Here on the Fertility Forward podcast, we often say that knowledge is power — and that you are your own best advocate! Joining Rena and Dara on the show today are two passionate guests who are dedicated to educating women about infertility, fertility benefits, healthcare coverage, and women's health. We're thrilled to welcome Dr. Taraneh Nazem, a double board-certified Reproductive Endocrinologist, Infertility Specialist, and Obstetrician-Gynecologist at RMA of New York's Westside office, along with Carissa Simek, one of Dr. Nazem's patients. In today's episode, Carissa opens up about how her desire for better fertility coverage led her to change jobs — and the incredible fertility journey she's walked with Dr. Nazem. They discuss the panel discussions they've led, the catalyst behind Carissa's decision to preserve her fertility, and how advocating for fertility education has empowered Carissa in her own life. We also explore the conversation around freezing eggs versus embryos, the biggest challenges Dr. Nazem faces in her field, and how infertility treatments can impact daily life. For all this and much more, don't miss this insightful conversation!
What if your pain, irritation, or sexual discomfort was being dismissed because no one ever taught your doctor how to look for the real cause? There's a reason vulvar conditions like lichen sclerosus are often missed or misdiagnosed—and it's not because they're rare. It's because most doctors aren't trained to recognize the signs. In this episode, I'm joined by Dr. Jill Krapf, a dedicated gynecologist and vulvar skin expert, to unpack the truth about vulvar dermatoses, why it takes so long to get diagnosed, and what can be done when scarring, pain, and sexual dysfunction start taking over your life.We don't just skim the surface—we go deep into the subtle (and not-so-subtle) symptoms, the stories of people who were told nothing was wrong when something clearly was, and what really works when managing chronic vulvar skin conditions. Jill also shares insights about how women of color are even more likely to be overlooked and misdiagnosed because of gaps in training and representation in medical education. If you've ever been told “it's just in your head,” or if you've silently dealt with vulvar discomfort, pain with sex, or itching that won't go away—this episode is a must. There is real hope and effective care available. You just need to know where to look—and what to ask.
Women go on hormone therapy to manage peri/menopause symptoms and feel better. But sometimes to their surprise, they feel worse. Or some symptoms go away while others pop up. That's because of the hormones! We have hormone receptors all over our body and whenever we switch up the signals they're receiving, whether through the menopause transition itself or through the hormones we use, they're going to respond to those changes, and sometimes not in the way we want. Most side effects are manageable with time or by dialing in delivery methods and/or doses. It's just important to know what to expect. We talk all about it this week with our resident menopause specialist Dr. Carla DiGirolamo.Carla DiGirolamo, MD, PhD, is a Double Board Certified Reproductive Endocrinologist and Obstetrician/Gynecologist specializing in infertility and menopausal medicine. Welcome to Menopause Research Review! Now that menopause has hit the mainstream, we're swimming in a steady stream of studies, which is great and can also be overwhelming. So for 2025, we're introducing a regular segment called Menopause Research Review where we break down key research as it comes out with resident reproductive endocrinologist and menopause performance specialist Dr. Carla DiGirolamo. Bioidentical Hormone discussion with Dr. Karen Kaufman, board certified OB/GYN. Karen Kaufman, MD is a board-certified Obstetrician/Gynecologist who has been practicing in Boulder, Colorado since 2003. Her medical school training was at Case Western reserve in Cleveland, Ohio followed by OB/GYN residency at Northwestern University in Chicago, Illinois. While practicing conventional medicine in Boulder, Dr. Kaufman became aware that patients needed and wanted more than what they were receiving from traditional Western Medical healthcare. She observed that while traditional medicine had its role, many of her patients symptoms and issue weren't being addressed with this approach. She pursued the idea of functional or integrative medicine and then completed fellowship training with the American Academy of Anti-Aging Medicine where she honed her skills using bioidentical hormones. We will be discussing:1. Bioidentical hormones do NOT increase breast cancer risk. If estrogen causes breast cancer then why don't 25 year women, bubbling with estrogen get breast cancer?2. VAGUE language creates confusion – HORMONES – ESTROGEN. We discuss the actual names of bioidentical hormones such as estradiol and distinguish it from drugs such as birth control pills or Premarin.3. Birth control pills, Premarin and Provera are not hormones. They are drugs. Drugs are useful but dangerous and are never a part of “Bioidentical hormone replacement”.4. Bioidentical means it is identical to the hormones you have made your whole life and need forever if you intend on having a vital life. 5. Top 4 benefits derived from using BHRT: Reduce risk of heart attack by 50%. Brain protective and reduces risk of Alzheimer's by 30-60%. Bones stronger and reduced risk of osteoporosis. BEST first line treatment for bone density. Longevity: mice studies show 19% increase in lifespan.6. Why doesn't my OB/GYN offer BHRT? Hormones are not “drugs” so it is not taught in traditional western medicine. The vast majority of OB/GYN physicians know next to nothing about true bioidentical hormones or how to use them.

