Tre unga män som ska skyddas från olika gäng placeras i samma stödboende. Lyssna på alla avsnitt i Sveriges Radio Play. ”SOS 112 vad har inträffat?”En ung kvinna, märkbart skärrad, har ringt 112.”Vad är det som har hänt?”Det är mitt i natten en kväll i september.”Ja men lugn, vi ska hjälpa till här.”Skrik och rop på gatan gör att grannar reagera och tittar ut. I mörkret ser de ett gäng killar och någon skriker ”Jag ska döda dig”.”Berätta för mig vad du ser. Vet ni om någon är skadad?”Personerna på gatan springer plötsligt iväg, men en kille ligger kvar.Den unga tjejen går ut med larmoperatören i telefon.”De är på väg, de måste ligga still där, håll tryck på blödningen där.”Ambulans och polis kommer till platsen och polisen Fredrik blir förundersökningsledare:– Och han är väl ganska illa däran redan ambulansen kommer dit. Men jag tror att de dödförklarar honom på sjukhus då, på akuten.”Ett rörigt ärende”Flera av de inblandade på båda sidor i bråket är skadade. En yxa och flera knivar har använts. Fredriks uppgift blir att försöka bevisa vem som gjorde vad den där natten.– Det var ett rörigt ärende och ska man erkänna så från början så tänkte man väl typ att det här kommer vi aldrig kunna reda ut.Det här är inte bara en vanlig gänguppgörelse i den långa raden av mord under våldsvågen – tre av de häktade är avhoppare som samhället betalar miljonbelopp för skydda.En av killarna kallar vi Adam, honom hittar polisen blodig i sängen i sin lägenhet – precis vid brottsplatsen.Adam kommer till Sverige som sexåring, han växer upp i en mindre svensk kommun med sin mamma, sin styvpappa och så småningom en lillebror. Han spelar fotboll i den lokala klubben, är musikintresserad och när han går på högstadiet startar han ett konto för att dela pranks och utmaningar.Det är mycket konflikter hemma och när han är 16 år placeras han för första gången i ett jourhem.Ett år senare stoppar polisen honom på en busshållplats och skriver i sin rapport att det doftade mycket kraftigt av cannabis. Urinprov från hans jourhem visar att han även använder kokain, benso och Tramadol.Mindre än ett år senare griper polisen honom igen, då på ett hotellrum med narkotika och pengar. Polisen larmar socialtjänsten om att Adam umgås med välkända kriminella.Under våldsvågen blir Adam vittne till ett uppmärksammat mord. Han är då 19 år och vill han bryta med sitt gamla liv. Och socialtjänsten börjar leta efter en skyddsplacering.Stora behovVåldsvågen - med allt yngre förövare - har lett till ett skriande behov av platser på Sis, HVB-hem och även i avhopparverksamhet.I en konferenslokal vid Odenplan i Stockholm befinner sig närmare 200 socialsekreterare och en rad privata omsorgsföretag.Annelie Blomqvist jobbar på HVB-guiden.– Socialtjänsten behöver ju hitta hela tiden nya verksamheter eller nya ställen där de kan placera sina klienter. Så här får ju de träffas på kan man säga neutral mark och kunna skapa kontakter egentligen ett nätverk.Här precis närmast ingången står företaget Patia, de bjuder på godis och energidryck. Vi frågar:Vad säljer ni här idag? Vad vill ni erbjuda socialsekreterarna?– Vi har både stödboende, träningslägenheter, speciallösningar, familjehem... Men sen har vi också... Vad har jag missat? Avhopparverksamhet!Patia beskriver att de har ett rikstäckande stödprogram för personer som vill lämna en kriminell livsstil. De kan få bo i skyddslägenheter och bemannade stödboenden på flera orter i Sverige. Här köper Adams socialtjänst en plats för att han ska få hjälp och skydd.Ska inte vara nära narkotikaPå stödboendet där Adam hamnar arbetar Hanna, som vi kallar henne. Hon berättar varför hon började jobba där:– Just att man kan få till en förändring i en människas liv, ge de här ungdomarna saker som de inte har fått under sin uppväxt, kontinuitet, stabilitet, någon som står kvar framför allt och inte sviker, få dem att lita på vuxenvärlden– Man är ju inte där för pengarna liksom då hade man varit någon helt annanstansI Adams vårdplan står det bland annat att han inte ska ”vistas i riskfyllda miljöer bland individer som brukar narkotika och begår brott.”I samma hus där Patia ger Adam en lägenhet har de också placerat två killar som också lever under hot, fast från andra gäng.