What if your pain, irritation, or sexual discomfort was being dismissed because no one ever taught your doctor how to look for the real cause? There's a reason vulvar conditions like lichen sclerosus are often missed or misdiagnosed—and it's not because they're rare. It's because most doctors aren't trained to recognize the signs. In this episode, I'm joined by Dr. Jill Krapf, a dedicated gynecologist and vulvar skin expert, to unpack the truth about vulvar dermatoses, why it takes so long to get diagnosed, and what can be done when scarring, pain, and sexual dysfunction start taking over your life.We don't just skim the surface—we go deep into the subtle (and not-so-subtle) symptoms, the stories of people who were told nothing was wrong when something clearly was, and what really works when managing chronic vulvar skin conditions. Jill also shares insights about how women of color are even more likely to be overlooked and misdiagnosed because of gaps in training and representation in medical education. If you've ever been told “it's just in your head,” or if you've silently dealt with vulvar discomfort, pain with sex, or itching that won't go away—this episode is a must. There is real hope and effective care available. You just need to know where to look—and what to ask.
Tim Tobin, CEO and Founder of Planatome, has a background in the semiconductor industry where precision polishing techniques were developed. Planatome has used that technology to create molecularly smooth scalpel blades that remove all the jagged edges on traditional surgical scalpels. These new nano polished blades significantly reduce scarring and inflammation from incisions and improve healing outcomes for patients, especially those with darker skin tones who are more prone to hypertrophic scarring. The challenge is disrupting an industry that has not changed in over 100 years and that has been driven by reducing the costs, not improving the blade. Tim explains, "So we picked the surgical scalpel because it's still the foundation, the primary incision tool, whether it's a minimally invasive procedure or a procedure with a lot of cutting. So we started with that. We've since applied our technology to many other surgical instruments. We started with the scalpel, which was patented in 1915. There's been no change, no technological change, just change to drag down the cost. So it's just completely commoditized, a scalpel blade somewhere in the 20 to 50 cent range, and nobody thinks about it because everybody just uses a scalpel, and they don't need to be more critical." "So we started looking, and if you look at a scalpel blade underneath magnification, it looks almost like it's highly serrated and jagged because they're made by taking a piece of stainless steel, and they grind it up to a point, and that's the cutting edge. But they leave it like that." "We took that, and we said, okay, well, what if we polish it and take out all of those jagged serrations, at least a thousand times smoother. Now if you measure the surface, it changes the cutting mechanism from tearing and snagging to a nice clean incision. So some of the challenges in that is when surgeons have been using the same thing pretty much their whole career, when they try something different, that's not always something they want to think about." #Planatome #MedTech #MedicalDevice #Surgery planatome.com Download the transcript here

In this episode, Dr. Carol McMakin and Kim Pittis discuss Frequency Specific Microcurrent (FSM) protocols for treating nerve pain, including post-herpetic neuralgia and ataxia. They explain the mechanisms behind nerve inflammation, polarization, and scarring, and shares case studies demonstrating effective FSM treatments. The conversation covers topics such as treating the psoas muscle, addressing ticklishness, and managing complex neurological cases. Dr. McMakin also provides insights on using FSM for patients with cancer and emphasizes the importance of comprehensive assessments in identifying the root causes of pain. 01:13 Understanding Voltage-Gated Ion Channels 02:42 FSM and Nerve Pain Treatment 05:59 Case Study: Cavernous Hemangioma 07:42 Nerve Pain Treatment Insights 12:03 Treating the psoas Muscle 17:04 Scarring and Tissue Treatment 18:14 Case Study: Knee Pain Post-Surgery 23:14 Holistic Approach to Pain Treatment 28:12 Addressing Ticklishness in Treatment 30:23 Nerve Inflammation and Pain Management 34:18 Case Study: Ataxic Gait and Neurological Concerns 42:02 Chronic Conditions and Treatment Approaches 47:08 Treating Cancer Patients: Guidelines and Considerations 50:04 Upcoming Courses and Final Thoughts As medical practitioners, it's crucial to constantly refine our approaches to treating conditions like nerve pain, ataxia, and other related dysfunctions. In a recent episode of the Frequency Specific Microcurrent podcast, various experts, including Dr. Carol and Kim Pittis, shared valuable insights that can benefit practitioners seeking to enhance their understanding of nerve-related treatments. Understanding Nerve Pain and Treatment Modalities Dr. Carol emphasizes the complexities of nerve pain treatment, explaining how conditions like postherpetic neuralgia and traction injuries can influence nerve function. Practitioners are encouraged to consider the underlying causes of nerve numbness or hypersensitivity. Dr. Carol suggests the combination of polarizing frequencies and specific microcurrent protocols to address these issues effectively. One primary focus is on using combinations such as 40 Hz and 396 Hz to alleviate inflammation and enhance nerve conductivity. Complementing this approach, Dr. Carol underscores the importance of addressing the surrounding scar tissue, often by applying frequencies such as 13 Hz to reduce adhesions and further alleviate nerve discomfort. Techniques for Addressing Complex Cases Throughout the podcast, both Dr. Carol and Kim Pittis share personal case studies to illustrate treatment strategies. For instance, Dr. Carol describes a particular case wherein a patient developed knee pain post-tummy tuck procedure. Here, the importance of a holistic approach is evident. Realigning the patella and addressing underlying scar tissues from the surgery was pivotal. Additionally, careful inquiry into the patient's history—identifying past surgical or traumatic events—can help in piecing together effective treatment plans. Addressing Ataxia and the Importance of Neurological Assessments In discussing ataxia—an often misunderstood and misdiagnosed condition—Dr. Carol stresses the potential need for thorough assessments beyond standard examinations. She shares anecdotal experiences highlighting that an ataxic gait may stem from spinal, rather than purely cerebral, origins. This revelation points medical practitioners toward the need for detailed diagnostics, including MRI assessments of both the brain and spinal regions. Promoting Safe and Effective Treatment Environments Kim Pittis touches on the critical aspect of patient safety and comfort. Ensuring the nervous system perceives treatment interventions as safe is paramount for achieving lasting results. Furthermore, practitioners are encouraged to adopt a patient-centered approach, considering the individual experiences of pain and discomfort and adjusting treatment protocols accordingly.

