Sometimes V-fib doesn't respond to the shock at all. Sometimes it converts and comes right back. Those are two completely different problems. In this episode, Sarah breaks down the science behind why defibrillation fails, how to recognize the difference between refractory and recurrent V-fib, and respond when shocks aren't working. You'll learn how to start approaching the electrical storm at the bedside — that means understanding transthoracic impedance, optimizing your pad placement, and knowing when to reach for double sequential defibrillation.Topics discussed in this episode:Refractory vs. recurrent V-fibThe physiology of ventricular fibrillation and re-entry tachycardiasMisconceptions about defibrillationTransthoracic impedance and how to reduce itAnterior-lateral vs. anterior-posterior pad placementDouble sequential defibrillation: evidence and objectionsKey findings from the DOSE VF trialAmerican Heart Association. Send us Fan MailThere are people in the hospital who walk into the room when everything is falling apart. Not to fix it or rush through, but simply to be there.Today's guest, Mark Wilson, lives in that space—right in the emergency room, where life can change in a single moment. Where nurses are moving fast, families are trying to make sense of the unthinkable, and patients are meeting some of the hardest moments of their lives. But what makes his story different… is that he's been on the other side of the bed. After a severe bicycle accident, Mark suffered a traumatic brain injury and spent months in a rehabilitation hospital, learning how to walk again, relearning his body, his independence—his life. And somewhere in that process, something shifted. What started as survival… became a calling. Now, he shows up for patients and families in crisis as an emergency room chaplain, bringing a kind of presence that only comes from someone who truly understands what it feels like to be vulnerable, uncertain, and afraid. His experience as a former EMT, firefighter and lead pastor, gives him a unique lens on faith, meaning, and what people actually need in moments of crisis—whether they consider themselves spiritual or not. This conversation is for those on the frontlines of healthcare—the nurses and others who journey through the tough times and often carry unprocessed burdens. It's for anyone seeking insight into what truly matters when life hangs in the balance. Send us Fan MailThere's a certain kind of pain a lot of people learn to live with.It starts as something small—a twinge, a tightness, a bad shift at work. And then, slowly, it becomes part of your normal. You stretch it out, push through it, tell yourself it's just part of the job… part of getting older… part of life. And if you work in healthcare, you probably know this story all too well. You spend your days lifting, turning, reaching, carrying—taking care of everyone else—while your own body keeps asking for attention you don't have time to give.So what happens when the pain doesn't go away? When the usual answers stop working? When you start to wonder if this is just how it's going to be?Today's guest has spent over 30 years asking those exact questions—and refusing to accept the idea that chronic back pain is something people just have to live with.Dr. Brigitte Rozenberg is a Doctor of Chiropractic and the founder of Spinatomy Spine & Disc Centers and the creator of Advanced Spinal Restoration Therapy. Her work has helped thousands of people find relief from chronic pain—without surgery, without medication, and without giving up on their lives. We talk about where traditional care gets it right, where it falls short, and why so many people—especially caregivers—end up carrying pain in silence for far too long.If you've been pushing through, powering on, or quietly wondering if things could feel different… this one is for you.In the five-minute snippet: Aye, Chihuahua! Send us Fan MailYou're going to love this one.Today I'm talking with Seth Radnothy, a fellow nurse whose path into healthcare is anything but typical. Before he ever stepped into a hospital, he was serving as a Reconnaissance Marine—and if that doesn't tell you something about his ability to stay calm under pressure, just wait.Seth's worked as a paramedic and in both emergency and perioperative nursing, so he knows what it's like when things get intense fast. These days, he's taken all that experience and built something really unique—Pulsepoint Prose Legal Nurse Consulting—where he helps attorneys make sense of complex medical cases. He also shares what he's learned as a legal nurse consultant with the next generation of nurses as a clinical nursing instructor. And because he values his work/life balance, in his off time, he plays guitar in his band, hits CrossFit, mountain bikes, and hangs out with his wife and their furry crew. This one is about the many ways a nursing career can evolve—and how all of your experiences can come with you. In 2024, she was honored with the Nevada Arts Council Literary Fellowship for her contributions to literature