Die falsche Dosierung eines Medikamentes, eine Verwechslung von Patienten bei der Behandlung, ein Instrument, das nach der Operation im Körper der Patientin vergessen wird: In der medizinischen Fachsprache spricht man bei solchen gravierenden Fehlern von sogenannten Never Events. Ereignisse, die in der Medizin eigentlich nicht passieren dürfen und dennoch vorkommen. Wie können solche Fehler vermieden werden? Und wie könnte der Umgang damit aussehen? Das steht im Fokus an der Internationalen Patientensicherheitstagung, die heute und morgen in Zürich stattfindet. Fachleute diskutieren dabei auch über ein nationales Melderegister, in dem schwerwiegende Fehler künftig erfasst werden sollen. Diese Forderung kommt von der Stiftung Patientensicherheit, die an der Tagung auch eine neue Liste mit Never Events präsentiert. What is the impact of nasogastric tube misplacement on a family? NG Tube related Never Events continue to occur, but most healthcare professionals don't get to see the impact on the lives of families. For episode 4 of the NGPODCAST, we speak to our dear friend Deahna Visscher, who's son Grant died at just 11 days old after being fed through a misplaced NG Tube, about what happened that day and what she has done since then to try and prevent other families suffering the same tragedy. Since his death in 2008 she has been on a quest to find a solution to prevent others from experiencing the loss that she was due to a misplaced feeding tube. She has become a parent partner on the Patient Safety Committee at the hospital where Grant died at and has also become a member of the American Society of Parenteral and Enteral Nutrition (ASPEN) sub-committee NOVEL (seeking New Opportunities for Verification of Enteral tube Location). We are also joined by Beth Lyman (MSN, RN, CNSC) who is a nurse consultant who formerly worked at Children's Mercy Kansas City. She has been a Nutrition Support Nurse for 36 years with 26 of those years at a children's hospital. In medicine there is a term NEVER-EVENTS. It means events that are NEVER supposed to happen while under medical care. For example, no patients is supposed to FALL DOWN while in the hospital. Never events are always bad things to be prevented. So lots of time, energy, design and protocols are utilized to try and make that a reality. Today's passage notes a spiritual/physical NEVER-EVENT. This is a GOOD thing. A great thing. A NEVER IN THE HISTORY OF MANKIND sort of thing: A blind man gets eyes to SEE. He seems to be the only one who realizes what a wonder this is. The pharisees miss it. The disciples seem to miss it. Even his parents miss it.Also, does God listen to sinners? It depends. What do they say and with what attitude. Of course God listens to sinners. Otherwise there would be no communication with Him, as we are all sinners. What are some of the security measures you need to have in place to protect your practice? In today’s day and age, security breaches can come from many different directions. A hacker will infect your system with an encryption virus, and to regain access you have to pay a ransom. This isn’t just a thing happening on TV shows anymore. It’s something that has to be investigated by the FBI, reported to the Office for Civil Rights (OCR), and potentially even media outlets. Is there anything you can do to prepare? What do you are attacked? Invest in a great IT company that will answer any questions you may have. Obtain adequate cyber-insurance coverage. Have legal counsel in place that you can call if it’s ever necessary. Prepare as well as you can—make sure you have more than adequate website security. Don’t let your practice be compromised and watch your reputation take a nose-dive. Do everything you possibly can to protect your practice and your clients. We give you a few options in this episode! HIPAA breaches and how to prevent them If you do indeed experience a breach of protected health information, you must have systems in place to handle the situation. You need a designated privacy officer and security officer. If you’ve done your due diligence and have systems in place, it can be taken into consideration if there is an investigation. A forensic investigation is recommended (especially in the case of a cyber-attack) to determine the scope of the breach and what information was affected. Assess where your vulnerabilities may be and take preventative action. Was there a change in technology? Where are your gaps? Don’t simply rely on your IT company telling you that you’re safe—ask questions so that you understand and know you’ve done everything possible. How to prevent “Never Events” “Never Events”, usually referred to as “Sentinel Events” are unanticipated extraordinary events that incur serious injury, illness, or the death of a patient. Unfortunately, events like these can and do happen (we talk about prevention and preparedness in depth in this episode). Even if you do everything right and are a great clinician, the litigation and stress involved in the process can be debilitating. You can’t turn back time—but you can be proactive. Have checklists in place to make sure life-saving medical devices are checked regularly. Practice life-saving procedures with your staff. Be prepared for the unexpected. Controlled substance rules, regulations, and safety measures A state board rule was enacted in Texas eff. September 1st requiring providers to check a patient's substance history before prescribing controlled substances. There are also whispers that a law will be enacted in 2020 that will only allow prescriptions for controlled substances to be done electronically. Do your research now to find a compatible software to have in place by the time this is state law! It’s an added safety measure that will protect you from some liability as well as protect your patient’s life. In the meantime, if you prescribe or utilize controlled substances they must be securely locked up (behind TWO locks and TWO keys). You must have a log to track dispensing that is stored safely with the drugs. When Terry Shaw’s wife was the victim of a car accident in 2016, it completely changed his outlook on care delivery. Even as president and CEO of AdventHealth, one of the nation’s largest faith-based health systems with 50 hospitals across nine states, Terry had a difficult time navigating his wife’s recovery. He spent six months calling in favors to find the right physicians and coordinate