Is it possible to be drunk without drinking alcohol? Actually, yes. There is a condition called auto-brewery syndrome whereby an imbalance in your gut microbiome can ferment carbohydrates to create alcohol. Blood alcohol levels can get surprisingly high. Though this condition is thought to be rare, it might just be underdiagnosed. Patients are dealing with legal ramifications, social stigmas, and disbelief by friends, family, and physicians. Today we introduce the topic of auto-brewery syndrome to raise awareness that your gut microbiome can produce alcohol. We discuss how it occurs, how to diagnose it, and some strategies to treat it. What to do when different folate markers don’t line up | Masterjohn Q&A Files #220

Mastering Nutrition

Play Episode Listen Later Mar 16, 2021 5:04

Question: What to do when different folate markers don’t line upYou know, if you have high figlu on the NutrEval, which is Formiminoglutamic acid, which rises when there is Tetrahydrofolate THF, unmethylated folate, to metabolize figlu, and in that case, I would be thinking about maybe you have a B12 deficiency that is leading to the trapping of folate as the methylfolate form, so that THF isn't regenerated for that reaction. So I would absolutely never use figlu as the only marker of folate. I would look at serum folate and red blood cell folate always as the first markers of folate. So in this case, I would look at your serum folate and your red blood cell folate. So before I would get folate injections, I would be looking at all those markers to see whether there's a coherent story between all of them that are agreeing that your folate status is low despite supplements, versus a divergence story between them that is telling me that one form of folate is the one that's missing rather than a total folate. Most people believe they eat healthy and make smart eating decisions, but the truth is - you can eat well-balanced meals and still be malnourished. So, what are the factors that contribute to our overall nutrition status? How do we ensure we achieve optimal nutrition and nutrient absorption when there are so many factors that affect our overall nutrition status? Today, we discuss the wide gamut of nutrition and how the food we eat fuels our cells, providing us with the energy we need to tackle the day. We discuss the connection between your nutrition and the gastrointestinal issues you may experience with poor nutrition and malabsorption. We discuss the role that the microbiome plays in your nutritional health and your body’s ability to absorb nutritional supplements. We discuss the difference between nutritional deficiencies and nutrition inadequacies and why it’s important to make the distinction. We also discuss why we believe most people have room to improve their nutritional status.   “The microbiome plays a BIG role in your nutritional status.” - Michael Chapman   This week on The Lab Report Podcast: The connection between your GI health and your nutrition status… and vice versa How the microbiome impacts your nutrition and ability to absorb vitamins, nutrients, and nutritional supplements The distinction between nutritional deficiencies and nutritional inadequacies Factors to consider to optimize your nutrition How conventional medicine takes nutrition into consideration compared to functional medicine Motivational issues deterring people from improving their nutritional health Nutritional risks involved in various diet plans Functional medicine assessments to help practitioners get to the root cause of nutrition inadequacies The best way to determine the status of nutrients   Question of the Day: Is the NutrEval test an intracellular test or an extracellular test and what is the difference?   Resources Mentioned: Methylmalonic Acid, Quantitative, Serum - MayoClinic citation   Subscribe, Rate & Review The Lab Report Thanks for tuning into this week’s episode of The Lab Report, presented by Genova Diagnostics, with your hosts, Michael Chapman and Patti Devers. So, it turns out that nutrition - like many other things - has a long, complex history and there are many things that contribute to optimal nutritional health. So many, in fact, that we forgot to mention some very important factors in our previous episode. Achieving optimal nutrition is more than eating the right foods, but the real question is - how should practitioners assess the nutritional levels in their patients? Today, we explore nutrient assessments and the most common nutrients clinicians and medical practitioners assess in patients. We discuss the challenges associated with assessing certain vitamins, minerals, and nutrients and how the NutrEval test can help clinicians determine which nutrients their patients are not absorbing adequately. Product Research and Development Manager and fellow Genovian, Ashley Gibbons, also joins us to discuss the various methodologies we use to test a patient’s nutritional health.   “By looking at those biochemical pathways, we know that the enzymes that are responsible for maintaining those biochemical processes and we know the co-factors for those enzymes.” - Michael Chapman   This week on The Lab Report: How practitioners should determine whether their patients have adequate nutrition The most common nutrients clinicians and medical practitioners are most concerned about The challenges associated with assessing certain vitamins, minerals and nutrients What exposures and oxidative stressors can help clinicians determine What is the NutrEval test and how it can help clinicians determine nutritional inadequacies and deficiencies The various testing sections included in the NutrEval test The benefit of assessing a patient's genomics How we assess the information collected in the NutrEval test to develop nutrition recommendations The various methods we use in the NutrEval test to test nutritional health   Question of the Day: If a patient is doing a comprehensive nutritional assessment, should they continue taking their nutritional supplements or stop them?   Subscribe, Rate & Review The Lab Report Thanks for tuning into this week’s episode of The Lab Report, presented by Genova Diagnostics, with your hosts, Michael Chapman and Patti Devers. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started! Megan: Hi Dr Cabral, as always, thank you so much for taking the time to answer my question. I wrote in a few weeks back about my 10 year old son and how we healed his leaky gut which in turn eleviated the eye tics he was having. His most recent Nutreval showed the yeast was elevating some again. We are back on the biociden and the biofilm buster. When you answered my first question about why the yeast returned, you explained how we have to get rid of the bad yeast and replace it with the good yeast. Makes total sense. We had stayed away from probiotics that have saccharomyces in them because it always caused his symptoms to flare. At first I thought the flare was because yeast was his highest sensitivity in the IgG. I realize the sensitivity is to the bad yeast but my question now is could the flare be from the saccharomyces (even though they are the good guys) or would the flare simply occur because we were adding the good guys while there were still too many guys which means too much yeast all together? Thank you so much for sharing with us each day, you are such a blessing. –Megan (Continued) Hi Dr Cabral, I just sent in a question yesterday about my 10 year old son and the sacchromyces. I am realizing I may not have posed all of my question clearly so I'm adding on, and I'm so so sorry if this makes it confusing for you. Please feel free to copy and paste for the purpose of your house call. As I mentioned in yesterday's question my sons eye tics used to flare when I would give him probiotics with saccharomyces in them, but what I didn't mention was he was always totally fine with probiotics that didn't include the saccharomyces. His highest sensitivity on the IgG was to yeast. I realize that's the bad yeast and saccharomyces are the good yeast but why do the good cause a flare in symptoms? Is it simply because I added the good before getting rid of the bad? I guess my real question is why would he have been ok on probiotics that didn't include saccharomyces but have a hard time handling the ones that do have them? Is it as simple as too much yeast all together? So maybe when we rid the gut of the bad yeast he won't flare when we add the good? I've avoided the saccharomyces due to the previous flare in symptoms but am now learning that we need to be replenishing the good to keep the bad out! Thank you thank you thank you for sharing all of your incredible knowledge with us. I have the highest respect for you and what you do and can not begin to tell you how much I have learned and how our family's health has changed for the better since we were introduced to you and your incredible podcast. You are a gift to us. Megan PS I again apologize for the mistake and having to submit the same question twice, but I am interested to hear what you have to say about why the saccharomyces in particular cause a problem for him and I left out of the first submission. Eliana: I love your podcast, it is so informative! I wanted to ask you about my workout program split. I typically do this kind of 5 day split: legs, arms, cardio and abs, back and chest, shoulders and cardio. Basically, I lift 4-5 days a week and get my cardio in either on the same day or on alternating days. Is this too much lifting? I have been doing it for awhile now and have enjoyed it, but I was listening to your recommendation about 3 days of resistance training per week. Thank you! Tyler: Hi Dr. Cabral, I know you have a background in body building and personal training and am looking for advice in regards to putting on more muscle. I am a 25 year old former collegiate athlete and am very lean but am looking to put more mass on in the safest way possible. I eat predominately vegan and GF with some wild fish and eggs. I work out 3-4 days a week and do a combo of lifting, swimming, and hatha yoga. I am 6’1” and 182 lbs. I have read on the importance of ingrsting protein shakes before and after workouts as well as BCAAs pre, during, and post workout for anabolic muscle growth. Do you have brands that you recommend for vegan protein (I do your Nutritional Support Powder in smoothies in the morning) and BCAAs? Or are they even necessary? I appreciate your insight. Thanks, Tyler Anthony: Hi Dr. Cabral, What are your thoughts on MRET water activators? Your products and podcasts have become a very positive part of my life, thank you for sharing your knowledge. Sincerely, Anthony James: Is a kenogentic diet good for you  Zach: What's up Doc, writing in for my 27 y/o girlfriend who has recently been to the "doctor" because she was having allergic reactions (tingling itchy lips, redness around the lips, and breakouts) whenever