It's important to learn through every experience and Sully certainly has learned a lot from the loss of his first associate. The Importance of Systems in the Dental Office Episode #360 with Dr. Sully Sullivan Whether you want better patients, better working hours, more vacations, or all of the above, it all comes down to one thing: you need to have systems in place. In today's crossover episode, Kirk Behrendt and Dr. Sully Sullivan (from The Millennial Dentist) chat about generational differences in dentistry, the systems you should have in place, and some important keys for the success of your practice. It is your responsibility to train patients how to treat you and your team. Invest in training your team so they can handle problems efficiently. Show your team and your patients that you value what you do. Have regular morning huddles with your team. When you can't change the person, it's time to change the person. Quotes: “Here's the thing. You go to dental school to learn how to do dentistry. And then, you get out and then realize, ‘Wow, I've got to run a business, and there are people attached to these teeth. And now, I have to manage the emotions of all these women.' And the true fact is that you've got to learn how to run a business.” (4:09—4:22) “You can do whatever you want to do [in dentistry]. You can hire whoever you want, and you can fire whoever you want. You can go, ‘This isn't working. Buh-bye.' We don't even call it firing. We call it freeing up your future and money.” (4:51—5:05) “A new dentist, rule number one, stay away from dentists. Just stay away from all dentists. And I mean that in a tongue-in-cheek way, but the truth of it is that as a young dentist, you've got to pick your neighborhood. You've got to get around great people that are going to influence your thinking, because [if] your thinking goes bad, your business model goes really bad. Because you're going to sit next to a dentist who goes, ‘There are no good people out there.' And you're going to go, ‘No, there are a lot of them. They're just not working in your office.' And then, people are going to go, ‘You've got to work Saturdays. Say goodbye to your kids and your family for a while. And sign up for all the PPOs.' And I'm like, that's dumb. Why would you do that? You earned the greatest profession, ever. You've got to surround yourself with people that think really well.” (7:08—7:49) “You can't work any harder [at a certain point]. You've got to think better.” (8:21—8:23) “When you open up your practice, you've got to train patients to pay. You've got to tell them when to come in. You can't let the inmates run the asylum. Because what happens over time is you go, ‘I hate this!' You've got to effectively teach people how to treat you in a business.” (8:59—9:19) “The seven most expensive words in business are, ‘That's the way we've always done it.' That's the worst phrase. ‘Why do you guys do that?' ‘That's the way we've always done it.' You can't do that. That's a dumb reason to do anything.” (13:28—13:41) “People love structure. I talk about the beginning of creating the podcast, releasing and producing the Millennial Dentist, and who the podcast is for. The owner was impressed by millennial dentists really wanting a lifestyle instead of just a paycheck for their work. “I feel that the millennials are more balanced than when I started out in dentistry when I burned through everything working 90 hours a week.” When you are looking at your dental practice, don't just look at your business, search, and see how you can balance work and family life. “There are one of three things that stop people from doing a treatment; that is time, fear, and money.” which is what my mentor practiced by. When you take away these three hurdles, you can assist your clients in moving forward with their dental improvements. “We don't have a fee issue, we have an affordability issue.” We need to find ways to make our procedures affordable for our patients, when we break this down into payments, now our clients can pay for their treatment. What is the risk associated with taking out a $30, $40 or $50,000 loan to provide treatment for your patients? What happens if the patient defaults? We talk about how to mitigate your risk with large loans for treatment payments. Many of the patients we send to apply for Compassionate Finance get approved. About 10% of our clients do not get approved, so we call for manual approval. With manual approval, they can look at applications on a case-by-case basis to see if they can be approved depending on their current financial status. Dr. Paresh Shah is here to talk about the importance of continuing education, treatment planning, and having a mentor. Dr. Shah was born in India, but has been in Winnipeg, Canada for 50 years where he has been a solo practitioner. He has recently added an associate to his practice. After being an associate, Dr. Shaw purchased a practice from a retiring dentist and built that practice up. He really became excited about dentistry when he found mentors and teachers and like minded people to grow with. In this episode, Dr. Shaw shares a lot of great information for young dentists and dentists starting out. He talks about the importance of getting or buying a patient base when you first start out. He talks about how to use technology in a smart, efficient, and profitable way. [02:26] His family and his practice are in Winnipeg and his daughter is a dentist in Chicago. [02:59] Dr. Shaw has been a solo practitioner, but he just got an associate. [03:06] He does restorative dentistry, implants, cosmetic dentistry, and interdisciplinary care. [03:20] He has a busy practice with three full-time hygienists. They have five rooms and their operation is very efficient. [03:32] Dr. Shaw loves what he does. He used to be a super dentist and do everything, but now he focuses on what he does best. [03:37] He has a great group of specialists that he works with. [04:06] When Dr. Shaw first started in dentistry, he was an associate. Although, he almost became an oral surgeon. [04:45] A distribution rep told him about a practice that was for sale, and he ended up buying his practice. He started with three chairs and no hygienist. He built the practice adding hygiene one day at a time. [06:15] How corporate dentistry is entrenched in the US. If you work hard, and you are smart, you can buy a practice and pay the debt off. [06:47] The