– Det ledde till att de blev vänner och precis som det blir när man sitter inne på anstalt att de lärde sig av varandra, de umgicks och påverkade varandra väldigt negativt.På stödboendet bor också andra ungdomar. Men en viktig skillnad finns – för avhoppare får Patia mer betalt:– Det är en mycket högre prislapp på en som är klassificerad som avhoppare än en vanlig stödboendeplacerad ungdom. Då var det att de skulle få x antal timmar terapi per vecka, strukturerad aktivitet per vecka...alltså det var väldigt mycket mer rigid planering, mycket mer noggrant med tanke på timpriset.Socialtjänsten betonar hur viktigt det är att en av killarna ska kunna gå klart gymnasiet, men trots det går det en hel termin utan att Patia ordnar en plats.Samma kontaktpersonPolisutredaren Fredrik får en bild av att de tre killarna inte har särskilt mycket att göra på dagarna:– Nej men jag tror att de hade väl lite för mycket fritid. De skulle nog behövt gå i skolan eller gå klart sin gymnasieutbildning eller någon lärlingsplats kanske.De har – alla tre - samma kontaktperson. Men i stället för att försöka hålla isär dem för han dem samman - på gemensamma aktiviteter. En gång åker de till badhuset tillsammans – en annan gång för att köpa pizza.På det här sättet kunde kontaktpersonen redovisa att han spenderat tid med var och en av sina klienter, trots att man umgåtts i grupp.– Det var ju helt klart för ekonomisk vinning att man sammanförde dem. De fick ju inget egentligen av det de behövde, inget alls.Stödboendet ligger i ett hyreshusområde med smutsiga fasader. Lekplatsen bredvid kallas av boende i närheten för knarklekplatsen, eftersom föräldrar brukar hitta kanyler där.En sommarkväll minns Hanna särskilt väl:– Det här var en lördag jag jobbade, jag hade fått både sms och samtal från grannar för att det hade luktat cannabis i trappuppgången, det var högljutt och det var uppenbart en fest som pågick sent på natten.Det är inte första gången grannarna klagar. Det har under flera års tid kommit in mängder med klagomål till hyresvärden och till Inspektionen för vård och omsorg – IVO – om högljudda fester och att det luktar hasch.”Helt okontrollerat”När vi försöker prata några av grannarna är det ingen som vill tala öppet. Det har varit stödboenden här länge men allt blev värre när Patia kom menar en granne:– Ja, det blev sämre. Det var helt okontrollerat. Det är helt okontrollerat.Hanna säger att hon får höra av sin chef att grannarna är gnälliga, men hon menar att klagomålen verkligen inte var överdrivna.Hon berättar att den här lördagsnatten ringer hon Adams kontaktperson om den stökiga festen men att han inte ser någon anledning att åka dit:– Och sen dagen efter så kommer kontaktpersonen till mig på kontoret och säger att nej, jag har tagit urinprov på de här tre killarna nu och alla är rena, så att det har inte försiggått något intag av droger.– Vilket vi tyckte var väldigt väldigt konstigt för vi vi kände ju på lukten och såg ju vad som pågick liksom.Efter den här festen flyttas Adam till en ny lägenhet, som inte ligger i samma hus, men i samma område.Men det dröjer inte länge innan det ringer hos Patia igen. Den här gången är det polisen.– Patia får ett samtal att det har varit en ett mord eller någon har dött som är placerad på Patia, först tror man att det är den här killen som har blivit mördad men sen får man reda på att det är han och eventuellt då två andra som har haft ihjäl en annan kille.Flera häktadeNu sitter plötsligt flera av Patias klienter häktade – misstänkta för mord. Polisutredningen leds av Fredrik och han upptäcker att de tre avhopparna inte bara tog droger:– Och utredningen visar ju att de sålde ju knark. De gick ju med sådana här, som vi säger, becknarväskor. Vi tog in mycket filmmaterial så jag ser att de går med sina becknarväskor och de säger ju själva i förhör att de sålde droger och köpte droger.Adam har försökt kräva in en drogskuld och det har lett till flera bråk. Den här natten tar killen – som senare blir dödad – med sig sina kompisar och letar efter Adam.De är runt tio personer från båda sidor som möts utanför Adams lägenhet, han har bett sina vänner ta med sig knivar och tar själv med sig en kökskniv. Adams kompis, hamnar på marken och får slag och sparkar.