she ate certain foods. She's tired all the time, never seems to have energy to do anything active, and could lay on the couch all day. We recently (within the last year) went vegetarian, and we limit dairy and eggs as much as possible, but this hasn't seemed to help with the issues she's having. She has had allergy problems (histamine issues) her whole life and found out she was allergic to nickel, as well as many different pollens/grass/trees when she was younger. About a year and a half ago she had a psoriasis "patch" show up on the back of her neck on her hair line, during a stressful period at work. It went away after she was prescribed some cream for it, but it bothers her randomly. Basically she's always had skin issues, and her mother and grandmother have thyroid issues that we now know have been passed to her. Here are the results of her first blood test...only going to give you what was "out of range" as well as her thyroid numbers. T3 uptake - 25.9 T4 - 8.1 TSH - 8.32 BUN - 5 CALC/BUN/Creatine - 7 On a food/skin prick test she was "allergic" to almonds, cherries, hazelnut, soy, and green peppers. I know she needs to do an IgG food sensitivity test, but unfortunately we can't afford that and insurance won't cover it. She cut out those foods, and had her blood work taken about a month and a half later. These were her results the second time: Free T3 - 3.2 Reverse T3 - 14.5 Free T4 - 1.11 TSH - 5.07 Thyroid peroxidase AB - 130 IU/ML Thyroglobulin AB - 6 IU/ML Positive for ANA ANA titter 1:1280 Cholesterol - 212 Calculated LDL - 127 So we know she has hypothyroid and an auto immune disorder. I believe its from some form of intestinal permeability (IBS, Leaky gut, etc) but I also think she has high levels of heavy metals, as she's always putting stuff on her skin/hair whether its make-up, hairspray, or dry shampoo. I've told her she needs to cut out gluten and dairy for sure, but what do you recommend for her as far as the next step in testing, diet, detoxing, and overall lifestyle changes? I just ordered a 21 day detox from your store...I'm going to start her on a 7 day detox whenever that comes in, and hopefully have her doing a 21 day detox in January. If money weren't an issue, after the IgG test I'd have her doing a heavy metal detox, candida/SIBO/yeast protocol, and then start rebuilding her gut. What do you think Doc? Thanks for everything you do. I listen to you daily, and I pass your podcast on to anyone and everyone because theres isn't a person it wouldn't help. If you ever want to take someone under your wing and create the next you, you have my email. ;] But then again, you're already creating more of us like you everyday! Welcome to our first Cabral HouseCall of the weekend! We're getting ready to answer some of our community's amazing questions on wellness, weight loss, and anti-aging. I'm looking forward to sharing those answers with you so let's get started! Scott: Hi Dr. Cabral - I recently accompanied my fiance on her visit to the Wellness Institute. I must say I'm thoroughly impressed with the whole experience, especially with the amount of time and effort you put into each appointment/consultation. My fiance has me hooked on the podcasts! Thank you very much for the knowledge you share with us. With that said, I have some questions of my own, but first, just a little background. I'm a 30 year old male who played at a very high level of soccer up until graduating from school, and since then, still play quite competitively. I've always been prone to injuries and most recently I've been diagnosed with bulging discs in my lower back (L5) as well as in my C3/C4, C4/C5 and stenosis in my C5/C6 - quite a mess right now. Long story short, I've been in a lot of pain over the last two weeks with the bulging pressing on the cervical nerves radiating down my shoulder and into my hand. My questions are which supplements do you recommend I take to help in the healing process? I've read about MSM, Omega 3, and Glucosamine and Chondroitin. I haven't heard of MSM, but read that it's a "miracle drug". Where does one purchase these or other types of supplements? And lastly, I'm hoping you have a recommendation on any exercises (I envision you as the Wikipedia of health - my apologies, I'm asking too much), whether that be an inversion board or some other type of exercise. I've been advised to go see a spinal specialist and will be making appointments today. Thank you again for the podcast service and such a great practice! Regards, Scott   Jen: Hi Dr. Cabral! Thanks for all the great information you've provided in your podcasts. I've learned so much! My question today is related to excessive sweating and body odor when doing so. My teenage stepdaughter is very active in sports. She's muscular and strong, bigger than most girls her age, but 'solid' not flabby. Based on my experience, she sweats more than most grown men and her body odor when sweating is quite pungent. Her clothes smell like a stale dirty gym locker room and must be washed immediately. Her father (my husband) and her sister (my other stepdaughter) do not have this issue at all. It might be something genetic from her biological mom's family, but that's unknown to us. Do you have any suggestions for what might be causing this and how we might help prevent this? Thanks for all the help you provide to your listeners!!   