The Brain on Pain: Why Women Hurt More—and What to Do About It with Rachel Zoffness, PhD (Episode 279)

When you're in pain, the first question someone asks is “Where does it hurt?” The answer—as anyone with phantom limb pain can tell you—is not always as straightforward as it seems. This week, pain psychologist and author Dr. Rachel Zoffness breaks down the “big fat lie” we've been sold about pain: it's not just a signal from damaged tissue, it's a brain-driven experience built from a mix of ingredients, including your nerves, thoughts, emotions, memories, and environment. She explains how the same brain regions that process emotions also construct pain, why that matters so much for women—who go through menstruation, childbirth, and menopause and are still disproportionately told their pain is “all in their heads”—and how understanding brain-based biology is the first step in changing how much you hurt. She also shares practical, evidence-backed strategies for active women to retrain their nervous systems, along with tools to help you advocate for care that goes beyond pills and procedures and actually reflects the modern science of hurt.Rachel Zoffness, PhD, is a pain psychologist and thought-leader revolutionizing the way we understand and treat pain. She's an assistant clinical professor at UCSF, lectures at Stanford, and consults on the development of pain management programs around the world. Dr. Zoffness's new book, Tell Me Where It Hurts, is out now and is being translated to more than 25 languages. Most of us have had that moment where we get our bloodwork back and shake our heads. We're still our active, health‑minded selves and out of nowhere—rising LDL, ApoB, A1C, and maybe blood pressure and Lp(a), too. This week, preventive cardiology dietitian Michelle Routhenstein joins us to unpack the cardiometabolic chaos and what's really driving it. She explains estrogen's protective role in lipids and blood pressure, why standard risk calculators and even calcium scores can miss women's disease, and which advanced labs are worth asking for. We also dig into how under‑fueling and low‑carb diets can worsen cardiometabolic health and plaque; why complex carbs, fiber, fermented foods, and gut health matter so much; and how to approach protein, red meat, electrolytes, nitric oxide, and statins in a personalized, empowering way—remembering that 80–90% of heart disease remains preventable when women get the right information and advocate for themselves.Michelle Routhenstein, MS, RD, CDCES, CDN is a preventive cardiology dietitian and founder of Entirely Nourished, a virtual practice focused on personalized, science-based nutrition for heart health. With over 14 years of experience, she helps people improve cardiometabolic risk and manage conditions like atherosclerosis, heart failure, and atrial fibrillation using a whole-person approach. She holds Bachelor's and Master's degrees in Clinical Nutrition from New York University, serves on the Forbes Health Advisory Board and the Medical Advisory Committee for the National Menopause Foundation, and is the author of The Truly Easy Heart-Healthy Cookbook and Simple Meal Solutions for High Blood Pressure. Active women are often blindsided by an osteopenia or osteoporosis diagnosis. This week, we explain why—and, more importantly, what to do about it—with Professor Belinda Beck, PhD, investigator on the groundbreaking LIFTMOR trial, which found that high-intensity resistance and impact training was both effective and safe for postmenopausal women with low bone density. This episode is a deep dive into how bone remodeling works and why mechanical loading—especially the right combination of impact exercise and progressive heavy strength training—is essential for building and maintaining bone strength. We also cover peak bone mass, estrogen, menopause, and the foundational role of nutrition, including calcium and vitamin D, in supporting stronger bones for life.Dr Belinda Beck is a Professor of Exercise Physiology at Griffith University (Gold Coast, QLD) and Director of The Bone Clinic in Brisbane. Her work, primarily related to the effects of mechanical loading on bone, includes a series of clinical trials (the LIFTMOR and MEDEX-OP trials) that changed the way osteoporosis is managed with exercise. In 2015, she established The Bone Clinic, a translational research facility and clinical practice in Brisbane providing the ONERO® exercise program for osteoporosis which has been licensed for delivery around the world. She has been awarded almost $8M in research funding and has published over 150 papers and chapters in the field. This week, former Ironman champion and lifestyle medicine practitioner Dr. Tamsin Lewis joins us to explore the evolution from "harden the F up" culture toward bioharmony—a compassionate, context-driven alternative to the more rigid biohacking that's popular today. Drawing from her high-performance athletic career and hard-won personal experience, she unpacks what she's learned about female biology, hormones, and recovery. We also dig into hormone therapy, nervous system regulation, autophagy, regeneration, and embracing midlife as a "second spring"—a season for new pursuits that expand our identity, horizons, and even cognitive capacity. Prepare for a deep dive into the menopause transition through the lens of mindset, biology, and the nervous system.Dr Tamsin Lewis, also known as Dr Tam, is a physician, longevity expert, and human‑flourishing advocate. Graduating with honours in Medicine from King's College London and Guy's & St Thomas' Hospitals, with a BSc in Neuroscience and the Biology of Ageing, her work bridges psychiatry, sports & exercise medicine, nutritional and functional medicine. As Founder and Medical Director of Wellgevity®, she leads a personalised, diagnostics‑driven approach to health and emotional regulation—the Wellgevity® Method—integrating biomarkers, brain‑body