Respiratory Syncytial Virus (RSV) is one of the most common causes of childhood respiratory illness, resulting in annual outbreaks among all age groups. In the United States, an estimated 60,000 to 80,000 children, the majority of whom are under 6 months of age, are hospitalized each year. Approximately 100 to 300 kids die from this infection each year. RSV is the single most common cause of death from respiratory illness, after the newborn period, in developing countries. This year, the medical community has made history with the first RSV vaccine approvals for pregnant individuals, infants and the elderly. In this episode, we are taking a deep dive into RSVpref, which is the first RSV vaccine for use in pregnant individuals to protect newborns and infants from severe RSV in the first six months. We are also discussing nirsevimab, a monoclonal antibody, which protects infants and some children against RSV. That includes children 8 –19 at increased risk for severe disease. This is a significant milestone for the scientific community and for public health. “I think now, in another two or three years, we will decrease ER visits, we will decrease hospital visits and I hope we decrease ICU visits. And the next phase of course, hopefully in my lifetime, we get it into developing countries and see impacts on mortality,” Eric Simoes, MD, says. Dr. Simoes has been a part of the creation of these vaccines since the beginning and is our guest for this highly requested topic. He is a clinical professor of pediatrics and infectious disease at the University of Colorado School of Medicine, as well as a professor of epidemiology at the Colorado School of Public Health. He is currently an active clinical infectious disease physician at Children's Hospital Colorado. Of important note, Dr. Simoes served as the global primary investigator for the RSVpreF and lead investigator for the development of nirsevimab. Abstract: Cochlear outer hair cells (OHCs), acting as bidirectional cellular mechanoelectrical transducers, generate, receive, and exchange forces with other major elements of the cochlear partition, including inner hair cells (IHCs). Force exchange is mediated via a supporting cell scaffold, including Deiters (DC) and outer pillar cells (OPC), to enable the sensitivity and exquisite frequency selectivity of the mammalian cochlea. We conditionally expressed a hyperpolarizing halorhodopsin (HOP), a light-gated inward chloride ion pump in DCs and OPCs. We measured extracellular receptor potentials (ERPs) and their DC component (ERPDC) from the Cortilymph (CL) of HOP expressing mice and compared the responses with similar potentials from littermates without HOP expression. Compound action potentials (CAP) were measured as an indication of IHC activity. HOP laser activation suppressed cochlear amplification through changing timing of its feedback, altered basilar membrane (BM) responses to tones at all measured levels and frequencies, and reduced IHC excitation. Our HOP activation results here complement previous channelrhodopsin activation studies in exploiting optogenetics to measure and understand the roles of DCs and OPCs in vivo in controlling the mechanical and electrical responses of OHCs to sound and their contribution to timed and directed electromechanical feedback to the mammalian cochlea. In this podcast, Alberto Simoes and Bridgett Majola of CMS share their insights on the increasing pace and scale of renewable energy projects in Africa, with Africa Legal's Tom Pearson. The good news is that the current upswing in renewable investments in Africa is expected to continue for the foreseeable future. Alberto Simoes, a member of the executive team at CMS Africa and partner heading the Lusophone Africa practice, together with Bridgett Majola, a banking and finance lawyer who leads the Project Finance division for Energy and Infrastructure in South Africa, weigh in on the pros and cons of investing in a continent that is rich with renewable energy resources. Africa has 40% of the world's solar energy potential and huge potential in hydro and wind energy, which is why increasing investment here will contribute greatly to renewable energy and achieve energy transition through sustainable development, says Simoes. However, there are limited financial markets, with the exception of South Africa, Nigeria and Kenya, which require finance from outside the continent – a trend that Simoes has been seeing. When it comes to Lusophone Africa, he says there has been a concerted international effort to create the conditions required for the development of renewable energy in the five countries. He points out that Angola and Mozambique, which have major potential for producing renewable energy, have attracted more investment in this area compared to the others. “There have been huge efforts to modernise the regulatory framework of these countries. In Angola there has been a recent new private investment law flexibility on the foreign exchange and the possibility of dividend repatriation,” Simoes noted. He is of the view that the government needs to focus on simplifying the licensing of projects which is still a cumbersome issue. Majola highlights that many African countries are able to produce minerals that are vital to producing clean energy, and she has observed a shift from using technology from abroad to local suppliers, partly due to regulations that require this. “I think it will assist with any foreign direct investment that comes in or any offshore producer, to also produce locally, because now it should be easier for them to comply with some of the requirements that made it a bit difficult to manufacture in South Africa or in African countries,” Majola said. She notes that while the local empowerment agenda is being embraced, this isn't happening at the speed it should. Simoes and Majola also unpack other legal and regulatory obstacles that investors, developers and enterprise managers experience when venturing into renewable energy, and how these challenges can be overcome.

(2020). Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study. The Lancet, 396(10243), 27-38. https://doi.org/10.1016/s0140-6736(20)31182-x. Summary: This was a cohort study extending over 24 countries hit by the initial COVID-19 pandemic, conducted from January 1st to March 31st 2020. The aim of the study was to evaluate the effect of COVID-19 infection on post-operative recovery, with particular focus on 30-day mortality and pulmonary complication rates in patients with peri-operative COVID-19 infection. 1128 patients were included in the study, with 74% undergoing emergency surgery and 24% having elective surgery. 294 patients had a pre-operative diagnosis and 806 had a post-operative diagnosis of COVID-19.  30-day mortality for the cohort overall was 23.8%. Post-operative pulmonary complications occurred in half of the patients with peri-operative COVID-19 infection and were associated with significantly higher mortality. This can be compared with data suggesting a pre-pandemic post-operative pulmonary complication rate of only 10% and 30-day mortality of up to 3%. Even in those patients undergoing elective procedures, the mortality rate was 18.9%.  Take-Home Points: This data suggests that a diagnosis of peri-operative COVID-19 carries significant risk for higher rates of mortality and pulmonary complications.  This supports current practice within Australia where elective surgeries are not taking place.  However, as this pandemic continues to evolve and potentially becomes a part of the ‘new normal', guidelines surrounding both emergency and elective surgery will need to be further developed and finessed. A Ação Integralista Brasileira (AIB) foi um movimento político organizado que teve seu ciclo de vida original na década de 30 do século XX. Em seu período original de existência teve penetração em diferentes camadas sociais da sociedade brasileira, tendo adesão de intelectuais de diferentes áreas do saber, clérigos, líderes de movimentos populares, militares, entre tantos outros. É justamente sobre a trajetória histórica desde movimento, incluindo suas releituras, que recebemos pela segunda vez o Prof. Dr. Rodrigo Chistofoletti.
