Later, Dan looks back at Andrew Luck's retirement on the 2 year anniversary of Luck's shocking decision. Dr. Hogan is the Vice Chair of Education and Residency Program Director in the Department of Orthopaedic Surgery at the University of Pittsburgh Medical Center. He oversees all educational and training programs within the department. He is an Associate Professor in the Departments of Orthopaedic Surgery and Bioengineering.  He completed medical school at Howard University College of Medicine in Washington, DC.  He was the recipient of a National Institutes of Health Training Grant and served as a Musculoskeletal Tissue Repair and Regeneration Research Fellow at the University of Virginia Health System, Department of Orthopaedic Surgery, in Charlottesville, Virginia, where he also completed his orthopaedic surgical residency training. This was followed by a foot and ankle fellowship at the Hospital for Special Surgery in New York, NY. Have you ever experienced an injury so severe you thought you would never come back from it?  Do you remember the thoughts that ran through your mind?  Or the disappointment you felt?  In 2017, Hillary Allen was ranked first in the Skyrunning World Series. This series consisted of high altitude endurance races spanning anywhere from 50k to 100k.  Hillary, sponsored by The North Face, was at the top of her running game when she suffered a catastrophic injury after a 150 foot fall during the Hamperokken Sky Race in Norway.  Hillary suffered two broken arms, two broken vertebrae, two broken ribs, ligament damage in her foot, and several bruises and lacerations.  While many people may have seen this as an impossible setback, Hillary took it as a challenge and faced it head on. What is Strength As runners, we define ourselves as runners. When you experience a runner’s high, it is an experience like no other. But when we can’t experience that high due to an injury, especially one that will keep us away from the sport we love for so long, we will experience both physical and emotional pain. It is in these moments that you can define yourself as someone who gives up, or you can define yourself as someone who is not only a strong athlete, but a strong person. Being a strong person may seem easier said than done. So, how do we find the strength to press forward when the situation seems so dire? Try starting a journal. Writing down affirmations to yourself will help pull you through these hard times, even if you don’t believe them. Write positive messages to yourself and then go back and read those messages. You can empower yourself and nurture a belief that your strongest athletic days are ahead of you. You may not be able to see the light at the end of the tunnel, you may even think it’s not there, but it is! The more you affirm this within yourself, the more you will buy into it. Strength is also being able to rely on other people. It is ok to be vulnerable. It is in moments of vulnerability that we find the most strength. We can ask for help and that’s ok. It takes strength to rely on other people. Strong isn’t always, “Let me do it myself,” but can be, “Please, I need help.” Be Honest with Yourself and Don’t Give Up One of the more serious injuries Hillary suffered in her fall was a Lisfranc injury. This type of injury typically takes close to a year to fully recover from. Hillary’s doctor told her she may jog again, but most certainly would not run again. As heartbreaking as this may be to some people, Hillary made the decision to not allow this diagnosis to eliminate hope that she may not run again. Instead what she decided to do was allow the impossible to motivate her and make her optimistic. If you sustain an injury and want to get back to running, you can do it. The first step is realizing it will not be easy. You and only you make the decision on how you are going to handle the hard times that are sure to come. It is ok to doubt yourself. It’s human nature to experience feelings of reservation and uncertainty; but doubt has the ability to foster belief. Think back to the last time someone told you that you couldn’t do something. How did that make you feel? Did it motivate you to prove them wrong or did you decide to believe what was being said? We can become so overwhelmed by focusing on our inabilities that we forget about how much we are truly capable of doing. Surround yourself with people that want to see you succeed, with people that will listen to your negative thoughts and fears. Put those scary thoughts out there and be honest with not only yourself, but with your support group. Sometimes honesty can be hard, and if you find that you can’t do it, fake it. And continue to fake it until that self-doubt is gone and you begin to believe in yourself. People are stronger than they give themselves credit for, but you can do it. If you give yourself permission to suffer a little bit, you’ll surprise yourself with how strong you really are. Don’t Call it a Comeback Your return to running following an injury or a break may insinuate you are coming back, but don’t call it a comeback. Coming back means you are coming back to the same place you left off. But you are not. The journey back from an injury should make you feel like a completely different person. You cannot compare yourself to the person you were before. By doing this, you will put a lot of unnecessary pressure on yourself which will diminish the joys you should feel while running. Know that while your pace may not be where it once was, you are now a stronger athlete, with new knowledge and a greater love for running. By calling it a comeback you put a lot of internal pressure on yourself. Pressure that is unnecessary and that can take away from the joys running should bring you. Not only do you put pressure on yourself, but it can also be frightening. However, the opposite of fear is courage. Getting back out there and running is what courage looks like. Lacing up your shoes and hitting the trail or the road and competing with yourself. Running is a celebration.  