and the arts. But what makes Amanda especially fascinating to me is that before she ever became a full-time writer, she spent 16 years as a registered nurse. And you can feel that in her work. Her stories don't just live in the past—they dig into the human experience of medicine, justice, and belonging… often in the overlooked corners of history where voices have been silenced or forgotten.Today, we talk about that journey—from bedside nursing to bestselling author, how her clinical experiences shaped her storytelling, and what it means to care for people… whether through medicine or through words. In the five-minute snippet: they are just so cute. In fact, Nicole's life as an artist might just be giving her full-time nursing career a run for its money.Since we last talked in episode 46, she hasn't just been creating beautiful art—she's been building pathways for others to reconnect with their own creativity. Her Brave Beginner Art course has expanded, and she's added Creative Thinking in Nursing, both offering continuing education units. I've taken her Brave Beginner course myself, and I can tell you—it's not just about art. It's about rediscovering a part of yourself you may have forgotten was there. And Nicole hasn't lost sight of the bedside. As a former ICU nurse, she's created two powerful resources—The ICU Diary and The Hospital Wellness Journal—designed to support patients and families as they navigate some of the most vulnerable moments of their lives. You've heard me say it before—I deeply admire nurses who lift up other nurses. Nicole has experienced firsthand the impact of creativity: less stress, less anxiety, more joy, and a deeper connection to emotion. Send a textIf nursing is consistently ranked as the most trusted profession in America…Why do so many nurses feel unheard inside their own workplaces? That question has been on my mind.On this podcast: We talk about burnout. We talk about staffing. We talk about leadership. But underneath all of it is something deeper — culture. And culture is hard to change. Not because nurses don't care. Not because leaders don't try.But because culture isn't just policy. It's identity. There are many layers to uncover here, so let's get into it.Notes on Nursing: What it is and What it is Not, by Florence NightingaleContact The Conversing Nurse podcastInstagram: https://www.instagram.com/theconversingnursepodcast/Website: https://theconversingnursepodcast.comYour review is so important to this Indie podcaster! ARDS is one of the more complex syndromes we manage in critical care. More than just pulmonary edema, we are battling stiff lungs, refractory hypoxemia, rising pressures, and frequently making decisions that can either protect the lung or make things worse. In this episode, I'm joined by respiratory therapist Melody Bishop for a deep dive into ARDS ventilator management through the lens of physiology, evidence, and real bedside practice. Send us a textThis week, my guest is Tracy Hicks, a doctorally-prepared and dual-certified family and psychiatric mental health nurse practitioner. She also holds an MBA, which is particularly useful in her role as a CEO.Tracy is the founder of C-Trilogy Comprehensive Clinical Care and C-Trilogy Outreach, a certified community behavioral health clinic that provides mental health services, substance use support, primary care, and 24-hour crisis help.I was excited to talk with Tracy, who is an expert in the care and treatment of individuals with schizophrenia, because in my 36 years as a nurse, I have encountered only two patients with this diagnosis, both of whom were teenagers. I knew very little about the condition and was eager to learn from Tracy, and she did not disappoint. We discussed long-acting injectables as a successful treatment option, the challenges of compliance, and the management of a long-term chronic disease. We also emphasized the crucial importance of a strong support system to help patients lead meaningful lives.A recurring theme in our conversation was Tracy's passion when discussing these complex cases. This emotion stems from her lived experience, as two of her beloved family members—her father and her daughter—both have the disease. Growing up with the stigma of having a father with schizophrenia has shaped her perspective, and even though her daughter is now grown, Tracy's concerns for her never waver.Tracy's days are incredibly busy. Between practicing in the clinic, serving on multiple boards that advocate for marginalized populations, and shaping the next generation of nurses as an Associate Professor, people often ask her how she manages it all. Her answer? When you align with your purpose from God, He will guide your steps. She truly is a blessing to her community. In the five-minute snippet: Be prepared to jump! The science is finally catching up to what clinicians have long known: more fluids aren't always the answer to septic shock. In this episode, host Sarah Lorenzini and Jaclyn Bond MSN-LM, MBA-HM explain what the ANDROMEDA-SHOCK 2 trial reveals about