– Och så hugger han en av de här killarna i ryggen. Två gånger... Sista gången så sitter kniven kvar.En av grannarna som ringt efter ambulans går ut på gatan.”Kan du fråga om han är skadad? Vi ska hjälpa till där”Hon ser en kniv ligga på marken. En kille blöder jättemycket, det är Adam som har huggit honom i ryggen:– Så att han själv erkänner ju. Men han hävdar nödvärn. Och vi kunde bevisa att det var han som hade gjort det här dödande hugget.Adams kontaktperson besöker honom i häktet och kallas även till polisen som vittne, i förhöret får han frågan om sin roll i verksamheten:– Jag är samordnare och kontaktperson på företaget Patia AB på deras avhopparverksamhet. Jag är beteendevetare och terapeut. Mitt uppdrag är att jobba med deras beteende, samt återintegrera i samhället.Enligt andra placerade ungdomar – som vi har pratat med – skryter killarna om att deras kontaktperson mörkade resultaten på deras drogtester för socialtjänsten.– Syftet var väl att ha kvar placeringarna för det här är ju ett drogfritt boende”Var ju jättefulla”Socialtjänsten hade som krav att placeringarna skulle vara drogfria. Men i domstolshandlingar kan vi se att socialtjänsten inte fick veta vad som pågick. Där står också att killarnas utveckling beskrevs som positiv av Patia.Trots att polisens tester visar att killarna rökt på före dödsmisshandeln, hävdar en av killarna i förhör att han varit drogfri under hela sommaren och att de kan kolla det med hans kontaktperson.Polisen Fredrik får bilden av att de här killarna har kunnat festa mycket:– Och de var ju jättefulla. Det säger de ju själva. Och de hade väl rökt på också. Och just i den lägenheten var det en hel del yxhugg inne i lägenheten och att man kan ha en yxa i lägenheten om man nu är placerad på ett speciellt boende. Det borde man ju kanske reagera på.Det visar sig att en av killarna inte ens bor i sin lägenhet på stödboendet – utan hyr ut den.– Jag tror det var fem eller sex tusen kronor i månaden. Och då kan man ju undra om man nu har kontroll på de här killarna och besöker dem regelbundet, så borde man ju inse att den här killen bor ju inte ens här.Killen bor istället hos sin flickvän som också är placerad hos Patia. Där hittar man också en skyddsväst slängd på golvet.– Sådana grejer har ju, ja det är klart att man tycker att det är anmärkningsvärt.På Patias stödboende erbjuds inget krisstöd och Hanna upplever att cheferna försöker tysta ner det som hänt.– Det är en människa som har dött – vad gör vi liksom – men det var en väldigt laid back manana-attityd.Hon har nu förstått att avhopparnas kontaktperson också är deras terapeut. Han har dessutom spelat en stor roll för att få hit klienterna:– Han hade bra kontakter, med tydligen socialsekreterare och polisen, och att han då kunde fixa in avhopparklienter till Patia.Kniv från IkeaDet var flera knivar som användes i bråket. Jag visar Hanna en bild på kniven som gav det dödande hugget:– Ja, det är den kniven man får när man flyttar in på Patia, i det här kökskittet så får de de här sakerna från Ikea, bland annat är de här två knivarna som ingår och ja…jag är omskakad och ja, jag har fortfarande inte smält att jag har jobbat där det känns det känns fruktansvärt.Enligt lag ska den som bedriver vård och omsorg anmäla allvarliga missförhållanden eller händelser där det funnits risk för det till Inspektionen för vård och omsorg, Ivo – en så kallad Lex Sarah-anmälan.Trots att en person dött i anslutning till Patias verksamhet, att Adam kunnat sälja droger under placeringen och att han och andra ungdomar har kunnat missbruka – så görs ingen anmälan.– Alltså det handlar ju bara om att undanhålla allt för Ivo, det är ju så man jobbar, det är ju ett systemfel liksom som är gigantiskt.Vi pratar med killarnas kontaktperson. Han förnekar att han skulle ha agerat på det sätt som beskrivs.”Fick inget gehör”Vi söker Patias VD Janne Menander, som vill att vi mejlar frågor.Flera tidigare chefer och anställda på Patia berättar hur företaget försökt tysta ner allvarliga händelser som sexuella övergrepp och våldsdåd, på andra platser i landet.De flesta vill bara prata med oss anonymt, de hänvisar till att de skrivit på tystnadsavtal – bara en vågar berätta öppet:– Lovisa Broström heter jag. Jag är socionom sedan 16 år tillbaka.Lovisa började jobba på Patia 2019.