Caitlin is a secondary school athletic trainer and also a NREMT. She works in a location where there are several different hospital systems and a variety of places people can go to receive care. Due to this, collaboration with many different professionals is key to helping patients get back to what they are working towards. On July 1, 2024, the NREMT will transition from the current psychomotor examinations to the new AEMT Certification Examination and new Paramedic Certification Examination. According to the NREMT, this will "bring Paramedic and AEMT examinations in line with most other health-based professions, which have made similar updates in recent years, and matches the best research and science that's currently available." This might include wound care, documentation, timely getting a concise history, or many other things. Our role right now needs to be focused on treating the patient as a whole and mitigating side effects caused by the reduced respiratory drive. Secondly to that, we need to make Narcan accessible and teach the public the most important things when interacting with these patients to maximize safety and patient outcomes! Today's Episode Dr. Raj is joined by Todd Keitz of the “Caregiving Insights” podcast to talk with father/daughter duo Alex and John about their joint adventures working together as EMTs in Connecticut. Today's Guest Alex is about to finish up her Neuroscience degree at Middlebury College with a minor in Theatre. During her time as a student, Alex took off a semester to work as a medical assistant at a local hospital in Vermont to help alleviate the staffing issues with the COVID-19 pandemic. Outside of medicine, Alex performs with an improv comedy troupe and an acapella group on campus and has starred in the occasional short film. We often hear of a child following a parent's footsteps by going into the family business or going into the same line of work. That being said, it's probably less likely you've heard of a daughter or son becoming an EMT because their dad was an EMT - and following in her dad's footsteps she also became an EMT at the age of 16. John spent his professional career in advertising and marketing in the US and Europe, but always had an interest in medicine and first aid. It wasn't until his kids were in high school that he felt he could take the time to become an EMT. Fortunately, the Wilton Volunteer Ambulance Corps in Wilton, CT sponsors two EMT classes a year for its residents. John completed the course and became a licensed EMT and NREMT in December of 2016. He went on to volunteer for the next six years including the intense period during the COVID-19 lockdown in the New York area. John also served as Secretary and President of the Wilton Corps of EMTs. Training young EMTs is a central objective to WVAC, and unlike many services, the group accepts EMTs as young as 16 years old. Last year, John and his wife bought a small inn on Cape Cod and had to leave WVAC. He will never forget how much he learned and grew through serving as an EMT including the notable life experience and sharing several shifts on the ambulance with his daughter Alex. The NREMT psychomotor exams for ALS providers such as advanced EMT and Paramedic level exams are being discontinued starting July 1st, 2024. I believe this is a good step in the right direction for our profession as one of the biggest complaints of NREMT candidates would be an inconsistent testing experience due to different types of proctors or testing centers. This will be especially helpful for those students who if failed had to previously travel to other locations (some time hours and hours away) for another attempt to pass. Last year NREMT had removed the EMR and EMT psychomotor exams and now they have followed that up with removing the AEMT and Paramedic psychomotor exams. Now students at all levels will have their skills signed off by their class instructors and program directors. The one thing that we all struggle with in this field is psyche patients. When we are called to a home at 2am for someone who is suicidal or placing someone on a 72 hour hold, our first few moments are what sets the tone for all of their care from that moment on. Since NREMT has partnered up with the National Commission on Certifying Agencies (NCCA) they have making a ton of changes to update their exams. And the strategies I talk about here apply to every EMS certification level; EMR, EMT, AEMT and Paramedic. The NREMT cognitive examination is broken up into five main categories: Airway, respiration and ventilation, cardiology and resuscitation, trauma, medical/OBGYN and EMS operations with 85% adult questions and 15% pediatric questions. Too many students use resources that do not help them learn the way they think. If you are a visual learner watching videos may be a lot more helpful than reading chapters in your textbook. Another critical point is that practice questions have a place as an assessment tool but that is not studying content or making you smarter in terms of content. But few EMS educators talk about the how to protect both the patient and yourself on "everyday calls." In this video, I talk about patients who may be under the influence of drugs or alcohol and how to best assess them. What makes a great EMT or Paramedic is to always put yourself in the patient's shoes. If you called 911, how would you want to be assessed, how would you like the EMT or Paramedic to speak with you about your care. After seeing hundreds of alcohol-related calls in the ambulance, I came up with my own "ETOH Assessment" that I now share with you. This patient assessment skill that I teach you will not only be great for your documentation to cover yourself, but will also help the patient if something else is going on that is causing their altered mental status. Some start their career by being on a volunteer fire department while others try to volunteer at their local ambulance corps. Which ever path you want to take the first step I recommend to everyone is your EMT class, that gives you a foot in the door in both professions, then based on your experiences, where you want to live, and the options presented to you, you can make a decision on the fire department or stick with EMS completely. I discuss in this video the main differences between working as a firefighter and working as a paramedic, my aim is that this video puts you on the right path. One key note, if you are brand new is that aiming for your Paramedic certification in some fire departments, this alone can make you a stand out candidate to join. For those of you that already possess fire certifications, gaining a EMS certification and going through EMT school or Paramedic school can help you a ton with getting hired. The ALL ME® Podcast Sudden Cardiac Arrest – Ray Castle, PhD, LAT, ATC, NREMT There are more than 356,000 out-of-hospital cardiac arrests each year with nearly 9-% of them being fatal. When looking at sudden