Of all our hormones, cortisol might be the most misunderstood and maligned. We're told this stress hormone is the culprit behind excess belly fat, high blood sugar, accelerated aging, and a host of menopausal and midlife woes. Menopausal and midlife women sometimes are afraid to exercise too much because they've been told it will raise their cortisol, which can already be elevated during this time of life. The problem with this narrative is that it's one-sided. It paints cortisol as a demon to be avoided at all costs, when actually cortisol is essential to maintain and improve fitness, and frankly for human life. This week we sat down with endocrinologist Dr. Carla DiGirolamo to dive into what cortisol is, what it does, and how menopausal women can manage–even befriend it–to optimize their health, performance, and well-being.Carla DiGirolamo, MD, PhD, is a Double Board Certified Reproductive Endocrinologist and Obstetrician/Gynecologist specializing in infertility and menopausal medicine. She is also a Crossfit Level 1 trainer, Certified Nutrition Coach, and member of CrossFit Health. In today's episode with Dr. LaKeischa McMillan, you'll hear us take a deep dive on: 1. Perimenopause versus menopause: Perimenopause is the time when the woman's body starts transitioning to menopause. The transition can happen ten to fifteen years before menopause, and the ovaries begin producing fewer hormones. Menopause is when a woman is without a period for twelve consecutive months. 2. Perimenopause symptoms: Though perimenopause can begin in some women in their 30s, usually it starts in women aged around 40 or 41. It's the time when testosterone and progesterone start falling and cause symptoms like sleep trouble, brain fog, headaches, vaginal dryness, mood changes, and many others. 3. What Can Cause Perimenopause and Menopause Symptoms to Worsen? Stress and trauma are the most impactful factors that can worsen perimenopause and menopause symptoms. Other things may include alcohol, smoking, processed food, or various chemicals. Now that menopause is mainstream news, interest in hormone therapy is exploding. And so is the confusion and controversy surrounding it, as menopausal hormone therapy (aka hormone replacement therapy), which is undeniably a game changer for millions, has also become a massive business. Some practitioners are reportedly prescribing “megadoses” of estrogen–in some cases twice or even 3 or 4 times the maximum licensed dose, causing myriad medical societies to sound the alarm. Meanwhile, researchers are working overtime to catch up to provide clarity on what hormone therapy does, what it doesn't do, who it benefits most, and for how long. This week we dig into it all with our favorite hormone doc, Dr. Carla DiGirolamo. Dr. Debbie Mueller thought she was not interested in painting or the arts and one day on a whim tried painting and found joy. She is now a prolific and accomplished painter and tells her story in this episode. About Dr. Mueller from her website: Late Bloomer Artist This is the story of an accidental artist, the story of an Obstetrician/Gynecologist, Mother, and Wife, who discovers, at the age of 56, that she is a painter. I am the daughter of an artist mother, and crafty father, and had always seen myself as having received my father's genes. I pursued hobbies of mosaic making, pottery, and even photography, leaving the fine art to my mother. However, when visiting my parents in Sarasota, Florida in February of 2016, my life changed. On a rainy afternoon, with nothing else planned, a set of acrylic paints appeared, and I produced my first painting of a dock at dusk. My life was forever changed. Since then, I have not stopped learning, and have not stopped painting. I have studied with generous teachers, and have taken away important lessons from each. I have fallen in love with painting from life, and have embraced plein air and still life painting, as well as painting the figure. I delight in having discovered this hidden passion, and look forward to living the rest of my life as an artist. While I am drawn to the still life and landscape in all their wardrobe, my heart sings when I can paint the dance between light and shadow. In this illuminating episode, we talk with Dr. Kristyn Brandi, an Obstetrician-Gynecologist, proud abortion provider, and board chair with Physicians for Reproductive Health, where she debunks the “biological clock” among other pronatalist myths and shares how the medical institution in one of many that fuels pronatalism. We also discuss what the overturning of Roe v Wade means in terms of safe abortion care for the already marginalized communities, including BIPOC, LGBTQ, people with disabilities, and those living in poverty, within the U.S.. We end the conversation by unpacking the dark history of reproductive control, including slavery, eugenics, and population control, and why centering reproductive autonomy through a deep understanding of Critical Race Theory (CRT), Reproductive Justice (RJ), and pronatalism, is essential to upholding reproductive rights and a sustainable environment. Ellement was actually a sponsor of the podcast last year and I invited them to come back because what they're doing with prenatal supplements is pretty cool. The short story is that, unlike most prenatals, these supplements are made specifically for each individual person based on their unique needs or dietary preferences. I love the idea of things being more personalized and less one size fits all; I think that's the future of where supplements and healthcare overall are headed. Please know that I have no financial interest here. They are no longer sponsoring the podcast - I just want to share things with you that I think are important and useful. In this Episode, You'll Learn About: -Why we recommend prenatal vitamins -When you should start taking them -What the difference is between folic acid and methylfolate -What role supplements should serve in pregnancy nutrition -How prenatals can be deficient -Why individualized care is important -How Doreen's personal experience with pregnancy and birth inspired her to start Ellement -How the cost of Ellement stacks up against other prenatals or mixing and matching vitamins --- Full website notes: drnicolerankins.com/episode191 Check out The Birth Preparation Course Register for the class How to Create a Birth Plan the Right Way
