In this episode of the PFC Podcast, Dennis and Doug delve into the complexities of pneumothorax and hemothorax, discussing when treatment is necessary, the implications of tension physiology, and the management of trauma patients. They explore the differences between spontaneous and traumatic pneumothorax, the importance of patient assessment, and the medical management strategies for these conditions. The conversation emphasizes the need for vigilance in monitoring patients and making timely decisions regarding interventions such as chest tube insertion. Takeaways Tension physiology occurs when blood return to the heart is impaired. Pneumothorax can be observed if it is less than 30% of lung volume in stable patients. Healthy lungs tolerate losing lung volume better than sick lungs. Trauma is a common cause of pneumothorax, but COVID-19 also contributed significantly. Ultrasound is the gold standard for diagnosing pneumothorax and hemothorax. Medical management of pneumothorax involves minimizing positive pressure ventilation. Chest tubes are indicated when pneumothorax significantly affects oxygenation. Tension hemothorax requires urgent intervention to prevent hypovolemic shock. Young practitioners should be aware of complications associated with chest trauma. Vigilance is key in monitoring patients with chest trauma for deterioration. In this episode we are talking again with our audio editor Jack Scaro.  The topic again is: spontaneous pneumothorax, or collapsed lung. Spontaneous pneumothorax is an abnormal condition of the lung characterized by the collection of gas in the pleural space between the lungs and the chest wall. This condition occurs without an obvious etiology and can be classified as either primary or secondary. Patients may present with symptoms such as tachycardia and dyspnea. The diagnosis is based on clinical suspicion and can be confirmed with imaging.  Jack had this condition which surprised everyone since he was a healthy 20 year old with no risk factors except.... you'll have to listen to this week's episode to find out what it is and whether you or a loved one could be at risk!

In this episode we're going to cover the ‘atraumatic' or ‘spontaneous' pneumothoraces and focus on some new key guidelines from the British Thoracic Society which came out in July this year and also look at the relevant evidence on the topic. There are pretty significant changes in the BTS guidance, it's no longer about finding a pneumothorax, working out if it's primary or secondary and then acting dependant on the size. It's now moved more towards looking at how the patient is clinically, taking into account the symptomatology, any big risk characteristics, whether it's primary or secondary and then thinking about the patients wishes and priorities and nuancing the management plan towards those.  This episode builds on some of the concepts we discussed in our Traumatic Pneumothorax podcast, so make sure you give that one a listen before clicking play on this one! We'll be looking at the presentation, evidence, management and follow up, along with some trials that you can get involved in to help develop practice even further. Today, we're diving deep into pneumothorax and hemothorax. Pneumothorax can quickly become an emergency, especially when it's not about the pressure but the declining SPO2. But where's the line between tension and hemothoraces? Our ICU doc expert clarifies that it's all about the impaired blood return to the heart. When your preload drops, it affects the cardiac output. If vitals are stable, though, it's not tension physiology. Also, did you know there are folks walking around with untreated pneumothoraces? The literature suggests only treating those affecting over 30% of the thoracic cavity. And, oh boy, many pneumothoraces come from trauma, but spontaneous cases exist. Especially with COVID, the lung scarring has made things fragile. Add trauma and narcotics, and things can get dicey. But remember, before deciding to treat a pneumothorax, be sure it's the right move. Better safe than sorry! In this Shoptalk, I will be sharing with you some critical advice regarding safety when needling GB 21 and the upper trapezius region generally.My advice comes from having served as an expert witness on a half-dozen cases in which well-intentioned and experienced acupuncturists, trained according to the current standards of the profession, nevertheless caused pneumothorax injuries–one of them fatal.What's an expert witness? A neutral professional who reviews evidence and renders opinions in a court proceeding as to whether the acupuncturists' care was up to standards of the profession–and if not, whether the substandard practice contributed to injuries, pain and suffering. Reviewing these cases has caused me to question some commonly-taught, widely-practiced, but unsafe techniques, and in my own practice, replace them with needle angles, lengths and depths, as well as screening protocols for risk factors, and other techniques that are much better at preventing pneumothoraxes, and yet just as clinically effective–if not more.Unfortunately, it's not just dry needlers who cause pneumothoraxes, but they are avoidable. This is the first of two episodes looking at pneumothoraces. In this episode we're going to start out by taking a look at traumatic pneumothoraces. Traumatic pneumothoraces are present in about a fifth of multiple trauma patients, so it's not infrequent to come across them and they can obviously occur in those with isolated chest injury too. Thoracic trauma occurs in around two thirds of multi-trauma cases and is classified as the primary cause of death in a quarter of trauma patients. The clinical assessment carries with it a fair amount of dogma, including looking for tensions with tracheal deviation, so we'll be running through what the signs we should look for actually mean. Then we'll move on to a detailed discussion about investigation strategies before finally looking at the guidelines and evidence on the topic, including which we have to intervene with, which we probably shouldn't and those in which there is much uncertainty... 2.20 Pneumothorax Pulmonary system review for the USMLE Step 1 Exam Pneumothorax is a collection of air outside the lung in the pleural cavity, caused by a hole in the pleurae that allows air to infiltrate and disturb normal respiratory function. There are three categories of causes: primary spontaneous, secondary spontaneous, and trauma. Trauma, such as blunt force or penetrating wounds, is the most common cause, as well as iatrogenic causes like transthoracic needle aspiration and central venous catheterization. Spontaneous pneumothorax occurs without any trauma and can be classified as primary (no underlying lung disease) or secondary (with underlying lung disease). Smoking greatly increases the risk for spontaneous pneumothorax, especially in tall men with Marfan's syndrome or Ehlers-Danlos syndrome. Tension pneumothorax is a more emergent and dangerous type of pneumothorax, where air enters the pleural cavity but cannot exit, causing pressure to build up and compress nearby structures like the lungs, blood vessels, and heart.

These are six major causes for dyspnea. Pulmonary bronchial constriction prevents the passage of air into the lungs which contributes to dyspnea. (Possible) foreign bodies can become trapped or logged within the trachea restricting air flow. Pulmonary embolisms can prevent complete oxygenation of the blood in the alveoli due to restricted blood flow. Pneumothorax collapses the lung and prevents full expansion, restricting oxygenation. Pump failure refers to the heart not beating appropriately. If the heart is not perfusing the lungs than the lungs will be unable to oxygenate the blood. Pneumonia leads to poor lung ventilation as well.

At the age of 15, I was going about my normal day, when all of a sudden, I started to get a pain in my right side, all the way down my back. I remember vividly where I was (Morrisons Car park) about to go with my dad to do the weekly shop. I didn't think too much of it and just carried on with my day, then later on that evening I was sick quite a few times from the pain. I put it down to a sickness bug and spent a couple of days in bed over the weekend. When it came to the Monday morning, I could no longer lie on my right side as it was too painful. Little did I know at the time, but my lung had collapsed on that Friday in Morrisons car park.
