Today's podcast is a natural follow up to our podcasts on Slow Codes and Unilateral DNR orders.Today we talk about a new study about how clinicians talk about potentially non-beneficial life-prolonging treatments, published in JAMA Network Open. Do they adhere to society guidelines, which allow as permissible approaches only shared decision-making and following institutional policy. Or do they take alternative approaches, like not offering interventions, not mentioning interventions, or simply stating a plan to limit interventions? Turns out doctors are using these alternative approaches frequently. Our guests are Jason Batten, Liz Dzeng, and Teva Brender, all clinicians, all of whom have been thinking about and wrestling with the ethical reasoning behind these approaches. We all admit to using these approaches. Are the alternative approaches wicked games (song hint), and our response should be to stop these behaviors, beginning with ourselves? After all, if you ask patients or surrogates, they're likely to say they want all the options and may not universally welcome recommendations. Or, as with slow codes, does the fact that these alternative approaches are in common use suggest that the guidelines should be revised? EDITORIAL: Are we starting to overreact to the weather? Now before anyone jumps on the phone — yes, I know what's happened up north. Yes, I know what happened with the cyclone and it caused real damage. Flooding across parts of Northland, the Coromandel, Bay of Plenty and Hawke's Bay. Yes I know that. States of emergency declared. Power outages. Evacuations. Roads closed. People genuinely affected. Up north. Those are the facts and they matter. But here's the bigger picture we're being told now: New Zealand is entering what experts are calling a more defined “cyclone season.” This isn't just a one-off. We've already had multiple major weather events in 2026 — storms earlier this year that caused deaths and serious flooding. Scientists are warning that while cyclones may not come more often, they could be more intense, last longer, and hit harder. So yes — the risk is real. But here's the question I can't shake this morning: are we starting to jump too quickly into full shutdown mode every time a system rolls through? Because we know this is the time of year. We know we sit in a part of the world where these systems track down. This is not new anymore — this is becoming part of the pattern. And yet — what are we seeing? Batten down the hatches. Cancel plans. Stay home. Prepare for the worst. Now again — authorities will say that's exactly the point. Early warnings save lives. And they're right. Since events like Cyclone Gabrielle, behaviour has changed. People take warnings seriously. Emergency management acts earlier. And in many cases that's the preparedness that we want and absolutely prevents tragedy. That's a good thing. But is there a tipping point? Because when every weather system is treated like it could be catastrophic, something else starts to happen. People stop going out. Businesses take a hit. Events lose momentum. And slowly, quietly, the economy feels it. And it starts to feel like it's clickbait for media. It starts to feel like a build up days before. And here's where it gets tricky — because you don't want complacency. You don't want people ignoring warnings when a genuinely dangerous system is coming. But you also don't want a country that freezes every time the forecast turns rough. We've seen the impacts — infrastructure under pressure, insurance costs rising, councils facing huge bills to build resilience. All of that is real, and all of that needs serious long-term thinking. But day-to-day? Are we getting the balance right? Or are we starting to default to worst-case scenario thinking every time will be a catastrophe — just in case? Because if this is the new normal… if this really is a “cyclone season” we're going to live with… Then the way we respond to it matters just as much as the weather itself. So, here's the question this morning — have we found the right balance between caution and overreaction? Or are we, bit by bit, becoming a country that shuts down too easily? Should we be more cautious of a boy who cried wolf situation in Wellington? Because that's the part that really scares me. Do we overreact too quickly? It was coming.How cruel a mistress is silver! She did exactly what she needed to do to suck everyone in, even the doubters like myself: went through $50 to $54, persuaded everyone that this time it's different, sucked them in and then slapped them down. Oh, silver!Today we consider what to do next, and we'll start with the big picture.There are several possible scenarios. Here are three of them:* That's it. The bull market's over. Silver reached $50, as it did it 1980 and 2011. Now it corrects, and it'll be another decade or more before it gets there again. Batten down the hatches: we are going into another bear market. What we saw in the miners was just a 9-month relief rally, much as we got in 2016. Normal gold mining behaviour - relentless declines, in other words - will soon resume.* You don't understand, gold is being remonetized. This is a new paradigm. Buy the dips. We'll be back at new highs before you know.* Nothing goes up in a straight line, not even gold when it is re-entering the money system. Bull markets shake you off. We are going into a sideways correction that is going to last as long as a year. Possibly longer. It is going to frustrate everyone, bull and bear alike. The weak, hot money needs to be purged, the system cleansed of excess, before this thing can get going again. It's a mid-cycle shake out.Which of these is it?And, most importantly of all, what do we do now?I'm going to give you my opinions on gold, silver, the miners, and the speculative positions in which we own stock.I'm also going to give you some target prices.This is what I think happens next.

This trip was solely undertaken to raise awareness of the rare disease called 'Batten disease'. Kristen Gray is the mother of Charlotte and Gwenyth Gray, who were diagnosed with Batten disease in 2015. Kristen and her husband, film producer Gordon Gray, founded the Charlotte and Gwenyth Gray Foundation to Cure Batten Disease. Doctors explained that there was no cure, nor any treatment options for this rare neurodegenerative disorder which would leave the girls blind, immobile, cognitively impaired, and ultimately gone between the ages of 6 and 12. Kristen and Gordon were unwilling to accept this prognosis. One year later, thanks to unwavering dedication and an overwhelming outpouring of support from all over the world, they built a foundation which coordinated and funded a first of its kind investigational gene therapy clinical trial, providing the first signs of hope for a cure to families impacted by this disease. Results are ongoing, but promising. The Foundation funded the pre-clinical research and treatment of 15 children to date.