physiology-guided sepsis resuscitation and why fixed-volume fluid strategies can lead to avoidable harm. They break down how dynamic fluid responsiveness testing helps teams stop guessing, and how tools like FloPatch support real-time assessment of carotid flow time and stroke volume. You'll leave with a clearer idea of when to give fluids, when to stop, and how to justify the decision.Topics discussed in this episode:The purpose and key findings of the ANDROMEDA-SHOCK 2 studyWhy dynamic measures of fluid responsiveness matter more than static vitalsWhat recent meta-analysis data shows about physiology-guided fluid strategiesCarotid flow time: what it is, how it's measured, and how it guides decisionsHemodynamic assessment and bedside limitationsHow FloPatch supports real-time assessment so you can make individualized fluid decisionsSEP-1 2026 guideline updates and why it's better for patientsHow to apply these principles to your workflow Website: www.flosonicsmedical.com See FloPatch in action: https://hubs.ly/Q03-68Hg0Mentioned in this episode:CONNECT
Pulmonary embolisms don't always announce themselves... sometimes they ambush. One minute your patient is walking with physical therapy, the next they're hypotensive, hypoxic, and coding. This re-released early episode dives deep into why PE patients can look deceptively stable… right up until they aren't.In this episode, I revisit one of my earliest case-based teachings on pulmonary embolism, updated with an added segment on vasopressin use in obstructive shock from PE. Through real bedside stories from my time as a rapid response and ER nurse, we break down the physiology behind PE-related collapse, why intubation isn't always the answer, and how to think through management when the right ventricle is failing in front of you. This is a sobering but essential refresher on one of the most dangerous diagnoses we encounter.Topics discussed in this episode:Why pulmonary embolism is a common cause of in-hospital cardiac arrest (even if it's not common overall)Classic and subtle PE presentations and why they're often missedA real-time rapid response case: stable to crashing in minutesRisk factors for PE and the anticoagulation double-edged swordObstructive shock explained: what's actually killing the patientRight ventricular failure, septal bowing, and the spiral of deathWhy intubation can worsen outcomes in massive PEVasopressors in PE: norepinephrine, epinephrine, and vasopressinThe unique benefits of vasopressin in obstructive shockThrombolysis vs. thrombectomy: when TPA helps — and when it's deadlyBedside echo findings that point to massive PEWhy PE patients can crash during transport (and what to always bring)Nursing vigilance, rapid escalation, and activating help earlyWhen perfect care still isn't enough and the heart of nursing in end-of-life momentsMentioned in this episode:CONNECT

Send us a textToday's guest is someone whose work sits right at the intersection of education, innovation, and human connection.Dr. Christine “Christy” Heid is a nursing educator, simulationist, innovator, and healthcare advocate with more than two decades of experience. As a Nursing Simulation Specialist, she brings an invaluable perspective as both a nurse educator and simulation expert who has personally experienced the challenges institutions face in preparing nurses for clinical practice. A recognized innovator in nursing education, she created the Heid ATE Guide for Clinical Teaching and Learning©, which is an Innovative, Progressive Tool to facilitate the Development of Clinical Judgment Skills for real-world nursing Practice. Brilliant. But wait, there's more. Christy has led a multi-site research project funded by an OADN Foundation Grant. She has held roles as a consultant, course developer, and faculty member, contributed to national committees, and published research on simulation and clinical judgment. In this episode, we talk about how simulation is transforming nursing education, what it really means to build clinical judgment, and why innovation is as much about empathy as it is about technology. Her work reminds us that innovation and empathy aren't opposites; they're partners.I wish, as a young nursing student in 1983, that Christy had been in my life. We didn't have simulations. Send us a textToday's episode is one that really hits close to the heart — because it's about the people who spend their lives caring for others, and what happens when no one's caring for them. You're about to meet Kevin Dickinson, the founder of GetVitals, and his co-founder and clinical advisor, Amber Smith. They developed a mental health app built specifically for nurses — starting with those working in emergency departments. Kevin didn't come from healthcare; he came from engineering. But after watching Amber, an ER nurse and educator with more than a decade of experience, struggle through exhaustion, compassion fatigue, and burnout, he decided to do something bold: he left his career to build a tool that actually supports the people on the front lines.Together, Kevin