Neil Ratner, MD, has played remarkably divergent roles in his life, initially as a rock-and-roll drummer and staging entrepreneur, working with the likes of Edgar Winter and Emerson, Lake & Palmer. But he always had an interest in medicine, so eventually he ditched his successful rock career and went to medical school, later becoming an anesthesiologist who embraced the then-novel concept of delivering non-OR anesthesia, which he helped pioneer.  It was through his work as an anesthesiologist for a plastic surgeon that he ended up meeting Michael Jackson, of all people, who immediately liked Dr. Ratner and his rock-and-roll roots.  His rock music and medicine careers enjoyed some precipitous highs but also significant lows, all of which he candidly details in his book “Rock Doc.”Perhaps surprisingly, however, our interview with Dr. Ratner is mostly not about his rock-and-roll past or his work doing non-OR anesthesiology, but instead about what he is up to today: serving as a kind of public health announcer for a radio station in, fittingly, Woodstock, N.Y. His work interpreting medical news—particularly as it relates to COVID-19—for the station (WDST; 100.1 MHz) has made a difference in the public's understanding of the complicated issues brought about by the pandemic.

When you think guitar gods, names like Hendrix, Frehley, Van Halen, Clapton, Iommi all come to mind, but one name in a very limited window of time will always stick out and that man was named, Randy Rhoads. In just a 5 year window, Randy Rhoads began his journey to guitar god with Quiet Riot, leading him to join Ozzy Osbourne on his first solo venture with Blizzard of Ozz. Sadly, at 25 years old, Rhoads lost his life in a tragic plane crash that left the entire music industry numb. Now, 40 years since his passing, director Andre Relis and rock legend, Tracii Guns (who narrates the documentary) revisit the life and times of Randy Rhoads, a musical prodigy from Southern California that within a few months was better than his own music teacher. In less than 90 minutes, Relis humanizes Rhoads who has been deified in the metal community and continues to live on in our hearts, minds and souls. If you can get your hands on an early release of Blizzard of Ozz or the first two Quiet Riot albums, you can hear the intricacies of Rhoads' musicality within each song. During our interview, I told Guns that I was terrified when initially finding out that he was the narrator, because although he is an incredible musician and vocalist, that does not always translate to voice over, but to my delight and everyone else's Tracii Guns knocked it out of the park and just like any L.A. After forming Quiet Riot with bandmates Kelly Garni, Drew Forsyth, and Kevin DuBrow in the 70s, the band quickly became one of the most popular acts in the Los Angeles club circuit. However, their music failed to land them a legitimate record deal. After playing with Quiet Riot for over 10 years, Randy received an opportunity to be the lead guitarist for former Black Sabbath singer Ozzy Osbourne. Randy played with Ozzy for a few short years, recording legendary songs such as Crazy Train, which includes Randy's groundbreaking guitar solo. After touring with Ozzy in the early 80s, Randy was tragically killed in a plane crash on March 19, 1982 at the young age of 25. His body died that day, but his soul and music live on forever. One of the differences between physicians and nurse practitioners is that physicians are regulated by a state Board of Medicine, while in most states, nurse practitioners are regulated by a state Board of Nursing.  This distinction is important, because while nurse practitioners are functioning in the same capacity as physicians—ordering and interpreting tests, making medical diagnoses, and writing prescriptions for medications—they are overseen by a regulatory body that includes nurses without experience in this type of practice, which can result in a lack of appropriate action when a nurse practitioner is practicing improperly.  Today we are going to discuss the case of nurse practitioner Jeffrey Young, the so-called, “Rock Doc,” who was indicted in 2019 for prescribing controlled substances to his patients “to obtain money, notoriety, and sexual favors.” Despite multiple reports to the Tennessee board of nursing, outside agencies took five years and action before Young finally lost his prescribing privileges. The patients of the nurse practitioner and aspiring reality star Jeffrey Young say he helped them like nobody else could. Federal prosecutors who charged him in a massive opioid bust say he overprescribed painkillers, often for “money, notoriety, and sexual favors.”  Young’s case provides a rare glimpse into the ways patients wind up addicted to the powerful painkillers fueling the national opioid epidemic. Branding himself “the Rock Doc” in a self-produced reality-TV pilot, Young would wear band T-shirts and blast music as he met with patients; he sometimes broadcast appointments and medical procedures on the live-streaming app Periscope. Off camera, Young allegedly prescribed 1.4 million addictive pills and had sex with female patients. Young was indicted on drug-trafficking charges in April 2019. He pleaded not guilty to the charges, and is currently in jail awaiting trial. “I had a lot of ‘Why on earth?’ questions,” the Atlantic reporter Olga Khazan says. “‘Why would he do this? Why would you go to this doctor? Good morning, RVA! It’s 26 °F, and today you can expect highs in the low 40s and some sunshine. Soak it in, because we’re under a Winter Storm Warning from 1:00 AM Thursday through 7:00 AM Friday. The National Weather Service at Wakefield says to expect a couple inches of snow followed by even more ice, and, yikes, “widespread power outages, significant tree damage, and difficult travel conditions.” Gather your supplies and loved ones and make sure all of your devices are charged!Water coolerAs of this morning, the Virginia Department of Health reports 1,770 new positive cases of the coronavirus in the Commonwealthand 21 new deaths as a result of the virus. VDH reports 251 new cases in and around Richmond (Chesterfield: 129, Henrico: 81, and Richmond: 41). Since this pandemic began, 722 people have died in the Richmond region. The number of new deaths reported each day has finally started to mirror the precipitous drop we’ve seen in cases over the last couple of weeks. The seven day average of new deaths, 19.9, fell below 20 for the first time since December 4th. Since that date, COVID-19 killed 2,877 people in Virginia. That’s shocking to me, and I had to triple-check my math to make sure it was right.In vaccine news, yesterday morning the Virginia Department of Health launched a statewide COVID-19 vaccine pre-registration form at vaccinate.virginia.gov. This replaces the interest forms you may have already filled out on your local health district website, and, if you already filled out one of those local interest forms, you do not need to fill out this new statewide form. In fact, you should have gotten an email last night (or maybe will get one shortly?) confirming that you’re in the system. You can also verify that you’re in the system by searching for yourself or a loved one by email or phone number on this page. One note: email search (at this moment) is case sensitive so pay attention if your browser autocapitalizes the first letter of your email address! I think, all things considered, the public rollout of this new form—at least from the technology side of things—went pretty well. Remember! This is NOT registering for a vaccine, this is getting in the system! Local health districts will then slice, dice, sort, and prioritize these massive lists of interested humans and work to get them all vaccinated as quickly as possible given the (extremely) limited supply of vaccine.So the technology parts went pretty OK, but from the accessibility side of things, Sabrina Moreno at the Richmond Times-Dispatch reports that VDH is still using Google Translate to provide a Spanish version of this new form: “Until about 11 a.m., three hours after the centralized portal allowing Virginians to add their names to a long waiting list went live, there was no Spanish translation…When a Spanish version of the form was added, it was powered by Google Translate and returned a decipherable but faulty product.” This is, to say the least, suboptimal: “On Tuesday, the registration system’s using Google Translate meant ‘your first name’ was translated to ‘nombre de pila.’ The term is rarely used on Spanish forms, and refers to the name given to children upon baptism, or on the ‘baptismal font.’” This should all sound super familiar, because literally one month ago, on January 17th, Moreno reported that VDH had used Google Translate on one of their vaccine FAQs and translated “the vaccine is not required” to “the vaccine is not necessary.”Governor Ralph Northam will hold a press conference today at 2:00 PM, presumably to officially announce the new vaccine pre-registration system and maybe the statewide call center that’s he’s mentioned a couple of times. One question I do have, that I’m sure someone out there knows the answer to, is: Are the additional cost for this new calendar included in the budget that the Board just passed?Wyatt Gordon at the Virginia Mercury has a piece about Virginia’s manufactured housing aka mobile homes or trailer homes. This quote from Jonathan Knopf, the ED of the Manufacture Home Community Coalition of Virginia, is fascinating: “[Manufactured homes] are the largest source of unsubsidized affordable housing in the country, but it’s the one that we know the least about.” I know I don’t spend a lot of time thinking about manufactured home communities, and that’s part of the problem, but these neighborhoods are often affordable places for folks to live. Plus they are naturally pretty dense—at least way denser than a couple single-family homes plopped down on the same acreage. Related, the RTD’s Michael Paul Williams has a column up about affordable housingthat includes this excellent quote from Laura Lafayette, the CEO of the Richmond Association of Realtors: “To be blunt, I think we need to have a long, hard, honest conversation about NIMBYism and the ability of neighbors to stop affordable housing developments in their tracks.”Today, City Councilmembers have the first day of their two-day virtual retreat. I don’t know why it takes place this deep into the year, but I’m sure there’s a good reason. Neil learned the music business working as a tour manager, assistant manager and producer for groups like Edgar Winter, Emerson Lake and Palmer, Pink Floyd and T-Rex, among others. After a number of successful years working in the music biz, he had an epiphany and decided to pursue his other childhood dream of becoming a doctor. He finished college, learned Spanish, spent four years at a medical school in Mexico, completed two years of surgical training and an anesthesia residency, and helped create the new specialty of office-based anesthesia in Manhattan. In doing so, he became an expert in the use of the new drug, Propofol. Although he encountered many celebrities in his anesthesia practice, one would change his life... Michael Jackson. One day Michael walked into an office in which Neil was the Director of Anesthesia. He became a trusted friend and personal physician of Michael's from 1994 to 2002, periodically going on tour and spending time with Michael at Neverland. He recently wrote the book called Rock Doc which talks about Rock & Roll, Michael Jackson, Nelson Mandela, a prison sentence with very unexpected results and how helping the poorest of the poor became a key part of his life.