In this bonus episode, Dr. Joel Kahn sits down with Dr. Isaac Eliaz, a renowned clinician and researcher based in California, to discuss Galectin-3 and its critical role in heart health. The conversation kicks off with a brief Hebrew exchange between Dr. Kahn and Dr. Eliaz, who hails from Israel, before diving into the science behind Galectin-3. Galectin-3 is a regulatory protein heavily involved in chronic inflammation and has been linked to the development of numerous conditions, including heart disease. Elevated levels of Galectin-3 are often observed in both acute and chronic heart failure, playing a pivotal role in the inflammatory pathways that lead to myocardial tissue remodeling after injury. At the Kahn Center, Galectin-3 blood levels are routinely measured as part of our comprehensive approach to heart health. Tune in to learn more about how this protein impacts the heart and the latest insights into managing inflammation to protect cardiovascular health.
rWotD Episode 2792: Myocardial scarring Welcome to Random Wiki of the Day, your journey through Wikipedia’s vast and varied content, one random article at a time. The random article for Wednesday, 25 December 2024 is Myocardial scarring.Myocardial scarring is the accumulation of fibrous tissue resulting after some form of trauma to the cardiac tissue. Fibrosis is the formation of excess tissue in replacement of necrotic or extensively damaged tissue. Fibrosis in the heart is often hard to detect because fibromas, scar tissue or small tumors formed in one cell line, are often formed. Because they are so small, they can be hard to detect by methods such as magnetic resonance imaging. A cell line is a path of fibrosis that follow only a line of cells. This recording reflects the Wikipedia text as of 00:45 UTC on Wednesday, 25 December 2024.For the full current version of the article, see Myocardial scarring on Wikipedia. This podcast uses content from Wikipedia under the Creative Commons Attribution-ShareAlike License.Visit our archives at wikioftheday.com and subscribe to stay updated on new episodes.Follow us on Mastodon at @wikioftheday@masto.ai.Also check out Curmudgeon's Corner, a current events podcast.Until next time, I'm generative Amy.

In this episode, Bahar Etminan and Danielle Renee from Bobbie Charles Clinics, explore the complexities of melasma and hyperpigmentation, discussing their differences, triggers, and effective treatments. They share personal insights into various solutions, particularly the Cosmelan treatment, which is highlighted as a leading option for pigmentation issues. The conversation covers the importance of understanding skin types and potential risks associated with treatments such as prescription hydroquinone. It also delves into the role of ingredients in skin health, the cost and accessibility of treatments, and the importance of understanding contraindications. Danielle and Bahar discuss combining treatments for optimal results, the evolving role of epigenetics in skin wellness, and the impact of sun exposure. Key Takeaways Melasma is often misunderstood and challenging to treat. Hyperpigmentation refers to increased melanin in specific areas, with forms such as sun-induced and post-inflammatory hyperpigmentation. Melasma penetrates deeper into the skin, making it harder to treat, and can be triggered by hormonal changes and genetics. Cosmelan is an effective treatment for pigmentation, containing ingredients like Arbutin and Kojic acid. The skin retains memory of past treatments, influencing its future responses. Proper aftercare is essential for maintaining treatment results. Understanding individual skin types is crucial for effective treatment. The cost of Cosmolan treatments has risen over the years. Pregnant or breastfeeding individuals should avoid Cosmelan treatments. Combining Cosmolan with LED therapy enhances results. The skincare industry is moving towards less invasive treatments. Education on sun exposure is vital for skin health. Thorough consultations are essential for personalised skincare. Perimenopause significantly affects skin health and appearance. Cosmelan is effective for pigmentation but not primarily for scarring. Innovative treatments for intimate areas are emerging in skincare. Is there a right surgical approach that completely transforms the lives of patients with myocardial bridges? This episode is a live recording of the first ever myocardial bridge meetup where Dr. Theo Kofidis, took the time to virtually join us from Greece for the first session of the day. We learned of his ambitious project of revamping a JCI accredited hospital into a cutting-edge medical facility. He takes questions from the group including attendee Jane's prolonged struggle with myocardial bridge symptoms and provides suggestions and support from his perspective. He shares insights into the significance of timely intervention and the crucial consideration of surgical options when medication fails. Questions from the group included best practices for unroofing a myocardial bridge. Dr. Kofidis outlines the meticulous surgical techniques that ensure optimal outcomes in his opinion. We dig deeper into the importance of global collaboration among healthcare