These citations directly impact patient safety by increasing risks of healthcare-associated infections (HAIs), and they appear frequently across major accreditors like CMS, AAAHC, and ACHC. For context: In AAAHC's 2025 Quality Roadmap (released September 2025), IPC remains a persistent top challenge, with deficiencies cited in nearly 90% of surveys for ASCs and office-based surgery settings. ACHC's November 2025 survey data highlights sanitary environment lapses (e.g., sanitation issues, incomplete policies) as a leading deficiency, often tied to inconsistent implementation of hand hygiene, glove techniques, and environmental cleaning. Historical CMS data shows patterns holding into recent years, with "Sanitary Environment" and "Infection Control Program" among the most cited, stemming from failures in basic practices like cleaning, sterilization, and safe injection protocols. These issues often arise from documentation gaps, staff inconsistencies, or overlooked details during busy operations, even in high-performing centers. Key Regulatory Requirements The foundation for sanitary environment and IPC in ASCs is CMS Conditions for Coverage (CfC) at 42 CFR § 416.51, which applies to Medicare-certified ASCs and influences other accreditors (deemed status via AAAHC, ACHC, etc.). Standard: Sanitary Environment (§ 416.51(a)): The ASC must provide a functional and sanitary environment for surgical services, avoiding sources and transmission of infections by adhering to professionally accepted standards (e.g., CDC, AORN, APIC guidelines). Standard: Infection Control Program (§ 416.51(b)): Maintain an ongoing, coordinated program to prevent, control, and investigate infections/communicable diseases. This includes: Designation of a qualified professional (e.g., infection preventionist) to oversee the program. Annual risk assessment to identify infection risks. Integration of nationally recognized guidelines (CDC core practices are mandatory if no other evidence-based standards apply). Policies for hand hygiene, environmental cleaning, sterilization/high-level disinfection (HLD), safe injection practices, and point-of-care devices. Nelle ultime settimana l'attualità ci ha offerto due spunti legati alla nostra società che commentiamo, con un' esperta, nella puntata odierna. Partiamo dalla Svizzera dove una persona su sei ha dichiarato di aver vissuto una situazione di discriminazione razziale nel corso degli ultimi cinque anni. Lo scorso 5 dicembre, il Consiglio federale ha così adottato la prima strategia nazionale contro il razzismo e l'antisemitismo, in risposta alla mozione della Commissione delle istituzioni politiche del Consiglio nazionale denominata «Per una strategia e un piano d'azione contro il razzismo e l'antisemitismo». La notizia di questa nuova strategia si aggancia allo studio dell'Organizzazione per la cooperazione e lo sviluppo economico (OCSE), pubblicato il 2 dicembre scorso, in cui si dice che la popolazione straniera in Svizzera è molto ben integrata. Più che in altri Paesi, le persone immigrate in Svizzera hanno un buon livello di formazione, un tasso di occupazione molto alto e s'impegnano in modo attivo per imparare una lingua nazionale. Lo studio rileva quale potenziale non interamente sfruttato l'integrazione professionale delle donne, soprattutto se entrate nel quadro del ricongiungimento familiare. L'integrazione nel mercato del lavoro svizzero funziona molto bene. Il tasso di occupazione della popolazione immigrata è pari al 77 per cento e figura tra i più elevati nei Paesi dell'OCSE. Quasi la metà di queste persone esercita delle professioni altamente qualificate e anche questa cifra è più elevata rispetto ad altri Paesi. La conoscenza della lingua locale è uno dei fattori determinanti per una buona integrazione. Dallo studio emerge che la popolazione immigrata in Svizzera è molto ben posizionata sotto questo profilo. Dopo cinque anni, circa la metà delle persone che inizialmente avevano una mera conoscenza di base di una lingua nazionale ha raggiunto un livello avanzato; anche qui, la Svizzera si situa un po' al di sopra della media dell'UE. Ritornando alla prima strategia nazionale contro il razzismo e l'antisemitismo, con questo progetto il Consiglio federale ha voluto lanciare un segnale forte a favore della tutela dei diritti fondamentali e umani in Svizzera. Lo scopo perseguito è migliorare la rilevazione dei casi di razzismo e antisemitismo, proteggere le persone colpite, rafforzare la prevenzione del razzismo a livello istituzionale e promuovere al contempo l'impegno sociale. La strategia definisce un quadro d'intervento comune per la Confederazione, i Cantoni, le Città e i Comuni e rafforza il dialogo con la società civile. Il razzismo e l'antisemitismo sono una realtà sociale in Svizzera.undefinedSecondo i dati dell'Ufficio federale di statistica, nel 2024 il 17 per cento della popolazione residente ha dichiarato di aver subito discriminazioni