synchrony, and evidence‑based recovery, while consulting at Reborne Longevity in London, where she delivers longevity medicine through a bioharmony lens. If you've ever leaked when you laughed, jumped, or sneezed–or felt like your core disappeared after pregnancy, injury, or into perimenopause–you're not alone. This week, fitness specialist Leah Keller joins us to unpack deep core dysfunction: what causes it, and why healing it changes far more than just your abs. Leah's breakthrough came when she realized she could help women actually heal the separation between their abdominal muscles–known as diastasis recti–even years after giving birth. Clients who came for core work started reporting that they'd stopped leaking, that sex felt better, that back pain eased, and that their bodies finally felt like their own again– issues they never mentioned at the start. She explains exactly how fixing the deep core resolves problems women didn't even know were connected. We'll give detailed descriptions of the exercises. You can also watch this one on YouTube to see the exercise demonstrations as well as the anatomical illustrations referenced in this episode, showing the difference between a healthy midline and one with significant diastasis recti separation.Leah Keller is a certified personal trainer specializing in core and pelvic floor health, and co-founder of Every Mother (formerly The Dia Method). Her approach earned medical validation in a 2013 Weill Cornell pilot study — published in the American Journal of Obstetrics and Gynecology — and was further supported by a 2021 Hospital for Special Surgery trial in the Journal of Women's & Pelvic Health Physical Therapy. She has been featured as a women's health expert by NPR, Goop, and Good Morning America. Women in their 50s, 60s, and beyond are crushing courses around the world. At a time when we've been preconditioned to believe we'll slow down, many women are holding strong—or even improving. This week's guest Anna Troup, 56, who has clocked many of her greatest ultrarunning feats since turning 50, is a perfect example. Last year she took first overall (including the men) at the 268-mile (430K) Montane Summer Spine race—Britain's toughest ultra marathon—in 84 hours, 56 minutes, and 37 seconds. This after a 2022 injury that left doctors questioning if she'd ever run again. This week, we talk about her most recent feat: completing the Montane Winter Spine Race this past January in 106 hours, 19 minutes, and 12 seconds—through brutal conditions including gale-force winds literally blowing runners off their feet. We dig into her journey to ultras, how she trains, races, and recovers, her menopause experience, and why more midlife women should line up and give it a go.Anna Troup took up ultra running in 2013 with the sole aim of completing the UTMB, which she did for the first time in 2014. Unexpected success followed, much of it after she turned 50, and she has since won and held course records for many of the UK's toughest single stage ultras including the Arc of Attrition 100, the Lakeland 100, Exodus 100, Wendover Woods 100, the Oner and the Summer Spine and Winter Spine. She has run the women's Fastest Known Time for the Pennine Way and was the first female British finisher in UTMB in 2022. Ultra running has been a family sport since the beginning and she normally competes with Richard Staite, her partner, whilst her two children can be found either running themselves or as volunteers in aid stations. She works as non executive director and has recently moved to the Lake District to be closer to the mountains she loves.Resources:Ultramarathon winner's 'tough and terrifying' race at BBCHow a 55-year-old woman won Britain's toughest ultra marathon outright at Running MattersSign up for our FREE Feisty 40+ newsletter: https://feisty.co/feisty-40/Learn More about our 2026 Feisty Events, including Bike Camps and Cycling Trips: https://feisty.co/events/Follow Us on Instagram:Feisty Menopause: @feistymenopauseHit Play Not Pause Facebook Group: https://www.facebook.com/groups/807943973376099Support our Partners: Eternal - Get 15% off their services with code FEISTY15 at https://eternal.coMidi Health: You Deserve to Feel Great. This week we welcome back protein and fat-loss researcher Dr. Bill Campbell to unpack early findings from what may be the largest menopause fitness survey to date, designed to finally study women who actively train. We dive into what thousands of fit midlife women report about calorie and protein intake, hormone therapy use, and the widely felt—but poorly studied—experience of weight-loss resistance during peri- and postmenopause. The conversation also explores why hormone therapy use appears far higher in fitness-engaged women than in national estimates, how muscle quality and power may decline even before we notice muscle loss, and why high-intensity interval work appears especially promising for reducing visceral fat.Bill Campbell, PhD is a Professor of Exercise Science and Director of the Performance and Physique Enhancement Laboratory at the University of South Florida. This episode was sparked by a simple text, a follow-up call, and an intentional act of kindness from my friend Craig Gottlieb. That small moment carried massive impact and led me to ask a powerful question: Do I make people feel great?Proverbs 18 gives us a clear blueprint for being a great friend—speaking life, choosing wisdom over gossip, diligence over laziness, humility over ego, and listening before reacting. When you fill your heart with wisdom, discernment, and time with God, you become refreshing to the people around you. Good friends don't just show up—they add value. If you've ever wondered how to be a better friend, how to build stronger relationships, or how biblical wisdom applies to everyday life, Proverbs 18 is where to start. Read it. Live it. Then go make someone feel great today.

