In this episode of Inside Oversight, Nicole Maxey, a nurse consultant with the Office of Healthcare Inspections, discusses the VA OIG's evaluation of the transition of clinical care for service members with opioid use disorder from the Department of Defense to the Veterans Health Administration. Nicole describes deficiencies in documenting patients' opioid use disorder, as well as the barriers faced by healthcare providers accessing records, during the transition.  “We want to make sure that all providers are aware of [opioid use disorder] to ensure that this vulnerable veteran population gets the care they need. The care of rheumatology patients can be complex, often requiring medications that need close monitoring and education to ensure best and safe outcomes. There is often minimal time within the busy clinical outpatient setting to holistically review the patient's needs. The rheumatology clinical nurse consultant telehealth service was introduced to achieve better patient outcomes and provide a comprehensive approach to the delivery of care to any individual anywhere. Ensuring that patients have a greater understanding of their rheumatological condition can lead to patient empowerment and better outcomes. The implementation of the clinic via telehealth was to provide an alternative way to receive specialist nursing care. This also helps to reduce petrol and parking costs as well as having positive environmental implications. It also means that patients can receive holistic care wherever they may live and is particularly useful for those living in rural and remote areas. By offering telehealth, patients have less time off work which leads to more work productivity and time spent with family. It also means that the interval between patients being reviewed by the clinician can be increased as the telehealth service allows for interim follow up. This therefore allows for more patients to be reviewed in clinic by the nurse practitioner and other clinicians.

As part of our Special Report series, we shift our focus from nursing burnout to an innovative nurse-based initiative to build vaccine confidence in California. In this episode, we speak to Lizett Leandro about her work as a consultant for the Trust A Nurse, Ask A Nurse program, a telehealth platform created by HealthImpact. The program relies on volunteer nurses to field vaccination questions from community members. Lizett helped create the initiative's informational template and shares her experience with this type of public health outreach. Lizett Leandro is the Director of Clinical Services and Quality for a senior living non-profit in Southern California and a Gerontology Nurse Practitioner and Nurse Consultant at HealthImpact. She oversees nursing education policy and procedures and quality improvement for skilled nursing facilities and assisted living communities.

Karen's PhD thesis ‘An interpretative phenomenological study of the therapeutic relationship occurring between women admitted to eating disorder services and their workers' established her position that the therapeutic relationship is pivotal to assisting the person with anorexia to commence on their journey to recovery and survival. Although EMRs software was designed for greater efficiency in documentation, the large number of different systems, many of which perform different functions, programs that don't “talk” to each other, and the frequent mixing of hand-written and electronic records can make an LNC's journey through the world medical records feel bewildering. Difficulty in deciphering electronic medical records is especially frustrating because it can hamper an LNC's efforts to discover crucial information about the merits of a case. That's why I've invited Diane Tomacelli, a nurse and expert on deciphering medical records, to explain some of the mysteries of EMRs. Diane describes a range of issues. She also explains the importance of knowing the different kinds of information various EMRs capture and how to interpret the results. In describing the importance of requesting an audit trail, she explains the importance of having clarity on what kind of information you want. If you only need five or six pages of information, you must be careful not to make a request that will give you five- or six- hundred pages.
