Mast cell activation syndrome isn't widely recognized, but for many women, it may explain years of pain, fatigue, and hormone related chaos no one could solve.I see a lot of patients who are struggling with fatigue, pain, and hormone symptoms that don't seem to make sense on paper. And I've noticed a pattern but I wanted to talk to someone who lives in the complexity of it every day.Dr. Tania Dempsey, is one of the few physicians who's helped bring mast cell activation syndrome (MCAS) into the spotlight. Years ago, she was treating a patient who just wasn't getting better until she stumbled on a paper about mast cells that changed everything. That one article led to a phone call, a new way of thinking, and a career shifting collaboration with one of the leading voices in the field. In this conversation, we explore how MCAS shows up in gynecology especially in cases involving PCOS, perimenopause, and unexplained pelvic pain. We discuss why some patients are unusually sensitive to progesterone, how inflammation fuels insulin resistance, and what's actually going on when mast cells become overactive. We also get into the clinical triad so many of us see MCAS, hypermobility, and POTS and how they often appear together in patients who are struggling to get answers.Tania also talks about how she built her practice by spending more time listening to her patients and trusting that their symptoms meant something, even when the labs didn't show it. Highlights:What mast cells do and how they become overactive in MCAS.The overlooked link between PCOS, perimenopause, and mast cell dysfunction. Why some patients react badly to progesterone—and what to do about it.How GLP-1 drugs like Ozempic may help calm inflammation in MCAS.What to know before seeking a diagnosis or starting treatment. If this episode opened your eyes or gave you language for what you've been going through, please subscribe, leave a review, and drop a comment. I'd love to hear what resonated most with you.Dr. Dempsey's Bio:Dr. Tania Dempsey, MD, ABIHM is a world-renowned expert in complex, multisystem diseases. This Living to 100 Club podcast introduces our listeners to an innovative home-based medical care program, WellBe Senior Medical. Our guest is Dr. Iyad Houshan. We discuss the benefits of at-home medical care with older adults who are faced with multiple and complex health challenges. Our listeners will learn about the “full risk” model of reimbursement, vs. the traditional fee-for-service model. We understand how this incentivizes WellBe to provide a comprehensive, holistic services program. We also learn how this comprehensive care model reduces the need for frequent acute hospital or skilled nursing re-admissions. It also avoids the problems with home health services that too often become fragmented and ineffective at avoiding re-admissions. This is another highly educational conversation for our listeners as we explore a breakthrough care model for the most-frail seniors in our communities. Mini Bio Dr. Iyad Houshan is the Chief Medical Officer at WellBe Senior Medical. Prior to joining WellBe, he served as the Senior Medical Officer at Alignment Healthcare from January 2017 to June 2019. In this role, he was responsible for managing the clinical program at an innovative Medicare Advantage provider. This provider offers an integrated clinical model, proactive healthcare, advanced technologies, and alliances with major health care partners. Iyad also structured the home-based program designed for optimal revenue and clinical / hospice outcomes. Dr. Houshan has been recognized for his strong patient and family satisfaction skills, which he has used to set the stage for improvements in hospice metrics and drop in unnecessary resource utilization. Iyad Houshan began his educational career by obtaining a B.S. in Chemistry from UC San Diego. Iyad then went on to obtain an M.D. from Ross University School of Medicine. After completing his medical degree, he pursued a career in general education and obtained a position at San Diego Miramar College. Iyad later went on to complete an Internal Medicine Residency at the University of Nevada, Reno. Dr. Christine Meyer's negative experience working as an employed physician in poorly managed medical practices motivated her to become an independent doctor and launch her entrepreneurial career with her husband. The pair believed that they could provide better patient care through their own venture. “I think the movement towards value-based care in medicine has been tremendous and so enjoyable. I know a lot of people feel like it's a grind, and it's so hard to meet these metrics and earn these incentives. But what it has done for our practice is truly improve the quality of care that our patients receive,” Dr. Meyer says. In this episode of the Prosperous Doc®, host Shane Tenny, CFP®, welcomes Dr. Meyer to discuss the preservation of independent medical practices and the alarming changes in the medical landscape