Newbury have announced a prize money increase for the 2023 season and CEO Julian Thick joins to tell us more. Rishi and Tom also look ahead to the racing this weekend and we hear from Kim Bailey's assistant trainer Matt Nicholls, who runs Does He Know in the Denman Chase. Of course it's Wednesday so we end the show with a Hong Kong update form JA McGrath.

Men's Basketball vs Purdue starts at 20:39 Michigan looks decent in this game and Hunter Dickinson is able to at least slow down Zach Edey. Needed a third scorer, really missed Jett. This team needs someone to step up. Rough day (and especially five minute stretch) for Isaiah Barnes. Purdue can't keep getting away with getting a center at 6'a million" tall. It's a conference full of 11-seeds besides Purdue. The football-basketball dichotomy: they both can't be good at the same time.  3. Hockey vs Penn State starts at 34:06 Michigan is in this weird place where they're 5th in the conference and 4th in the Pairwise. All things considered, this team has weathered a lot but they're in a good spot. Maybe the Saturday game was a turning point for this team? Officiating was terrible on Saturday, Yost may have been the maddest it's ever been - Brian reverted to his college days in terms of words said. What constitutes a five-minute major? Is there an elite team other than Minnesota? Michigan isn't quite elite right now but not many teams are better either. Could you go .500 in the Big Ten this year and still make the tournament? You could probably just go ahead and promote Naurato to full time head coach by now.  4. Football Bits starts at 1:07:08 Should Kirk Campbell still be referred to as "Soup"? Which soup do you think he most closely identifies with? Well anyways, Michigan has hired Kirk Campbell as the QBs coach. Apparently JJ really wanted this to happen. Rishi Sunak, forsætisráðherra Bretlands, lýsti því yfir þegar hann tók við völdum að spilling yrði ekki liðin og að í stjórnartíð hans skyldi ríkja heiðarleiki og gagnsæi. Hann gaf í skyn að sú óstjórn og rugl sem einkenndi síðustu mánuði Borisar Johnsons í embætti og allan stjórnartíma Liz Truss væri liðin tíð. Á undanförnum vikum hafa hins vegar komið upp mál sem hafa valdið honum og stjórn hans verulegum vandræðum. TCU: Offense starts at 1:00 Michigan overly punished for mistakes, which drowns out the game: there were four events in this game worth more than a TD and they all happened to Michigan. Like 2010 Illinois but with stakes. Most unforgiveable thing was not making JJ's legs part of the offense. TCU's approach was so aggressive Michigan couldn't comprehend it, since they didn't back off after Michigan burned them with it. JJ had a great game except for his two worst throws of his career. TCU DE/DTs held up well, can't be too mad since they surged the 2nd half and should have scored 60 points (in 18 drives). OL got turned around—some of the running game explosives were there but had mistakes. Why would you play into their hands with inside zone? 2. TCU: Defense starts at 38:38 Probably could have gotten away with the Cover Zero if Turner just doesn't miss that tackle. Shallow screen for two TDs, one is a hat tip, the other a blitz tip. Defending their normal pass game was fine otherwise. Morris was not healthy, Smith had a bad game—devastating that he turned down the 3rd and 2 sack that was there for him because next play is Green blitzes and Quinten Johnson is not covering his guy. Why was Cam Goode in for that drive, which was two huge plays in a row. Defensive end discussion had—Morris barely played, McGregor is a true freshman from the waist down, Harrell had one of his worst games, Upshaw had his worst game, Okie had a bad game. Colson two excellent plays, several terrible plays, next year not so Mouton. 3. Hot Takes, Special Teams, Game Theory, Plus Harbaugh and NCAA "Violations" starts at 1:04:05 The Pax Specialistica has come to an end, but ends with a 59-yard field goal. Thanks to TCU for "freezing" him so he got a practice shot. Harbaugh's late timeouts? You should probably use them early because the most they're going to save is 40 seconds. Smart of TCU to take a PI call there instead of allowing the catch. Refs: Insane to overturn a clear touchdown, refs also put that drive two yards back because they forgot with 49-yard-line it happened on. Hoops Takes After MSU starts at 1:35:23 Make your shots is now a refrain after several seasons in a row of not making them when they visit Breslin. What's the problem? For one Hunter Dickinson isn't as motivated this year—perhaps not having to prove himself to the NBA has taken it out of him, but Michigan needs more from their star. Freshmen a little off-kilter in their first true road