importance of finding a practice that already has a patient base. [07:20] Ask what the dollar value of those patients are? [09:18] You can take the approach and build it and people will come, but to be successful you need a patient base. [09:53] You could also transition the patient base of a retiring dentist. [11:05] Find a mentor that will teach you how to diagnose and treatment plan and communicate with patients. [11:54] Millennials want a practice that has a luxurious look. [13:30] When you buy a new practice, there are aesthetic things you can do to make the practice look better. [14:35] The point of buying an existing practice is cash flow. [15:12] At first, Dr. Shah wasn't enjoying his work. Then he met a group of mentors and teachers and learned how to properly diagnose and treatment plan. He spent 5 or 6 years at the University of Minnesota. [16:40] He saw the great lives and practices of these older dentists and their passion for dentistry. [16:54] He is still friends with these guys. By surrounding himself with this group and taking several continuing education courses, he became enthusiastic about his practice. [17:58] Like minded individuals can be inspiring.   [19:40] Hanging out with people ahead of you closes your information gap. [21:07] Learning the foundation by going to the Kois Center and Spear Education. [22:45] To understand and learn everything a young dentist really needs a continuum course. [24:53] How you don't know what you don't know is absolutely true. [25:45] The importance of updating and learning new techniques. [27:38] How important it is to meet people and be exposed to people in other cities and other countries. [28:34] The importance of creating a nonthreatening environment and creating accountability. [30:15] The Seattle Study Club was founded by Michael Cohen. This is a university without walls that meets every four to six weeks. [31:08] They sit down and have treatment planning sessions. There are about 270 Seattle Study Clubs around the world. [31:34] The importance of finding a study club that wants to mentor you. [34:04] Dr. Shah lectures about current techniques for doing everyday stuff. People have to have a foundation. [36:30] How you can learn from your mistakes and decide what you can improve if something doesn't look good. [37:43] He also shows advanced treatment plan cases in his study clubs. [38:31] Digital workflow and new scanners and software. Dr. Shaw is trying to teach the digital workflow to get from here to there and be efficient about it. [44:28] How important it is to not make learning experiences sales pitches. Dr. Shaw and a friend are doing a two-day program and will have the products of several different brands represented. [47:33] Team members also need to be part of the education and the workflow training and creation. [50:24] Dr. Shah incorporates changes and checklists for a reason. He doesn't change procedure without having a reason and sharing it. [55:50] Buying a scanner, having a great lab, and getting a model printer is a great way to operate. [56:32] The key to digital is time. You can pay for your scanner in a year. [57:56] The value of saving time whether it's 15 minutes a crown or a week a year. [01:01:53] Interview labs and find a good lab that you can communicate with. Dr. Shah found a great lab in Canada through Instagram. [01:04:13] Send photos to your lab and keep the lines of communication open. You both need to be accountable with each other. In our final interview from the Voices of Dentistry Podcast Summit 2018, we go out with style!  Our guest this episode is none other than THE Tarun Agarwal, DDS— commonly known as T-BONE!  He came all the way from Boston, Mass. to be with us in Scottsdale, Arizona. On this episode, Justin and T-Bone are joined by Mike Freimuth, Randy Houska and Gabe Olson (who is still standing at this point, which is amazing).  In it they go into a bit of banter back and forth before going into details about a charity campaign that will ultimately raise $50,000 for a charity of Sully Sullivan's (from the Millennial Dentist) choice, in exchange for shaving his head!  More details on that to come. Dr. Peyman Raissi DDS AKA Dr. Peyray as he is more affectionately known in the Social Media, is one of the biggest rising stars in Dentistry today. Don’t let his young age fool you, Graduated from the University of Tennessee, he began his career with Affordable Dentures and Implants in Nashville TN. In just a few weeks he went from the new young graduate dentist in town to the biggest producing Implant Dentists in the corporate. In 2015, he ranked Nashville office within the corporate the Number 1 office out of 220 for placing and restoring the most number of implants. He has placed over 1000 implants, performed over 220 full arch rehabilitation using dental implants, over dentures and fixed hybrid bridges. Dr. Peyray has been trained by some of the most respected dentists in the world while completing over 900 hours of Dental CE education specific for implant surgery, bone and tissue grafting, prosthodontics as well as IV sedation. In 2016 His passion to keep pushing himself to conquer new challenges led him to build his very own dental practice from the ground up and call it “My New Smile Dental” in Nashville, Tennessee which focuses on Implants and full mouth rehabilitation.  Dr. Raissi proudly serves on the clinical advisory board for Zest Dental solutions, tests out new implant related products before they’re introduced to public and gives his professional opinion. He has contributed case series for Biohorizons Implants and Teeth Express Magazine. Dr. Raissi co-hosts a podcast called the Millennial Dentist that focuses on the new generation and the experienced dentists, interviews famous speakers to focus on dentists role for the future world of dentistry. With his few thousand of dentists followers online, Dr. Peyray regularly performs Live implant educational surgeries for domestic and international dentist overseas using media apps and social media.  Peyman is very involved in organized dentistry. He received his fellow in the International Congress of Oral Implantologists (FICOI), a fellow in American Academy of Implant Prosthodontics. He has passed the written exam for American Academy of Implant Dentistry (AAID) and is on his route to become the youngest dentist to receive the Diplomat Status from the Board of Oral Implantology.