care in the out-patient setting. And that’s with a lifetime of healthcare knowledge and the connections to match. If the road was so steep for Terry and his family, how is the average person supposed to handle similar circumstances? That question hit home and continues to be a key catalyst for improvements in care delivery that AdventHealth is pioneering under Terry’s leadership. He understands that 95% of contact with patients takes place in the out-patient setting, but knows from first-hand experience just how disaggregated, complex, and difficult to navigate it can be for patients. In this episode of the Healthcare Is Hard: A Podcast for Insiders, Terry recounts the expansion and evolution of AdventHealth, including its growth from a $2 billion to a $12 billion system over the past 20 years. He also talks to Keith Figlioli about the initiatives underway at AdventHealth to revolutionize the future of patient care, including: · Reinventing the Consumer Experience – In most other industries, it’s been essential to build a business around customers, but according to Terry, healthcare is just on the cusp of being required to figure out how to design care around the consumer. This includes processes for making sure people have the right care at the right time – especially after a patient is discharged – and making out-patient care just as efficient as in-patient care. It involves rethinking the whole care process, including the social determinants of health, and putting people in the right environment so that mentally, physically, spiritually and socially they have a better opportunity in life than they had before. · Focusing on “Never” Events. As part of its initiative to rethink the consumer experience, AdventHealth has defined 20 “never events” – situations that it’s working hard to help patients avoid. For example, a patient should never be unable to tell how much a procedure will cost, and should never have to work hard to find care. If they want to make an appointment with their doctor, have someone come to their home, stop by a clinic, or seek care on an iPad, those events may not be traditional primary care, but should be recognized and recorded as such if it works for the individual. To support this, AdventHealth is in the process of rolling out a new app that will enable patients to text their doctor, schedule appointments, access their health records and take a number of other actions that allow them to direct their healthcare in a way that’s convenient for them. · Looking at Patients as More Than a Diagnoses. Do you have someone at home who loves you? Do you have a sense of peace today? Do you have a source of joy in your life? AdventHealth now asks these questions when patients enter their facilities. If the answers are not what a physician would expect to hear, they’ll work to understand what is going on and refer the patient for additional treatment, therapy and/or care. Terry talks a lot about how a person is more than the sum of their diagnoses, but rather should be looked at as a sum of their thoughts, feelings, and beliefs. DiGioia, MD, Founder, The Orthopaedic Program and Innovation Center, Magee-Womens Hospital of UPMC Most of us are familiar with the National Quality Forum’s list of Serious Reportable Events in health care — often referred to as “Never Events.” There’s a wide consensus that everything from performing surgery on the wrong patient or wrong site, to a medication error-induced death, to a physical assault aren’t only tragic and harmful, they are not supposed to happen. Period. It’s a strong statement about patient safety and what the system as a whole should not be willing to tolerate. And, by extension, it’s a call to action to do better and to take care of patients differently so that terrible things do not occur. There are, of course, many ways to draw a line in the sand or to envision the health care system patients deserve and that providers want to work in. One of the most innovative in the last few years has been the Picker Institute’s development of a concept they’ve dubbed “Always Events®.” First conceived in 2009, Always Events®are activities and processes that should routinely be part of patient care and the patient and family experience, to ensure optimal communication, discharge, handoffs, transitions, health literacy, and more.WIHI welcomes the Picker Institute’s Executive Director, Lucile Hanscom; consultant Dale Shaller, who has an extensive history developing benchmarks and measurement systems for patient-centered care; Martha Hayward, who has been working with IHI to help shape public and patient engagement, drawing on her own history as a patient and as a strong and effective leader in Massachusetts; and dynamic leaders from two organizations that have received Always Events Challenge Grants: Gaye Smith of Vanderbilt University Medical Center and Tony DiGioia of the University of Pittsburgh Medical Center (UPMC).Dr. DiGioia’s groundbreaking improvements at UPMC are anchored in a first of its kind patient- and family-centered methodology. Picker’s support is helping to integrate these processes into the hospital’s transplant program. The grant-funded work at Vanderbilt, under Gaye Smith, is targeting better communication and collaboration between patients, family members, and providers to prevent patient falls during hospital stays.
Mr Radu Mihai and Dr Peter Hambly give a talk about rare/never events in surgery. Treating rare diseases raise the challenge of minimal previous exposure to similar operations. Defining a learning curve is therefore controversial as many such 'events' seem to occur in clusters. Similarly, rare complications might occur in 'clusters' hence the response to rare/never events should be interpreted with caution. Mr Mihai is a Consultant Endocrine Surgeon and Dr Hambly is a Consultant Anaesthetist at Oxford University Hospitals NHS Foundation Trust. Mr Mihai is also an Honorary Senior Clinical Lecturer at Nuffield Department of Surgical Sciences, University of Oxford.
Dr Clare Dollery describes a retained swab Never Event that happened in the Churchill Hospital and reflects on how we respond and learn from it. Dr Dollery is Deputy Medical Director at Oxford University Hospital NHS Foundation Trust and chairs the Trust’s Clinical Governance Committee which is a sub-committee of the Trust Management Executive. She has worked as a consultant cardiologist since 2003.
