Longevity isn't just about what you eat or how you train—it's really more about whether you believe your body is worth investing in. On this episode of The Dr. Hyman Show, I'm joined by my longtime friend and colleague Dr. Gabrielle Lyon to explore why muscle may be one of the most overlooked drivers of long-term health. We focus on practical, accessible ways to build strength as you age, and why muscle plays a much bigger role in your health than most people realize. We explore: • How strength training supports your blood sugar and metabolic health, beyond what the scale can show • Why muscle quality—not weight—is a stronger predictor of long-term health • How building strength helps you stay mobile, capable, and resilient as you age • What most people miss about protein and muscle health Aging well is something you can actively support, starting with how you care for your body today. And strength is one of the most powerful places you can begin. Skip to the start of the episode: 11:52 In today's Friday solo recap on the Optimal Protein Podcast, I break down the most important takeaways from my recent conversation with Dr. Gabrielle Lyon — and why this episode has major implications for fat loss, metabolic health, and women's body composition.
In this conversation, Cheryl McColgan discusses the significance of prioritizing protein intake in meals, particularly as one ages. She emphasizes the benefits of starting meals with protein to enhance satiety, meet nutritional needs, and support muscle health. Cheryl explains the concept of anabolic resistance and the importance of leucine in protein synthesis, providing practical advice on how to incorporate more protein into daily meals. The discussion highlights the challenges of meeting protein requirements and offers strategies for achieving a balanced diet without feeling restricted. Takeaways Eating protein first can help you meet your protein needs.Protein is highly satiating and can prevent overeating. As we age, our protein absorption efficiency decreases.Aim for 30-40 grams of protein per meal for muscle health.Leucine is crucial for stimulating muscle protein synthesis.Tracking protein intake can help ensure adequate consumption.Eating protein first can change your overall eating habits. It’s important to balance protein with carbohydrates and fats.Consider adding a protein snack before bed for muscle recovery. One meal a day focusing on protein can be a great start. So today we’re going to talk about one of my favorite things, one of my favorite hacks, if you want to call it a hack, and that is today’s habit, and that is to eat protein first at one meal. So you don’t have to do it every meal, just one meal today. It’s not about perfection. This is just about sequencing. So doing this one meal totally counts, if all you do is that, you win today. The reason that you want to start with protein is because it really highly satiating, it increases fullness. And one of the things that I’ve been working on my protein intake, I’ve been tracking it for about the last five or six years now, where I’ve really been more focused on intentionally getting more protein each day. And even after all that time, if I am not focused on it, I will tend to not get enough protein on any given day. So one of the things that eating protein first does for you is if you’re if you’re one of those people that they’ll say when they learn what their optimal amount of protein is, you’re like my gosh how would I eat that much protein it’s so difficult. Well it’s probably difficult because you’re eating other things first that take up this, I always say when I’m talking about a salad or bread before meals, I’m like, no, that’s just gonna take up the space where the meat should be, right? So I always keep that in my mind. That’s how really I approach every meal is I know how much protein I’m supposed to get each meal. I get that out of the way, I put that on my plate, I weigh it out, and then I know that I’ve gotten that. And then I use carbohydrates and fats to fill in the energy calories after that. So that’s… Really the main reason to do it is because protein is very satiating. Most people don’t get enough of it. And eating it first really allows you to make sure that you get it in. This is important for a few reasons. First of all, as we age, there’s this thing called anabolic resistance. So basically it means that the protein that we eat is not absorbed properly or might not be broken down effectively. I don’t know if they really know what the exact mechanism is, why that happens. Cheryl McColgan (02:17.934)But basically, it’s like if I eat 30 grams of protein as an 18-year-old person, that’s almost getting 100 % utilized in the body. Whereas if I eat 30 grams of protein as a 45, 55, 65-year-old person, maybe only 80 % of that gets broken down into amino acids. so especially as you get older, that getting the proper amount of protein is really important. Unfortunately, what tends to happen, people’s appetite sometimes get less or over time or they just don’t eat as well as they get older and they just get less and