Women go on hormone therapy to manage peri/menopause symptoms and feel better. But sometimes to their surprise, they feel worse. Or some symptoms go away while others pop up. That's because of the hormones! We have hormone receptors all over our body and whenever we switch up the signals they're receiving, whether through the menopause transition itself or through the hormones we use, they're going to respond to those changes, and sometimes not in the way we want. Welcome to Menopause Research Review! Now that menopause has hit the mainstream, we're swimming in a steady stream of studies, which is great and can also be overwhelming. So for 2025, we're introducing a regular segment called Menopause Research Review where we break down key research as it comes out with resident reproductive endocrinologist and menopause performance specialist Dr. Carla DiGirolamo. Of all our hormones, cortisol might be the most misunderstood and maligned. We're told this stress hormone is the culprit behind excess belly fat, high blood sugar, accelerated aging, and a host of menopausal and midlife woes. Menopausal and midlife women sometimes are afraid to exercise too much because they've been told it will raise their cortisol, which can already be elevated during this time of life. The problem with this narrative is that it's one-sided. It paints cortisol as a demon to be avoided at all costs, when actually cortisol is essential to maintain and improve fitness, and frankly for human life. Now that menopause is mainstream news, interest in hormone therapy is exploding. And so is the confusion and controversy surrounding it, as menopausal hormone therapy (aka hormone replacement therapy), which is undeniably a game changer for millions, has also become a massive business. Some practitioners are reportedly prescribing “megadoses” of estrogen–in some cases twice or even 3 or 4 times the maximum licensed dose, causing myriad medical societies to sound the alarm. Meanwhile, researchers are working overtime to catch up to provide clarity on what hormone therapy does, what it doesn't do, who it benefits most, and for how long. Kelly and his work have been featured on 60 Minutes, The View, The Joe Rogan Experience, CBS Sports, Outside Magazine, Men's Health, Men's Journal and dozens of other podcasts, magazines, and books — including Tim Ferriss' The 4-Hour Body and Tools of Titans. On top of co-founding The Ready State, Kelly and Juliet also started San Francisco CrossFit and StandUpKids together. Founded in 2005, San Francisco CrossFit was the 21st CrossFit affiliate in the world. And StandUpKids is a non-profit dedicated to combating kids' sedentary lifestyles by bringing standing and moving desks to low-income public schools. To date, StandUpKids has converted 95,000 kids from sitting to standing. Earlier in their careers, Kelly and Juliet also co-founded a kayaking camp for children with HIV called Liquid. In his athletic career, Kelly paddled whitewater slalom canoe on the US Canoe and Kayak Teams. He lead the Men's Whitewater Rafting Team to two national titles and competed in two World Championships. In his free time, “KStar” likes to spend time with his wife, Juliet, and two daughters, Georgia and Caroline. He also loves to mountain bike, paddle, and sauna. Caffeine: It's likely the most widely used mood- and performance-enhancing drug. However, have you ever noticed that some people are wired all day from one cup of coffee in the morning while others can drink it right before bed? The CYP1A2 enzyme is largely responsible for caffeine metabolism, and our genes may give some information about our individual response. Interested in harnessing the performance-enhancing benefits of caffeine? Mark Goodman, MD, recommends you start by eliminating regular caffeine consumption. Then you can reintroduce a small amount approximately 60 minutes before your workout to observe its effect on your performance. Experiment with timing and dosage, and keep in mind that some individuals might experience a neutral or even negative effect, so it may take a few trials to find the best formula for you. The average American spreads their food consumption out over 15 hours a day, which basically means they're eating from the time they wake up until the time they go to bed and fasting only during sleep. Increasing this fasting window by even just a few hours can provide a multitude of health benefits, (1, 2, 3) including: Decreased rates of obesity Improved blood pressure Decreased insulin resistance Better lipid profiles Decreased risk of cardiovascular disease Decreased cancer risk Decreased inflammation These benefits are seen with many different fasting practices. With time-restricted feeding, for instance, an individual eats for only a portion of the day, say 8-12 hours, and fasts the remainder of the day. Periodic prolonged fasting, on the other hand, ranges from as little as 24 hours to up to 3-5 days during which an individual is consuming only water. Keep in mind that fasting isn't for everyone. Those who have a history of disordered eating or diabetes, are pregnant or breastfeeding, or are participating in high-volume training should all consult with a doctor before fasting. However, if fasting is right for you, it can be a small, easy-to-implement change that can have a significant positive impact on your health. It probably comes as no surprise that alcohol has a negative impact on sleep quality, but why does something that seems to help you drift off actually turn out to be problematic? Mike Stone, MD, shares two main reasons why alcohol and sleep don't mix: Alcohol disrupts normal sleep architecture, leading to nighttime waking and reduced sleep quality. Alcohol suppresses REM sleep, which is critical for learning and memory. For an eye-opening experiment, try using a wearable sleep tracker and observe the results. You're most likely to notice decreased HRV, increased resting heart rate, and decreased REM sleep. Still want to enjoy a cocktail or glass of wine? Dr. Stone suggests these strategies to help mitigate the negative effects of alcohol consumption: Minimize the total amount of alcohol you consume. Keep your alcohol consumption earlier in the evening, further away from sleep. Use a sleep tracker to run experiments and determine which types of alcohol and what circumstances result in the smallest impact possible for you personally. “We like to say heart rate variability is the range of motion of your nervous system,” Lindsey Mathews, DC, says. Put another way, heart rate variability (HRV), or