It is liberating! Overcome your mental block and allow your body to do what it knows how to do: run. You can tell yourself you are going to give up, but don’t ever give up. Look at challenges as an opportunity to dig deep into yourself, to pull from within you something you never knew you had. This will create experiences for you to draw from as you encounter future trials and will give you the strength to conquer setbacks, as improbable as they may seem. Author: Susan Ryan, DO Educational Pearls: General orthopedic principles: Examine above and below the injury Document neurovascular status X-ray imaging typically requires three different views Fracture description should include name the bone, location of fracture, degree of displacement, and if it is closed or open Osgood-Schlatter (tibia) and Sever’s (calcaneus) disease are apophyseal injuries caused by ligaments that are “stronger” than the bones they attach to   When looking for scaphoid injuries, get extra (turned) views of the wrist. Remember that the scaphoid has a reverse blood flow and is prone to avascular necrosis Acute carpal tunnel syndrome can occur in forearm fractures. Again, don’t forget your neuro exam.  Distal radial-ulnar joint (DRUJ) injuries are caused by tears in the ligaments that stabilize the wrist. They cause chronic pain with pronation and supination. Posterior effusions in the elbow in the 90 degree view nearly always indicate a fracture  Lisfranc injuries are commonly missed, especially if the mechanism is perceived as low energy. Look for the “fleck sign”, which is an avulsion fracture at the base of 2nd metatarsal  Syndesmotic injuries of the ankle (a high ankle sprain) can be identified through the squeeze test Knee dislocations are neurovascular emergencies

Heads 'N Tales Podcast - Talking Sports Injury Rehab, Prevention, Perseverance, Concussions & Athlete Transition

Rebecca Schumer Lee is a Sports Physical Therapist at MedStar National Rehabilitation Network and was a dual sport (soccer and softball) division-1 athlete at The George Washington University. Currently, Rebecca primarily works with athletes of all levels and patients with orthopedic injuries and/or post-surgical recovery at the US Lacrosse Headquarters in Sparks, Maryland.  Rebecca reached out to me after coming across our Instagram account. She was understandably intrigued by our podcast after having 23 surgeries of her own throughout her athletic career.  Rebecca believes her injury-proneness stemmed from the natural hyper-mobility in her joints. During our interview we learn about how although mobility and range of motion are important for both performance and in reducing the risk of injury, but joint stability is also important.  Additionally, Rebecca feels her competitive drive and not knowing when to stop was also a contributing factor of her high injury rate.  I can certainly relate to Rebecca on that one!  Rebecca wishes she knew more about the importance of active recovery during her athletic career, which is a topic she touches on more than one occasion throughout the episode.  Sixteen of the 23 surgeries were on her left foot after suffering a Lisfranc injury on October 6th, 2004 during her Junior year soccer campaign at GW.  Rebecca remembers this day vividly and recalls her athletic trainer encouraging her to find a hobby, knowing her injury would required surgery. Easier said than done for such a competitive athlete who saw no life outside of sports.  During our conversation she described her left foot as looking like a bag of screws (see picture below). I think that is a pretty accurate claim. Although she admits that it has taken time to come to grips with her various injuries and the resultant limitations those have placed on her active lifestyle, she has learned to make light of the situation (see below).  "Pain is your body's warning signal that something is not right" With all these injuries, Rebecca talks about the struggle of having the injury prone tag.  She remembered having both a walking boot and an arm sling at one point while still in college.  Rebecca gives advice in this episode on how to use your injury to your advantage.  This isn't referring to the use of elevator services and getting sympathy from your friends, teachers and family.  There is a lot that can be learned from watching from the sideline. Believe it or not, since high school Rebecca knew she wanted to be a physical therapist even before all the injuries. Below, she is pictured treating one of her patients with dry needling. As Rebecca states in her Instagram caption (above), there is not one tool that is going to solve everything.  During our conversation, I asked Rebecca about some things injured athletes and their families should look for when choosing a PT facility.  Not all PT practices are created equal. What to look for in a PT facility: What is the population? Athletes or hip replacements? Take a tour of the facility to see their space and equipment options Rebecca's new favorite tool is the Owens Recovery Science blood flow restriction device featured in episode 84.   The most commonly treated injury in Rebecca's practice are ACL injuries Are you progressing? Are your exercises always the same? Are you always off on your own or working with an aide? If any of those are true, you should look into finding a new facility. Throughout the recoveries from all her surgeries, Rebecca has found peace in her thirst for knowledge.  Reading books and listening to podcast are two of her favorite hobbies outside of physically active activities.  Below are some of her favorites.  Personally, I am a huge fan of fifth one on the list, The School of Greatness, by Lewis Howes. Getting contralateral XR may help you identify this. Even if you don’t see that widening on the XR, the patient could still have a Lisfranc injury. If they cannot walk due to pain, get a weight bearing XR or CT scan to look further. Once the injury is identified, the patient must be strict non-weightbearing. Place them in a posterior splint and get orthopedics involved either in the ED or for prompt follow up as the patient will probably need surgery.
