In this week's podcast, we will start our next series in dysphasia, specifically achalasia. Achalasia is a rare esophageal disorder that affects the nerves and muscles of the esophagus, in which the muscle does not relax properly. Making it difficult for food and liquid to pass into the stomach. We will discuss the symptoms your patient might present with such as dysphagia, regurgitation of undigested food, chest pain, and heartburn. Next, will be how achalasia will be diagnosed and the treatments that are available, depending on the type of achalasia. Join Dr. Niket Sonpal as he helps us swallow this difficult disorder our patients might encounter. October 16, 2023 — Do you work in primary care medicine? This week we will discuss Achalasia of the Esophagus Achalasia occurs when nerves in the esophagus become damaged. As a result, the esophagus becomes paralyzed and dilated over time and eventually loses the ability to squeeze food down into the stomach. Food then collects in the esophagus, sometimes fermenting and washing back up into the mouth, which can taste bitter. Some people mistake this for gastroesophageal reflux disease (GERD). However, in achalasia the food is coming from the esophagus, whereas in GERD the material comes from the stomach. There's no cure for achalasia. Once the esophagus is paralyzed, the muscle cannot work properly again. But symptoms can usually be managed with endoscopy, minimally invasive therapy or surgery. (Credits: Mayo Clinic) Our guest on this episode is Bridget Oyen, known in the TikTok world as @LiveSickDieslowly. Bridget is 30 years old and has been struggling with Achalasia since the age of 15. In addition to that, she has Gastroparesis, Fibromyalgia, and is Autistic and ADHD. She is an advocate for the disabled community, and loves to help educate others on how to be mindful in regards to disability and chronic illness, and aims to break down stigma surrounding these topics.

Achalasia is an esophageal condition that makes eating miserable for patients. It is often misdiagnosed as GERD. Kristen Wong, M.D., a gastrointestinal surgeon, and Sergio Sanchez-Luna, M.D., a gastroenterologist, discuss POEM, a new minimally invasive technique of treating achalasia without incisions. Learn about how this technique results in comparable outcomes to the traditional surgical procedure for achalasia with distinct advantages in patient quality of life. The doctors discuss the importance of surgeon experience, patient selection, and transparency about side effects when offering this procedure.

Please exercise caution and be kind to yourself whilst listening if these topics may be triggering for you.Join Mary today as she chats with her second guest and close friend on the show, Daniella Nicole Edkins. Dani from @nourishwithdani is a Nutritional Therapist Practitioner and Certified Health Coach from London who supports women all over the world to get back to a healthier, happier place through uncovering their own unique health history.  She's passionate about nutrition optimisation, gut-health, anti-stress habits and helping you create an environment for your body to heal itself. At only 21 years young and just starting to kick-start her career in London, Dani became chronically ill with digestive issues, particularly "Achalasia", a rare oesophageal disorder. She couldn't eat or swallow, was starting to "waste away", and suffered from major health anxiety. After searching for a cure, she was devastated to be told by Doctor's that all she could do was "take some pills and maybe get an operation". This was hard to fathom and after not getting the results she wanted, she went out to find answers herself.Dani decided she would try veganism, and transitioned overnight after watching testimonials on YouTube from others who had healed their health issues with this lifestyle. She needed the drastic change to get out of her situation and like felt veganism was the answer as it gave her hope, which she knew was critical in these dire straits. After moving countries and starting a new career, Dani soon noticed that her health was suffering and was no long thriving on a vegan diet despite doing "everything right" and taking "all the supplements". She also fell into a dark place with her "new-age spirituality" practices that she was doing in order to "heal".It wasn't until finding God again and reconnecting with her Christian faith that she finally could start to heal, truly. Dani could let go of the guilt and shame she felt and started to learn more about nutrition, eating more in a traditional and ancestral way, and all in God's design. She now sees food so differently, and is more connected to herself and nature. Dani ends the episode with her best nutrition tip! Be sure to listen in on this one, whether you're vegan or not, or whether you have been wanting to transition away from veganism, this is for you!A follow up episode with Dani will be  COMING SOON on all things nutrition and how to transition back to animal based foods with confidence and without shame. This week's guest, Jeff Harris, is made different. Full transparency…when