Atrial flutter, especially with 2:1 conduction, is commonly missed in both inpatient and outpatient settings so look carefully at that 12-lead EKG so you can mitigate the stroke and tachycardia induced cardiomyopathy risk  Decremental conduction of the AV node makes it more challenging to rate control atrial flutter than atrial fibrillation  Catheter Ablation is the first line treatment for atrial flutter and is highly successful, but cardioversion can be utilized as well prior to pursuing ablation in some cases.  Class I AADs like propafenone and flecainide may stability the atrial flutter circuit by slowing conduction and thus may worsen the arrhythmia. Therefore, the preferred anti-arrhythmic medication in atrial flutter are class III agents.  Atrial flutter can be triggered by firing from the left side of the heart, so in patients with both atrial fibrillation and flutter, ablating atrial fibrillation makes atrial flutter less likely to recur.  BONUS PEARL: Dr. Cooper's youtube video on atrial flutter is a MUST SEE!  Notes Notes: Notes drafted by Dr. Akiva Rosenzveig  What are the distinguishing features of atrial fibrillation and flutter?  Atrial flutter is an organized rhythm characterized by a wavefront that continuously travels around the same circuit leading to reproducible P-waves on surface EKG as well as a very mathematical and predictable relationship between atrial and ventricular activity  Atrial fibrillation is an ever changing, chaotic rhythm that consists of small local circuits that interplay off each other. Consequently, no two beats are the same and the relationship between the atrial activity and ventricular activity is unpredictable leading to an irregularly irregular rhythm  What are common atrial flutter circuits?  Cavo-tricuspid isthmus (CTI)-dependent atrial flutter is the most common type of flutter. It is characterized by a circuit that circumnavigates the tricuspid valve.  Typical atrial flutter is characterized by the circuit running in a counterclockwise pattern up the septum, from medial to lateral across the right atrial roof, down the lateral wall, and back towards the septum across the floor of the right atrium between the IVC and the inferior margin of the tricuspid valve i.e. the cavo-tricuspid isthmus. Surface EKG will show a gradual downslope in leads II, III, and AvF and a rapid rise at end of each flutter wave.   Atypical CTI-dependent flutter follows the same route but in the opposite direction (clockwise). Therefore, we will see positive flutter waves in the inferior leads   Mitral annular flutter is more commonly seen in atrial fibrillation patients who've been treated with ablation leading to scarring in the left atrium.  Roof-dependent flutter is characterized by a circuit that travels around left atrium circumnavigating a lesion (often from prior ablation), traveling through the left atrial roof, down the posterior wall, and around the pulmonary veins  Surgical/scar/incisional flutter is seen in people with a history of prior cardiac surgery and have iatrogenic scars in right atrium due to cannulation sites or incisions  How does atrial flutter pharmacologic management differ from other atrial arrhythmias?  The atrioventricular (AV) node is unique in that the faster it is stimulated, the longer the refractory period and the slower it conducts. This characteristic is called decremental conduction. In atrial fibrillation, the atrial rate is so fast that the AV node becomes overwhelmed and only lets some of those signals through to the ventricles creating an irregular tachycardia but at lower rates. In atrial flutter, the atrial rate is slower, therefore the AV node has more capability to conduct allowing for higher ventricular rates. Therefore, to achieve rate control one will need a higher dose of AV blocking medications. Atrial tachycardia may require even higher doses due to the increased ability of the AV node to conduct, as the atrial rates are slower than in atrial flutter.  Sodium channel blockers (Class I) such as flecainide and propafenone slow wavefront propagation, making it easier for the AV node to handle the atrial rates. This will end up leading to increased ventricular rates which can be dangerously fast. That is why AV nodal blockers should be used in conjunction with flecainide and propafenone.  What is the role of cardioversion in atrial flutter management?  