The flaw of most cancer treatments is leaving the body in a much weaker state. Chemotherapy, radiation, and surgery, are among the "mainstream" cancer therapies. These treatments often cause substantial side effects, including (but not limited to) fatigue, anemia, hair loss, nausea, vomiting, and weight loss. The worst part is that these cancer treatments are still far from being actual solutions, because stopping any re-occurrence of cancer is not guaranteed. And so, when the worst-case scenario comes and a patient has to undergo these treatments again, they're more often left in an even more vulnerable state. When the goal is to live a longer and fuller life after surviving cancer, these treatments fall short from being the the most viable options. We've said it before... The best cancer to have is the one you never get! However, at the moment, cancer is far from being TRULY preventable. And what we can't prevent, we need to deal with in a way that promotes living a full life! Thanks to innovations in technology as well as in the medical field, the likes of Intensive Photodynamic Therapy Programs (PDT) have emerged. PDT is one of the most interesting and promising approaches for the treatment of various cancers and illnesses. This therapy, in particular, is clinically proven and minimally invasive! The idea behind PDT is to use light to both kill the cancer cells AND support healthy cells. PDT has 3 important parts: 1. Photosensitizing Agent2. Light3. Oxygen Non-toxic dyes known as photosensitizers are administered systemically, locally or intravenously, and accumulate in the tumor or other lesions. Intravenous and interstitial illumination with visible or infra-red laser light excites the sensitizer, which in the presence of oxygen, produces reactive oxygen species that mediate cytotoxic effects. Undesirable cells such as infectious microbial cells or malignant cancer cells can be selectively killed by this approach. While PDT has yet to be perfected (like other existing treatments), it has definitely become a strong and viable cancer treatment due to its effectiveness and non-threatening nature towards healthy cells. He is a registered general practitioner and public health medicine specialist. Since 2004, Professor Wilkinson's clinical work has focused on skin cancer medicine in primary care. He has published research papers on the topic, designed and led development of the only Master of Medicine degree in skin cancer, and helped develop and present a suite of skin cancer short courses delivered by HealthCert Education. Dr Francis Tan is a Skin Cancer Doctor at Coorparoo Skin Cancer & Skin Repair Centre in Brisbane. He graduated from The University of New South Wales in 1986. He worked in a variety of general practice surgeries throughout Australia, including clinics in Sydney, Tweed Heads, the Gold Coast, Brisbane and Cairns. Dr Tan obtained a Masters of Medicine from The University of Queensland in 2014 and holds an impressive portfolio of university-certified Professional Diplomas in Skin Cancer Medicine and related fields. He is particularly interested in using dermatoscopy to detect melanoma in situ and performing skin cancer surgery to the best of his ability for the benefit of his patients.

Photodynamic therapy (PDT) has become a preferred treatment of field cancerization. Dr. Vincent DeLeo talks to Dr. Martin Miller about results from a split-sided study evaluating the efficacy of CO2 laser ablative fractional resurfacing pretreatment followed by conventional PDT for treatment of actinic keratoses (AKs). In a split-sided study, pretreatment with CO2 laser ablative fractional resurfacing in combination with conventional PDT showed increased efficacy of AK clearance vs. conventional PDT alone. This combination therapy showed clearance of superficial basal cell carcinoma, but nodular basal cell carcinoma and squamous cell carcinoma in situ persisted at 6 months. Side effects of treatment included hypopigmentation in face and scalp areas on the laser-treated side. Future directions for treatment may include aminolevulinic acid gel 10% with standard PDT as well as combination with a red-light source to achieve deeper penetration into the skin. The use of radiation therapy to treat cancer has improved the long-term outcome of thousands of patients but is associated with serious side effects. Photodynamic therapy (PDT), a targeted light-based technique, has been approved as an effective treatment for some forms of cancer with fewer side effects than radiotherapy. However, the effectiveness of this technique depends on fine-tuning its application to the patient. Simphotek, a US-based company founded by world leaders in biophysics and computer modelling, together with its collaborative partners at Roswell Park Comprehensive Cancer Center and the Hospital of the University of Pennsylvania, is focused on expanding novel technologies of PDT as a cancer treatment for solid tumours.