less protein and that just creates a number of health problems. Sarcopenia is low muscle mass that can happen as you age without strength training and without the proper amount of protein. There’s also this other idea called the protein leverage hypothesis, which is basically that your body will keep seeking out. eat until your protein needs are met. So if you are eating a lot of either processed foods or a lot of foods that might be good for you, but don’t contain a lot of protein, it tends to make you overeat because your body will still be telling you, no, I still need protein. I still need those amino acids. And so you just keep eating and eating until you get to that point. So that’s some of the reasons that it’s very important. And since we’re on the topic, I’ll just go ahead and talk about that. You know, you heard me mention 30 grams. 30 grams of protein is basically what’s required to stimulate muscle protein synthesis. again, if you’re an older person, you might need more like 35 to 40 grams per meal. the real key there is not necessarily the grams of protein, it’s the amount of leucine in the protein that stimulates muscle protein synthesis. And they say that that’s two to three grams of leucine required to start that process. So again, as we get older, it might not be broken down as efficiently. So you really want to get like 35 to 40 every time you eat a meal. Every time you eat is an opportunity to stimulate muscle protein synthesis. And for most people, even the smallest women, you need about 100 grams of protein a day. An easy way to figure this out and kind of what’s been going around in the health space and people that are focused on this work, people like Dr. Don Lehman that do this kind of literature about protein, Dr. Gabrielle Lyon. Cheryl McColgan (04:36.558)is that so the minimum is about one gram of protein per pound of ideal body weight. So say you are a woman, you might have a few maybe your 10 or 20 pounds of weight, you have little weight to lose. And so whatever you weigh now, you feel like your ideal weight for your height is 150. Well, that’s the amount of protein you’d be trying to get in a day is 150 grams of protein. So for me, what that has meant is I’ve had to add a fourth meal into my day, basically like an afternoon protein hit. Because I find it challenging to, number one, wake up and get something like 50 or 60 grams of protein. I mean, that would be ideal, but I just don’t have the appetite for that. So I always go for like 35 to 40 my first meal of the day. And then lunch again, it’s kind of hard to eat something that’s going to be that amount of protein. So it’s usually lands around 40 to 50. And you can see how that’s still by the end of the day, if I’m shooting for 150 grams of protein, I’ve still got like 60 or 70 to eat and you don’t want to be eating ideally a giant meal before dinner. having that little afternoon protein and then the evening meal is kind of how it’s worked out for me. I’ve also had people talk or heard interviews where they talk about having, you know, a protein snack in the evening before bed is very muscle stimulating and just kind of If you’re a person that’s working on your body composition, working on building muscle, working out a lot, maybe having, you know, that eight o’clock protein shake or something before you go to bed might be a good way to get that extra amount in. I did just mention protein shake. Ideally, that’s not what we’re leading with in this challenge. You’re eating a real food protein for your first part of your meal, and then you’re filling in with carbs and fats. Anyway, that is all the reasons why you want to start working on eating protein first. And you’ll see that if you do that, it really changes the way you eat pretty naturally. So instead of having to restrict a lot of things or change a lot of what you eat, if you really just eat the protein first, you’ll find that the things, the rest of it kind of balances itself out pretty easily. And it also becomes very hard to overeat on protein because say you’re eating a chicken breast, you’re not really going to, you know, magically overeat chicken breast. It’s pretty hard to do. Cheryl McColgan (06:51.106)But we quite often magically overeat other things like that have that carbon fat combination. If you just start eating some chips or you know, those kinds of foods, bread or whatever it is, those are much easier to overeat for that reason. And also I think going back to the protein leverage thing, that’s probably why as well. So that was probably more than you wanted to know about protein, but I really wanted to make a good case for why eating protein first is a habit that you really should take on in your life. And Hopefully you will maintain this one. I think doing it one meal a day at first is pretty dang easy. So maybe you carry that out for the rest of challenge, or again, maybe this is just one of those you try, you’re not sure it works for you, or maybe come back to it later. But for today, it’s just eat protein first at one meal. So that is it for now, and I will see you again tomorrow.