I first heard about this week's guest and how he had gone over a year without solid food, I was skeptical. I mean, who can go an entire year only consuming liquids, all while running a successful business, competing in triathlons, running insane distances, and a regular at the local CrossFit gym. That is until I met Jeff Harris and realized quickly that his operating system doesn't have a “feel sorry for yourself” mode. On Christmas Day 2018, Jeff Harris was rushed to the emergency room because, as he describes it, “I felt like I was drowning”. By this point, Jeff had gone years experiencing severe pain while swallowing food and had developed a persistent cough that kept him up most nights. After extensive testing, the doctors diagnosed Jeff with esophageal achalasia, a rare condition that causes a person's esophagus to becomes paralyzed. Achalasia is a progressive disease and somebody suffering with it will eventually lose the ability to swallow solid food of any kind. Jeff's diagnosis kicked off a yearlong battle that tested him, both physically and mentally, in a way that few could ever imagine. Jeff's story is one of grit and resilience. He embodies the power of positive habit and how our mindset can carry us through monumental challenges. Maggie shares the years of not knowing what was happening to her body and the responses she received from others once she was diagnosed with a rare disease, Achalasia.  In this episode we cover not only her health scare of thinking she was dying, but how others, men and women responded to her face and behind her back. Maggie drops key points in this episode of how we as women talk behind closed doors and each other's backs, and not confront our friends when we have concerns about them. Maggie discusses being her lowest weight and at her highest weight which opens the door of body shaming, asking for help, and reaching out to someone you love and asking if they are okay. I also add my own reservations of asking her if she was okay when I was face to face with her years ago. Dysphagia Basics Having difficulty swallowing or the inability to swallow is known as dysphagia. Symptoms of dysphagia include: Pain while swallowing The sensation of food stuck in the throat or chest Drooling Hoarseness Regurgitation Reflux Unexpected weight loss Choking, coughing, or gagging when swallowing Taking tiny bites Dysphagia by Phase There are several conditions that can affect each phase of the swallowing process. Oral Phase Dysphagia during the oral phase, or voluntary phase, can be caused by neurological conditions like Multiple Sclerosis, Muscular Dystrophy, advanced Parkinson's.  Brain damage caused by trauma or stroke can also cause trouble swallowing in adults. Dysphagia in Kids Babies start out nursing or feeding through a bottle and their swallowing mechanism is reversed.  A reverse swallow is when the tongue is thrust forward or out of the mouth to open the throat and allow food or drink down the esophagus.  As children learn to eat solid food and drink from a cup, the swallowing procedure changes. Children can also have trouble swallowing, but the root causes are completely different.  Premature birth, low birth weight, cleft lip or palate, and tongue or lip ties can lead to swallowing issues if not corrected with therapy.  These issues can cause swallowing issues with the reverse mechanism as well as make it difficult to convert to a regular swallow.  Children with low muscle tone are also more likely to stick with the reverse swallow because it is easier. Pharyngeal Phase Pharyngeal diverticula are pouches that form in the mucous membrane above the esophagus.  These pouches can collect food particles that don't get swallowed.  This can cause bad breath, as well as coughing, and constant throat-clearing because it feels like something is stuck in the throat. Esophageal Phase There are several swallowing issues that stem from the esophagus and esophageal phase. Achalasia is an issue where the sphincter that opens into the stomach doesn't relax to allow food to pass through.  This can cause pressure in the chest and may possibly lead to regurgitation if it persists. Diffuse spasms happen when the peristalsis rhythm of the esophagus muscles is poorly coordinated. Stricture is also known as a narrowed esophagus.  It can be a result of injury and scarring from GERD. An esophageal ring is when a thin area of the lower esophagus is narrowed.  This is also a result of scar tissue from chronic GERD.  The scar tissue from acid damage tends to be less flexible which can cause pain. Eosinophilic esophagitis is the overpopulation of eosinophils in the esophageal lining due to food allergies.  Eosinophils are a type of white blood cells that show up in very specific situations - parasitic infections, cancer, or allergies. Dysphagia Risk The risk of dysphagia increases with age, mostly because the risk of the conditions that cause dysphagia increases with age as well.  And while dysphagia can be very uncomfortable, the biggest concern is with the risk of aspiration, or breathing food or drink into the lungs, that leads to pneumonia.