and Amber are bringing a deeply personal and powerful perspective to the conversation about mental health in nursing — one that blends love, lived experience, and a shared mission to change the culture of healthcare. We'll discuss what burnout really looks like inside an ER, what inspired GetVitals, and how they're helping nurses take care of themselves with the same compassion they provide their patients every day.So settle in. Send us a textToday, I'm honored to welcome on the podcast, Lori Schellenberg, a registered nurse with nearly 45 years of experience. Lori's career is a true testament to the power of embracing change. She began at the bedside and followed her curiosity into home health, occupational nursing, and workers' compensation—discovering along the way just how transferable nursing skills really are.Through her journey, Lori built partnerships, mentored colleagues, and helped healthcare businesses grow. When retirement left her seeking a new purpose, she turned her focus to patient advocacy, founding Stepping Stone Advocacy Services. And when she noticed nurses themselves struggling with burnout and limited career vision, she created the Stepping Stone Nurse Academy—a platform offering courses, mentorships, and internships to help nurses transition into fulfilling alternative careers.Lori's mission today is dual: to support patients navigating the healthcare system and to empower nurses to realize their untapped potential. Nicotine, NDT Bans, and the Truth Big Pharma Doesn't Want You to Hear In this episode, I sit down with Dr. Bryan Ardis to challenge the mainstream narrative on nicotine and expose what's really behind the push to ban it worldwide. Backed by history, published research, and clinical data, Dr. Ardis explains how nicotine may support recovery from long COVID, reduce inflammation, improve autoimmune conditions, sharpen brain function, and even play a role in cancer prevention. We also cover the FDA's plan to ban natural desiccated thyroid medications and how it connects to larger efforts to restrict patient access to bioidentical hormones and other natural treatments. Dr. Ardis breaks down the financial and political forces at play, and I share my own cancer experience and how this information could shift your approach after a diagnosis. If you're ready to hear the research, the risks, and the real reasons behind these bans, this conversation will give you the clarity you need to protect your health and your options. Why would a healthy newborn suddenly stop eating and start vomiting? When a fussy baby comes into the ER, it's easy to assume it's nothing urgent — like colic or gas. But in today's case, there were small signs that pointed to something more. This episode unpacks the high-risk condition and treatment of neonatal cardiogenic shock with Natalie Pleiman, pediatric nurse and clinical coach. From the early (often misleading) signs to critical assessments, you'll learn what makes cardiogenic shock different in neonates (specifically ductal dependent lesions), what to look for in patient labs, and how to manage the risks of treatment.Tune in for insights that will help you on your next pediatric cardiac emergency!Topics discussed in this episode:Neonatal triage: initial assessment and red flagsPhysical exam and early diagnosticsDifferences in neonatal anatomy and physiologyPreductal vs. postductal vitalsSigns of coarctation of the aortaAssessing for sepsis vs. cardiogenic shockThe dangers of fluid bolusesHow to effectively administer prostaglandinsThe risk and process of neonatal intubationPathophysiology of coarctation of the aortaICU stabilization to optimize cardiac functionKey signs of congenital heart conditionsNatalie's framework for understanding congenital heart defectsConnect with Natalie:https://www.instagram.com/chatwithnat_rn/Listen to Chat with Nurse Nat on Spotify:https://open.spotify.com/show/7Jh2qe44KipudVKkdXFwWHListen to Chat with Nurse Nat on Apple Podcasts:https://podcasts.apple.com/us/podcast/chat-with-nurse-nat/id1815541418Mentioned in this episode:Listen to the In The Heart of Care Podcasthttps://link.cohostpodcasting.com/6598429e-e927-45b0-9b57-7dd34a09d803?d=seASyqjs7CONNECT
Traditional Chinese Medicine and Thyroid Healing: A Conversation with Dr. Chris Motley In this episode, I sit down with Dr. Chris Motley to explore how Traditional Chinese Medicine (TCM) can uncover and address the root causes of thyroid dysfunction. This is the first time on the show we're going deep into the science of acupuncture, meridians, and herbs - explained in a way that connects directly to your thyroid health. Dr. Chris combines the best of Eastern and Western approaches, showing how TCM can work alongside your medications, lab testing, and conventional care. He explains how energy “roadblocks” in the body's electrical pathways can disrupt thyroid function long before labs reveal a problem, and how those disruptions can be linked to unresolved emotional trauma or patterns passed down through generations. We discuss his own mother's