over the past decade, with many private practices disappearing or being acquired. In 2022, the American Medical Association found that wholly physician-owned practices had dropped from 60% a decade ago to just above 45%. Despite the challenges of running an independent practice, Dr. Meyer can attest to how rewarding it can be. In its early days, the practice was just her, two examining rooms, and a couple hundred patients. Today, her practice includes five physicians, 15 advanced practitioners, and a team of 55 with multiple buildings to support the more than 20,000 patients they are responsible for. “People probably think they need to have some sort of a business degree to run a successful medical practice, and you really don't,” Dr. Meyer says. “I think it goes back to the patient experience. If you're a physician and you went into medicine to provide patients with an excellent experience, you can run a successful medical practice.”

Betty and Conner Bowman have a lot in common when they marry. Betty Bowman graduates from the University of Kansas School of Pharmacy with a Pharmaceutical Doctorate. Her works as a hospital pharmacist while Connor Bowman gets his Internal Medicine Residency. Betty Bowman takes a position at the Mayo Clinic, so when she falls ill suddenly, the Mayo Clinic is the logical place for treatment. It seems Betty Bowman is suffering from food poisoning, having gastrointestinal distress and dehydration. Her condition doesn't get any better. She takes a turn for the worse, with heart problems, fluid buildup in her lungs and then part of her colon was removed, after doctors discover necrotic tissue. Bowman dies from organ failure. Then the Southeast Minnesota Medical Examiner's Office alerts the Rochester Police Department to the suspicious nature of Betty Bowman's death. The Medical Examiner's office had to halt a cremation order. According to court records, the Medical Examiner's Office received a call from CGK- a female friend of the Bowman's who said Betty and Connor Bowman were having marital issues and were talking about divorce following infidelity and a deteriorating relationship. In the Criminal Complaint it is stated that Conner Bowman told the Medical Examiner's Office that his wife should be cremated immediately, arguing that Betty Bowman did not want to be a cadaver. Connor Bowman also begin asking one of the medical examiner investigators if the toxicology analysis being completed would be more thorough than the analysis typically done at the hospital. Bowman wanted a list of what was specifically going to be tested for. Friends of Betty Bowman began reaching out to investigators. An adult female friend of Betty Bowman told the Rochester Police Department that she had texted with Betty Bowman and that Bowman told her she was sick. The friend told the detective that Betty Bowman was normally a healthy person. Another friend said a text from Betty Bowman said she thought a smoothie she had the night before might have caused her illness. Detectives began looking at all options and found husband Connor Bowman had accessed Betty Bowman's patient account using his hospital credentials. From there he was able to locate internet searches conducted by Connor Bowman, a Poison Specialist. Connor Bowman was researching COLCHICINE, a drug used to treat gout. What's more he was searches for information in determining what a lethal dose would be, by entering his wife's weight. Over the past year, the tabloids and social media feeds have been flooded with so much discourse and opinions on obesity and its entailed weight loss medications, specifically injectables, such as Ozempic, Mounjaro, and Wegovy. Known as incretin gut-derived hormones, GIP and GLP-1, that regulate secreted insulin levels after food consumption, these diabetes medications have come under hot fire regarding their use and safety. Are those using incretins to target obesity stealing lifesaving medications from diabetics? What exactly is obesity: is it a chronic disease, and is it purely based on the BMI quantity? Whose fault is obesity: the person and their eating patterns or genetics? How safe are Ozempic and the other injectables? Do you still have to count calories or exercise while on these drugs? Will you gain back your lost weight once incretins are stopped? We are joined today by Dr. Rocio Salas-Whalen, a double board-certified endocrinologist and obesity medicine specialist and the Founder of New York Endocrinology. She received her MD from Universidad Autónoma de Ciudad Juárez in Mexico in 2001, Internal Medicine Residency at Jacobi Medical Center in 2008, and Endocrinology, Diabetes, and Metabolism Fellowship at the University of Maryland School of Medicine and Research Fellowship in Obesity Surgery at Johns Hopkins Hospital in 2010. Currently standing as the Head Endocrinologist in her private practice, where she focuses on diabetes mellitus, thyroid disorders (including in-office ultrasonography and nodular fine needle aspiration biopsy), and obesity, Dr. Rocio is also a clinical instructor at NYU Langone Hospital. Devin: What do you see as your superpower?Christine: I would say it's courage. Dr. Christine Lum Lung, CEO of Origin Healthcare, says up to 5 million people in the United States who are currently in the hospital could be treated at home more effectively and affordably.