Today we revisit a topic we last discussed in a 2020 podcast with Laura Mosqueda: elder mistreatment.  Our guests today are geriatricians Carrie Rubenstein and Julia Hiner, and Tony Rosen, an emergency medicine doctor. They talk about where we are now, in 2026, with elder mistreatment, including: Terminology: elder mistreatment vs. abuse and neglect The need to incorporate prevention and solutions into how we talk about mistreatment This is not rocket science. Studying elder mistreatment is much harder than rocket science. Highlighting the reasons they focus on elder mistreatment, including inspiring words for why this led them to geriatrics and aging research Should we screen for elder mistreatment? The US Preventive Services Task Force doesn't see enough evidence to recommend screening. Our guests may differ… Which clinicians should assess for elder mistreatment? Hospitalists? ED docs? Primary care providers? Tony published a study in JAGS showing older adults who experienced elder mistreatment were as likely to visit primary care as those who did not, also great accompanying editorial by Mara Rosenberg and Lena Makaroun gets a shout out. Early evidence that supporting caregivers can reduce elder mistreatment (in one small study of the COACH intervention, rates of mistreatment were reduced to zero) Borrowing from pediatrics: many/most hospitals and emergency departments can call a Child Protective Services Team. Tony is piloting a parallel team for older adults - the Vulnerable Elders Protection Team (see JAGS paper).   We talk about key members of interdisciplinary teams across sites, systems, and counties. Despite its many benefits, few technological developments of the 21st Century have had a larger negative impact on the overall mental health and wellbeing of American young people than the rise of social media. And while no one can imagine putting the social media genie back in the bottle from which it came, a growing chorus of parents, educators, mental health professionals and advocates is calling for better and tougher regulation of the giant corporations that dominate this field. And it's in light of this that many are welcoming a pair of recent court rulings in California and New Mexico that held corporations liable for damages to children injured on their platforms. One of those advocates is Sam Hiner, the co-founder of a national organization birthed in Chapel Hill known as the Young People's Alliance. And recently Hiner joined NC Newsline to discuss the debate over social media as well as several other formidable challenges facing young people in the modern world. And while it's not difficult to identify the positive impacts of these phenomena, the worrisome impacts are also numerous. And this is a trend that seems certain to intensify in years to come with the rapid rise of artificial intelligence. Thankfully, many advocates have started to speak out in favor of stronger laws to protect vulnerable people – especially young people – from exploitation like predatory algorithms, social media bullying, artificial intelligence and other impacts from the online world that endanger people's mental and even physical health. "Older adults have this special clarity about who they are and what they want, which is incredibly inspiring," says Dr. Julia Hiner, explaining, in part, why she loves her work as a geriatrician in Houston, Texas. She also enjoys the challenge of the medical complexity these patients present and the opportunity it creates to see the patient as a whole person. Get to know the hands, head and heart behind Lyn Hiner, an accomplished fine artist whose pallet knife paintings of florals and automobiles have been featured and collected all over the world, from Beverly Hills, California, to Japan and Saudi Arabia. After studying art at the Pratt Institute in New York, Lyn pursued a career in business accepting a job as tradeshow manager for Performance Racing Industry (PRI), where she worked for nearly a decade. Unfortunately, a horrific and unrelated accident found Lyn in the hospital where she underwent days of surgeries and a year of occupational therapy. It was at this time where she dove back into her passion for art and used it as a form of healing. Eventually, she birthed the ideas for her signature collections, now highly sought after, called “petals” and “petrol”. Today, Lyn, with her husband and teammate, Rob, travel the world showcasing her oil paintings at a multitude of distinguished art and car events. Among her impressive list of accolades, Lyn is also a loving mother of two grown daughters, one of whom is employed by Lyn Hiner Studios. Together, they have opened an appointment-only studio in San Clemente, California, as well as launched an international collaboration officially called, Lyn Hiner Studios, Japan. Does the emotion someone experiences immediately preceding an episode of nonsuicidal self-injury (NSSI) influence how painful that episode of self-harm feels? How do you go about starting a relationship to find a team dentist? -A lot of AT's have nobody.  -Find someone who is available to come after hours and loves it. -Can cold call offices or Google search.  -looking for sports dentistry -Ask other ATs if they know a dentist since the community is small.  What resources are available to the AT with dental emergencies, especially after hours? -Cell phone number of the dentist -Tooth SOS app. How do you know who to refer to for dental injuries? -General dentist first, then general dentist can refer to the proper personnel.  Special consideration for athletes with braces? -If you have braces, ligaments are already loose and can put you at risk for trauma/injury. -Mouth guard. -Mouth guards specifically for braces. And because teeth move they might have to get mouth guards frequently.  -Custom has more compliance than boil and bite.  -Can get a custom mouthguard to go over Invisalign.  -See more soft tissue trauma How have patients felt with mouth guard around braces? -Tough sell because nobody wants to wear a mouthguard at that age.  What would you like the AT to do to help with the referral process? -Send pictures. -Backstory is nice to know -Timing -Don't need past dental records Team dentist be utilized for PPE's? -Have the athletes come in groups. -Visual exam  -Oral cancer screening -cavities/wisdom teeth -Get scans for mouthpieces.  -Able for the athletes to ask random questions -Establish a dental home, especially for college students who move away from home.  -Dentist can give a card Fun stories -Mowing lawn and trying to tie a string and it took the tooth.  The little girl had her silver cap come off and there was a Jolly Rancher glued to it.  -Kids are very resilient and heal a lot quicker than adults.  Action Item: What should an AT have in their bag? -gloves, gauze, light -Guidelines on what to do when it happens.  Cheat Sheet -Cell phone number of the dentist,  - Tooth SOS app - Save a tooth solution - Spit in a cup - Do not put the tooth in water