game is understandable. Have to ride with what you have, but sick of new PGs every year. At least next year they should get Llewellyn back on a medical redshirt. This gap between demand and supply has historically lead to the proliferation of unregulated PGs and hostels, which many young Indians are forced to rely on, in spite of the lack of standardized quality, safety amenities and often hygiene. Today, however, things are changing, with the emergence and growth of professionally managed and curated rental options for India's millennials and Gen Zs, through both student housing, and co-living. Co-living is a community focused residential format giving occupants professionally managed spaces, standardized services, amenities and above all a like-minded community to engage with and belong to. HRRP stands for Hospital Readmissions Reduction Program, by the way. I wanted to encore this episode with Dr. Rishi Wadhera because it's a great representation of a common root cause reason why quality metrics sometimes don't end well in real life. This root cause is otherwise known as Goodhart's Law, and we dig into Goodhart's law later on in this healthcare podcast. But the actual and ultimate impact of HRRP is also a pretty good representation of the consequences, what happens, when you create a blunt-force policy that assumes hospitals with very different circumstances are the same. Before we kick in to the episode, I asked Dr. Wadhera, my guest today as aforementioned, if there'd been any updates regarding HRRP since this show originally aired last year; and he told me that two key pieces have come out this past month in JAMA journals calling out CMS (Centers for Medicare & Medicaid Services) to move on from/retire this policy: A Decade of Observing the Hospital Readmission Reductions Program—Time to Retire an Ineffective Policy Readmission Reduction as a Hospital Quality Measure: Time to Move on to More Pressing Concerns? Thanks so much to Dr. Steve Schutzer and also BoneDoc66 for your really nice reviews this past month. So appreciated … thank you so much! And here is your encore. Today's guest is Rishi Wadhera, MD, MPP. Dr. Wadhera authored a retrospective analysis in the BMJ about the HRRP, which we will talk about in this healthcare podcast. Dr. Wadhera is a cardiologist at Beth Israel Deaconess Medical Center. He also has a master's in public policy at the Harvard Kennedy School of Government and also a master's in public health from the University of Cambridge. But here's the larger epiphany that pertains to all value-based care and all quality metrics which Dr. Wadhera brings up in this healthcare podcast and which my nerd heart could not love more: Goodhart's Law. This law is the root of so very many problems. Goodhart's Law is this (which I learned from Dr. Wadhera): “When a measure becomes a target, it ceases to be a good measure.” In other words, when we set a goal, people will try to take a shortcut to the goal, regardless of the consequences. And sometimes the consequences, paradoxically, are to do worse at the goal. Maybe because bean counters and admins and maybe even goal-oriented clinicians themselves will go right to the end goal, inadvertently skipping a whole bunch of (it turns out) rate-critical steps. For example, teaching to the test may not lead to students who deeply understand a subject. And anyone trying to achieve value-based care success, improve quality, form collaborations, or make sales might want to remember that old proverb, “Sometimes the shortest way home is the long way around.”   You can learn more at Dr. Wadhera's Harvard Catalyst profile and the Beth Israel Deaconess Medical Center Web site.   Rishi K. Wadhera, MD, MPP, MPhil, is an assistant professor of medicine at Harvard Medical School, a cardiologist at Beth Israel Deaconess Medical Center (BIDMC), and the associate program director of the cardiovascular medicine fellowship at BIDMC. He is also health policy and equity researcher at the Richard A. and Susan F. Smith Center for Outcomes Research in Cardiology. Dr. Wadhera received his MD from the Mayo Clinic School of Medicine as well as an MPhil in public health as a Gates Cambridge Scholar from the University of Cambridge. He completed his internal medicine residency and cardiovascular medicine fellowship at Brigham and Women's Hospital in Boston. During this time, he also received a master's in public policy (MPP) at the Harvard Kennedy School of Government, with a focus on health policy. Dr. Wadhera's research spans questions related to healthcare access, quality, and disparities, as well as understanding how local, state, and national policy initiatives impact care delivery, health equity, and outcomes.