While we often focus on muscle mass for longevity, new research published in Nature Aging reveals that our immune system undergoes a similar decline, with youthful sentinel cells (naive CD8 T-cells) dropping by 75% as we reach age 50. Dr. Singh explains how mitophagy —the targeted recycling of damaged mitochondria—is the key to "rewiring" an aging immune system to fight infection 20% more effectively. In this episode: Alzheimer's of the Muscle: Why sporadic inclusion body myositis (sIBM) is often misdiagnosed as simple aging. The Power of Urolithin A: How this postbiotic compound induces mitophagy to increase mitochondrial abundance by 20% in just 28 days. Immune Imprinting: Why children get 6-8 colds a year while adults only average 2-4. Repurposing Longevity Drugs: Dr. Singh's take on Metformin, Rapamycin, and GLP-1s for life extension. The 1:1 Exercise Rule:Why 150 minutes of moderate activity is the threshold for mitochondrial biogenesis. If you're a woman over 40 trying to build muscle, burn fat, and not lose your mind in the process, this episode is for you. I'm breaking down the biggest takeaways from Dr. Gabrielle Lyon's interview with Thomas DeLauer - specifically her approach to body recomposition and what it really takes to grow muscle and lose fat at the same time. As an IFBB bodybuilding pro who's coached women for almost 20 years, I'm giving you my honest thoughts, what actually works in the real world, and where most women go wrong once hormones, aging, and metabolism shifts start kicking in! We're talking protein targets that actually make sense, how much you really need, what supplements are worth your time, and why creatine isn't scary. I'm also getting into resistance training basics, how many sets you actually need per muscle group, and what a realistic weekly training plan looks like if you're trying to change your body in your 40s, 50s, and beyond. If you want to build muscle, burn fat, stay healthy, and look hot doing it without drowning in misinformation, you're in the right place! You've been running on empty long enough. Subscribe to Forever Strong Insider: https://foreverstrong.supercast.comIn this episode, Dr. Gabrielle Lyon sits down with Dr. Heidi van den Brink; one of the only scientists in the world studying the ovarian transition from fertility to menopause. Together, they unpack what textbooks and wellness influencers often miss: the biology behind how your ovaries, uterus, metabolism, and nutrition interact across every stage of a woman's life. Dr. Heidi reveals the never-before-seen dynamics of the ovarian cycle, how follicles develop in “waves,” and what really happens during the menopause transition. They discuss how undernutrition, obesity, and metabolic health shape reproductive hormones; why early menarche and delayed ovulation matter for long-term health; and the surprising connection between your gut microbiome, bile acids, and fertility. If you've ever been told to “balance your hormones” with seed cycling, apps, or supplements, this episode separates myths from mechanisms. It's a masterclass in understanding the science of your hormones and how to truly support reproductive health through every decade of life.Chapter Markers0:00 - Can You Get Pregnant While on Your Period? 1:21 - Intro to Dr. Heidi & Ovarian Cycle Research 3:39 - The Uterine Cycle vs. The Ovarian Cycle 6:19 - Follicle Waves: The Hidden Drama in Your Ovaries 10:38 - Novel Discovery: Follicle Waves in the Luteal Phase 12:00 - Why One Follicle is Selected for Ovulation 13:17 - Does Nutrition Affect Ovarian Function? 14:48 - The Variation in Menstrual Cycles (It's Not Always 28 Days) 17:23 - The Impact of Obesity on the Ovarian Cycle 18:11 - The Problem of Luteal Phase Defects & Fertility 24:52 - Ovarian Morphology as a Bio-Marker of Health 26:54 - How Undernutrition Affects Ovarian Health 31:56 - The Metabolic Tipping Point of PCOS (Polycystic Ovary Syndrome) 33:26 - The Myth of the "String of Pearls" Ovary 35:25 - The Menopause Transition & Rogue Follicles 39:04 - Can We Predict Menopause by Scanning Ovaries? 45:59 - Early Menarche (First Period) and Long-Term Disease Risk 48:33 - The Surprising Link Between Puberty Timing and Environment 52:28 - The Role of Leptin in Reproductive Health 55:50 - Debunking the Myth: Can Nutrition Cure PCOS? 59:07 - PCOS Diet Myths (Fruit, Timing, and Supplements) 1:04:40 - Fish Oil for Menstrual Cramps (Dysmenorrhea) 1:08:37 - Breakthrough: Bile Acid, The Gut Microbiome, and Ovulation 1:11:50 - Closing RemarksWho is Heidi Vanden Brink:Dr. Vanden Brink is a reproductive physiologist with over a decade of research in nutrition, metabolism, and female reproductive health. In this highly requested AMA, Dr. Gabrielle Lyon addresses the most critical questions surrounding male hormone health. Dr. Lyon tackles the fear, stigma, and misinformation surrounding Testosterone Replacement Therapy (TRT), revealing why a low testosterone level is a major predictor of serious health issues like heart disease, bone fracture, and obesity. She breaks down the current medical consensus on TRT, the risks of replacement, the truth about dosing, and what every man over 40 needs to know about their hormone health. This is a must-watch to separate the bodybuilding myths from the medical facts.Chapter Markers0:00 - Intro: Low Testosterone is a Predictor of Heart Disease1:12 - The Penile Shrinking Myth: What Caused the Viral Frenzy2:20 - Testosterone Levels: The Truth About Aging in Men3:44 - The First Step: What Labs to Ask Your Doctor for4:22 - Why Men Need to Check Their Estrogen (Estradiol)6:13 - The Medical Cutoff for Low