Due to high success rate with atrial flutter ablation, ablation is the first line treatment. However, sometimes cardioversion may be utilized in patients depending on how symptomatic they are and how long it will take to get an ablation. Cardioversion may also be utilized preferentially when the atrial flutter was triggered by infection or cardiac surgery to see if it will come back.   If cardioversion is pursued, the patient will need to be anticoagulated due to the stroke risk after the procedure due to post-conversion stunning.  How effective is atrial flutter ablation?  The landmark Natale et al study in 2000 demonstrated 80% success rate after radiofrequency ablation as compared to 36% in patients on anti-arrhythmic therapy. The LADIP study in 2006 further corroborated these findings. Contemporary data shows above 90% success rate of atrial flutter ablation.  In patients who have had both atrial fibrillation and atrial flutter, most electrophysiologists would ablate both. However, in patients with atrial fibrillation, the atrial flutter usually is initiated by trigger spots firing in the left atrium. Once the atrial fibrillation is ablated, the flutter will become less likely. Therefore, there are those who say there's no need to ablate the flutter circuit as well. Alternatively, if a patient has severe comorbidities and/or is high risk for ablation, one may consider performing the atrial flutter ablation only since atrial flutter is harder to manage medically compared with atrial fibrillation.   How do you manage atrial flutter in the acute inpatient setting?  In the inpatient setting, electrical cardioversion is often limited by blood pressure and the hypotensive effects of the sedatives required. If one is awake and too hypotensive, chemical cardioversion can be pursued. The most effective anti-arrhythmic for this is ibutilide. Amiodarone is not effective for acute cardioversion. Since ibutilide prolongs refractoriness in atrial and ventricular tissue, there's a risk of long QT induced torsades de pointes. Živahno dogajanje v okviru srečanja Dobrodošli doma v Novi Gorici in Gorici je bilo tudi v znamenju kulturnega in gospodarskega povezovanja. Formalno je bila nadgrajena Slovenska globalna poslovna povezava. Komu je namenjena? Spoznamo mlade rojake z avstrijske Koroške, s Tržaškega in iz Porabja, nagrajence Urada za Slovencev v zamejstvu in po svetu za magistrska in diplomska dela. Ustavimo se v državnem zboru, kjer je potekalo tradicionalno vseslovensko srečanje in je s svojim razmišljanjem o negovanju slovenske identitete navdušil še en up porabskih Slovencev Mate Labritz. Goran Gorše, ki je pred 15 let ustanovil danes zelo uspešno in uveljavljeno tolkalno zasedbo Sudar Percussion, pa pripoveduje o praznovanju jubileja, povezovanju in tudi političnem delovanju Slovencev na Hrvaškem. Piše Marija Švajncer, bereta Igor Velše in Lidija Hartman. Tatjana Pregl Kobe, pesnica, pisateljica, umetnostna zgodovinarka in esejistka ter dobitnica številnih, tudi mednarodnih nagrad, je v zbirko Pisan cvet na vrtu belih rož uvrstila dve vrsti pesmi: prve, natisnjene z večjimi črkami in brez velike začetnice, napisane v prostem verzu in nekajkrat grafično oblikovane v polkrog, druge pa kot poezijo v prozi. Tako v prvih kot v drugih pesmih je umetničino pesnikovanje subtilno, izrazno bogato in izpovedno globoko. Avtorica trosi življenjske modrosti, filozofska spoznanja in usodne resnice; dokopati se skuša do temeljev stvarnosti. V pesmih v prozi nagovarja različne ustvarjalke, se pogovarja z njimi in izraža razumevanje do njihovih, tudi bolečih življenjskih izkušenj, in pri tem priznava, da pravzaprav govori sama sebi. Sogovornice, ki jih prikliče predse, so bodisi znane in uveljavljene osebnosti bodisi neznane, nekatere od njih dejavne na različnih področjih, pesnice, slikarke, igralke, novinarke, kraljice in še mnogo drugih, tako iz sedanjega časa kot iz preteklosti. To so ženske, ki so se je dotaknile s svojimi življenjskimi zgodbami, iskanjem prostora pod soncem ter razočaranji in krivicami, ki so jih doživele. Med njimi je tudi dekle, ki ji je nekdo storil silo; ubogo, zaznamovano, ranjeno bitje, ki potrebuje objem. Toda pesnica ne izraža samo sočutja in razumevanja, temveč nakazuje tudi upornost. Nekateri verzi govorijo o grdem in nasilnem svetu, vse prevečkrat