– Jag upptäckte ganska snabbt att det var väldigt dåligt på väldigt många olika nivåer. Utan det var väldigt tydligt att det var fokus på profit och marknadsföring. Och inte på den kvalitet som ändå utlovades.– Och jag fick inget gehör när jag tog det uppåt eller tog det vidare. Det var så mycket som lovades i ramavtal som faktiskt inte fanns. Eller som man sa att ”vi kan skriva så här”.Lovisa hade hand om deras skyddsplaceringar och besökte deras boenden i hela landet:– När jag var där så blev det också tal om att ta emot kriminella avhoppare. Då kände jag att alla varningssignaler blinkar för fullt. För det är ju just det här att man gamblar med liv och död. Att man tar risker. Att man placerar någon i skyddade boende som inte är tillräckligt säkra.– Och samma sak där, att stoppa ihop personer som kanske inte ska ha samröre med varandra till exempel heller. Det kan ju också bli potentiellt farligt.”Man tackar ja först”Det går bra för Patia, företaget växer i hög takt och omsatte 2023 mer än en kvarts miljard.Lovisa säger att det hela tiden handlar om att få placeringar, företaget påstår att man har personal – fast man inte har det.– Utan man tackar ja först. Och sen så med alla medel så ser man till att det löser sig till varje pris.– Det fanns inte tillräckligt kompetent personal till exempel med den erfarenhet som krävs för att hantera sådana här komplexa ärenden.På stödboendet där Adam placeras har Patia anställt två personer som båda är dömda för brott. För drygt 10 år sedan dömdes en av dem för narkotikabrott och vapenbrott, den andra för grov kvinnofridskränkning – hans ex och barn har själva bott på ett skyddat boende för att skyddas från honom, men han fick jobb på Patia Skydd. Något som Hanna reagerade på.– Det känns ju inte jättebra, det känns helt galet.De dömda är vänner till Adams kontaktperson.Lovisa stod till slut inte ut med alla brister hon såg i skyddsverksamheten hos Patia. Och innan de startade sin avhopparverksamhet slutade hon.– Ja, jag sa upp mig utan att ha något annat jobb. Det har jag aldrig gjort förut, men det gjorde jag då.Varför gjorde du det?– Därför att jag som både socialarbetare och medmänniska inte kan stå för den här typen av arbete. Och att skattemedel behandlas på det här viset är i min värld inte förenligt med en socialarbetarkodex. Eller hur samhället ska fungera.Jag berättar vad som hände när Adam och de andra killarna.– Det är väl exakt det här som är liksom skräckexempel på vad som faktiskt kan hända. Här kan det bli en fara för allmänheten. Om man sätter en person i en lägenhet. Och inte liksom lever upp till det man lovar, säger Lovisa.Patia tillbakavisarVi får mejl från Patia. De skriver att de inte vill kommentera händelserna vi granskat, med hänvisning till sekretess, men att man alltid utreder allvarliga händelser i samverkan med berörda myndigheter, och att man inte känner igen den bild som förmedlas.Patia tillbakavisar uppgifterna om att klienter förts samman av ekonomiska skäl, och skriver att det alltid görs en riskanalys inför en placering. Patia har ”en nolltolerans mot våld och kriminalitet och arbetar systematiskt med säkerhet, oavsett sammanhang”.Företaget uppger dessutom att deras verksamhet är präglad av engagemang, kvalitet och professionalism och att man har ungefär 1000 uppföljningsmöten, tillsyner och granskningar varje år från olika myndigheter, och att det visar att man inte mörkat några missförhållanden.Adams kontaktperson slutade på Patia efter den har händelsen och företaget väljer också att flytta avhopparverksamheten från stödboendet.”Blev sämre för killarna”Adam, som ville lämna en kriminell livsstil och frivilligt sökte hjälp, var tidigare dömd för narkotikabrott. Nu är han dömd till 6 års fängelse för dråp. De andra inblandade dömdes för grovt brott mot knivlagen och narkotikabrott. – De fick ju inte den hjälpen som socialtjänsten har köpt. Det slutade med ett dödsfall och fängelsestraff och domar för alla tre så det blev ju mycket sämre för de här killarna än vad det var när de kom, deras framtid är ju mörkare nu än den var då.Granskningen bygger på en rad intervjuer, samtal och olika typer av dokumentation.Hanna och Adam, heter egentligen något annat. Adams och kontaktpersonens röster är utbytta.