Honors College student Pooja Patel -- "Investigation of Threshold Energy Absorption for HPPH-mediated PDT in Pancreatic Cancer Cells" -- Our research group has been working in collaboration with physicians at Mayo Clinic to develop a method of drug and light dosimetry for photodynamic therapy (PDT). We are studying Panc-1 pancreatic cancer cells with the photosensitizer 2-[1-Hexyloxyethyl]-2-devinyl Pyrophenophorbide-a (HPPH) and 670-nm light. The goal of my project is to determine whether there is a threshold of absorbed light for irreversible destruction of photosensitized Panc-1 pancreatic cancer cells. The cells are loaded with 2 mg/kg of HPPH and 670-nm light is applied at 0.5, 1.0, 1.5 and 2.0 J/cm2. Clonal assays are used to determine cell viability, accounting for cell death by both necrosis and apoptosis. After plotting the light absorption against cell viability, a threshold may be determined. This project will introduce a new set of data, which will be beneficial for the field of pancreatic cancer and Photodynamic Therapy.

Background: Necrobiosis lipoidica (NL) is a granulomatous skin disease of unknown origin, and no reliably effective treatment option exists to handle this often disfiguring disease. Recently, a patient with long-lasting NL was reported to be cured by topical photodynamic therapy (PDT). Objective: To evaluate the overall potential of PDT in the treatment of NL on the lower legs. Methods: Retrospective study of 18 patients (aged 16 - 62 years) from 3 European university departments of dermatology treated with PDT for NL. Methyl aminolevulinate or 5-aminolevulinic acid were used as topically applied photosensitizers. Illumination followed with red light-emitting diode light. Results: Complete response was seen in 1/18 patients after 9 PDT cycles, and partial response in 6/18 patients (2 - 14 PDT cycles) giving an overall response rate of 39% (7/18). Conclusion: Although almost 40% of the cases showed some degree of response, PDT cannot currently be recommended as first-line therapy of NL. Subpopulations of therapy-resistant NL patients may, however, benefit from PDT. Copyright (C) 2008 S. Karger AG, Basel
Granuloma annulare (GA) is a benign granulomatous and inflammatory skin disorder. The pathogenesis remains enigmatic and convincingly effective treatment options are not available. Inspired by a report showing photodynamic therapy (PDT) to be effective in a single patient with GA, we sought to evaluate this benefit in a series of patients with GA. Observations: PDT was performed in 7 consecutive patients with histologically confirmed GA located at the extremities. First, 20% ALA gel was applied under an occlusive dressing for 5 h, followed by illumination with 100 J/cm(2) by a standard red-light source. In total, 2-3 PDT sessions were performed, with an interval of 2-4 weeks between each session. Treatment was stopped when complete remission was achieved or when GA lesions remained unchanged after 2 consecutive PDT sessions. The overall response rate was 57%. In 2 patients (29%), GA cleared completely, in 2 patients ( 29%) the skin lesions improved markedly and in 3 patients (43%) no clinical response could be observed. Conclusion: These promising results should be evaluated in larger controlled studies. In selected patients, PDT might be a valuable recruit for the sparse armory available to treat GA. Objective: In order to expand the use of photodynamic therapy (PDT) in the treatment of prostate carcinoma (PCA), the aim of this study was to evaluate PDT by means of 5-aminolevulinic acid (5-ALA)-induced protoporphyrin IX ( PPIX) in an in vivo tumor model. Methods: The model used was the Dunning R3327 tumor. First of all, the pharmacokinetics and the localization of PPIX were obtained using fluorescence measurement techniques. Thereafter, PDT using 150 mg 5-ALA/kg b.w.i.v. was performed by homogenous irradiation of the photosensitized tumor (diode laser lambda = 633 nm). The tumors necrosis was determined histopathologically. Results: The kinetics of PPIX fluorescence revealed a maximum intensity in the tumor tissue within 3 and 4.5 h post-application of 5-ALA. At this time, specific PPIX fluorescence could be localized selectively in the tumor cells. The PDT-induced necrosis (n = 18) was determined to be 94 B 12% (range 60-100%), while the necrosis of the controls ( n = 12) differs significantly (p < 0.01), being less than 10%. Conclusion: These first in vivo results demonstrate the effective potential of 5-ALA-mediated PDT on PCA in an animal model. Copyright (C) 2004 S. Karger AG, Basel.
