And you know, the consequences are pretty high. They don't have another skill. We're basically saying, stop self-injuring. What are they going to do with those emotions? What are they going to do with them? Clearly, if they had another way to manage them, they would. I've never met somebody that started self-injuring that has said I had other coping skills, but I chose this instead. Usually when they start self-injuring, it's because they don't know what else to do.” ~Gillian C. Galen, PsyD Our guest, Gillian C. Galen, PsyD, is a senior child and adolescent psychologist specializing in dialectical behavior therapy (DBT). She is the director of training for the 3East continuum, an array of programs for teens that use DBT to target self-endangering behaviors and symptoms of borderline personality disorder. She has extensive experience diagnosing and treating adolescents and young adults who struggle with emotion dysregulation, anxiety, depression, trauma, and self-endangering behaviors, such as self-injury and suicidal behaviors. Dr. Galen has a particular interest in the use of mindfulness in the treatment of borderline personality disorder and psychiatric illnesses. She is the co-author of the books “Mindfulness for Borderline Personality Disorder: Relieve Your Suffering Using the Core Skill of Dialectical Behavior Therapy”, “Coping With BPD: DBT and CBT Skills to Soothe the Symptoms of Borderline Personality Disorder”, and “DBT for Dummies.” Our host, Gabe Howard, is an award-winning writer and speaker who lives with bipolar disorder. In this episode of The Art Career, Emily travels to Galerie Lelong & Co in Chelsea to sit down with gallerist Mary Sabbatino. Mary Sabbatino is Vice President and Partner of Galerie Lelong & Co., New York. She was appointed director of the New York location of Galerie Lelong & Co. in 1991 and became a gallery partner in 2007. In 2021, she was awarded the prestigious Chevalier de L'Ordre des Arts et des Lettres (Knight of the Order of Arts and Letters) from the Government of France. Championing a diverse roster of contemporary artists from throughout the world, the gallery has pioneered the community both in presenting a balanced roster of male and female artists, and artists from the Global South. Contributing to the overarching contemporary art scene, she served on the boards of ArtTable and the Executive Board of the Art Dealers' Association of America, during which she initiated and co-authored the ADAA's first code of ethics. Sabbatino served on the Selection Committee for Art Basel Miami Beach (2013-2020), the Art Basel Joint Selection Committee, and is a founding member of the Council for the Elizabeth Sackler Center for Feminist Art at the Brooklyn Museum, New York. In 2020, along with colleagues from international galleries, she initiated the platforms Galleries Curate and SOUTH SOUTH. In addition to fostering the careers and legacies of the gallery's artists, Sabbatino co-curated Art from Brazil in New York (1995) which presented the first solo exhibitions of the region's most vital figures—Waltercio Caldas, Cildo Meireles, Hélio Oiticica, Mira Schendel, and Tunga—and curated Juan Downey: Video Installations and Drawings (1995), at the Museo Bellas Artes, Santiago, Chile. Agoraphobia is the fear of being away from a safe place. People with agoraphobia have trouble being in certain types of public places. They also have problems with enclosed areas that don't seem to have an escape path. There are people that do just fine traveling only to a home, the store, work and maybe the church. Anything more than that causes them distress.    There are people that cannot ride a bus or the subway. It is the fear of being trapped without an escape. Then you have people that cannot leave their homes or they can only do so with someone accompany them.    In this episode a quick look at the condition, some of the symptoms and resources to get you started on your recovery journey. If you need support contact the National Suicide Prevention Lifeline at 988 or 1-800-273-8255, the Trevor Project at 1-866-488-7386 or text “START” to 741-741. There are times when you just have to keep putting one foot in front of the other. You will get to a destination. Hopefully the one that you intended to get to.  In this episode I wanted to help make clear the difference between mindfulness and the practice/tool/application of meditation. I have to be honest, it isn't for everybody. There are people that have weaponized the terms. Or feel that it isn't strong enough for what they are going through.    Or they don't have time to sit and breathe. They have real life problems and asking them to take an hour to take care of themselves it a bit much.   I understand.    I would submit that you have to be ready to do this. I think there can be great benefits in reducing symptoms. So in this episode I wanted to put a bit more context as to why sucking sour candy could work to stop panic attacks. Or not. Or not for the long term.   Here is how it started. I have news feeds for anxiety. This story popped up.   Delish is a food recipe and kitchen gadget website. I've seen videos on YouTube. I was confused when I saw the story about sour candy and panic attacks.   It didn't match their usual beat. Sometime that can lead to good stuff. Like a fishing website that reviews a movie about Vets, fishing and PTSD.    It isn't an instant reject for me unless it is one of those way past NC-17 websites, but my radar is on high alert.    Publisher these days they need to draw people to the website. Delish saw a fit with there mission. They did have a series called Suck It Up. They would ask celebrities questions and if they refused to answer they would have to eat an increasing strong sour candy.    So this was in the Delish wheelhouse. Kinda. They got the story from Tik Tok.   USA Today did a story with more info from mental health professionals. That is how Delish found out about it.   As far as our neck of the woods is concern, this could be thought of as a form of grounding or interrupting the signals that generated the panic attack.    There are a lot of them. It is not treatment.   