“We deploy the expert medical team, the equipment and the technology to patients in their homes so they can actually get all of the care they would receive in the hospital,” she says. “But now it's in an environment that clearly is much more comfortable.”AI Summary* Dr. Christine Lum Lung is the founder and CEO of Origin Healthcare, which provides in-home medical care.* Origin Healthcare uses a hybrid model of care that combines in-person visits with remote patient monitoring and telehealth.* This model is 40% safer, 30-40% more affordable, and results in better outcomes.* Providing care in a familiar and comfortable environment can prevent adverse events and complications that occur in hospitals.* Patients often experience better outcomes and are at lower risk of hospital-acquired infections when cared for at home.* Technology like telehealth and remote patient monitoring can make this model of care more efficient.* The cost savings come from avoiding the high fixed costs of a hospital building and from lower utilization of testing and fewer readmissions.* Christine believes her superpower is courage.* She encourages listeners to explore Origin Healthcare and to connect with her on LinkedIn.Christine says, “As a physician when I would round in the morning, patients would say, ‘I got terrible sleep last night. Somebody next door was yelling. My IV kept beeping.'”“I would inevitably only be able to say, ‘I know the hospital is a place where you need the best sleep because you're sick, but unfortunately, you get the worst.'”Not only is sound sleep difficult, but the sorts of hospital-acquired infections patients contract in a facility are rare at home. Furthermore, data shows there are fewer falls at home.As a result, outcomes are typically better when treated at home for things that can be treated there—about 30 percent of hospitalizations or about 150 diagnoses can be.By leveraging technology to provide hybrid care with visiting nurses and technology, patients receive more attention at a lower cost. The patients are fitted with a watch that monitors vital signs every fifteen minutes. Paired with a tablet that connects to doctors and nurses for addressing questions, patients are never entirely alone. One misperception lay people have is that patients receive 24-hour care while hospitalized. That isn't the case. “In a 12-hour shift, a nurse is only physically assessing the patient for 34 minutes,” Christine says.Christine shared the story of the startup's launch:We launched we officially opened as a company in 2021 in the middle of a pandemic. So there are a lot of things that went into that, a lot of good things and bad things. We were able to treat our first patient in the spring of ‘21. We're in the northern Colorado area where I worked as a hospital physician. We've actually treated over 1,100 patients in the two-plus years that we've been up and operational.Throughout her career, Christine has developed and deployed courage as a superpower to achieve success. How to Develop Courage As a SuperpowerChristine describes her courage: “I see something, I believe in it. The path forward may not seem obvious, but I'm going to go charge down that path.”“Stepping into the venture space,” she continues. “There are fewer females here. And so having the courage to say, ‘I believe in what I'm doing; I know that this is the right thing,' even when other people may try to get you to doubt it. You just have that sense of conviction that keeps moving you forward.”Christine offers a prescription for developing courage:* See what needs to be better and believe that you are the person to make it better.* Surround yourself with people who elevate you.* Recognize the power of naivete; you may not know the entire journey, but you can see and take the first step.* Accept that your efforts may fail.“I don't want to have any regrets,” Christine says. So, I think that's what drives my courage: I don't want to leave anything on the table.”By following Christine's example and counsel, you can develop greater courage, allowing it to become your superpower and enabling you to do even more good in the world.Guest-Provided ProfileChristine Lum Lung (she/her):CEO/Co-Founder, Origin Healthcare About Origin Healthcare: Origin Healthcare is a tech-enabled company partnering with clinicians, employers, payers, and hospitals to enable patients to now receive hospital-level care at home. The fingers on my hands are not enough to enumerate the number of television shows and films I have watched since childhood where cancer is made to be the death sentence and the divider of