Disability Series, #4 of 4. The Tuskegee Syphilis Study was an ethically problematic, to say the least, medical research project conducted in Alabama. Officially titled “The Effects of Untreated Syphilis in the Negro Male,” this government-sponsored research project was conducted by the United States Public Health Service in Macon County, Alabama, between 1932 and 1972. For four decades, researchers observed the progression of untreated syphilis in approximately 399 African American men without their informed consent. Many of the men thought they were being treated for “bad blood,” which had a variety of connotations. They were not aware that they were being actively blocked from receiving effective treatment, even after penicillin became the recognized standard of care for syphilis in the 1940s. The origins of Covid-19 still remain somewhat hazy, over three-and-a-half years since the virus began spreading across the world. That has led to a number of prominent conspiracy theories, with 25% of people in the UK believing that Covid was definitely or probably a hoax. That finding was part of an April 2023 survey by Savanta for King's College London and the BBC. It also established that belief in other conspiracy theories is relatively high too, like the idea that the cost of living crisis is a government plot to control the public. Carl Elliott is a bioethicist at the University of Minnesota who was trained in medicine as well as philosophy. For many years he fought for an external inquiry into a psychiatric research study at his own university in which an especially vulnerable patient lost his life. Elliott's efforts alienated friends and colleagues. The university stonewalled him and denied wrongdoing until a state investigation finally vindicated his claims. His experience frames the six stories in this book of medical research in which patients were deceived into participating in experimental programs they did not understand, many of which had astonishing and well-concealed mortality rates. The Occasional Human Sacrifice: Medical Experimentation and the Price of Saying No (Norton, 2024) is an intellectual inquiry into the moral struggle that whistleblowers face, and why it is not the kind of struggle that most people imagine. Carl Elliott is a bioethicist at the University of Minnesota who was trained in medicine as well as philosophy. For many years he fought for an external inquiry into a psychiatric research study at his own university in which an especially vulnerable patient lost his life. Elliott's efforts alienated friends and colleagues. The university stonewalled him and denied wrongdoing until a state investigation finally vindicated his claims. His experience frames the six stories in this book of medical research in which patients were deceived into participating in experimental programs they did not understand, many of which had astonishing and well-concealed mortality rates. Beginning with the public health worker who exposed the Tuskegee Syphilis Study and ending with the four physicians who in 2016 blew the whistle on lethal synthetic trachea transplants at the Karolinska Institute, Elliott tells the extraordinary stories of insiders who spoke out against such abuses, and often paid a terrible price for doing the right thing. Clinical trials can be a ticket to better healthcare, so breaking the barriers that limit People of Color to participate is essential. In this episode of the Straight Out of Health IT podcast, Jayne Morgan, executive director of Health and Community Education at Piedmont Healthcare Corporation, has a chat with Christopher Kunney about the current lack of minority participation in clinical trials and how to overcome the obstacles that directly impact it. Diverse populations have a mistrust towards the healthcare systems as a result of multiple care disparities and events like the Tuskegee Syphilis Study. Hence, how physicians approach these patients and build trust with them is incredibly important when offering research opportunities. Amongst the many effects of COVID-19, there was an increase in confidence in health providers and participation of communities of Color in clinical trials, but the challenge lies in maintaining the momentum now that the public health emergency has ended. Having established the gravity of the problem, Christopher and Jayne discuss ways to ensure equitable access to research in healthcare. Trial diversity will ensure treatment applicability for a broader range of people instead of a population predominantly composed of white men or other majority groups. Jayne explains how clinical trials offer access to tomorrow's medications and treatments. Being able to participate in a trial means having access to maximum therapy quality and attention, and in cancer trials, participation has led to increased remission rates. For her, representation is a crucial step toward rebuilding trust and moving beyond exploitation toward health equity. One of the very first things to accomplish to encourage participation from People of Color is to involve physicians who have a cultural congruence and trusting relationship with the community in leadership roles within clinical trials, and it needs to happen at all levels of research to establish cultural understanding and trust when interacting with patients. Even though community involvement through leaders, churches, and civic organizations can have impactful results, direct dialogue with physicians will ultimately open the first gates to clinical trials for diverse people. This is why Jayne encourages listeners to find providers who communicate effectively and engage in conversations with the healthcare system to understand and learn the necessary information firsthand. So let's talk about the intersection of African-American history and medicine... Today's episode is more than just medicine; today, we will talk about something that many people fail to recognize. Something that is written in the records, yet many people ignore or refuse to acknowledge. Today, we will talk about history – the history between medicine and the African-American community in particular. When we talk about chronic diseases, African Americans are always at a higher risk than any other community. The same is the case when talking about lack of trust in the healthcare system; only 3% of Black Americans believe that doctors will do what is best for them, and 50% of them believe that the healthcare system as a whole is racist. It is sad, but that's the reality, and there's no way we can address the health disparities and the distrust if we don't understand why the disparity and distrust are there to begin with. That's why in line with Black History Month, we will be talking about the connection between medicine and African-American history. Join me as I shed light on why we can't discuss the advancements of modern medicine without mentioning all the atrocities, deceit, and exploitation that it did on the African-American community. After being slammed with multiple strikes from YouTube for "medical misinformation" violations, Dr. Drew's account received a review from a human moderator and was reinstated. Most creators have no recourse when their voices are censored by big tech platform algorithms.  These events also add an alarming layer to recent news that California's state Senate passed AB 2098, a bill that threatens doctors with disciplinary action if they share "misinformation". Though seemingly well-intentioned, the bill's vague terms could easily be weaponized to censor medical experts simply for disagreeing with their peers. Who decides what is misinformation?