experience moving from Hashimoto's to Graves disease and back, the key herbs he uses to support thyroid recovery, and why liver and colon health are critical to the process. He also shares how remote consultations can help identify energy imbalances that standard testing often misses. If you've been looking for an integrative approach that blends clear science with time-tested Eastern practices, this episode will give you practical strategies you can start exploring right away. Methylene Blue, GABA, and What Your Mitochondria Actually Need In this episode, I sit down with Dr. Scott Sherr to unpack two of the most talked about tools in metabolic and brain health: methylene blue and GABA. If you're dealing with supplement fatigue, burnout recovery, post-surgical healing, thyroid issues, or perimenopausal symptoms that no one seems to have answers for, this conversation is the reset you've been looking for. We start with methylene blue and why it's gaining attention, what it actually does at the cellular level, and how it supports energy, detox, and overall resilience. Dr. Sherr breaks down how to use it safely, what to look for in a quality product, and how to avoid common mistakes. Then we shift into nervous system support, specifically GABA, and how it influences anxiety, sleep, and focus. Dr. Sherr shares his approach to calming down the stress response and why supporting both energy and relaxation is essential for long-term health. We cover how these two tools can impact everything from mood and metabolism to brain clarity and hormone balance. If you've been trying to piece together a supplement plan that actually works for your body, and your life, this episode is designed to help you simplify and move forward with clarity. This episode is your straight-up crash course on perimenopause... what it is, how to know you're in it, and what to do when your body starts rewriting the rules without your permission. I'm joined by Dr. Amy Killen, who doesn't sugarcoat a thing. Together, we unpack the early signs of hormonal shifts, the symptoms most doctors dismiss, and why perimenopause deserves real medical attention, not another vague suggestion to “just get more sleep.” We get into the real reasons behind the mood swings, sleep disruption, joint pain, and motivation crashes. We cover which labs are worth running, how to interpret them, and why testosterone and progesterone matter long before menopause officially shows up. We also talk through the decision-making process around hormone therapy, when to start, how to cycle it, and what the latest research actually says about safety, cancer risk, and long-term benefits. If you've been told to wait until menopause to care about your hormones, this is your correction. No confusion, no guesswork, no being dismissed. Just the facts, the options, and the tools to make informed decisions that actually support your health. This is the conversation you should've had years ago. Let's get you caught up. I finally get to dive deep with the queen of hormones herself, Dr. Carrie Jones! We're breaking down estrogen dominance, estrogen detoxification and what all this really means for women navigating perimenopause, menopause, or recovering from hormone-driven cancers. If you've ever stared at your Dutch test results like they're written in another language or questioned whether your hormone therapy is safe, this is the episode you need in your life. Carrie explains how estrogen is processed in the body, why detoxification matters for symptoms and long-term health (spoiler: it's about way more than just estrogen), and why you don't need to fear hormone therapy, even with a cancer history. Carrie then unpacks the difference between estrogen dominance and bad detox pathways, how to test and interpret your results, the exact nutrients and supplements to help your system handle hormones like a pro, and why constipation is basically your mortal enemy when it comes to hormone health. We bust the myths on “bioidentical” versus synthetic hormones, discuss the hot debate on estradiol vs. estriol, and share real solutions if you're tired of bloat, moodiness, or stubborn pounds. If you want actionable advice, up-to-date info, and want to make sense of hormone chaos, tune in now and start fixing what's really driving your symptoms! We crack open why rates of depression, anxiety, ADHD, autism, and even Alzheimer's are skyrocketing in our modern world. No, it's not just “hormones”, it's a tangled web of gut health, toxins, heavy metals, processed foods, and, believe it or not, little parasites controlling more than just your digestive tract. But I'm not leaving you swirling in stress, we've got solutions! We get practical, talking about targeted supplements, the undeniable gut-brain connection, and the absolute essentials you need to detox and fuel your brain for peak performance. We expose hidden threats in your “healthy” foods and break down realistic, actionable steps to reclaim your mind, your mood, and your quality of life, even if your thyroid's already optimized.