Nope. It is rare that The Anxious Morning and The Anxious Truth podcast match perfectly on any given day. Today they do, and the topic for both is the fear of vomiting. This is known in clinical circles as emetophobia. Emetophobia is more common than you might think in our community. Many anxious people harbor a fear - sometimes a very intense fear - of vomiting. In some cases, the avoidance driven by emetophobia can be the key factor in developing agoraphobia. Ken Goodman, LCSW (Goodman, n.d.) wrote on the ADAA website:People who suffer with this phobia often experience significant social and occupational impairment, going to great lengths to make sure they don’t vomit (similar to someone who fears elevators and walks up 10 flights of stairs). Kids refuse to go to school or visit a friend’s house, and adults miss work and stop eating at restaurants. Much of life is avoided all due to a fear of vomiting. This means missing out on much of life and a great deal of worrying and strategic planning — all to avoid something uncomfortable that seldom happens. The uncertainty of not knowing when it will happen is what causes so much distress. And because you don’t know when it will happen, you are constantly on guard, rearranging your life to ward off any possibility of puking.An emetophobic person may try to do exposures to overcome the fear of panic and anxiety itself, but when the base fear is that one might vomit when anxious or in a state of panic, these exposures can become far less productive, or even counter-productive in some cases. This is why for an emetophobe that suffers with panic attacks or has become agoraphobic, often it is VERY important to look beyond the idea of “floating, accepting, and surrendering” to panic. When there is a steadfast insistence that exposures are literal torture chambers based on the possibility that anxiety may trigger nausea or vomiting, that fear really needs to be addressed directly.This is both good news and bad news for our emetophobic friends. The good news is that there is a good explanation for why your exposures feel like torture that never help you. You’re not broken or beyond help. You’re just afraid of something more specific, which can be addressed. The bad news is that … you’ll have to address it. This is something that many emetophobia sufferers steadfastly insist is impossible because vomiting is in fact the very worst thing that can happen to a sentient being in the known universe. This may sound like an exaggeration. It is not. Trust me. I have been chastised by many of our emetophobic friends for daring to suggest that even that fear can be faced. But even when you insist that there is no possible way to overcome the fear of vomiting, you would be wrong. Many people overcome emetophobia. It’s treated successfully every day using the basic premises of cognitive behavioral therapy. That treatment can certainly be difficult. Nobody is denying this. Before talking about Dialectical Behavior Therapy,  I want to give a brief reminder of Cognitive Behavioral Therapy aka CBT. Cognitive Behavioral Therapy is a short term treatment that provides skills on how to transform negative thoughts and behaviors.   Traditionally, CBT is offered to people with an anxiety disorder, depression and other mental health conditions.   There are a number ways to do this but the general format is to:   Assess where you are right now. What is your view of yourself, your attitude about yourself and how you function with those beliefs. Skills and tool building process to help you deal with your symptoms and other blocks you experience. Dialectical Behavior Therapy (DBT) is a variation of CBT, the goals are the same but there are some differences.    DBT was originally created as treatment for suicide treatments, Borderline Personality Disorder, then eating disorders, some types of addictions and is now used for PTSD and anxiety conditions.   It is actionable focus, meaning you actively apply the skills and techniques as you learn them.    There is a connection with mindfulness. There is also a focus on acceptance and finding ways to change what you can to improve the quality of your life..   In this episode, a review of CBT and a brief dusting on the concepts of DBT. It happens that I start in on an idea and think I've done my research. Only to get whacked in the head about a question I did not ask. Like how to find the specific vendor on a state website.   For example, I start to think about the episode and then a thought came into my head.    "How are they supposed to do that?"    And because I'm hardheaded and I don't listen to the spirits from up on high, I respond that they should go to their mental health or behavioral health board in their state.    Then the spirits from up on high say, "you better double check that."    I'm hard-headed, but I'm not stupid.    I got humbled right quick. There are some states to provide mental health services and resources to the public. Every nook and cranny of their website has resources and plain English explanations. There are other states that work as a conduit between public and private resources.   But when it comes to seeking licensing info, it may not be a state mental health site you need to be.   Some of the states have their licensing information in an entirely different site. May is Mental Health Awareness Month. Cases are on the rise and many experts are pointing to the CoronaVirus Pandemic as it's amplifier. According to ADAA.org (The Anxiety & Depression Association of America) there are nearly 450 million people worldwide currently living with a mental illness of some sort and only two-thirds of those individuals seeking out help for their condition. How can we break the stigma around mental health issues? Let's start by letting people know it is okay to acknowledge them and to talk about them with others. We want you to know you are not alone. When you seek help, you move one step closer to overcoming any shame and living a well-balanced life.We must create a safe space for people to talk about mental health and be willing to share their stories with others. For decades, there has been a negative connotation around the phrase Mental Health, deeming those dealing with it as “Crazy”. We must learn to take care of our mental state, in order to take care of others, but more importantly ourselves.  We encourage anyone that is struggling with any type of mental condition to seek the help they need in order to live a purposeful life.