ohranjeni patriarhalnosti in odrinjenosti žensk. Kot humanistki se ji zahoče drugačnosti, sožitja med ljudmi, sprejemanja in dobrih medčloveških odnosov. Tatjana Pregl Kobe je tudi angažirana pesnica, in to v žlahtnem pomenu besede. Čeprav pozna grozo, samoto, strah in žalost, ohranja upanje o uresničevanju boljšega sveta. V prelepih metaforah se razdaja s posebno živahnostjo in strastjo, predana je ljubezni in stvarem, ki življenju dajejo smisel, ter je pesnica z veliko začetnico in čarodejka lepote. V naravi je zanjo vse resnično in čudežno razgibano. Simbolika dreves in reke slika pesničino notranjost in govori o dogajanju zunaj nje. Toliko vsega vidi, hkrati domišljijsko oblikuje tisto, kar je za druge oči nevidno. Spominja se igrivega otroštva, s posebno milino in v izbranem slogu prikliče v spomin nežnost in drobne radosti iz tistega časa. Poezijo v prozi oblikuje tako, da srečanja z ženskami ujame v kitice, vendar pa se zadnja vrsta nadaljuje v novi kitici ali odstavku. V ležečem tisku navaja dobesedne navedke ter tako znanim in neznanim ženskam omogoči, da same spregovorijo, pomenljivo ali otožno, morda z opozorilom ali napovedjo. Avtorica navede natančen vir in letnico dogajanja oziroma podatek o ženskah, ki jih spominsko ohranja. Prikliče naslednje besede slikarke Ivane Kobilce: » Videla sem svet in življenje. Bilo je lepo in polno sonca. Filozofinja Simone de Beauvoir je v pesničinih očeh iskalka enakopravnega položaja žensk. Igralki Mileni Zupančič je hudo, da ljudje iz njenega življenja kar izginjajo. Grozljivo je, ko ugotavlja, da je napočil dokončen odhod; nikoli več ne bo srečavanja in pogovorov. Literarna zgodovinarka Katja Mihurko je prepričana, da ženska veliko tvega, ko se odloči razpeti krila, toda če tega ne stori, trpi. Tatjana Pregl Kobe občuti doživljanje vseh teh žensk in priznava: »če ne povem resnice o sebi, je ne morem povedati o drugih.« Njene resnice so iskrene, povedno zaokrožene in tudi strastne: »še vedno sem vidna, resnična in živa. / strast se še ni osula. / zardela od ljubljenja / vonjam cvetlični prah, ki lebdi na ramenih / upognjene trave.« Pisanje jo ohranja, pomeni ji odrekanje, samoto in predanost. Vztraja, tudi če ne ve, ali bo kdo prebral njene pesmi. Dokler piše, je to vse, kar ima. Življenje zanjo pravzaprav postane pesem: » Nikoli nisem sama, niti v temnem tunelu niti onkraj noči. Ker je z menoj in v meni pesem. In mi vladajo čustva. In poezija, popolnoma jasna in globoko skrivnostna, čeprav je ni mogoče v celoti pojasniti in je ni mogoče izčrpati.« V postmodernistični maniri citira in oživlja stvaritve drugih. Ponuja verze iz Baudelairovih Rož zla, pogled Mone Lize, Tisnikarjeve podobe smrtne groze in igrivost pravljičnih junakinj. Omeni Kierkegaardove besede o bolezni za smrt. Spomni se na svojo mater, pisateljico Milenko Pungerčar, in na njene zadnje ure življenja. V slovo ji poje in jo drži za roko. Druga svetovna vojna je mater strašno zaznamovala. Njena generacija je bila nadčloveško pogumna in pokončna, pravi avtorica. V pesničinih očeh so ohranjene podobe in barve s platen velikih slikark, glasba zveni in jo izpolnjuje. Minevanje premaguje s predajanjem človeški dobroti, likovnim stvaritvam, naravi, tišini in duhovnemu iskanju. Verjame, da je pomembno tisto, kar ljudje dajejo drug drugemu. V nežnih verzih pojasni naslov zbirke: »ni lahko biti edini pisan cvet / na vrtu belih rož. / vedno sama s seboj. / nenehno osredotočena na bistvo stvari …« V kositrnih jutrih se zave, da se vse začne in konča z rojstvom in smrtjo, življenje pa je nekje vmes, »ko se počutiš božansko, / ko doživljaš trenja in razpoke, / ko vzletaš in lebdiš.« Za hip se ji zazdi, da je vse tako, kot mora biti, z vzponi, padci in zdrsi, toda v eni naslednjih pesmi zapiše, da je čisto sesuta in tudi svet je tak. Povsod vidi smrt in temo ter predvideva, da ju bo slej ko prej še več. Kaj kmalu lepota pripomore, da se vanjo vrneta moč in želja po vztrajanju. Zbirka Pisan cvet na vrtu belih rož je zagotovo ena najlepših in najbolj pretresljivih pesniških stvaritev, ki so v zadnjem času izšle na Slovenskem. Tatjana Pregl Kobe daje svoj glas različnim ženskam. Če pa pomislimo, kako morajo moški današnjega časa zaradi odločitev svojih oblastnikov in nerazumnih okoliščin v vojno in tam pustiti najdragocenejše, kar imajo, svoje življenje, potem bi bili pesničini verzi lahko namenjeni tudi njim in vsem, ki žalujejo za nesmiselno umrlimi svojci. Dejavniki in odločevalci, ki sejejo smrt in druge pehajo vanjo, sami pa so pri tem na varnem, ji na žalost ne bi hoteli prisluhniti.

