meals, avoiding carbs, even skipping breakfast? Seems like there's a lot of science behind it (although as a distance runner, I wouldn't skip meals or avoid carbs). Mark Sisson at marksdailyapple.com is big on this. Listener David asks: Question on headaches and working out. One of my clients complains of headaches halfway through the workout. I train him pretty hard, usually circuits with minimum breaks in between sets. He doesn't drink coffee/tea. He keeps caffeine intake to a minimum (under 30 milligrams), and most day avoids all caffinated drinks, including his workout days. He's 30 years old, male in good shape and of average weight with no special conditions, ie no diabetes, hypertension, high lipids, etc.. I'm working to build his upper body strength and improve his core. I think it could have something to do with his breathing technique. But I do watch him and make sure he's exhaling on concentrics and inhaling on eccentrics. So maybe he's not breathing correctly or deep enough. What other possibilities do you think could be causing his headaches and if its with his breathing, do you have breathing tips/techniques? Could be low carbs or lack there of? In my response, I direct David to this breathing article I wrote...and also recommend Topical Magnesium. Listener Mike asks: Question 1: I was hoping you could point me in the right direction on your recommended brands for the suggested 4 supplements (Vit D, Magnesium, Omega 3's, and Greens Multi-Vit). I'm 32 and developed a severe case playing volleyball a couple years ago and still struggle with it (even after physical therapy and avoiding any activities that cause patella inflammation/ pain). I can run just fine, infact I'm training for the Hood to Coast again this year, but I want to be able to play other sports and jump again -- any suggestions? BTW thanks for the vitamin D question, it was exactly what I was wondering. 2) In Podcast 92, I was puzzled by your fat recommendations. The research I have looked at indicates we need single digit percentages of daily calories from omega 3 and 6 EFAs. Why do you recommend 15-25%? Is there a benefit that outweighs the cardiovascular risk? Listener Josh asks: It's been a mantra in my family that "slow metabolism" is a curse. Is it true that some people struggle with a "slower" metabolism? Or does those people (like me) just eat a little bit (or a lot) more and slowly add to their body fat? I ask because I monitor what I take in, and most days, I get in 1800 calories (I'm 5'10", 190) and I exercise 6 days a week. No diet or meal plan seems to really be able to help me drop my body fat. My physician says my blood tests don't indicate a "slow metabolism", but even his dietary advice doesn't work. Can you help? Second question, much easier: how far in advance do you plan your workouts? How do you anticipate muscles that you're overtraining (and thus avoid overtraining that muscle) when you're planning ahead? I notice a dramatic change in form throughout May, June, July in my running, cycling and rowing times and recover more slowly after a tough session. If a shorter load was best, would it be using the same technique you described, just for a shorter period of time? Listener Chris asks: I just got my butt royally kicked this past weekend at the Devil's PunchBowl bike race. I believe that the altitude (3,800ft at the start) had a lot to do with that, as I live and train at sea level. My heart rate was sky rocketing the entire race much higher than it ever had in the past. Do you have any practical advice on how to prepare for 3 days of hard racing at a similar elevation in mid-June? Also, is it possible to improve one's lactate threshold? If so, how might I do so? Listener Ken asks: I am having problems staying asleep. I am going to bed around 10-10:30. I am waking up to go to the bathroom and I am not able to fall back to sleep. Very wide awake after 4-5 hrs. Also, I am drenched in sweat when I wake up. I do not work out before bed. Most of my stuff is done morning or early evening. Is this normal? What can I do about it?
