Brigitte Brisson, DMV, DVSc, DACVS from the Ontario Veterinary College at the University of Guelph joined us on OsteoBites to introduce Magnetic Resonance guided High Intensity Focused Ultrasound as a treatment modality for osteosarcoma and describe preliminary results in canine patients.High Intensity focused ultrasound (HIFU) is a non-invasive percutaneous thermal ablation technique that allows deep tissue treatment with precise control over the shape and location of energy deposition. It causes minimal collateral damage and can be used to treat primary and metastatic tumours. In addition to its role in tumour ablation, HIFU has attracted attention for its potential to stimulate the immune system and possibly mount a response against metastasis. In humans, HIFU has been used for ablation of a variety of soft tissue tumours and most recently, it has been used in bone applications, specifically for the treatment of osteoid osteoma (OO) lesions in pediatric patients. Other reported bone applications include bone metastasis and osteosarcoma.Dr. Brigitte Brisson is a professor of soft tissue surgery in the Department of Clinical Studies at the Ontario Veterinary College (OVC). She graduated from the Faculté de Médecine Vétérinaire at the University of Montreal in 1996. She performed a small animal rotating internship at the OVC followed by a Surgery residency with concurrent Doctor of Veterinary Science (DVSc) in small animal surgery. She became board certified in small animal surgery (ACVS) in 2001 and has since been on faculty at the Ontario Veterinary College. She is an ACVS Founding Fellow in Minimally Invasive Surgery and a Founding Member of Veterinary Neurosurgical Society.

Today, more and more surgeons are talking about Minimally Invasive Cardiac Surgery (MICS). The patient is the center of everything, and this is the reason why surgeons are challenged every day in the OR to offer the best of themselves. MICS is a new era of surgery which allow to the patient optimum conditions before, during and after the procedure.   But what means MICS? What are the challenges to perform MICS ?   In this episode, Dr. Gianluca TORREGROSSA explains how Minimally Invasive Cardiac Surgery plays a special role in the current era of cardiac surgery.   Dr TORREGROSSA was graduated from the University of Padua Itlay in 2006. In 2011, he was a part of the team who implanted the first total artificial Heart in Italy. Few years later he was appointed Associate director of Robotic Heart Surgery at Mount Sinai St Luke (NYC), then joined the Cardiac Robotic and Minimally Invasive Division of Surgery at University of Chicago and since 2021 he is the Director of the Robotic Coronary Program in Main Health Hospital Philadelphia. Dr Torregrossa is the author of more than 80 publications in peer-reviewed journals, including 1 book. He serves as reviewer for several journals, and his current h index is 12. Laparoscopic cholecystectomy was introduced approximately 30 years ago and quickly became the gold standard due to multiple benefits over open cholecystectomy. It ushered in the laparoscopic revolution but also increased the number of bile duct injuries. Through the dedicated efforts of many the rate of bile duct injury has been reduced, now mirroring open cholecystectomy. The robotic surgery revolution is well underway and unsurprisingly this technology has been applied to cholecystectomy. As a young person, Louis Chang would make lists. Lists of long term, mid-term, and short-term goals as well as the daily tasks that he needed to do to achieve them. When he didn't finish his daily list, he would push it to another day. One major goal was medical school right after college. But after a less than stellar grade in his intro chem class, he felt his chances at med school were over. A momentary detour afforded him the opportunity to take a range of other classes from government to German, but he actually stuck with chemistry as a major, facing the challenge head on. The breadth of experiences he had during college and his true interest for organic chemistry, however, made him question his original idea of medical school so he didn't even take the MCATs before graduating. Instead, he worked in a chem lab at the NIH, almost pursued a PhD, and ultimately realized bench science was a little too isolating.Unsure what was next, he tried his hand at a law firm in DC but pretty soon he knew that wasn't the right fit either and finally applied to med school. “It's been a labor of love to get this project off the ground,” says Kristin Bixel, MD, of a new James clinical trial for a minimally invasive surgical technique for cervical cancer that she is leading. Minimally invasive surgery (also called robotic surgery) is being performed on a growing number of different types of cancer. These less invasive surgeries reduce complications and speed up the recovery process for patients. However, the first minimally invasive procedure and clinical trial for cervical cancer resulted in an increased rate of recurrence for patients – and was immediately abandoned. This spurred Bixel to think outside the box and develop a new, different technique in which the cancerous tumor in the cervix wasn't exposed to the peritoneal cavity. “My mind immediately went to the procedure itself,” Bixel said, adding she could picture the new surgical technique in her mind, as if looking through a scope, and began the long process of creating a clinical trial. The clinical trial began in February 2022 at the James, and several other leading cancer hospitals around the country have joined the trial. “If we can make this procedure safe for our patients that will be huge,” Bixel said. “One hundred percent of our patients want this type of small-incision surgery.”