John Wilson's guest is the pioneering American artist, author and educator Judy Chicago. Having run the first ever feminist art course in California, she established herself as a powerful advocate of women artists in the early 1970s. She is best known for a ground-breaking installation piece called The Dinner Party, a monumental work which was made with the help of a team of ceramists and needle-workers over five years and first displayed in 1979. Now enjoying her sixth decade as an artist, Judy Chicago is regarded as a trailblazing figure in the art world.Judy recalls studying at the Art Institute of Chicago's children's classes at the age of five, and afterwards wandering around the galleries upstairs where she was particularly drawn to the Impressionists. It was here that she first decided to become an artist. As a young woman she moved to the west coast to pursue her dream. Although she found the art scene there "inhospitable" to women, she was inspired by a group of male artists including Ed Rucha, Larry Bell and Bill Al Bengton, associated with the LA-based Ferus gallery. Judy also cites discovering Christine de Pisan, the Italian-born French medieval poet at the court of King Charles VI of France, as a turning point in her own research and art practice. Like Judy herself, de Pisan had faced obstacles because of her gender and sought to challenge contemporary attitudes towards women by creating an allegorical City of Ladies. The works published by Galileo expounding on these findings eventually led to him accused of heresy by the Roman Catholic Church. One of the most common gripes that foreigners have about us Tuscans is that we're so proud of our rich history to never miss the chance to sneak it into any conversation. While this has some merit, it would be quite difficult to act any differently. You see, history for us is not a distant thing, something you read in a book. It's literally everywhere around us, sneaking up on you when you least expect it. Need an example? Let's talk about the odd hospital that is just a stone throw's away from the Leaning Tower in Pisa. No matter how impractical and old-fashioned, Pisani love it so much to have delayed for decades its transfer to a more modern facility. They love it so much because it's possibly the oldest in the world, founded almost eight centuries ago. Not only it's very old, but it's the only one in the world that was born out of a massive naval battle and the clash of two giants of European politics who just hated each other. People in Pisa don't care much about the other hospitals in London or Paris that claim to have been around even longer: as far as they're concerned, the Santa Chiara is the template that was used all over Europe to create hospitals as we know them today. As we promised a couple of weeks ago, time for our second instalment of the mini-series of episodes dedicated to the festivals that populate the calendar of Pisa during the month of June. After waxing lyrical on the Luminara, the amazing light festival that turns the city of the Leaning Tower into a fairytale scenario, time for the big daddy, the main event that closes the month. On the last Saturday of June, teams from the North and South side of the city fight each other on the most ancient bridge trying to push a very heavy cart, in a sort of reverse tug of war. This is probably one of the oddest historical celebrations in the whole of Italy and this should surprise exactly no one. This weird competition was created in the 1940s to replace the real Gioco del Ponte, a simpler and much more violent contest that had been held in the ancient harbour city for centuries. Calling it a "game" is definitely a misnomer: it was in all effects an organised brawl that involved hundreds of fighters per side and could last up to one hour. And yet, Pisans loved it so much that not even a devastating flood could stop them from having their beloved game. Follow us as we delve deep into the history of this brutal clash and how it was crucial in maintaining alive the fighting spirit of the ancient sea power after the devastating defeat against Florence. Looking back at the episodes