In this episode, Max discusses the crash of N1125A, a 1991 ASTRA/GULFSTREAM 1125 SP business jet at Ingalls Field Airport in Hot Springs, Virgina. It was the third fatal business jet crash in the U.S. in just five weeks. Five people on board were killed. The aircraft had been purchased just 3 ½ months early. ADS-B data shows it had a normal approach on the ILS down to about 1000 feet AGL, where the data stopped. Strong winds prevailed, and it's likely that there were strong downdrafts on short final, which may have brought the jet down. Several airline pilots send feedback about the United flight 2477 taxiway overrun at Houston. They also talked about the FOQA programs which would have provided automatic reporting, if this approach to landing met the airline's criteria for an automatic go around. Host Max Trescott discusses the fatal crash of a Falcon jet in Georgia with aviation expert Rob Mark. The NTSB final report revealed a series of factors contributing to the accident. These included misreading a NOTAM regarding the ILS glideslope, difficulty entering the initial approach fix into the navigation system, high and fast arrival at the final approach fix, unauthorized use of airbrakes. It's also possible that they didn't realize the ILS approach required flying a procedure turn. They were also flying a black hole approach in dark night conditions with minimal ground lights, but apparently weren't referencing the PAPI visual indicator. The captain, aged 73, had extensive flight experience but had undergone retraining due to unsatisfactory performance in certain areas. The first officer, aged 63, had a significant number of flight hours, but received only a Second in Command (SIC) type rating, because of performance issues. The podcast delves into the transcript of communications between the flight crew and Atlanta Center, highlighting confusion regarding NOTAMs and the approach procedure. The crew, flying a cargo route from El Paso to Thomson, Georgia, requested information on the ILS approach, but there was a misunderstanding regarding the status of the glideslope and localizer. The approach required a procedure turn, which the crew seemingly missed, leading to an unstable approach. There were delays in programming the initial approach fix (IAF) into the navigation system, possibly due to confusion over the fix's identification. The crew ultimately crossed the IAF at an altitude significantly higher than prescribed, leading to a steep descent to intercept the glideslope. Max created a software simulation of the final minutes of the flight that revealed a rapid descent rate and an unstable approach. Despite warnings from the captain about being high, attempts to correct the descent were ineffective, ultimately resulting in impact with trees just short of the runway. This was a classic black hole approach, in which there are few lights on the ground before the runway. A Boeing simulator study of a black hole approach showed that pilots consistently crash short of the runway, and so they must have either an electronic or visual glide slope. I've been in a bit of a funk. Not really flying, not really participating in the industry, not really inspired to put it bluntly. And I know I'm not the only one. And it's kinda funny, since by all counts, we're actually coming out of this disaster time and getting the industry on the long road to recovery. Will it ever be the same. Well, probably not exactly, but who knows, maybe it'll be better? But I finally started flying again on the regular. Every Saturday around noon you might find me in a Diamond Star somewhere over Southern California and it feels pretty good. But there were a few first flights back that felt more than a little uncomfortable and awkward. And I don't like feeling that way in an airplane. Now I know, I know, this might be the last thing anybody wants to talk about ever again. Except maybe politics, that's usually worse, and I usually make a point to steer clear of that as much as possible, but the conversation we have isn't about that, although it may border dangerously close few times... It's about what I call “getting it back.” Our lives, our inspiration, our mojo. Getting our groove back so that we can put this all behind us as quickly and healthily (I'm sure that's a legitimate medical term) as possible. It's been almost a year and a half since the Atlas crash, and that's about how long it takes the NTSB to complete a formal investigation, and to issue a Probable Cause statement. And boy did they ever… The idea that a 767 could just fall out of the sky vertically, with no warning, was weird enough to suggest that this wouldn't be a typical case, but now that the truth is out, the weirdness doesn't disappoint. There's a great article out there from Rob Mark, at FLYING Magazine, and there's a link in the show notes, just in case you missed it. But today Rob's here, in the flesh, and we have a pretty cool talk for you. And it's about more than just what happened, it's about why it happened, and whether or not we can even prevent something like this from happening in the future. And I sure hope we can. But as you'll hear, it'll take a lot more than just a few tweaks in the training Department… From Rob's Article: The NTSB issued six new safety recommendations following the investigation into the Atlas Air 3591 accident. The NTSB said the FAA should issue a safety alert for all Boeing 757/767 operators warning of the possibility of an inadvertent go-around activation when the pilot flying is in close proximity to the speed brake handle. The NTSB also wants to see the vulnerabilities of the current