Jake Johnson is Western North Carolina to his core.  Born and raised in rural Polk County, he knows just about everyone in his close-knit district spanning over Rutherford, McDowell, Henderson, and his home county. The 30-year old Republican talks about his public service, including his important position as co-chair of the House Oversight and Reform Committee, also called Gov-Ops for short.   However, the conversation isn't all business as Skye and Brian discover Rep. Johnson has a love for (almost) all things James Bond, and he has family ties to moonshine, racing, and coon dogs.   Plus, NC is inside two weeks of Election Day, early voting numbers are coming in strong, Mark Robinson amends his lawsuit, #TOTW, Skye's appearance on State Lines gets unwanted attention to her feet, and more.   The Do Politics Better podcast is sponsored by New Frame, the NC Travel Industry Association, the NC Beer & Wine Wholesalers Association, the NC Pork Council, and the NC Healthcare Association.

In the state of Oregon, there is a glut of grass. A wealth of weed. A crisis of chronic. And, jokes aside, it's a real problem for people who work in the cannabis industry like Matt Ochoa. Ochoa runs the Jefferson Packing House in Medford, Oregon, which provides marijuana growers with services like drying, trimming and packing their product. He has seen literal tons of usable weed being left in marijuana fields all over the state of Oregon. Because, Ochoa says, there aren't enough buyers. There are just over four million people in Oregon, and so far this year, farmers have grown 8.8 million pounds of weed. Which means there's nearly a pound of dried, smokable weed for every single person in the state of Oregon. As a result, the sales price for legal marijuana in the last couple of years has plummeted.Economics has a straightforward solution for Oregon's overabundance problem: trade! But, Oregon's marijuana can only be sold in Oregon. No one in any state can legally sell weed across state lines, because marijuana is still illegal under federal law. On today's episode, how a product that is simultaneously legal and illegal can create some... sticky business problems. Joe Biden's refugee resettlement program is moving illegal alien minors across state lines to get abortions. In some cases, it appears, the government is paying non-governmental organizations (NGOs) to move the children. It looks like these NGOs are also paying to facilitate the abortions. We have the documents to prove it.Goldstar families are speaking out against Joe Biden, and the Afghanistan withdrawal pull out. They all have very harsh words for the so-called commander-in-chief. Hear their shocking testimony for yourself.  From one debacle to another. The open border crisis is out of control. Wait till you see the gates in our border wall that are chained open!  Apparently to let wild animals move freely across our southern border.  We are talking real wild animals, not the illegal criminals. Should they uproot their lives and leave the state?Kat is 14 and transgender. The Utah law banned the medical care that Kat was considering.Around 20 states have passed similar laws — meaning many families could face the same tough decision: whether to leave their homes and where to go. Often it's to a state like Minnesota, where elected officials have protected trans health care for patients and providers. We speak with reporters Saige Miller from KUER in Salt Lake City and Dana Ferguson, a political reporter with Minnesota Public Radio to hear how this patchwork of laws in both states affects trans patients and their doctors. In participating regions, you'll also hear a local news segment to help you make sense of what's going on in your community.Email us at considerthis@npr.org.