the variance in time between the beats of your heart, can provide insight into how your body is responding to the many different stressors in life. This is a valuable tool to help guide women's training through pregnancy and postpartum. When HRV is low, it opens up a conversation on what other life stressors are going on and indicates a need to reduce training load. When HRV is stable or trending upwards, pregnant and postpartum women can use it as an opportunity to push training a little bit harder within the parameters of what is safe for their pregnancy. HRV values are individually unique, but by establishing a personal baseline, women can bring awareness to how different lifestyle factors impact their physical readiness. They can then use this knowledge to inform training throughout their pregnancy and postpartum. Mealtime with kids — Is it possible for it to be low-stress AND nutritious? Nicole Christensen wears many hats: CF-L4, owner of CrossFit Roots, Seminar Staff Flowmaster, CrossFit Nutrition Course Director, and, most importantly, mom of two. Her best tip: Keep it simple and offer a small selection of healthy options to keep the decisions and negotiations to a minimum. At breakfast, she keeps it quick and easy by offering a fruit and a protein, which vary based on what's in the house. For lunches and snacks, she likes to have a variety of good choices available so her girls can make their own selections. At dinner, the kids eat what the adults eat: typically grilled meat, some form of potato, and a vegetable. If this all sounds too good to be true, Christensen shares that when her girls come across foods they don't want to eat, oftentimes it simply takes patience and time for them to change their minds. Fats and oils are important parts of a healthy diet and support functions such as vitamin absorption, brain function, and nerve function — but did you know not all fats and oils are created equal? When choosing the right oil for your meal, consider two factors: -The degree of processing the oil has undergone -Smoke point (the temperature at which the oil starts to burn and smoke) Seed oils such as sunflower, vegetable, and canola oil require a high degree of processing and can be pro-inflammatory. Better choices require less processing and include avocado oil, coconut oil, olive oil, and grass-fed butter. Olive oil has a low smoke point and is best saved for dressings. Avocado oil and butter have higher smoke points and are more appropriate for cooking. By selecting minimally processed oils with appropriate smoke points, you can maximize the health benefits of including fat in your diet and minimize factors that contribute to the risk of chronic disease. Focusing on QUALITY food can have a huge impact on health, but many individuals reach a point where they hit a plateau. That's when food QUANTITY comes into play. Stephane Rochet, CF-L3, shares a simple method to begin measuring food portions. At each meal, load your plate with: Protein: Between 1 and 2 palm-sized servings, depending on your size and goals Vegetables: Double the size of your protein serving Starch: Equivalent to the size of your protein serving Fat: Approximately the size of your thumb Ready for more precision? Start with between .8 and 1.2 grams of protein per pound of body weight and set a macronutrient ratio from there — 40% carbohydrates, 30% protein, and 30% fat is a good place to start. The practice of measuring doesn't have to last forever. Once you do it often enough, you'll have built the skill to look at your plate and generally know if you're hitting the correct quantities, Rochet says. From there, you can return to measuring as needed when you find you need to recalibrate your eye. Can giving birth increase your heart rate variability (HRV)? Research conducted by OB-GYN Shon Rowan, MD, indicates that it can! In a study following women who were already exercising and planning to become pregnant, Rowan used a wearable device pre-, during, and post-pregnancy to track HRV and resting heart rate. The results provided several fascinating insights: Several participants were able to detect conception prior to taking a pregnancy test. Women experienced a statistically significant increase in resting heart rate and decrease in HRV with each trimester. Approximately 56 days before delivery, those trends reversed. Resting heart rate began to decrease, and HRV increased with the approach of delivery. This trend continued into the postpartum period to the point that postpartum HRV exceeded the pre-conception state. Adopt a positive mindset. Recognize the habits that aren't moving you toward your goal. Set up your environment to support your goal. Identify challenges or barriers to success and plan ahead for those scenarios. Stephane Rochet, CF-L3, offers tips for getting started and overcoming plateaus. “There's nothing harder than taking that first step,” Rochet says. His advice: Start small. Master the basics before moving on to more advanced steps. It's well recognized that lifestyle factors are key in preventing cancer — quitting smoking, limiting alcohol, avoiding obesity — but did you know incorporating regular exercise has an impact as well? Exercise reduces the risk of developing cancer by approximately 20% and decreases cancer mortality by about 20% as well. Exercise oncologist Allison Betof, M.D. and Ph.D., shares some of the many ways exercise can function as a component of cancer treatment, including improving: Aerobic fitness Mental health Bone health Metabolic health And overall functionality She notes open communication with an oncologist is important so patients and trainers have guidelines for modifications and safety precautions. Kelly Starrett, DPT breaks down the most common causes of shoulder injuries in CrossFit athletes and explains what can be done for prevention. Starrett attributes most injuries to: Poor preparation or taking insufficient time to warm up Athletes lacking the necessary range of motion Generally not treating the shoulder with the same care as the rest of the body To address shoulder discomfort before it becomes a problem, he recommends: Increasing soft tissue work by adding mobilization between sets during training sessions and doing myofascial release with a foam roller and ball at home Expanding your repertoire of warm-up exercises to restore normal range of motion in the shoulder joint Checking in with your breathing. Sarah Hopping Estrella started CrossFit in 2009 after narrowly missing qualifying for the 2008 Olympic Games in the hammer throw.
