In the push for competency-based education where faculty verify the skills of what a student can do, rather than their knowledge, our series focuses on the application of didactic content with a practical approach so that you can learn nuances of clinical skills before you reach the bedside.    This episode begins with a brief review of malnutrition and pediatric nutritional assessment in acute care settings. Next we begin a choose-your-own-nutrition adventure by asking a series of questions that aid in medical decision-making for which nutrition route is appropriate, and, if enteral feeding is best, then determines the type of tube indicated. A case-based discussion with examples helps you to apply the concepts to a complex scenario. Noah rang the bell and ended his therapy after 18 months of chemotherapy.“Fortunately for us, we had a really positive outcome,” Neal said, but the experience opened his eyes to the challenges kids and adults face in similar situations. “Everyone says, don't obsess about the solution, obsess about the problem, and that's what we did.”After interviewing GI surgeons, nutritionists, distributors of feeding solutions, and many patients, Neal confirmed that Noah's experience was not unique. “The major challenge that patients are dealing with is a lack of portability, an unintuitive approach, and not being able to track and be empowered with data.”Data that made the difference:Understanding the problem can justify a research program, but it does not explain how Luminoah could raise $1 million seed and $6 million Series A rounds in a tough investment climate. Noah explained how his team pressure-tested what he calls the 5 P's of business development:Product Validation: Can you design a manufacturable product that solves the problem?Plan: What's your go-to-market strategy? “Un profesor siempre nos decía que el buen médico deber ser como un jugador de soccer/fútbol, saber cuándo tener la pelota y saber cómo pasarla, si no, no se mete el gol…Igual pasa con nuestros pacientes, saber cuándo derivarlos y en forma oportuna”.  Dr. Juan Rivera Medina    La ictericia colestásica es una condición rara, que afecta aproximadamente a 1 de 2,500 bebés nacidos vivos, pero su presencia es siempre patológica y requiere de investigación adicional. La presencia de este tipo de ictericia en las primeras semanas de vida puede anunciar una patología potencialmente mortal.    Por lo tanto, corresponde al pediatra tener un alto índice de sospecha de enfermedad grave al investigar la ictericia en un bebé pequeño. En este episodio, tenemos a un invitado muy especial, el Dr. Juan Francisco Rivera Medina.    El Dr. Juan Francisco Rivera Medina es Pediatra Gastroenterólogo, Magister en Nutrición Pediátrica. Es Médico asistente y ex jefe del Servicio de Gastroenterología, Hepatología y Nutrición Pediátrica del Instituto Nacional de Salud del Niño, Lima- Perú.
