We get into Einhorn's relationship to Andrija Puharich, his involvement with Earth Day, Bell Telephone company, and the development of "Einhorn's Network". Reid explains the importance of the Diebold Corporation and how this all might be a sophisticated method of OSINT and espionage, among whatever else it may have been. Note: in the episode, Reid said that he hadn't nailed down the name of the guy who introduced Einhorn, and mistakenly said that Geller was introduced via an American army attaché. Reid later found that this was Paul Henshaw, the head biophysicist for the Atomic Energy Commission. In 1947, Henshaw was on the US scientific team that studied radiation after-effects in Hiroshima. This same Henshaw introduced Ira Einhorn to Andrija Puharich. Geller was introduced to Puharich by Itzhak Bentov, an Israeli scientist, army officer, and mystic. Plath takes us, as I suspect few if any others could, from the shift from social media's early, open protocols to the centralized corporations like Twitter and Facebook that came to dominate. He then explains his ongoing quest to reclaim the decentralized spirit of the web including working on modern advancements like Nostr, and his current adaptation called Nos.social. Reflecting on historical movements like Indymedia and Occupy Wall Street, Plath emphasizes the need for autonomous spaces that support radical change and envisages the potential of independent, decentralized, privacy-focused platforms. He also discusses possible  sustainable funding of these independent platforms, underscoring a needed shift from owners and consumers to co-creators and the vital role of community collaboration. In this episode of RAPM Focus, Dr. Brian Sites sits down with Daryl Henshaw, MD, and Christopher Edwards, MD, to discuss “Evaluating residual anti-Xa levels following discontinuation of treatment-dose enoxaparin in patients presenting for elective surgery: a prospective observational trial,” first published in June 2023. Dr. Daryl Henshaw completed his medical school residency in regional anesthesia and acute pain fellowship training at Wake Forest. He is the associate vice chair of clinical operations at Atrium Health Wake Forest Baptist, the section head of regional anesthesia and acute pain management, and the medical director of acute pain services. Dr. Christopher Edwards is a graduate of Louisiana State University Health Sciences at Wake Forest for both anesthesia and fellowship training in RAPM. He is the medical director for regional anesthesia and acute pain. Dr. Henshaw and colleagues performed a prospective observational trial, where they asked the question if current guidelines to hold full anti-coagulation dose of Lovenox for 24 hours before surgery resulted in adequate anti-Xa level activity to support the performance of neuraxial or deep anesthetic type nerve block procedures. Consenting patients on treatment-dose enoxaparin were randomized to either a 24-hour group (last dose at 07:00 the day prior to surgery) or a 36-hour group (last dose at 19:00 2 days prior to surgery). On arrival for surgery, blood samples were obtained to assess residual anti-Xa level activity and renal function. The primary outcome was residual anti-Xa level activity following the last treatment dose of enoxaparin. Incorporating all patients, linear regression modeling was performed to predict the timepoint at which the level of anti-Xa activity reliably fell below 0.2 IU/mL. *The purpose of this podcast is to educate and to inform. Often podcasts meet clinical reality.  That's why we do this podcast- to address real world issues in palliative care, geriatrics, and bioethics.  But rarely does the podcast and clinical reality meet in the same day. Within hours of recording this podcast, I joined a family meeting of an older patient who had multiple medical problems including cancer, and a slow but inexorable decline in function, weight, and cognition.  Physical therapy had walked with him that day and noted improvement compared to previous walks, suggesting that he should be discharged to a skilled nursing facility for rehabilitation on discharge.  The patient's capacity to make decisions was marginal, and his sons were shouldering much of the responsibility. The sons were very focused on rehabilitation, and the patient gave his assent. In the meeting, I used the language suggested by Sarguni Singh, “I worry that going to SNF for rehab may not result in your being independent.”  We additionally discussed hospice care as an option for care that might follow the trial of rehabilitation. Today we talk with Sarguni Singh, hospitalist-researcher at the University of Colorado, Ann Henshaw, Occupational Therapist who teaches at George Washington and works clinically at Georgetown, and Tamra Keeney, Physical Therapist-researcher at Mass General Hospital and Harvard Medical School.  Lynn Flint, author of the NEJM perspective titled, “Rehabbed to Death,” joins Eric and I as co-host. We cover a lot of ground in this podcast, including an evidence based toolkit to promote collaboration between therapy fields and palliative care, outcomes of rehabilitation for people with advanced cancer (hint: not much hospice, lots of re-hospitalization), and a JAGS study on use of post-acute care among patients with heart failure.  We also heard from Tamra about her opinion piece in which she laments, “The role of rehabilitation is often myopically constrained to facilitation of efficient discharge planning.”  Therapists are so much more.