Dr. Jaime Seeman, known as doctorfitandfabulous on social media, is a board-certified Obstetrician-Gynecologist with a background in nutrition, exercise, and health science. She is also a current Fellow in Integrative Medicine, a Board Certified Ketogenic Nutrition Specialist, and the Chief Science Officer at BioCoach. Dr. Jaime has a passion for preventative medicine and is an expert in women's health. In this episode, we dive into a few very critical and hard-to-discuss topics like birth control, difficulties getting pregnant, and the importance of muscle-building for women. Dr. Jaime talks about the ways women can optimize their health if they are taking birth control including the specifics of certain nutrients that are often depleted in these circumstances. We also talk about the importance of nutrient-sensing pathways that play a critical role in signaling that the female body is in an optimal environment to reproduce and how this is a big reason why so many people are struggling to get pregnant. Dr. Jaime puts a big emphasis on the importance of muscle building for women when it comes to anti-aging, hormones, and even metabolic health. Dr. Jaime also gets into the nitty gritty of her new book Hard to Kill and how important it is for us to take control of the 4 pillars of health to increase our resilience and set us up a longer, happier, and healthier life. In addition to Dr. Seeman's book, she offers an academy for Hard to Kill where she leads you through her program providing you with the support you need to make the incredibly beneficial lifestyle changes she recommends.
Since we started this podcast, many women have gone from being afraid to take hormone therapy to being afraid not to take hormone therapy, as some in the menopause field are saying that menopause is really a “hormone deficiency” and we can't age well without it. This week we dive in on what hormone therapy is; what it can do (especially for performance and health); what it can't do, and what we're still learning about all of it with Dr. Carla DiGirolamo.Carla is a Double Board Certified Reproductive Endocrinologist and Obstetrician/Gynecologist specializing in infertility and menopausal medicine. She is also a Crossfit Level 1 trainer, Certified Nutrition Coach, and member of CrossFit Health. She runs an online personal training and nutrition coaching business, FitforLifeMD, specializing in the care of women in all age groups, but with a special interest in the comprehensive care of peri/postmenopausal women. She also writes a regular blog on Athletic Aging. The Dobbs decision that overturned Roe v. Wade has, in many states, decimated access to abortion care and prevented abortion providers from treating patients. Dr. Leah Torres, Obstetrician/Gynecologist practicing in Tuscaloosa, Alabama, sits down to talk with us about the state of abortion access in Alabama and how the United States' healthcare system layout is generally preventing access to quality care. Currently, there is no abortion access in Alabama. Prior to the overturning of Roe v. Wade, the rate of people dying in pregnancy and childbirth in Alabama was twice the national average. Lack of abortion access will only make these extremely upsetting numbers worse. In addition, the United States' current healthcare system is compounding these dangers for pregnant and birthing people in this country. From transgender healthcare to abortion to vaccination, the politicization of modern medicine comes from the select few embedded in our current healthcare system who have money and power to gain by exploiting the framework and those who seek care within it. Some states, such Alabama, continuously refuse to expand their Medicaid program blocking insurance coverage for those who need it most, while also limiting which providers and clinics can receive Title X funding. Medical students continue to face barriers to training in abortion care. Many residency programs state the miscarriage management training is the same as abortion care training, which is not true. Fellowships, which offer training in certain areas, are often years-long. This means after medical school and residency, many who want to learn about abortion care feel the need to take on another two-year long fellowship in family planning.A lack of sex education across the United States and the gatekeeping of comprehensive information about healthcare intensifies the inability to realize sexual and reproductive health care. When powerful institutions control the narrative about our health and bodies, often informed by “religious freedom,” this can lead to interpersonal relationship problems and mis- and dis-information. While this may seem big-picture, it has a very pin-pointed impact, leading to providers like Dr. Torres to treat individual patients who are coming in for treatment because of lack of access to education and basic care elsewhere. Many who seek care at Catholic hospitals (entities that don't often make their religious affiliation clear), may not receive the reproductive healthcare they want or need because of religious directives.