If you like what you are reading, please consider subscribing and/or sharing!Content note: this piece will discuss research around binge eating, including behaviors and frequency. In part 1, we looked at the authors and methodology of a 2024 study called “Evaluating dietary restriction as a maintaining factor in binge-eating disorder” by Bartholomay et al., whose authors claim that the findings “challenge the assumption that dietary restriction maintains BE [binge eating] among all individuals with BED [Binge Eating Disorder]. Rather, results suggest that dietary restriction may be largely unrelated to BE maintenance in this population, and that reducing dietary restriction generally does not have the intended effect on BE frequency.”Too Long Didn't Read Version:When analyzing research, it's important to check to see if the research that the authors cite to support their claims actually does support their claims. In this case, these authors are drawing large, broad-based conclusions that would support a weight-centric/weight-loss paradigm based on small, short-term, and decades old studies that offer only weak conclusions. They are also completely ignoring the success of a weight-neutral intervention that is included in on of the studies they cite. Let's dig in!When I teach research analysis, I recommend checking something I call “The Do-Do's” which encompasses two questions: Do the citations support the claims? Do the study data support the results?In this part of our three-part analysis, we're going to examine the first question. Regular readers may have noticed me pointing out in my analyses how often the studies that are cited don't support the claims. In the case of this study, the question “do the citations support the claims” is particularly important since they are using them to bolster a very limited experimental design, so I've devoted this section to some examples.Bartholomay et al. claim:“randomized controlled trials conducted among individuals with overw*ight and ob*sity demonstrate that assignment to a low-calorie weight loss diet (e.g., 1200 calories per day) produced greater decreases in binge-eating symptoms than assignment to a wait list control group (Goodrick et al., 1998; Klem et al., 1997; Reeves et al., 2001).”What I want to note here is that the study authors have created a false binary between low calorie weight loss diets and wait list control group which would be bad enough. In this case, they've done so by blatantly ignoring the inclusion of a weight-neutral intervention in the very first study they cited.Let's look at the three studies they cited to, in theory, support this claim:The first study they cite is1998 Nondieting versus dieting treatment for overw*ight binge-eating womenG K Goodrick 1, W S Poston 2nd, K T Kimball, R S Reeves, J P ForeytDOI: 10.1037//0022-006x.66.2.363This study evaluated non dieting vs dieting treatment for what they described as “overw*ight binge-eating women.” There were 219 subjects and they were assigned randomly to diet treatment, non-diet treatment, or wait-list control (which is a form of control group in which participants are told that they were on a wait list for the intervention, while being offered no intervention.)The diet intervention was a restricted diet “reinforced with behavioral strategies” and the non-diet treatment was “therapy designed to help participants break out of their dieting cycles.” It involved 18 months of contact - 6 months of treatment followed by 26 bi-weekly maintenance meetings. They checked in on the subjects 6 months later and 18 months later and at both follow-up points the Binge Eating Scale of both the diet and non-diet group was significantly better than the weight list group and at 18 months both experienced weight gain. They concluded that “Results indicate that neither intervention was successful in producing short- or long-term weight loss.”I would suggest that this means that the weight loss intervention is a failure since its goal was weight loss and binge eating reduction but the non-diet intervention was a success since it's goal was to reduce binge eating disorder. I would also suggest that it may have been more effective if the therapy included more specific techniques to reduce binge eating.What it