we released in the past year, we've come to realise with some horror that we've provided you with plenty of legends but not much practical info, the kind of local knowledge that might help anyone who finds himself landing in an airport miles and miles away from his final destination. If you're interested in a bit more than what you can find on the front page of your tourist guide, this land of ours has plenty to offer - probably too much to offer. That's why starting from this week, we will try to help you plan interesting day trips starting from the main tourist centres of Tuscany, giving you the chance to discover little known corners of this land that are able to offer a lot. Our first suggestion is the town of San Giuliano Terme, a few miles from the Pisa airport, a place that is overlooked by many despite having had quite a successful past in terms of tourism. The local spa had been active since the times of the Etruscans but was the third most visited in Europe in the 1700s, when it was the choice of royalty and aristocrats alike. All sorts of beauty treatments are available, including a vapour bath in the secret hammam of the Grand Duke of Tuscany, only recently rediscovered. A mile or something away you can find one of the most beautiful Renaissance villas in Tuscany, Corliano, owned since the 1550s by the same family, which has now turned it into a boutique hotel with a renowned restaurant. The main attraction, though, is the ghost of Teresa, a remarkably intelligent, progressive woman that in the second half of the 18th century was raising eyebrows in town by driving her own carriage and being an outstanding member of many cultural circles. The villa of Corliano hosted in the centuries many famous people, from kings to generals, artists and writers, including the very Mary Shelley, who spent several weeks there in the early 1800s. According to many, it was in Corliano that she knew a Pisan surgeon whose innovative experiments with natural electricity gave her the inspiration to write her most famous book. "Enchantment - a useless thing, but as dispensable as bread" - Gio PontiWelcome to Episode #27 Pisa, Tuscany....I have always been a little intrigued about Pisa.  Wondering to myself what it was all about, so much so I thought we better go.  And it is a curious place, a Tuscan city not far from the sea, a maritime power in its hey day.  The city was built on the legacy of trade and its location on the Fiume Arno (River Arno) the one that connects it to its Tuscan neighbour Florence.  It was a little fun going down the rabbit hole and discovering this place.  The Leaning Tower - Torre Pendente is obviously what sets this city apart, what magnetises the curious traveler.  The thing I love about travel is when something happens that throws you off your tracks, our day in Pisa was full of lovely surprises.  The beauty of the Piazza dei Miracoli - the square of Miracles is something I will not forget.  A wandering city, the river Arno is wide, the beautiful gothic cathedral stands by the river ....the Santa Maria della Spina is a marvel as you meander along the beautiful bridges connecting the two sides of Pisa.The idea of travel now has a luminous quality, after a year of knowing we cannot go, our rosy glasses have had to come off.  We have to appreciate what is within our own personal radius.  I don't mind this, and shall reap the benefits of my own beautiful country, yet Italy still lures me, calls me.  It is only through books and art right now that I can fly away, escape the day to day.  If you have any great books or film or ideas in general on Italy send me a little note on the email.  I would love to hear from you....Send to 23colours@gmail.comMusic Composed by Richard Johnston, Copyright 2021.Please find all Show Notes and details mentioned at: www.michellejohnston.lifeInstagram:  @awriterinitaly and @theyellowhouse__About A Writer in Italy Podcast:Five years ago Michelle Johnston traveled to Italy to create space for herself and her writing.  Michelle traveled solo giving herself time to recalibrate after many years of parenting and life in general.  Leaving her husband to take care of the family she reconnected with her writing, her art and her love of travel.The adventure turned into the book and travel memoir...............Support the show (http://michellejohnston.life)