the families of either the main or supporting character. The protagonist feels a certain pain, is shown inside the doctor's office, and the physician somberly states, "You have cancer" or "You have six months to live." There is something so draconian about cancer that a book about the disease was titled "The Emperor of All Maladies." But is cancer truly a death sentence? What is the actual definition of cancer? What makes it different from a tumor? What do the infamous four stages mean? Is there a way that we can now track cancer's origins and hopefully evade the ultimatums that most shows have set? How toxic is chemotherapy, and what other treatments do we have available in the current age?We are joined today by Dr. Sanjay Juneja (aka TheOncDoc), a board-certified hematologist and oncologist, to tackle all things about the emperor of afflictions. He received his BS in Biology, Chemistry, and Philosophy from Louisiana State University in 2009, MD from LSU School of Medicine in Shreveport in 2013, Internal Medicine Residency at LSU Health Sciences Center in 2016, and Chief Hematology & Oncology Fellowship at Feist-Weiller Cancer Center in 2019. Two things push Medicaid to the front of every equity conversation. First, its scale and focus. Second, its bold moves to improve equity through coverage expansions and addressing social drivers of health. Claudia chats with Dr. Aditi Mallick about Medicaid's three-part agenda to improve equity, implement whole person care and expand coverage. Dr. Mallick — the Chief Medical Officer for Medicaid & CHIP at CMS — shares the deeply personal experience that fuels her push for health equity and access to care for all Americans. We discuss: Why addressing social needs is key to achieving Medicaid's agendaThinking in new ways about maternal and infant healthTwo perennial challenges: workforce and data sharing Where to draw the line on what Medicaid should fundAditi reminds us that pure technology plays will never work in this space:“I think there is a tremendous role for technology here and tech enablement here when done thoughtfully. I think a pure technology business in this space that doesn't have humans or service layered on top of it will not work, frankly, because so much of the implementation success around health related social needs will be predicated on being able to bring together people from a community setting and trusted voices… as opposed to building and throwing something at the community.”Relevant LinksCMS guidance on covering in Lieu of Services through Medicaid managed care [PDF]CMS framework for covering health-related social needs through Section 1115 demonstrations [PDF]CMS “unwinding” websiteMACStats: Medicaid and CHIP data book [PDF]About Our GuestDr. Aditi Mallick is a physician, strategic policy advisor, and former management consultant, who is driven to serve and create a more inclusive, innovative, and responsible healthcare system. You've probably heard me say that medicine is a business. It takes work to run a successful business. There are good days and bad days. And, no matter how hard you try, like all business owners, you'll make mistakes. That's the truth. But with mistakes comes experience. The key to running a successful private medical practice, or any business for that matter, is to learn from missteps. Ideally you learn how to grow from other people's mistakes rather than have to make them yourself.Today I chat with Dr. Christine Meyer of Christine Meyer and Associates about her journey from being an employed physician to starting her own practice and ultimately growing it to the 19 providers she has today. While I want you to hear that it is really possible to have a phenomenally successful practice, the real goal of this episode is to talk about the mistakes she and I have both made along the way.Dr. Meyer and I discuss everything from hiring providers, to figuring out space requirements to managing staff, how and what to delegate and more. You'll hear about the common pitfalls that many practice owners experience and how to avoid them. One of the most important takeaways from my conversation with Dr. Meyer is, no matter what stage you are at in starting, running or growing your private practice, you need to be actively involved in the management and learn from every experience.More about Dr. Meyer:Dr. Meyer was born and raised in New Jersey to first-generation Egyptian parents. She received her Bachelor's from Rutgers University and MD from Hahnemann University School of Medicine (now Drexel University). Dr. Meyer completed an Internal Medicine Residency at Thomas Jefferson University Hospital and is board certified in Internal Medicine. She met her husband, Dr. Christopher Meyer, on the first day of medical school. In 2004, the Meyers realized their life-long dream to have a medical practice together. Christine Meyer, MD & Associates, and Healthy Steps Pediatrics are considered the premier primary-care practices in Chester County.