means for sure is that a higher-weight cis woman (the only group included in the sample and thus the only group we can draw conclusions about) who wants to reduce binge eating without risking the experience of another failed diet (and the physical and psychological risks that can come with it) would be best served by a non-diet approach.Second, they cite:1997 A descriptive study of individuals successful at long-term maintenance of substantial weight lossM L Klem 1, R R Wing, M T McGuire, H M Seagle, J O HillDOI: 10.1093/ajcn/66.2.239The study discusses the ridiculous National Weight Control Registry (NWCR) which I often use as a peak example of the embarrassingly poor methodology that gets passed off as research in the weight-centric paradigm. I've written about it in detail previously but the short story is that they collect commonalities among an incredibly small group of dieters (literally 10,000 out of what is estimated to be over a billion attmpts) and then make unsupportable claims about those commonalities. I am unclear why Bartholomay et al cited this since the only reference to binge eating disorders is two studies that happened to include those who binge that tracked the mean lifetime weight loss of participants in order to compare it to the mean lifetime loss claimed in the NWCR. Not only could I find no claim here about whether weight loss attempts impacted binge behaviors, what I did find was that:”20% of the sample indicated a worsening in time spent thinking about weight and 14% reported a worsening in time spent thinking about food.”I would argue that, being as generous as I possibly can, this study has nothing to do with the authors claims and, being a bit less generous, this could be seen as a bit of a refutation of the claims.And this is why we check the references. Finally they cite2001 Nutrient intake of ob*se female binge eatersR S Reeves 1, R S McPherson, M Z Nichaman, R B Harrist, J P Foreyt, G K GoodrickDOI: 10.1016/S0002-8223(01)00055-4In this very small, short study 46 cis women were given a behavioral self-management intervention (6 months of weekly, 1-hour classes taught by registered dietitians) and the other group was a wait list control group. The researchers wanted to measure any change in calories consumed, percentages of calories from fat, protein, and carbohydrates, grams of fiber per 1,000 calories and change in number of self-reported binge days.After 6 months they found no significant difference between nutrients in either group. The behavioral self management group reported a greater reduction in binge days than the control group.Their conclusion was quite weak, finding that “Our results suggest that collecting dietary information from participants identified with binge eating disorder is challenging. Dietitians who conduct behavioral weight management programs may require additional training in identifying and understanding the psychological characteristics of participants with binge-eating disorder.”For this conclusion to have merit, behavioral weight management programs would have to be effective and there is no research supporting that and plenty suggesting that it is not.Overall these studies are small, short term, and quite old. The most recent was conducted about 24 years ago. It makes me wonder if there really isn't more recent data, or if more recent data exists but didn't support their conclusion? Also, remember that Barholomay et al. claimed “randomized controlled trials conducted among individuals with overw*ight and ob*sity demonstrate that assignment to a low-calorie weight loss diet (e.g., 1200 calories per day) produced greater decreases in binge-eating symptoms than assignment to a wait list control group.”I think their claim is seriously overstated. First of all, they should have been clear that this was the case in short-term, small sample studies. They also failed to mention that a non-diet treatment also produced greater decreases in binge-eating symptoms than in weight list control group - I wonder why they left that out of everything?Ok, let's look at another claim from Bartholomay et al.. They claim that adolescent girls with BN (which they explain is “bulimia nervosa, a disorder characterized by both binge eating and