As we wrap up 2025 and start dreaming about our goals for the New Year, this week's guest, ultracyclist Paula Ralph, has one simple message: Just go for it. We often wait until we're 100% ready—which usually means we never start. But whether you succeed or fall short, you'll be better for having tried. Paula knows this firsthand. She set out to break a world record riding solo from Cape Town, South Africa to Nordkapp, Norway. Sponsors pulled out. Mechanical issues piled up. She got heat stroke…and hit by a truck. Safety concerns forced reroutes. The record slipped away. But she didn't stop. She kept going—ultimately riding 12,290 km across 17 countries in 126 days. And what she gained was far bigger than a record: proof that she's stronger than she ever imagined, and a reminder of what midlife women can do when we dare to go big.Paula Ralph is a solo endurance cyclist who rode unsupported from Cape Town to Nordkapp, Norway — a 12,290 km journey across 17 countries in just 126 days. What began as a world record attempt quickly turned into a raw test of resilience. Navigating relentless challenges, both on and off the bike, Paula had to let go of the record and hold on to something more powerful — the will to keep going. Her journey is one of survival, courage, and what it means to find freedom at the edge of your comfort zone. She was recently named a "sporting hero" as one of the top 100 female cyclists by CyclingUK. You can learn more about her and her mission @thelongrideglobal on Instagram. Muscle, Hormones & the Menopause Transition with Carla DiGirolamo, MD, PhD (Episode 253)

As active women, muscle is the engine that powers us through our lives, no matter what sport we love. When we hit midlife and menopause, it can feel like that 8-cylinder engine drops to six—less punch when we hit the gas, even though we're still training. We may notice less muscle and more body fat when we look in the mirror. Muscle and strength loss during the menopause transition is multifactorial. It's hormonal changes and estrogen decline. It's inflammation. It's symptoms like poor sleep, joint pain, and fatigue. It's body fat gain. It's how we recover and fuel. And, of course, it's aging itself, as both women and men lose muscle with age. Our approach to making and maintaining it has to be multifactorial as well. This week we talk all about muscle and menopause–from basic science to menopause symptoms to hormone therapy–to understand what's going on under the hood and help keep that engine strong.Carla DiGirolamo, MD, PhD, is a Double Board Certified Reproductive Endocrinologist and Obstetrician/Gynecologist specializing in infertility and menopausal medicine. The menopause transition is a critical tipping point for bone health. Bone loss accelerates rapidly in late perimenopause and continues for roughly 3 to 5 years after menopause begins–a window in which some women lose up to 20% of their total bone mass. For too long, society accepted this decline as inevitable. That fragility is our fate. That as women, our stature–and our lives and our worlds–just shrink as we age. Dr. Lisa Moore, DPT, founder of Brick House Bones, is on a mission to change that. After a surprise osteopenia diagnosis of her own, she rebuilt her bone density by 3% in a single year. Now she's helping tens of thousands of women reclaim their skeletal strength, too, including those navigating uniquely challenging circumstances like cancer recovery. She shares her recipe for maintaining and building strong bones this week.Dr. Lisa Moore, DPT, is on a mission to help over one million people with low bone density turn fear into action—and build the strength to do the things they love with the people they love! She's a Doctor of Physical Therapy, a certified yoga and mindfulness educator, and a BoneFit™ Trained Healthcare Professional specializing in evidence-based exercise for osteoporosis. Lisa is the founder of Brick House Bones, a global movement helping women build stronger bodies and stronger bones. She also runs her own clinic, Strong Bones Physical Therapy, just outside of Seattle, Washington. With degrees from the University of Maine, the University of Washington, and the University of South Dakota, Lisa brings together science, movement, compassion, and community to help her clients thrive at every age.ResourcesBrick House Bones with Dr. Lisa Moore, DPT, hereHigh-Intensity Resistance and Impact Training Improves Bone Mineral Density and Physical Function in Postmenopausal Women With Osteopenia and Osteoporosis: The LIFTMOR Randomized Controlled Trial, hereStronger back muscles reduce the incidence of vertebral fractures: a prospective 10 year follow-up of postmenopausal women, hereSkeletal site-specific effects of jump training on bone mineral density in adults: a systematic review and meta-analysis, hereWeighted Vests in Midlife: Hype? Unpredictable cycles. Sleepless nights. Mood swings. Body composition changes. And performance that mirrors all of it. As this week's guest menopause run and strength coach Becky Croft explains, you can still get stronger and faster, but it won't come from constantly pushing through or grinding it out. It takes smart planning, a different kind of periodization, and recovery that respects your shifting hormones and the cumulative stress that comes from this time of life. We dive into all of it: hormones, nutrition, hydration, strength, load management, and much more. Becky also opens up about her struggles with endometriosis, surgical menopause, and younger women in menopause can feel left out of the conversation.Becky Croft is a certified running and strength coach who specializes in guiding masters and menopausal athletes through the complex intersections of performance, hormones, and identity.
