Denne uken tar vi for oss noe som 30 % av Norges befolkning lider av: kroniske smerter.  Og en av disse er faktisk Rita. Hun har levd med dette i snart 30 år og her kommer en episode som er ble litt mer personlig enn planlagt..  Men det er mye læring å hente i andres historier, og det er håp å finne gjennom andres tilheling.  Rita ble inspirert av dokumentaren This Might Hurt, og boken "The way out" av Alan Gordon, som fokuserer på hvordan smerte kan ha psykologiske komponenter, og hvordan ulike teknikker kan hjelpe oss å “avlære” smerte. Link til studie rundt PRT.  Appen vi snakker om heter "Curable"  Det viser seg at når en skade er fysisk leget, og smerten fortsetter eller forverres, kan det noen ganger skyldes at hjernen har "lært" smerten. Dette er kjent som "sentral sensibilisering," hvor nervesystemet overreagerer på smerte, selv om den opprinnelige årsaken er borte. Det er som om hjernen har blitt overfølsom for signaler og fortsetter å sende smertesignaler, selv når de ikke lenger er nødvendige. Ikke minst er det viktig å forstå smerte-frykt-syklusen. Når smerter fører til frykt for å forverre tilstanden, unngår vi gjerne visse aktiviteter eller bevegelser. Dette kan skape spenninger i kroppen, som igjen forsterker smerten. Over tid kan denne syklusen forverres - og en negativ forventning vil også kunne gjøre dette mer kompleks. Om du lider av kroniske smerter - kan dette være en god ide å se nærmere på  Pain Reprocessing Therapy (PRT)  og her er noen steg fra boken: Få en nøyaktig diagnose: Feildiagnoser kan føre til mer smerte og misforståelser. Det er viktig å utelukke strukturelle årsaker som kreft eller brudd, før man vurderer om smerten er hjernegenerert. Forstå at kronisk smerte kan være reversibel: Nyere forskning viser at kronisk smerte kan være en lært tilstand, skapt av hjernen. Ved å trene hjernen kan man ofte redusere eller eliminere smerte. Deaktiver smerte-frykt-syklus: Når smerte skaper frykt, forsterker det smerten. Teknikker som somatisk sporing og mindfulness kan bidra til å bryte denne syklusen. Gjennomfør emosjonell bearbeiding: Smerte kan ofte være en erstatning for underliggende følelser som skyld, sorg eller sinne. Å bearbeide disse følelsene kan redusere smerten. Gjør nødvendige livsendringer: For å fremme helbredelse kan det være nødvendig å gjøre livsstilsendringer som reduserer stress og fremmer mental og fysisk helse. Økt frykt setter nervesystemet i alarmberedskap, slik at det skal mindre til for at et stimuli oppleves som smertefullt. Hvis du i stedet for å unngå smerter/smertefulle situasjoner, kan eksponere deg for dette i kontrollerte former, så vil du kunne øke smerteterskelen og øke toleransegrensen din.  Og til slutt - aldri gi deg!   Livet skal leves ikke bare gjennomføres...

Dr. Howard Schubiner is board-certified in pediatrics, adolescent medicine, and internal medicine. He was a professor at Wayne State University for 18 years, worked in Ascension Providence Hospital for 20 years, and currently works at Cormendi Health in Chicago, IL. He is a clinical professor in the department of internal medicine at Michigan State University - College of Human Medicine. He has authored over 100 publications in scientific journals and books, given over 500 lectures to scientific audiences regionally, nationally, and internationally, and consulted for the American Medical Association, the National Institute on Drug Abuse, and the National Institute on Mental Health. He is the author of Unlearn Your Pain and Unlearn Your Anxiety and Depression, the co-author of Hidden From View with Dr. Allan Abbass, and the co-editor of two textbooks on psychophysiologic disorders. As you will hear in the interview, I discovered Dr. Schubiner's work at a pivotal time in my life and his book supported my journey tremendously. Dr. Schubiner and colleagues developed Pain Reprocessing Therapy and Emotional Awareness and Expression Therapy, chronic pain treatments that focus around curing pain rather than managing pain. In fact, I did an episode on lowering pain signals through calming the brain and nervous system back in 2019 but I've been on a continuously evolving research journey since. The film follows three chronic pain patients who have spent years searching for answers. Desperate for relief, they enter a new medical program — run by Dr. Howard Schubiner — that focuses on uncovering hidden causes of pain, and retraining their brains to switch the pain off. Kent and Marion spent over five years filming and editing this feature documentary in their off time between paid film & TV projects. Since it was challenging to track down funding, they invested their own free time and savings into the film on nights and weekends, with the help of a few key supporters. We talk about their film that addresses the fact that sometimes pain can have emotional causes and how you can take the steps to heal and eliminate pain. “There's no such thing as negative emotions. Emotions are there to help, protect, and alert us.” - Dr. Howard Schubiner “Roughly 30 - 40% of people who come to see a doctor do not have a structural problem for which their symptoms are attributable to. Much of the drive to make This Might Hurt came from his own struggle with pain as a 22-year-old. Unable to type or even turn a doorknob, he was forced to drop out of college. Although he saw several doctors and tried physical therapy, strength training, and was put on opioids—his pain grew steadily worse. On this edition of Crushing Doubt's interview series, I got to sit down with Kent Bassett and Marion Cunningham, the directors and producers of the mind body film This Might Hurt, which features Dr. Howard Schubiner in his work with a particular group of pain sufferers. Kent and Marion put so much hard work and financial investment into this wonderful film, and I celebrated what an achievement this was and how many lives it can alter for the better. This Might Hurt was inspired by Kent's own mind body experience, in which he developed arm pain that crippled him to the point of having to drop out of Swarthmore College. It took a year before someone mentioned Dr. John Sarno's books to him and, like so many before him, he was shocked to achieve a book cure to his chronic pain. With his film career under way, as he states in the interview, he felt a strong need to tell the story, in some way, of how people can use new information to make changes to their physical experience. Marion got connected with Kent and has found a very powerful mind body journey of her own, which opened up her life in ways she hadn't expected either. Come find out how Kent and Marion made their choices in how to tell the story of the nine sufferers in this particular group of Dr. Schubiner's practice as it unfolded and their take on what they saw during those weeks of clinical work, from powerful emotional awakenings, changes in how to live, and the role of doubt in each of the sufferers. Director Kent Bassett is an Emmy-nominated editor and filmmaker from Arizona. He's edited a number of feature documentaries that have premiered at Tribeca Film Festival, AFI Docs, Doc NYC, and the PBS World Channel. His most recent film as editor is Not Going Quietly (Tribeca), about 32-year-old father, Ady Barkan, who after being diagnosed with ALS travels the country to fight for universal health care.Much of the drive to make This Might Hurt came from Kent's own struggle with debilitating arm pain as a 22-year-old. Unable to type on a computer or even turn a doorknob, he had to drop out of college. Although he saw several doctors and tried physical therapy, strength training, even narcotics—the pain grew steadily worse.