extreme compensatory behaviors; American Psychiatric Association, 2013”) who were assigned to participate in a healthy dieting intervention promoting weight control through moderate reductions in caloric intake, exhibited greater reductions in binge eating than girls who were assigned to a wait list control group.The first issue here is the claim that there is any such thing as a “healthy dieting intervention.” This is, in fact, a claim and not a fact, and it requires evidentiary support, which they don't offer. They don't even offer a definition of “healthy” nor proof that this intervention is, in fact, healthy by any definition. Again as we often see in research propping up the weight loss paradigm the authors have substituted “just saying stuff” for anything resembling scientific precision. In this case they are repeating a mistake originally made in the 2006 study they are citing by Burton and Stice called “Evaluation of a healthy-weight treatment program for bulimia nervosa: a preliminary randomized trial.” This study included 85 cisgender female participants with “full- and sub-threshold bulimia nervosa” who were randomly assigned to a 6-session “healthy dieting intervention” or a waitlist control group and assessed after a 3 month follow up. Their conclusion was that “These preliminary results suggest that this intervention shows potential for the treatment of bulimia nervosa and may be worthy of future refinement and evaluation. Results also provide experimental evidence that dieting behaviors do not maintain bulimia nervosa, suggesting the need to reconsider maintenance models for this eating disorder.”In using this study to support their claim, Bartholomay et al seem to be heavily glossing over words like “preliminary” and “may be worthy.” Also, even if we believe the results (without a deep dive into them) the fact that people on diets can suppress binging (or report suppressing binging) over 9 months is a far cry from showing what impact that restriction has over the long term.Back to Bartholomay et al., they make another claim that“Importantly, although longitudinal studies indicate that self reported dietary restraint predicts the future onset of binge-eating symptoms (Field et al., 1999; Killen et al., 1994; Stice, 2001; Stice et al., 2002), these results stand in stark contrast with findings from experimental treatment studies testing the causal effect of dietary restriction and restraint on the maintenance of binge eating.”This is an odd claim that, to me, is made with far too much confidence/bias. Longitudinal studies look at a longer time period, often quite a bit longer, than experimental treatments. Bartholomay et al. do not have a method to determine whether any difference is due to experimental design or simply due to the fact that experimental treatment studies capture a much shorter time frame. If what is true is that people who are on diets can suppress binge behaviors short term, but that the restriction drives additional binge behavior long-term, then the longitudinal studies could be more accurate.Let me offer an example to help illustrate the issue: It takes from 10-40 years for symptoms of asbestos conditions to appear. Let's stay that a study claimed “Importantly, although longitudinal studies indicate that exposure to asbestos causes mesothelioma, these results stand in stark contrast with findings from [6 month - 1.5 year] experimental treatment studies testing the causal effect of asbestos on mesothelioma, which found no relationship.”If the effect takes longer to appear than the time over which the experiments are conducted, there is a significant risk of missing the effect. That effect may be identified by longitudinal studies.Overall, I don't think the studies they are citing come anywhere close to, as they claim:“challenge the assumption that dietary restriction maintains BE [binge eating] among all individuals with BED [Binge Eating Disorder]. Rather, results suggest that dietary restriction may be largely unrelated to BE maintenance in this population, and that reducing dietary restriction generally does not have the intended effect on BE frequency.”In part 3 we'll investigate to see if the rest of the article's data supports their conclusions.