Navigating Cross-Jurisdictional Practice Permissibility: Insights and Considerations for Therapists  Expert Tips from Person-Centered Tech In this episode, I welcome Liath Dalton, the director of Person-Centered Tech, a valuable resource for therapists when it comes to all things tech-related.    Liath dives into the recent changes in telehealth regulations and the impact on therapists, specifically focusing on the expiration of the federal public health emergency (PHE) and its effects on cross-jurisdictional practice and telehealth coverage.   She discusses how the expiration of the PHE does not significantly impact the temporary practice provisions or the expanded telehealth coverage under Medicare. However, and highlights the importance of checking with private payers as coverage may vary, especially for out-of-network benefits.   The conversation shifts to HIPAA compliance and the requirement for therapists to use HIPAA-compliant video platforms and BAA's with phone service providers, including cell phone providers, as s an alternative.   Throughout the episode, Liath stresses the need for therapists to ensure the security of their devices and the importance of safe harbor from HIPAA's breach notification rule.    Overall, this episode provides valuable insights and practical advice for therapists navigating the changing landscape of telehealth regulations and HIPAA compliance. To learn more, listeners are encouraged to explore Person Center Tech's website and resources. Liathana Dalton Director, Person-Centered Tech https://www.personcenteredtech.com   Helping the Helpers Since 2010 PCT's Start Here -- includes an overview of the PCT Way system for optimizing and fortifying your practice, including an interactive tool for identifying your highest priority-need and where to begin (we believe in taking an incremental approach, so that it's a manageable process) PCT's Practice Care PCT resources for Device Security (included in Practice Care Premium) 50 State Tool, Teletherapy Practice Rules by State Cross-State Due Diligence Worksheet PCT's Telemental Health Certificate Program PCT's Service Selection Workbook & Worksheets (FREE!) which provides: a process for identifying all your practice's functionality needs for client care delivery and communications, as well as internal operations. identifies the best -- efficient, effective, and affordable --specific HIPAA-compliance-compatible services to meet those functionality needs. can be utilized to refine and optimize an existing "tech stack" (the combination of services that provide for meeting a practice's functionality needs) or to create your tech stack from the ground up. This is a friendly reminder that there are always as many as three jurisdictional authorities that have a say in determining whether or not you have permission to practice across jurisdictions: the state where client is physically located at the time of session the provider's state(s) of licensure the state where the provider is physically located at the time of session You must have permission to practice/lack of restriction/prohibition to practice from *each* applicable licensing board/jurisdictional authority. For all the details on this, check out our most recent on-demand CE training on cross state practice, Cross Jurisdictional Teletherapy Practice: Case Law, Rules of Practice, and Working with Minors across State Lines, presented by Eric Ström, JD PhD LMHC -- which also includes our cross-jurisdictional permissibility to practice due diligence worksheet (that includes all 3 potentially applicable jurisdictional authorities.) ... but, "what if I do 'coaching' or 'consulting' instead of 'therapy' when I'm physically located in a jurisdiction that restricts practice like Alaska? Or, what if I do 'coaching' instead of 'therapy' when a client's located in a jurisdiction that I don't have permission to practice in?" is a question we've, understandably, had posed to us a great number of times. Well, it's not as simple as that... and all those nuances and 'devil in the details' aspects of that are something Eric expertly addressed, supported by case law examples, in the Cross Jurisdictional Teletherapy Practice training. If you're wondering about what's current in terms of how cross-jurisdictional practice works, that training contains the information and conceptual framework to empower you to navigate cross-jurisdictional practice legally. Liathana Dalton Director, Person-Centered Tech https://www.personcenteredtech.com    Key points  The expiration of the federal public health emergency does not change cross-jurisdictional practice permissibility or temporary practice provisions. Licensing boards in certain states may restrict teletherapy practice from within their borders, requiring providers to be licensed by that specific board. Permission to practice is determined by the state where the client is located, the provider's state(s) of licensure, and the state where the provider is physically located during the session. It's important to understand and comply with the jurisdictional authorities and licensing board requirements to ensure legal cross-jurisdictional practice. Abortions are now illegal in 12 states, according to the Guttmacher Institute. That means many pregnant people now have to travel hundreds of miles to get an abortion. When Roe v. Wade was still the law of the land, it took on average 30 minutes for an abortion patient to get to a clinic. Now it takes an hour and ten minutes, according to a 2022 study from the Journal of the American Medical Association.Abortion patients now have to go to "safe haven" states like Kansas. But can these states keep up with the increased demand? We discuss safe haven states, and how grassroots groups are helping those seeking abortions get there. This conversation is part of our Remaking America collaboration with six public radio stations around the country. The series explores Americans' trust in institutions and the health of our democracy.Want to support 1A? Give to your local public radio station and subscribe to this podcast. Have questions? Following a normal first trimester, a pregnant patient starts spotting. The patient's care provider books an urgent ultrasound to see what is causing the bleeding. The main concerns are if the spotting is caused by a miscarriage or something else.Following the ultrasound, the technician calls in obstetrician Dr. Ashley Brant to review the results with the patient. There is a problem with the fetus' heart — specifically, a condition called hypoplastic left heart syndrome. Essentially, the left side of the heart doesn't develop normally and can't pump blood in the way that it should.Hypoplastic left heart syndrome causes poor oxygenation, meaning the skin can be bluish or with dark discolorations. It also causes difficulty breathing, feeding, and lethargy. Treatment includes multiple surgeries after birth, and can even require a heart transplant. Without treatment, the condition is fatal. The patient is offered genetic testing to determine if the heart condition is a symptom of a larger genetic disease. Regardless, the prognosis is grim. The patient meets with Dr. Brant to discuss all of the options, including continuing or ending the pregnancy. “I think everybody who's in a situation where they're thinking about ending a pregnancy because of a major fetal anomaly, they are thinking about what is the kindest decision, the most loving decision that they can make for their baby,” shared Dr. Brant. “Nobody wants to be in this position. And they're thinking about what the experience is going to be like for this child.”The patient makes the decision to end the pregnancy through the dilation and evacuation method. However, the procedure cannot be performed in the state because of a heartbeat law in place at the time. And so Dr. Brant refers the patient to an out-of-state clinic where the initial procedure to stop the heartbeat can be performed. But, in order to be where the patient has the support of the medical team she knows and who has been by her side, the patient returns to her home state for the final procedure. “No one ever envisions themselves needing an abortion. No one ever thinks, ‘I want to have an abortion,' before they're in a position of needing one. I would just encourage compassion and empathy and trying to understand the life that someone else might be walking in.”

In states where abortion is severely limited or illegal, clinicians face imminent prosecution if they continue to provide abortions. What is much less clear is what happens if providers in blue states offer telemedicine abortions to women in states where that's against the law. These clinicians, too, could be arrested or sued or lose their medical licenses. To protect themselves, they may have to give up traveling to certain parts of the country — and it's still no guarantee. In the face of so much uncertainty and an invigorated anti-abortion movement, large organizations and most clinicians are loath to gamble. But some providers think that the end of Roe v. Wade calls for doctors to take bold action. This story was recorded by Audm. To hear more audio stories from publications like The New York Times, download Audm for iPhone or Android.
