He described it not just as a sexual problem, but as one of the earliest "check engine lights" for heart disease. The blood vessels leading to the penis are much smaller than those in the heart. As we age, plaque can build up in these vessels. This process, known as atherosclerosis, affects the smaller vessels first.This means if you're experiencing persistent issues with erections, it's a strong indicator that you may have vascular problems that could soon affect your heart. As Dr. Kohler put it, "If the penis is failing, you should assume you're going to have problems with your heart soon until proven otherwise." The good news is that anything good for your heart—like exercise and a healthy diet—is also good for your penis.The "Use-It-or-Lose-It" OrganThe conversation also tackled the surprising phenomenon of penile shortening. When the penis goes into a "garage" for an extended period—meaning there is a consistent lack of erections—it can lose length. Dr. Kohler shared that a consistent lack of erections for just three months can lead to a loss of 1-2 centimeters in length due to the formation of scar tissue. This is why he calls the penis a "use-it-or-lose-it" organ. The key to maintaining penile health is to exercise the penis through erections, and tools like a vacuum erection device can be used for penile rehabilitation.Muscle Mass, Testosterone, and Sexual FunctionFor many men, this is where the conversation gets personal. In this episode, I share my full DEXA scan results, including a visceral fat score of 0.38 pounds, which I’m determined to drop to “0.0 fricking zero” through sprinting—shown in research to preferentially burn visceral fat. My body fat is 21 pounds (12.5%), and I break down the fat and lean mass distribution throughout my body, including minor asymmetries. I was advised to gain 2–3 pounds of muscle for longevity, though my sprinting and high jump goals favor staying lean. I discuss why chronic endurance training can increase visceral fat due to cortisol overproduction, while short, explosive workouts like sprints or strength training create a healthy stress response. You’ll also learn why bone density suffers in cyclists and astronauts, what makes muscles a glucose sink, why Dr. Ron Sinha and Dr. Gabrielle Lyon emphasize maintaining muscle as the key to long-term health and metabolic flexibility, and I end the show with a plug for muscle-centric medicine—focusing on building and maintaining muscle as the key to longevity and leanness. I had a huge response on Instagram when I shared the best information I've ever heard when it comes to why protein is such an important macro nutrient. For that reason, I thought I'd share it with you again in case you missed it first time around.Right now, you can't move for protein printed across most food packaging. Even chocolate bars have got in on the action, producing ‘high protein' varieties of their sweet snacks to try to sell them as a key component of a healthy, balanced diet. How can this be junk food when it contains protein?What's the truth? In 2021 I interviewed Dr Gabrielle Lyon, a double-board certified family physician trained in geriatric nutritional sciences who focuses on skeletal muscle to support longevity. It was her work with geriatric patients that highlighted the importance of maintaining lean muscle in order to live well into older age and to avoid a plethora of health issues.The fact this conversations pre-dates the obsession with protein adds, I think, real value to Gabrielle's expertise because it was long before every ‘health expert' was talking about protein on podcasts.