Lisa: Hi Dr. Cabral, I have 2 questions for you that I hope you can help me with. I recently read a book, Eating on the Wild Side, and I loved learning about eating the right foods. In this book they say the skins of fruit and veggies are nutritious......However, now Im reading The Plant Paradox and its about lectins and how harmful they are for us and its in the skins of our fruits and veggies. What is your opinion on lectins? My other question is about the OATS testing. I did this testing, however, I also do the NutrEval testing....can you explain the difference? The OATS test showed no yeast, but the NutrEval showed high yeast. This is confusing for me and would like your input. Thank you so much for everything you do and God bless your family! Lisa   Mariellen: Thanks for everything you do for your listeners. I previously wrote in and ordered the hormone test and had my consult with your health coach and just ordered the DIM Evail and adrenal response complete care. I am on month 2 of the candida protocol and haven't stuck to the diet as well as I should, but have noticed reduced bloating and other benefits. I also take your Daily Activated Multivitamin. I feel like everything and anything gives me headaches.. I have been using the netti pot and when I have my bad headaches it does make me feel a little better, but notice after I use it when I bend over.. water pours of my nose and not sure if it's normal when I use it I can sometimes feel it in my throat. My headaches are always the back of my head, my ears, my sinuses and my eyes always hurt and burn when I get them. They are usually pulsating. I also had trouble swallowing and getting food down. I have been choking here and there. I also have heart burn and do not talk anything for that. Sometimes when I swallow my pills for the candida protocol and my vitamin, the side of my stomach will start throbbing with pain. Sometimes it even feels like it's hard to breathe and I always feel the need to clear my throat. My thyroid is ok I have had that checked out. Any thoughts would help and any natural remedies would be great seeing as I do not want to take anything for acid reflex etc.   Ewa: Hello, thanks dr Cabral for all that you do, so grateful and appreciate you daily! I would like to know what you suggest I do regarding me very intense yellow urine. This has been the case for about 6 weeks. I live mostly a healthy lifestyle, with food exercise and weight under control. I drink plenty of water , Dr Cabral daily support shake f breakfast, and your probiotic. I take 120 mg extra magnesium glycinate daily , can that cause the discoloring, even tho I dont notice any difference the days I forget the magnesium... I cant recall any particular diet change when this all began ... appreciate your opinion on this, thanks a bunch! Ewa (like Eva, but you can spell it out any way you like ;) Im going to the bathroom A LOT, at least every hour and have been doing so even before this color change happened. Thought that should be included in the bigger picture in case that helps as a clue... Thanks a bunch! Ewa

Dr Ann Hathaway, MD has been successfully treating women and men with bioidentical hormones and other natural remedies since 1995. She is a member of the prestigious Institute for Functional Medicine and is a director of the Orthomolecular Health Medicine Board. Tommy and I met Dr Hathaway at the Buck Institute for Research on Aging where she presented this excellent and incredibly well-referenced talk on the role of oestradiol in cognition for women. Dr Hathaway is primarily using blood testing to assess hormone levels. However, urinary metabolites can be very helpful for mapping out the oestrogens. At around the twenty-minute mark, this interview gets quite technical, and I think you'll find it useful to look at this section of a DUTCH report while listening to the audio. Notice the enzyme names are written on the arrows indicating the direction of metabolism. The word "hydroxy" is abbreviated OH, so when you hear Ann say "four hydroxy E1," look for 4-OH-E1 on the map. Here’s the outline of this interview with Dr Ann Hathaway, MD: [00:01:35] Health problems not addressed well by the traditional system. [00:03:13] A 1.5h first appointment in Functional Medicine is typical. [00:04:25] Different types of practitioner. [00:05:20] American Academy of Environmental Medicine. [00:05:38] Jeffrey Bland, PhD. [00:06:57] Buck Institute for Research on Aging. [00:08:21] Principles for addressing hormone imbalance. [00:09:41] Underlying root causes. [00:10:40] Menopause and cognition. [00:11:04] Oestradiol less than 20 pg/ml. [00:13:11] The brain has oestradiol receptors. [00:13:55] All of the neurotransmitter systems are favorably impacted by oestradiol. Acetylcholine, which is the neurotransmitter most associated with memory, serotonin, norepinephrine, dopamine. All are enhanced by oestradiol. [00:14:30] Rasgon, Natalie L., et al. "Prospective randomized trial to assess effects of continuing hormone therapy on cerebral function in postmenopausal women at risk for dementia." PloS one 9.3 (2014): e89095. [00:19:27] Metabolites of oestrogen (see the diagram above). [00:21:13] Some of the things that you can do to increase the 2-hydroxy pathway are eating a high cruciferous diet, taking a supplement called diindolylmethane or indole-3-carbinol. [00:21:32] Iodine sufficiency. [00:21:39] Lignans in flaxseed. [00:22:04] COMT enzyme and methylation. [00:22:47] Genetic mutations. [00:23:55] CYP1B1. [00:24:14] Xenoestrogens.
