This episode features Irene, who shares her personal journey of caring for her husband diagnosed at just 51. Irene discusses the initial warning signs she noticed, including social withdrawal and increased forgetfulness, and the difficulty of balancing work, family, and caregiving. She candidly describes the emotional and physical toll of caregiving, the lack of limits, and the point at which she realized she couldn't do it alone, and the difficult but loving decision to move her husband to memory care. Irene emphasizes the importance of self-care for caregivers and the need to recognize personal limits. A Bit From Irene's Book We left the doctor's office and my head started spinning. I couldn't remember where I had parked the car. Quite miraculously, Robert managed to navigate us to the correct floor of the parking garage despite my that all hope had been sucked out of me. I had been struggling to cope since our last appointment with the family doctor, but receiving this final blow was brutal. I put my left hand on the side of the car, leaned over, and threw up. It was just bile. I had to push something out of me—denial, anger, angst—I didn't know. Wiping my mouth, my snotty nose and my tear-stained face with jittery hands I leaned heavily against the back of the car. I took a long, shaky breath in and out in an attempt to pull myself together. So began the journey with her husband who is diagnosed with early onset Dementia at the age of 51. It's amazing how resilient the human spirit can be. I've spoken to folks living alone with dementia who find ingenious ways to adapt. One gentleman, a former engineer, rigged up his whole house with reminders – lights that flashed when it was time to take his medication, a talking clock that announced mealtimes... it was incredible! Of course, not everyone has that knack for invention, but there are simpler things that can make a world of difference. Things like..." Safety First...making sure the home is safe. You'd be surprised how a simple rug or a poorly lit hallway can become a hazard. One woman I spoke to, bless her heart, tripped over a phone cord and ended up with a broken hip. It's heartbreaking, but preventable!" Memory Reminders...little things to jog the memory. Like, labeling drawers with pictures of what's inside, or using a whiteboard calendar for appointments. I remember this lovely lady who used to lay out her clothes for the next day every evening, just like she did for her kids when they were small. It gave her a sense of control, you know?" Simplicity Makes it Happen...sticking to a routine. It's comforting, like a familiar path in the woods. This one fellow, a retired teacher, swore by his morning walk. Rain or shine, he'd be out there, getting his exercise and saying hello to his neighbors. It allows them to acknowledge and validate their emotions, fostering a sense of acceptance and healing. Furthermore, poetry and spoken word provide a safe space for caregivers to explore the difficult emotions that may be associated with anticipatory grief, such as sadness, anger, guilt, and confusion. However, with the right support, they can be an invaluable supportive caregiver to their loved one. Here are some tips on how to help them navigate that role. Open Communication: Talk Honestly: Explain dementia in an age-appropriate way. Address their fears and frustrations openly. Listen Actively: Validate their feelings. Let them know it's okay to feel sad, angry, or confused. Supportive Caregiving: Find Age-Appropriate Tasks: Let them help with simple tasks like setting reminders or engaging in familiar activities with their loved one. Focus on Strengths: Highlight their unique ability to connect with their loved one through shared interests or hobbies. Maintaining Balance: Set Boundaries: Acknowledge their need for social time and hobbies. Schedule respite care or involve other family members. Our Guest Dr. Allison Applebaum is an Associate Attending Psychologist in the Department of Psychiatry and Behavioral Sciences, Memorial Sloan Kettering Cancer Center (MSK), and an Associate Professor of Psychology in Psychiatry at Weill Cornell Medicine. She is the Founding Director of the Caregivers Clinic at MSK, the first program of its kind to provide comprehensive psychosocial care to family members and friends of patients who are in the caregiving role. Dr. Applebaum's program of research focuses broadly on the development and dissemination of supportive services for family caregivers. She has published over 100 articles, reviews, and book chapters on caregiving, and is the editor of the textbook Cancer Caregivers (Oxford University Press, 2019). Dr. Applebaum has received competitive funding for her research, including awards from the National Cancer Institute, the National Institute of Nursing Research, and the American Cancer Society. She is also the author of the recently published narrative nonfiction book, Stand By Me: A Guide to Navigating Modern, Meaningful Caregiving (Simon Element, 2024). Our Guest: Valerie Drown When it comes to caregiving, Valarie Drown MS, NCC, LMHC is an expert. Drown currently serves as the project director of the Alzheimer's Disease Caregiver Support Initiative (ADCSI), which provides free support services to those caring for people with Alzheimer's disease and other dementias in northern New York. She is also the caregiver for both her mother with Alzheimer's disease and son who at age six was diagnosed with mental and physical developmental disabilities. They may assist with tasks such as coordinating medical care, communicating with healthcare providers, and coordinating with other care team members. End-of-life doulas also provide emotional support and companionship to individuals and their loved ones during this difficult time, helping to create a sense of peace and comfort. They may offer relaxation techniques, such as guided meditation or gentle touch, to help individuals cope with physical and emotional pain. They may also support families as they navigate the grieving process, helping to facilitate conversations about end-of-life wishes and offering resources and support as needed. Dr. Elizabeth Landsverk is on the show to discuss overcoming medication-induced challenges. This challenge is one area of focus in her new book, Living In the Moment - Overcoming Challengees & Finding Moments of Joy. In addition to learning how to overcome specific challenges, I also learned that there are only 3500 geriatricians in the country. Unfortunately, few geriatricians can be found in suburban and rural areas. Dr. Liz is trying to provide information and training to help alleviate this. The focus of Liz's book is to help guide family caregivers to understand what doctors should be thinking, and looking for, medications that may work, others that may cause other problems, and more. Essentially, this book is "what to expect when you have dementia." Most of us know that correctly treating pain can significantly reduce agitation. Did you know that older adults are at higher risk for high blood pressure, stroke, heart attack, edema, and kidney failure if their pain is treated with Motrin? However, Motrin is suggested regularly. To get the most out of our doctor visits, explain what's been happening in the past week or so, what was happening when the symptoms started, what you have tried, and what has worked. Sometimes you need to find the right doctor for your needs. This episode is chock full of great advice, tips, and chunks of knowledge that pair well with her book.
