The ST segment on an ECG represents the interval between the end of ventricular depolarization (QRS) and the beginning of ventricular repolarization (T-wave). It should appear isoelectric (flat) in a normal ECG. What if the ST segment is elevated? This is evidence that there is an injury that goes all the way through the muscular wall of the heart (transmural) This is very concerning for a heart attack (STEMI) but can be occasionally caused by other pathology, such as pericarditis What if the ST segment is depressed? This is evidence that only the innermost part of the muscular wall of the heart is becoming ischemic This has a much broader differential and includes a partial occlusion of a coronary artery but also any other stress on the body that could cause a supply-and-demand mismatch between the oxygen the coronaries can deliver and the oxygen the heart needs This is called subendocardial ischemia What else should you look for in the ECG to identify subendocardial ischemia? The ST-depressions should be at least 1 mm The ST depressions should be present in leads I, II, V4-6 and a variable number of additional leads. There is often reciprocal ST elevation in aVR > 1 mm The most important thing to remember when you see subendocardial ischemia is…history Still, keep all cardiac causes on your differential, such as unstable angina, stable angina, Prinzmetal angina, etc. Also consider a wide array of non-cardiac causes such as severe anemia, severe hypertension, pulmonary embolism, COPD, severe pneumonia, sepsis, shock, thyrotoxicosis, stimulant use, DKA, or any other state that lead to reduced oxygen supply to the subendocardium and/or increased myocardial oxygen demand. Central Otago ratepayers are facing a rates increase of an average of just over 24 percent. The district's mayor, Tim Cadogan says that's a lot better than what the council had considered, which included a rise of around 40 percent. He describes the situation as a "train wreck" brought about by the funding system for local government but adds there's nothing the council can do to prevent it. Cadogan spoke to Ingrid Hipkiss.

Barrie Cadogan (also known as "Little Barrie") is one of the world's first call electric guitarists when it comes to classic soul, rock, punk, and rhythm & blues. He has worked with some of the biggest indie artists performing today including Morrissey, Paul Weller, Edwyn Collins, Liam Gallagher, and Primal Scream. That's a hell of a CV by anyone's standards, but Barrie is perhaps best known for the theme music from the Netflix show 'Better Call Saul.' That glorious guitar intro? That was him! Quite simply, he is very cool. He inhabits a world of pristine big E vintage Levis, Cuban heels, and 1960s Jazzmasters; bespoke hatmakers name their latest designs after him; and, if you are planning a global stadium tour and you need a guitarist that will bring the goods, then you had better make sure that you have Little Barrie on speed dial. In addition, her work has been published in WIRED, Poets & Writers, Catapult, LitHub, Tin House, Longreads, Kenyon Review, The Washington Post, The Atlantic, CNN, Guernica, McSweeney's, Fast Company, Quartz, Colorlines, Romper, and elsewhere. Tensions in a small Clutha town, where residents pay vastly different levies from those set 25 years ago. The Waipori Falls Village- which has about 33 properties- attempted to have a meeting to resolve the problem on the weekend, but got nowhere. Clutha mayor Bryan Cadogan says there's vast discrepancies in costs that aren't reflecting what a leaseholder should pay. He says the first landholders did their due diligence years ago when the houses were first sold off.