pilot record system that would include the results of all pilot training once it begins, regardless of the result and that the system be easily searchable by pilot certificate number. The Board reiterated its call for advanced cockpit video recorder systems for Part 121/135 aircraft. On the human side of the equation, the NTSB reiterated to the FAA the urgent need to “require all Part 121 and 135 air carriers to obtain any notices of disapproval for flight checks for certificates and ratings for all pilot applicants and evaluate this information before making a hiring decision. To also require Part 121, 135, and 91K operators to document and retain electronic and/or paper records of pilot training and checking events in sufficient detail so the the carrier and its principal operations inspector can fully assess a pilot's entire training performance. The Board also wants to see all training records provided to hiring employers as required under the PRIA and to develop a method of verifying the accuracy of those training records.” I'm sure you've got at least one opinion on something you've heard today. What’s Narrative?No, honestly what is it. And why the heck are we talking about it on an aviation podcast?! Well, because I’m repurposing content from another thing I do. No, I’m kidding… Sort of. Narrative affects everything we hear. It’s that thing news outlets have that affects how they’re going to spin a story. And if you ask me, it’s a bummer that has to be a thing at all.Today’s a really cool episode, because we’re stepping a little outside the usual mold (as if there was one here), and pulling back the curtain a little bit. And for those in the business of making content, it’s something we struggle with sometimes. It’s been almost a year and a half since the Atlas crash, and that’s about how long it takes the NTSB to complete a formal investigation, and to issue a Probable Cause statement. And boy did they ever…The idea that a 767 could just fall out of the sky vertically, with no warning, was weird enough to suggest that this wouldn’t be a typical case, but now that the truth is out, the weirdness doesn’t disappoint.There’s a great article out there from Rob Mark, at FLYING Magazine, and there’s a link in the show notes, just in case you missed it. But today Rob’s here, in the flesh, and we have a pretty cool talk for you. And it’s about more than just what happened, it’s about why it happened, and whether or not we can even prevent something like this from happening in the future.And I sure hope we can. But as you’ll hear, it’ll take a lot more than just a few tweaks in the training Department…From Rob's Article:The NTSB issued six new safety recommendations following the investigation into the Atlas Air 3591 accident. The NTSB said the FAA should issue a safety alert for all Boeing 757/767 operators warning of the possibility of an inadvertent go-around activation when the pilot flying is in close proximity to the speed brake handle. The NTSB also wants to see the vulnerabilities of the current pilot record system that would include the results of all pilot training once it begins, regardless of the result and that the system be easily searchable by pilot certificate number. The Board reiterated its call for advanced cockpit video recorder systems for Part 121/135 aircraft.On the human side of the equation, the NTSB reiterated to the FAA the urgent need to “require all Part 121 and 135 air carriers to obtain any notices of disapproval for flight checks for certificates and ratings for all pilot applicants and evaluate this information before making a hiring decision. To also require Part 121, 135, and 91K operators to document and retain electronic and/or paper records of pilot training and checking events in sufficient detail so the the carrier and its principal operations inspector can fully assess a pilot’s entire training performance. The Board also wants to see all training records provided to hiring employers as required under the PRIA and to develop a method of verifying the accuracy of those training records.”I’m sure you’ve got at least one opinion on something you’ve heard today. A Unique Around-the-World Journey Heads East As you read this story, Mason Andrews should be winging his way eastward out of Italy toward Croatia while sitting in the left seat of his dad’s Piper Lance (a link to the full podcast is at the bottom of this story). Andrews was one very lucky young man when he asked his dad to borrow the airplane for a trip and received a thumbs up. OK, that’s not completely accurate unless you understand the context, that the senior Andrews did actually express a few reservations when young Mason mentioned the length of the trip … around the world. And yes, his dad definitely raised an eyebrow when Mason told him he wanted to make the trip alone. Mason Andrews, a newly minted instrument pilot and Louisiana Tech student just recently turned 18. When Mason Andrews completes his round-the-world trip, he should become the youngest person to complete a global trip solo. Mason wasn’t making the trip to become famous, although he likely will. The trip was actually designed to raise money for MedCamps of Louisiana to fund summer camp for kids with special needs, a summer event where Andrews also serves as a counselor. About the Aircraft The Piper Lance Mason will fly has been modified to carry enough fuel for legs as long as 18 hours. The Lance cruises at about 140 knots burning 13.5 gallons per hour. The first leg of the flight began last week from Republic Field on Long Island NY. The first leg took him from Republic to St. Johns