razziali negli ultimi cinque anni. Questo dato corrisponde a quasi una persona su sei, ovvero circa 1,2 milioni di persone di età compresa tra i 15 e gli 88 anni. Le vittime sono discriminate, in particolare, a causa della loro origine o nazionalità effettive o presunte, del colore della pelle o dell'appartenenza religiosa. Le conseguenze sono spesso gravi – minori opportunità sul mercato del lavoro e degli alloggi, svantaggi economici e sanitari ed emarginazione sociale – con ripercussioni sull'intera società. Ma cosa definisce, in concreto, questa strategia che si articola in quattro ambiti d'azione e quali a risultati si prefigge di arrivare? Quali sono i suoi obiettivi? Miljöpartiets språkrör om varför det är så svårt att nå breda överenskommelser om skogen, förslaget om sänkt straffbarhetsålder och regeringsfrågan. Lyssna på alla avsnitt i Sveriges Radios app. Sverige täcks till 68 procent av skog, det finns fler än 300 000 skogsägare i landet, och skogsindustrin står för cirka tio procent av Sveriges export. Skogen är också en plats för rekreation, och har en viktig roll för Sveriges biologiska mångfald samt klimatet, genom att binda koldioxid i marken. Amanda Lind (MP) menar att Sverige behöver en ny inriktning i skogspolitiken för att klara av att förena dessa olika intressen. ”Det forskningen pekar på är att vi behöver se till att skogen får stå lite längre innan man avverkar den. Just nu avverkas skogen allt tidigare, och det är problematiskt för just kolupptaget. Sedan behöver vi också skydda mer skog för att värna den biologiska mångfalden”, säger Amanda Lind (MP). Hon fortsätter: ”Sedan behöver vi också se till att det vi använder skogen till, det vill säga de produkter som faktiskt kan ersätta fossila produkter, blir mer långlivade.”Hur stor avverkning ska det vara tycker ni?”Om man tittar tillbaka och ser vilka avverkningsnivåer vi hade i början av 2000-talet. Ungefär på den nivån skulle vi helt säkert klara LULUCF -målen. Det var inga små avverkningsnivåer det heller. Men vi kan inte bara göra det, utan vi måste också satsa på att stötta de små skogsägarna som vill prova nya metoder och stötta förädlingen inom industrin”, säger Amanda Lind (MP).Vad handlar det om? ”De förslag vi har lagt fram som faktiskt skulle klara LULUCF-målen innebär 15-20 procents mindre avverkning. Det här baseras på att vi de senaste åren har haft en väldigt hög avverkningstakt inom skogen. Det har bidragit till att skogarna i dag inte fångar upp lika mycket koldioxid. Därför är vi i det läge vi är i i dag”, säger Amanda Lind (MP).Vad ser du för konsekvenser för skogsindustrin om avverkningen behöver minska och vara på de nivåerna som du nämner? ”Skogsindustrin har sedan 1990 tappat över 60 procent arbetstillfällen. Det är inte på grund av klimat eller miljöpolitiken utan av andra saker. Därför ser jag att med vår politik finns det potential att öka antalet personer som jobbar inom skogen”, säger Amanda Lind (MP).Amanda Lind (MP) menar att många jobb i branschen försvunnit för att man brukar skogen med mera effektiva metoder, och att där inom industrin där skogsprodukter förädlas som jobben kan öka.Breda överenskommelser om skogspolitiken?Skogspolitiken har utretts vid flera tillfällen under senare år, och många skogsägare saknar tydligare mål och regler för skogsbruket. Amanda Lind (MP) tror att klimatförändringarna är en anledning till att de politiska partierna har svårt att komma överens när det gäller skogspolitiken. ”Status quo i skogspolitiken och i skogsbruket i dag kommer inte att fungera. Inte minst på sikt när klimatförändringarna kommer att leda till mer torka och översvämningar. Mer stormar som gör att vi måste ha en mer robust situation i skogen”, säger Amanda Lind (MP).Vilket ansvar har ditt parti att komma överens och hitta spelregler? Ni kanske behöver kompromissa. ”Så är det inom alla politikområden. När vi satt i regering förra gången förhandlade vi skogspolitiken med S och C och hittade vägar framåt. Jag tror att det kommer att fungera framåt också, men någonstans krävs det också att man måste se över sin egen skogspolitik och inse att man måste vara beredd att ompröva gamla sanningar som har funnits kring skogen”, säger Amanda Lind (MP). Sänkt straffbarhetsålderRegeringens förslag om att sänka straffbarhetsåldern till 13 år vid grova brott har fått omfattande kritik från en lång rad remissinstanser. Inte heller Miljöpartiet vill sänka straffbarhetsåldern. De anser att unga förövare ska tas om hand av Sis, trots den skarpa kritik som också funnits mot hur Sis-hemmen fungerar. ”Det är väldigt mycket som inte funkar tillräckligt bra när det gäller unga som rekryteras in i kriminalitet. Det man håller på att göra nu med att inrätta ungdomsfängelser är något som vi i grunden var emot. Vi hade velat gå vidare med Sis i stället. Stärka rehabiliteringen, stärka skolan. Vi vet att skolan är jätteviktig. Det ska finnas konsekvenser när du har gjort så grova brott.