T and Androgen Sensitivity7:27 - Low T is Linked to Fatigue, Depression, and Fat Deposition9:27 - TRT and Cardiovascular Health: Is it Protective or a Risk?10:36 - Erectile Dysfunction Predicts Heart Attack Within 3-5 Years12:30 - The Stigma: Why Doctors Don't Check Testosterone13:00 - TRT vs. Steroids: Defining the Line14:21 - Infertility and TRT: What You Must Know15:18 - Safety: What are the Risks of TRT for Men?16:33 - The Stigma: Why Doctors Don't Treat Low T17:15 - TRT and Obesity: Treat Concurrently or Wait?18:10 - Menopause vs. Andropause: Why Men Don't Have to Decline19:34 - Testosterone Boosters (Ashwagandha, Tongkat Ali)26:11 - Different Forms of Testosterone Administration28:09 - Intranasal Testosterone for Less Impact on Fertility30:38 - Managing Expectations: How Long Until You Feel Better?33:00 - TRT and Muscle Gain: The Highs and Lows of Dosing36:47 - The Win-Win: Low Risk, High Reward39:00 - Blood Flow & Erectile Function: Cialis and Exercise40:17 - Semen Quality as a Marker for Overall Male Health41:29 - Parenting and the Cost of Unhealthy Humans41:45 - How to Raise Testosterone Naturally (The Pillars)42:29 - Low-Dose Cialis for Cardiovascular Health (Preemptive Strategy)43:59 - Dr. Lyon's TRT Journey and Early Concerns46:00 - OutroKey Takeaways:Low Testosterone is a major health risk. Low T is linked to an increased risk of bone fracture, heart disease, obesity, and diabetes.TRT is often protective. Contrary to the long-standing myth, replacing low testosterone to a healthy range does not increase the risk of cardiac events. It is, in fact, protective against cardiovascular disease.Erectile Dysfunction (ED) is a canary in the coal mine. ED can predict a heart attack within 3 to 5 years.Men can maintain high T levels. Testosterone levels in a healthy 75-year-old can be just as good as in a man in his 20s.The solution is often concurrent. For an obese or overweight man with low T, concurrent treatment with TRT and a lifestyle intervention is often the most responsible approach to provide the motivation needed for weight loss.Low-dose daily Cialis (2.5mg to 5mg) may be a viable preemptive strategy for cardiovascular health due to its blood flow benefits. This episode is brought to you by:Manukora - Get $25 off your Starter Kit at

View the Show Notes Page for This Episode Become a Member to Receive Exclusive Content Sign Up to Receive Peter's Weekly Newsletter In this special episode of The Drive, Peter hosts a strength and conditioning roundtable with three experts in the field—Gabrielle Lyon, Jeff Cavaliere, and Mike Boyle. Together they explore why maintaining muscle mass, strength, and power is essential for healthspan and longevity, and examine how resistance training supports metabolic resilience and injury prevention across the lifespan. The conversation covers practical strategies for getting started and staying consistent, the importance of a protein-centered diet tailored to age and activity level, and approaches to resistance training for peri- and post-menopausal women—including tendon care and optimal programming. They debate single-leg training versus heavy bilateral lifts, share knee-friendly lower-body options, and highlight exercises that belong in the “graveyard” due to poor risk-reward ratios. Finally, the group discusses youth sports specialization, emphasizing the long-term value of variety for developing lifelong athletes. We discuss: Personal career journeys and philosophies of each guest that shaped their approaches to strength, conditioning, and lifelong health [3:30]; Why so few people engage in resistance training despite its proven health benefits [8:30]; Mike's low-stress, high-consistency method for training beginners to resistance training [18:45]; How resistance training enhances metabolic health and body composition even in lean, highly active endurance athletes [25:30]; The dominant role of nutrition in shaping physique and achieving low body fat, the synergistic effect of resistance training, and the unrealistic expectations about muscle growth and fat loss [28:30]; Why Jeff advocates for five meals a day, and why meal timing matters less than overall protein intake and caloric consistency [38:00]; Optimizing protein for every stage of life: quality, quantity, and guidelines for diverse diets and body types [44:15]; The advantages of unilateral lower-body training over heavy bilateral lifts: impact on strength, athleticism, recovery, and functional movement [51:45]; Rethinking heavy squats and deadlifts: risk-reward, aging bodies, and the case for reverse lunges and other single-leg alternatives [1:01:15]; Adapting with age: Gabrielle on injuries, hip dysplasia, and the shift to smarter training [1:10:15]; Exercises that belong in the “iron graveyard”: unnecessarily risky exercises and their safer alternatives [1:19:15]; The downside of early sports specialization in children and the long-term benefits of encouraging kids to play multiple sports [1:25:00]; Advice for preventing an Achilles injury: calf strength, ankle mobility, and listening to pain signals [1:33:15]; Shoulder pain: how to keep training the upper body when shoulder pain limits pressing movements [1:40:45]; Effective strength training strategies for women before, during, and after the menopausal transition [1:43:15]; Best practices for strength training and athletic development in children, and the pitfalls of early sports specialization [1:50:30]; How to foster healthy habits and an interest in fitness beyond playing sports [2:00:00]; Something each guest has changed their mind about in the last five years [2:04:00]; and More.