Newfoundland. Mason’s flight plan includes a stop at Paris LeBourget, site of Charles Lindbergh’s arrival following his record-setting solo flight in 1927. Mason Andrews said he believes the flight’s biggest challenge will be “weather.” The Lance is much better equipped to keep him informed of the weather than Lindbergh could have ever imagined. My EAA Radio co-host Amy Laboda and I managed to convince Mason to join us for an interview last week during our regularly scheduled “Attitude Adjustment,” show at AirVenture 2018. One of the first things Mason mentioned the day I met him was that he was still celebrating the two-year anniversary of his first solo as a student pilot from Monticello Airport (LLQ) in Arkansas. Can Airports Help Revive the Aviation Industry? Dear Reader / Listeners – You now have the option to listen to The Aviation Minute podcast or just read the script of the show below. If you’re not yet a subscriber to The Aviation Minute, Click Here to sign up … it’s free. ______________________________ I knew we had a lot of landing areas here in the United States … but 19,315 according to the FAA? Wow. That number is of course broken down into traditional airports, heliports, seaplane bases. No matter what you call them or what they look like, they all have one thing in common. They represent a place where airplanes, helicopters and seaplanes come home to roost from time to time. Some of those airports represent much more of an opportunity to me than simply as landing areas though. The aviation industry is still suffering from an economic recession of sorts. In the early 1980s, we produced 15,000 piston aircraft. Last year we produced 1,328. In the 1990s we had over 700,000 pilots on the FAA register. Today that numbers in the high 500,000s. Student pilot starts are down from the old days too with nearly 7 in 10 students quitting long before they ever earn a pilot certificate. Aircraft maintenance technician numbers have been flat since 1990, which equates to zero growth. Worst of all, 75% of the AMTs today are over the age of 50. As an industry, our ship has been taking on water for sometime despite a number of conscientious initiatives to increase the pilot, mechanic and airplane supply, most of which haven’t moved the needle much. If we don’t figure out a way to start bringing new blood into the industry soon, there won’t be enough people to fly the airplanes we build, or fix them, or service them at those thousands of U.S. airports. The question is how to fix the problem we all know about, but that many people still seem to believe is someone else’s problem? Rather than another national initiative, what if we focused our triage efforts locally … at our neighborhood airport? When people think of learning to fly, they go to the airport. If they want to buy a plane they often visit the airport first. When they need one fixed, or they want a hangar, they head to the airport. This is where I think airport managers can help. Traditionally, they focus on keeping the airport alive with solid pavement, newly mown grass and runway lights that work … all very necessary tasks. But marketing the industry is not something airport people normally think about. But what if airport managers started thinking a bit more about marketing, I think they might just transform their airport into a local industry beacon of sorts, one that encourages people to learn to fly, or become involved in any of a half dozen other careers within the industry? I’m not asking airport managers to fix the industry’s personnel woes all alone, just help coordinate local efforts with the airport tenants whose companies need the boost as much as the industry. Imagine organizing a couple of career days, or a Young Eagles Rally or two to stimulate interest. What about an airport Facebook page or a blog to tell the local community about the value of the airport, one that regularly posts photos or stories gathered from airport tenants? Before you know it we might even convince the community around our airports to ignore those 8-foot barbed wire fences and stop in for a visit. I think airport managers are up to the challenge of coordinating the local marketing efforts for our industry. We’ll talk more about the actual tactics in another episode too. And for those naysayers who are already saying that will never work … tell me what you’d suggest instead because I haven’t seen much working lately. If we don’t all start realizing that fixing what ails our industry is everyone’s concern … flight schools, FBOs, flying clubs, maintenance and avionics shops, trust me … we may one day find that the airport managers who are still around may have a lot fewer airports to watch over. Hello Everybody. This episode is my recollections and imperessions of my first trip to Oshkosh.We spent 3 days at AirVenture, camping at Camp Scholler. I participated in Podapalooza on Friday night along with other aviation podcasters. This trip was very special for me since I was able to share my passion of aviation and my first trip to AirVenture with my sons. This was our first "boys" trip together and what a trip it was.The one thing that I will take from this trip is how social media is helping to bring people together, especially the aviation community. I met many of my "social media" friends for the first time in person at Oshkosh. We have all become friendly through Twitter, forums and listening to each other's podcasts. Even before I arrived at Oshkosh I already had meetups scheduled with my friends. So instead of not knowing anyone there, I had parties and meetups to go to. This is what our small community is all about.