professionals to enhance treatment efficacy and work toward identifying long term, symptom free patients to better support proper procedures for "unroofing". We spoke of advanced diagnostic tools like 3D reconstruction and high-frequency ultrasound that aid in post-operative evaluation, and heard why minimally invasive endoscopic approaches, complemented by tactile feedback, offer significant benefits. The group got expert advice on achieving comprehensive and precise surgical results for myocardial bridges that we're now sharing with anyone interested. Finally, we ventured into the realm of advanced diagnostics and the role of AI in cardiology. Dr. Kofidis discussed the underdiagnosed prevalence of myocardial bridges and their connection to coronary artery disease and coronary artery spasms. We conclude with an engaging discussion on medical tourism in Greece, highlighting its potential advantages and the robust support system within the medical community. Dr. Kofidis, as always, offers his expert advice and invites any concerned or interested party to reach out for a discovery call to see if Greece might be an option for you. In this episode, I discuss skin health appearance and why both are important indicators of the health status of your immune system, gut microbiome, and other organ systems. I explain why sunlight is essential for skin and hormone health and how excessive sunlight can accelerate skin aging and cause certain skin cancers. I discuss the different types of sunscreens (physical, chemical, and mineral-based) and potential health concerns of the chemicals found in some (but not all) sunscreens. I also discuss the importance of getting your skin (and not just moles) checked for pre-cancerous and cancer growths, the role of nutrition and lifestyle factors that improve skin health and appearance, and how to improve your skin by reducing local and systemic inflammation and supporting your microbiome. I explain what works to improve your skin's youthfulness and appearance, including reducing wrinkles, sagging, and pore size. I review the data on ingesting (or topically applied) collagen, vitamin C, niacinamide, hyaluronic acid, and retinol, and what is known about the use of peptides (e.g., BPC-157, copper peptides) and red and far-red light phototherapies for improving skin health and appearance. I also discuss the causes of acne, rosacea, and psoriasis and explain nutritional, skin care, and prescription-based approaches to treating these common skin conditions. This episode ought to help everyone better understand the biology of the skin and help them make the best possible decisions for their skin health, care, and appearance according to age, goals, and current skin conditions. Supporting hair growth through chemical burns, scarring alopecia and menopause. Angela Fields is from Florida. She first experienced issues with her hair when she suffered from chemical burns, following the use of relaxer to straighten her hair. She then began to experience thinning in perimenopause, and discovered that she had a deficiency in Vitamin D. After these issues were resolved, she then developed a scarring condition, central centrifugal cicatricial alopecia, known as CCCA, and has had to undergo scalp injections and biopsies to help to identify and treat this . Hair health at this point became extremely important to Angela, but, as she has an allergy to fragrances, she found that she was unable to use 96- 98% of haircare on the market that contains fragrance. She decided to make something for herself in her kitchen, but after being laid off from her job in the corporate world she was pushed to take the product range to market. She now works to help & support those with hair health concerns, particularly leading up to the menopause, and has created a supportive community where she offers education and advice. Welcome back to another episode of the Evidence-Based Hair Podcast. This week, we continue our June theme of JAK inhibitors, focusing on the use of topical JAK inhibitors for treating frontal fibrosing alopecia (FFA) and lichen planopilaris (LPP). In this episode, we delve into a significant study by Dr. Maryanne Senna and colleagues from Boston, exploring the effectiveness and side effects of topical 2% tofacitinib for LPP and FFA. The study, involving 41 patients, provides valuable statistics on improvement and stabilization rates, offering practical insights for clinical application. We discuss the potential of topical JAK inhibitors, given their less systemic absorption compared to oral forms, and their specific relevance to scarring alopecia where inflammation is closer to the skin's surface. Despite some challenges with compounded topical formulations, the study's promising results highlight a third of patients showing improvement and over 60% achieving disease stabilization. Join us as we wrap up our June series on JAK inhibitors and prepare for July, where we'll focus on the antiandrogen spironolactone and its use in androgenetic hair loss in women. Thank you for tuning in, and I look forward to our next discussion on the Evidence-Based Hair Podcast. STUDY REFERENCED Li-Chi Chen, Chino Ogbutor, Kristen J Kelley, Maryanne M Senna.