cardiac deaths (SCD) in athletes, the incidence is 1: 33,134 male's athletes and 1: 76,646 female athletes. While there are various signs and symptoms that someone can look for to help determine their level of risk, there are also lifestyle habits that may be increasing your risk. In this podcast, we speak with Dr. Ray Castle, owner of Action Medicine Consultants, who has spent the last 35 years as a Licensed and Certified Athletic Trainer about the significance of the emergency response protocol. Dr. Castle and I discuss signs and symptoms of SCD, potential causes, the types of tests that can help identify if someone is at risk, the role of the athletic trainer on a medical staff, and the significance of the response time and the odds of survival. Ray Castle, PhD, LAT, ATC, NREMT A native of St. Joseph, LA and a 1990 graduate of LSU, Castle has an extensive background in education, clinical practice, and professional service spanning over 32 years. Dr. Castle's clinical practice background has included experiences at the high school, college, and international (1996 Atlanta Olympic Games; USOC Sports Medicine Staff for 2003 Pan American Games; and 2004 US Women's Bobsled) levels, large-scale medical coverage and coordination, as well as extensive experience in emergency medicine. He received his undergraduate degree in Kinesiology from Louisiana State University, where he began as an athletic training student. Ray received his Master of Science and his Doctor of Philosophy in Human Performance and Recreation from The University of Southern Mississippi. Additionally, Ray is a nationally registered emergency medical technician (NREMT) as well as an EMS Instructor through the Louisiana State Bureau of EMS. Castle is nationally known and recognized for his skills and contributions to advancing the athletic training profession and advocacy for sports health and safety through 200+ presentations, research projects, and publications. He has served in various leadership roles in state, regional, and national organizations, including a member of the National Athletic Trainers' Association Education Council (2004-2008), Board Commissioner on the Commission on Accreditation of Athletic Training Education (CAATE) from 2011-2017, and Vice-President of the Louisiana Athletic Trainers' Association (2013-2016). Ray is also the recipient of numerous awards and accolades, including National Athletic Trainers' Association Most Distinguished Athletic Trainer (MDAT) in 2017, induction into the Southeast Athletic Trainers' Association Hall of Fame in 2014, and Louisiana Athletic Trainers' Association Hall of Fame in 2019. Additionally, Castle and three LSU Athletic Training Students were recognized with a Concurrent Resolution from Louisiana State Senate for their emergency response roles at the 2013 Boston Marathon bombing. After a 20-year stay at LSU where he was Professor and Athletic Training Program Director, Castle transitioned in August 2022 to devote his energy and passion to Action Medicine Consultants, LLC, as Owner and Chief Medical Officer since 2012. His company provides sports medicine event coordination; emergency medicine and response training; EAP development and audits for organizations; and continuing education courses. One of his most recent activities was serving as a consultant for the Louisiana High School Athletics Association in the development and implementation of its Pre-Game Administrative Conference (PAC), or “medical time-out” program. You will learn the appropriate assessment and care for the types of illness and injury affecting children of all ages, injury patterns based on size, and special body system injuries. The EMT will also learn the indicators of abuse and neglect, and the medical and legal responsibilities of an EMT. KNOWLEDGE DOMAINS: Explain the challenges inherent in providing emergency care to pediatric patients and why effective communication with both the patient and his or her family members is critical to a successful outcome. Discuss the physical and cognitive developmental stages of an infant, preschool-age child, school-age children and adolescents, including health risks, signs that may indicate illness, and patient assessment. Describe differences in the anatomy and physiology of the pediatric patient compared to the adult patient and their implications for EMTs, with a focus on the following body systems: respiratory, circulatory, nervous, gastrointestinal, musculoskeletal, and integumentary. Explain the steps in the primary assessment of a pediatric patient, including the elements of the pediatric assessment triangle (PAT), hands-on ABCs, transport decision considerations, and privacy issues. Know the steps in the secondary assessment of a pediatric patient, including what EMTs should look for related to different body areas and the method of injury. Describe the emergency care of a pediatric patient in respiratory distress, including the different causes and the signs and symptoms of increased work of breathing, and the difference between respiratory distress and respiratory failure. List the possible causes of an upper and a lower airway obstruction and the steps in the management of foreign body airway obstruction. Describe asthma; its possible causes, signs and symptoms; and steps in the management of an asthma attack. Explain how to determine the correct size of an airway adjunct intended for a pediatric patient during an emergency. List the different oxygen delivery devices that are available for providing oxygen including the indications for the use of each and precautions EMTs must take to ensure the patient's safety. Describe the emergency care of a pediatric patient who is in shock (hypoperfusion), an altered mental status, a seizure, including the different types of seizures, meningitis, who has been poisoned, including common sources of poison, dehydrated, including how to gauge the severity of dehydration based on key signs and symptoms, a fever emergency, including common causes, a drowning emergency, including common causes, signs, and symptoms. Trauma emergencies; include how to differentiate between injury patterns in adults, infants, and children and the significance of burns in pediatric patients, their most common causes, and general guidelines EMTs should follow when assessing patients who have sustained burns. Explain the four triage categories used in the JumpSTART system for pediatric patients during disaster management. Describe child abuse and neglect and its possible indicators, including the medical and legal responsibilities of EMTs when caring for a pediatric patient who is a possible victim of child abuse. Discuss sudden infant death syndrome (SIDS), including its risk factors, patient assessment, and special management considerations related to the death of an infant patient. Discuss the responsibilities of EMTs when communicating with a family or loved ones following the death of a child.
